Eligibility

Employees regularly working 30 or more hours per week are eligible to enroll in benefits.

You can enroll the following members in our plans:

• Your spouse (the person whom you are legally married to under state law); assuming they are not eligible for insurance through their place of work. If they are eligible for insurance through their place of work but join Serigraph's plan, a surcharge may apply.

• Your children up to the age of 26 are eligible to enroll in coverage. They are also able to enroll over 26 if they are incapacitated due to disability and primarily dependent on your support.

• Evidence of insurability may be required when adding dependents.

Enrollment Annual Open Enrollment

Open enrollment for current full-time employees is held at the end of every year. It is the one time each year that employees can make changes to their benefit elections without a qualifying event.

Qualifying Life Event

Election changes outside of an annual open enrollment period can only be made within 31 days of a qualifying event. A qualifying event includes, but are not limited to:

• Birth or adoption of a child

• Marriage or divorce

• Loss of other healthcare coverage

• Your child reaches the maximum age limit

• You or your child loses coverage under your spouse's employer plan

• Your spouse or child gains eligibility for benefits through his/her employer

• Your eligible family member gains or loses coverage under Medicaid or a state children's health insurance program (CHIP)

Make sure you notify Human Resources right away if you have a qualifying event and need to make a change to your coverage. Expect to provide documentation to support the qualifying event (i.e. marriage certificate, birth or adoption certificate, divorce decree, proof of loss of coverage). If changes are not submitted within 31 days of the qualifying event, you will not be allowed to make changes until the next open enrollment period.

How to Enroll

The UKG Benefit Events are set up to automatically take you through each step of the enrollment process for your eligible benefits.

If you have not updated your dependents and beneficiaries, you will need to do this BEFORE enrolling in your benefits. You will need the following information:

• Full legal name(s)

• Social Security number(s)

• Date(s) of birth

Enrollment Instructions

1. Log into Ultipro: https://NW12.Ultipro.com- This must be entered directly into the internet browser (Do not enter WWW.).

2. Username: SERI00XXX (4-digit employee number).

3. Password: Enter your password. If you do not have a password or haven't logged in before, enter your birth date (format mmddyyyy) as your default password.

4. Complete two-factor authentication process.

5. In the upper left-hand corner, click on 'Menu'.

6. Click on 'Myself' from the available options.

7. If a new employee or you have a qualifying event, select 'I have a qualifying event'. If completing benefit elections for open enrollment, select 'Open Enrollment'.

8. Select the matching plan year, which run on calendar years.

9. Welcome to enrollment- Please read the information before clicking next to move to another screen.

10. Move through the prompted pages to enroll in your benefits or waive coverage.

11. Review and submit your benefits.

Making the Most of your Benefits

Our goal is to help you and your family members stay healthy and use our benefits program to your best advantage. Here are a few things to keep in mind:

Preventative Care

Getting preventative care is one of the most important steps you can take to manage your health. When a condition is diagnosed early, it is usually easier to treat. Regular checkups can help you and your doctor identify lifestyle changes you can make to avoid certain conditions. Our plan covers preventative visits at no cost.

Review your medical bills and explanation of benefits (EOB) Make sure you always check your medical bills and explanation of benefits for accuracy. Medical billing is complicated and mistakes can easily happen. Make sure to contact UMR if you believe there may be an error.

Emergency Room vs. Urgent Care Clinics

Emergency visits should be used for a true medical emergency- such as any situation of a life-threatening condition, chest pain, shortness of breath, serious bodily injury, severe abdominal pains, or loss of consciousness. Otherwise, for nonemergencies call your doctor, our onsite clinic, or EZaccessMD for basic illness and injury. This will save you a lot of money and time.

Tier 1 Provider

Your copay will be the lowest and coinsurance coverage highest when you go to a tier 1 provider. Providers in this network have agreed to provide services at special negotiated rates. When you receive care from one of these providers, the reduced rates are passed on to you.

Medical

To better meet the needs of you and your family, Serigraph provides two medical plan options. Under all medical options, you will have access to NexusACO, a UMR Network, (within the designated 8 counties), and United Healthcare Choice Plus Network (outside of the 8 county area).

Prescription

Serigraph has partnered with CVSCaremark to provide you with the best cost savings for your prescription drugs. Members must be enrolled in a Serigraph medical plan to utilize these savings.

Mandatory Mail-Order Service

Medications taken on a regular basis (known as maintenance medications) are to be filled through the mail service or CVS Caremark Mail Service Pharmacy. How it works:

Ask your doctor to send an electronic prescription to CVS Caremark® Mail Service Pharmacy. This is the easiest way to get started- you can expect to get your medication in 7 to 10 business days. or

Request that CVS Caremark contact your doctor and get the process started for you. Once we reach your doctor and receive approval, it will take 7 to 10 business days for your medication to be delivered.

