Word of mouth training regime led to warehouse worker’s 14 m fall A logistics management company has been sentenced over the death of an employee who plunged 14 m from his material handling vehicle in Northamptonshire. to alert drivers to nearby controllers. The operators’ driving positions necessitated them turning 180 degrees to see through the rear window while reversing.
Andrej Grzeszczak was working at Great Bear Distribution’s premises in Desborough on 14 January 2016 when his vehicle was struck by another warehouse operations controller working in the same aisle. Grzeszczak’s vehicle toppled and he fell, sustaining fatal injuries. Great Bear Distribution pleaded guilty to breaching s 2(1) of the Health and Safety and Work Act and was fined £323,000 plus £30,000 costs. Northampton Crown Court was told there was no electronic or physical system to prevent two vehicles entering the same aisle. Nor did the vehicles have sensors
The company had failed to provide adequate training for its operations controllers, which the court considered to be a significant fault and directly causative of the accident because operatives ignored prompts on the computer system used to control vehicles that showed when one was already in an aisle. Shirley Plenderleith, head of public services at Kettering Borough Council, which brought the prosecution, said: “While Great Bear Distribution had numerous health and safety policies and procedures in place, they had failed to recognise that their operations controllers were being trained by word of mouth rather than through a structured and supervised training process. As a result, a culture had been introduced
which allowed a key safety control to be ignored. “By ignoring a clear warning on the operations system that Mr Grzesczak’s vehicle was already in the aisle, another vehicle was sent in and the collision occurred. This sad loss of a life is a clear demonstration to all businesses that proper structured training of all staff is key to a safe working environment for staff and others.” After the accident Great Bear Distribution, which says it “puts employees’ wellbeing at the heart of its culture”, installed a new version of the operations software, increased the number of CCTV cameras in the depot to 95, improved lighting on site, reviewed training and improved the visibility of vehicles and their controllers The company operates more than 7 million sq ft of warehousing space and an inhouse distribution fleet of more than 300 commercial vehicles.
Depressed workers with supportive managers take less time off Employees with depression who feel they are supported by their managers are less likely to take time off work than those whose bosses do not talk to them about their condition, a recent study from the London School of Economics and Political Science has found. Researchers examined survey responses of more than 16,000 employees and managers in 15 countries, including Brazil, Canada, China, Japan, Mexico, South Africa, South Korea and the US as well as seven in Europe, including Great Britain. They looked at the relationship between the way managers tackle depression and workplace
productivity. Findings showed that on average, people who lived in countries where managers avoided talking to them about their depression took an additional 4.1 days off work.
“It may be that a supportive manager is most important in helping employees to remain motivated and feel valued while performing their duties in the workplace,” the researchers said.
On the other hand, countries with a greater prevalence of bosses who actively offered to help employees who were depressed were associated with lower absence levels.
“Working in a setting where managers actively avoid employees with depression on the other hand may encourage avoidant behaviour among those employees, resulting in a tendency to take more days off work.”
Absenteeism was assessed using the question: “The last time you experienced depression, how many working days did you have to take off work because of your depression?”
The study, “Is Manager Support Related to Workplace Productivity for People with Depression?”, published in the BMJ, noted that managers in Asian countries tended to avoid talking about mental health.

Chubb Honoured with Two Gold Awards for Occupational Health and Safety Chubb Systems Limited has received a Gold Award from the Royal Society for the Prevention of Accidents (RoSPA) for the sixth consecutive year, and Chubb Fire & Security Limited received the Gold Award for the third time, following its first in 2016. RoSPA Gold Award winners are recognised for achieving a high level of performance while demonstrating well-developed occupational health and safety management systems and culture, outstanding control of risk, and low levels of error, harm and loss. Bonfire night funfair injury leads to £30,000 fine for Cambridge charity Cambridge City Council reviewed the charity’s planning arrangements and, taking advice from the HSE, decided to cancel the event. At Peterborough Magistrates’ Court last week, Cambridge Live pleaded guilty to breaching Section 3(1) of the Health and Safety at Work Act and was fined £30,000, plus costs of £6654. Matt Gowen, for the defence, said there had been “a wholesale change” at Cambridge Live since the incident. A process had been introduced for closing footpaths across the common during the setup and take-down of events, in line with the HSE improvement notice.
A charity set up by Cambridge City Council to hold live events has been fined £30,000 after a cyclist was struck by a fairground lorry during the set-up for a funfair held alongside the city’s annual Bonfire Night fireworks display. The accident, which resulted in a woman in her 50s losing her leg, took place on 4 November 2015 at Midsummer Common, Cambridge. According to the BBC, the cyclist was stationary when she was struck and became trapped under the lorry carrying dodgems. She received serious leg and abdominal injuries, spending several months in hospital where she underwent surgery. The accident also caused psychological trauma. The bonfire event went ahead as planned but organisers
cancelled the associated funfair, on the grounds that it would be inappropriate for it to go ahead. The HSE issued an improvement notice on 10 December 2015 that highlighted segregation failings, saying that Cambridge Live had failed to organise the set-up and take-down of events “in a way that allows pedestrians and vehicles to circulate in a safe manner”. The notice required compliance by 17 June 2016. However, in advance of the annual Midsummer Fair in June 2016, also organised by Cambridge Live,