Keep tabs on your order

For peace of mind, check the status of your order. You’ll know when your order is processed, on its way and delivered.

Refill by text message Get automatic refills

Opt in for secure text alerts. you can reply with a simple “YES” when we text you a refill reminder.

If the price of your medication is cheaper through versus running through insurance, the cheaper price is automatically applied and still counted towards your deductible.

With auto refills, you’ll always have your prescription when you need it. We’ll keep track of refills and send your order automatically.

Onsite Clinic & Wellness Center

The Serigraph Health & Wellness Center is operated by Marathon Health, an onsite wellness center vendor, that provides primary care and prevention services, health risk intervention, health coaching, chronic disease management, and occupational medicine to Serigraph employees and dependents enrolled in a Serigraph Medical Plan.

Our providers have a passionate focus on promoting overall well-being, allowing you to form a trusted bond with them. This inspires healthier habits, keeps patients accountable for their own health goals, and empowers the patient experience.

Monday: 7:30am - 4:30pm

Tuesday: 7:30am - 4:30pm

Wednesday: 7:30am - 4:30pm

Thursday: 6:00am - 12:30pm

Friday: CLOSED

Onsite Fitness Center

All Serigraph Employees are eligible to use the onsite fitness center; including part- time and flex employees. To access the gym, employees MUST sign a waiver. Please see HR to sign the waiver.

Spouses or adult family members who are on our health insurance can also utilize the gym but must be accompanied by the employee of Serigraph when using the facility.

Sign-In

There will be a sign-in sheet located on the table in the fitness center and we ask you to please print your name with the time you entered and left the center. This will help us keep track of when the center is most utilized.

Security and entrance to the fitness center

The fitness center will be unlocked for your convenience to use 24 hours a day, 7 days a week.

- All members using the center on weekends or hours outside your normal shift must use the employee entrance on the west side of the building.

- If you have not been exercising on a regular basis before using the fitness center, start slow. You may wish to check with your physician if you have any health concerns such as high blood pressure, diabetes, etc.

- If exercising during the hours of 7:00 a.m. to 5:00 p.m., we ask that you keep the volume down on the TV or radio to not disturb our Graphic Services office area that is located on the other side of the east wall.

- If you are the last to leave the center, please turn off lights, TV, and fan.

Please clean and disinfect equipment after use.

Hearing Aid Benefits are covered at 100% for employees and children to age 19. Contact United Healthcare at 855-523-9355 between 8:00 am - 8:00 pm, Monday through Frida, or visit www.uhchearing.com to learn more about the ordering process and for a referral to a UnitedHealthcare hearing provider location.

Imaging

Bundled Procedures

Serigraph offers a variety of bundled procedures with our medical plan, such as orthopedic surgeries and various imaging options. These bundles provide our members with medical services at a cheaper rate.

Serigraph has partnered with Medical Diagnostic Imaging to provide you with no-cost imaging options. Mention you have Serigraph Insurance and give them your UMR insurance card. This allows MDI to know you are actively enrolled in Serigraph's medical plan; however, you will not be billed for these services.

Serigraph has partnered with Orthopedic Hospital of Wisconsin to provide our health plan members with low-cost orthopedic surgeries at one of the best Orthopedic hospitals in Wisconsin.

If you or a member of your family on our health plan needs orthopedic surgery, talk with a member of HR so we can begin the process of connecting you to the right information and set up an appointment. Covered surgeries through this bundle have a minimum co-pay; much less than you would through the medical plan.

Dental

Regular visits to your dentist can protect more than your smile; they can protect your health. Serigraph partners with Delta Dental to provide three levels of providers to offer you the best savings possible. Our plan covers all dental services and covers preventative services at 100%.

Vision

Regular eye exams can help detect other health problems. Your eye doctor may even spot other conditions such as high blood pressure or diabetes. We utilize Superior Vision as our Network Provider.

Flexible Spending Accounts (FSA)

A Flexible Spending Account (FSA) lets you set aside money before it's taxed, through payroll deductions. The money can be used for eligible healthcare and dependent care expenses you and your family expect to have over the next year. Unused funds at the end of the Plan Year may be forfeited under the IRS "Use-it-or-Lose-it" rule.

Your full elected amount is available at the start of the year, regardless of if you have contributed that amount.

-Over the counter medications

-Dental and vision expenses

-Medical care items such as crutches, bandages, diabetic testing kits and contact solution

-Orthopedic Expenses

You can access money only after it is placed into your account

-Child or adult care center that complies with State and Local regulations (not nursing homes)

-Sitter inside or outside the home

-Daycare during school vacation

-Nursery school even if the school provides education services

IMPORTANT: This is a fixed indemnity policy, NOT health insurance

This fixed indemnity policy may pay you a limited dollar amount if you're sick or hospitalized. You're still responsible for paying the cost of your care. The payment you get isn't based on the size of your medical bill. There might be a limit on how much this policy will pay each year. This policy isn't a substitute for comprehensive health insurance. Since this policy isn't health insurance, it doesn't have to include most Federal consumer protections that apply to health insurance.

Critical Illness & Cancer Insurance

When life takes an unexpected turn, your focus should be recovery. Critical illness and cancer insurance helps relieve financial worries by providing a lump- sum benefit payable directly to you to use as needed.

Medical Bridge Insurance

No matter how well you plan, you can't predict when sudden medical expenses could impact your life. If you're admitted to the hospital for a covered accident or sickness, Colonial Life could help pay for outof-pocket expenses such as deductibles, copays, and other expenses.

Wellbeing Assistance Benefit

The wellbeing assistance benefit is available to you with group accident coverage, critical illness & cancer coverage or medical bridge insurance as well as all your covered family members. This benefit can help reduce the risk of serious illness through early detection of disease or other risk factors, giving you more protection from the unexpected.

Protecting your income can provide you and your family with peace of mind if you have become disabled and cannot work for a period of time. Serigraph partners with RelianceMatrix to offer short-term and long-term disability plans for employees.

Life and AD&D Insurance

We know that financial security is important to you and your family. That's why we are providing you with a basic life insurance plan with accidental death and dismemberment coverage at NO COST to you. All full-time employees are automatically enrolled in company-provided coverage equal to 1X your salary. The original amount of coverage will reduce as you age and terminate upon your retirement or termination of employment.

Beneficiary Reminder

You must name a beneficiary for your life insurance benefit. It's a good rule of thumb to review your beneficiaries on an annual basis.

Voluntary Life Insurance

You have the option to purchase additional life insurance to supplement your basic benefits already provided by the company. These benefits are available for both you and your dependents at group rates, in which you pay 100% of the premium costs through convenient after-tax payroll deductions. You can only add coverage for your dependents if you elect coverage for yourself. If you are electing coverage above the guaranteed issue amounts listed below, an evidence of insurability application will be required.

Retirement Savings

One of the best ways to ensure a secure retirement is to start saving as early as possible. Our retirement savings program allows you to save for retirement on a post- or pre-tax basis.

Enrollment

Eligible employees will automatically become a participant in the elective deferral portion upon the hire date. After 6 months of service, eligible employees will automatically become a participant in the employer match portion of the plan.

Contributions

At the time of hire, you will automatically be enrolled in the Roth (after-tax) portion of the 401K plan at 5% of your compensation and invested into a target rate fund. Each year, on September 1st, the amount of your deferral will be increased by 1%, on a post- tax basis, up to a maximum of 15% in total contributions. You may contribute up to 100% of your eligible compensation up to the annual IRS limit. Elections can be divided between two plans:

- Traditional 401(k) account Pre-Tax

- Roth 401(k) account Post-Tax

Employees aged 50-59 and over 64,may also make a catch-up contribution up to the maximum of $7,500, after you have contributed the maximum level into the regular plan. If you are between the ages of 60-63, employees can make a super catch-up contribution up to $11,250. If you are interested in changing your deferral percentage, the funds you want to invest in, or opt out of the program or automatic increase, please visit 401k.associatedbank.com.

Vesting

You are immediately 100% vested in your own contributions. Serigraph will contribute 50% on the first 4% of employee contributions. Company contributions are vested on the following schedule:

Paid Holidays

Eligible full-time employees will be paid 80 hours of holiday pay annually or 8 hours of holiday pay at their regular hourly rate (a day's pay if salaried) for the following holidays:

- New Year's Day

- Good Friday

- Memorial Day

- Independence Day

- Labor Day

- Thanksgiving Day

- Day after Thanksgiving

- Christmas Eve

- Christmas

- Floating Holiday (to be decided annually by the Company)

Vacation Purchase Program

After one year of service, full-time employees can purchase up to 5 days of additional vacation per fiscal year. From the date of purchase, employees have one year to use. All purchased vacation must be scheduled in accordance with standard vacation scheduling procedures.

Employees are not eligible for this program if:

- they have received a suspension within the last 6 months

- were out on approved leave for more than one month and have not worked a full two weeks upon return

- already purchased maximum allowable hours within the fiscal year

- they have sold vacation within the last 6 months

Employee Assistance Program

All full-time Serigraph Employees and their family members are eligible to use Advocate Aurora Employee Assistance Program. EAP is designed to help you and your family find solutions that can help deal with the challenges of busy lives- solutions that get results quickly, conveniently and confidentially.

How EAP Works:

Help begins as soon as you make the first call. They will listen to your concerns and ask questions about your current situation.

Based on your needs, EAP will either:

Connect you directly with an EAP counselor

Schedule a consultation at a convenient time for you, or

Link you with specialized work-life services.

When to use Advocate Aurora EAP:

Consider calling the EAP when a problem:

Occupies too much of your time

Interferes with normal activities

Persists for more than 2-3 weeks