The magistrate noted that a substantial fine would have an impact on the Cambridge community, but said there were a number of people using the common at the time of the incident who were at risk. In 2016/17, 272,488 people attended Cambridge Live Events, according to the company’s annual review. It had a turnover of £5.66m in 2016/17, of which £4.39m was income from charitable activities and £1.1m from trading activities such as catering.

The Health and Safety Executive for Northern Ireland (HSENI) has warned manufacturers and engineering companies who do not sufficiently manage exposure to metal working fluids (MWF) that they could face enforcement action. Designed to instill greater individual responsibility for OSH, the campaign also aims to encourage operatives to consider their responsibilities for wellbeing, use of personal protective equipment, traffic safety, and plant and equipment use.
The HSENI has announced a focused inspection campaign for October and businesses will be visited to check that they have sufficient safeguards in place. The inspection checklist includes making sure a suitable risk assessment is available for controlling risks and that machinery is adequately guarded.
The campaign, launched on 12 September, will feature a topic a month and will also seek to highlight good practice methods, encourage operatives to follow them and to benchmark themselves against good performers. The CIWM SIG also wants to encourage members to share successful workplace initiatives.
Inspectors will also check that businesses hold current local exhaust ventilation (LEV) records to prove that the system has been examined in the past 14 months, which is a legal requirement.
Earlier this year, the UK’s Health and Safety Executive (HSE) published Common Human Factors Underlying Worker Fatalities in the Waste and Recycling Industry, which found that knowledge-based errors appeared to be the most significant human factor underlying fatal incidents.
Businesses will need to show inspectors that they have either prevented or adequately controlled the exposure to MWF by inhalation and skin contact. Sometimes referred to as suds, coolants, slurry or soap, MWFs are used during the machining of metals to provide lubrication and cooling. As part of next month’s inspections, employers will also need to demonstrate the quality of MWF has been maintained and that they have taken measures to minimise the bacterial contamination of metalworking and associated washing fluids. Inspectors will expect businesses to show they have carried out health surveillance where necessary. The HSENI has warned that manufactures and engineering firms that have failed to comply could be handed improvement and prohibition notices to improve standards. Exposure to MWFs has been linked to serious and debilitating conditions, including dermatitis, occupational asthma, bronchitis, irritation of the upper respiratory tract and, in rare cases, a more serious lung disease called extrinsic allergic alveolitis.
HSE statistics show that the fatal injury rate for waste and recycling workers is 15 times greater than the average rate across all industries over the fiveyear period 2012/13-2016/17, even though the sector accounts for about 0.4% of employees in Great Britain. The research identified two human factors areas to prioritise to reduce worker fatalities – the design, use and maintenance of equipment and safety management systems.

Protection for National Rail College A key driver in the successful delivery of the UK’s £42.6 billion HS2 project, the new National College for High Speed Rail, is now protected by industry-leading fire panels from Advanced. The sheer scale of modern facilities, density of occupation and growing user demands present particular challenges for fire safety and protection. The speed of detection is crucial as it enables users to intervene at the earliest possible opportunity and prevent further escalation. Intelligent smoke, flame, heat and carbon monoxide detectors will detect minute smoke particles and will provide an instant fire alert. Importantly, they will also determine false fire phenomena due to issues such as toasters, poor housekeeping, airborne particles, welding, humidity and temperature change, and will not alarm in these instances. The impact of false alarms on the workplace is immense, as lost working hours and the upheaval of evacuating premises adds up to significant financial loss. Modern smart devices can even be adapted to reflect environmental changes in room usage. In the event of a fire or major alert, rapid and orderly evacuation is of the highest priority. Emergency evacuation highlights the need for greater control during a major incident and voice alarm systems play an integral role in managing these critical situations. Studies reveal that many people do not know how to react to conventional alarms such as bells or sirens. Many assume that it is a test or false alarm; others remain confused and unsure of what to do. Comprehensive messages about the nature of the incident, and the appropriate action to take, using clear language will minimise the potential for panic or confusion. Zoned voice alarm involves the delivery of highperformance messaging across multi-level, multi-occupancy buildings, offering controlled evacuation where selected floors are evacuated, while others are simultaneously alerted. Evidence has shown that multiple alerts are more effective; the combination of audio and media such as SMS has a greater success rate. Automatic coloured lighting systems can then be employed to direct personnel to the most appropriate emergency exit routes to further ensure personnel safety during the evacuation. It is vital to only evacuate those at risk and this combination of verbal instructions, messaging alerts and wayfinding will maximise safety procedures.

Could Be a Lifesaver GEZE UK has revised a piece of lifesaving learning - its popular RIBA-approved CPD on specifying, installing and maintaining ironmongery for fire doors.
The 40-minute seminar explains what fire doors are, identifies their various components, how they work, the standards and regulations which surround them and why smoke is so dangerous. Crittall windows help keep troubled youngsters safe
A state-of-the-art residential unit for vulnerable children and adolescents in the North West of England features Crittallâ&#x20AC;&#x2122;s Fendor CleanVent security windows. Junction 17, designed by architects Gilling Dodd, was commissioned by Greater Manchester West Mental Health NHS Foundation Trust to provide facilities for the care of 24 inpatients aged between 13 and 17. The unit, at Prestwich, is a Tier 4 CAMHS (Child and Adolescent Mental Health Services) facility for young people with significant mental health needs and who may pose a high risk to themselves and others. Typical symptoms or conditions may include self-harming behaviour and psychosis. For this reason windows in rooms to which patients have access must have an antiligature feature. Junction 17 has been designed with full input from the professional users of the building with the intention of providing a modern, uplifting and non-institutional appearance. In addition to the inpatient accommodation there is an intensive care suite, therapy and activity spaces and a fully-equipped college and learning zone. The aluminium CleanVent windows specified complement this modern, non institutional feel while satisfying the anti-ligature requirements, meeting the security level of the building as well as providing natural

INTRATONE ACCESS CONTROL TECHNOLOGY INSTALLED AT OVER 900 SITES FOR HOUNSLOW COUNCIL Door entry and access control equipment from Intratone, one of Europe’s leading manufacturers, has been installed at over 900 different sites across Hounslow Council’s estate. The access control systems across the Council’s residential and commercial portfolio had become obsolete with the desire to move from keys to a fob-based system. Residents had been complaining at the delays in receiving new keys – in some cases taking up to six weeks for them to be delivered. New fobs can now be obtained straight away by residents when visiting their local housing office through the new cloud-based technology. Since late 2016 the Intratone fob readers have been installed at low and highrise apartment blocks and Council offices, commercial buildings and operational sites, and complaints have dropped significantly. Ian Williams, Electrical Engineer for Hounslow Council is in no doubt as to the benefits of the new Intratone proximity readers: “We hadn’t heard of Intratone before it was recommended by the installer. So far, we have been very impressed – although it is a sizeable investment for the Council initially, the build and technology of the readers means that they will last for at least ten years. “At one point the fob programming was carried out by two full-time members of staff who would visit site daily to program residents’ fobs. This can all now be programmed remotely by the relevant housing officers at the local housing office which will be a large long-term saving to the council.” The cloud-based fob-reader system has significantly enhanced control over who has access, for how long and to which properties. Because it is cloud-based, changes made to key fobs are almost immediate and can be facilitated from anywhere where there is internet access – whether that’s at a PC, laptop or a smartphone on site.
“The simplicity of the database is such that some of our concierge and caretakers are being trained to manage the system themselves which frees up even more of the engineer’s time,” Ian adds. “With the older technology residents were also able to copy fobs in some local shops which did not give the council full control over who could access certain blocks.

Birmingham City Council’s vision for its Cycle Revolution is to make cycling an everyday way to travel by improving road safety and cycling conditions, with 10% of all trips in the city to be made by bike by 2033[1]. The initiative includes creating high quality cycle routes on two key roads into Birmingham: the A34 and A38. Amey, the company delivering the cycling infrastructure improvements in partnership with Birmingham City Council, have installed 299l/m of Charcon’s innovative Cycleway Segregation Unit to create the first section of a 4km, two-way, fully segregated route which shields cyclists from passing traffic, even when on the blind side of a lorry or bus. The standard, high-performance segregation unit has been used for
the majority of this section. And whilst it delivers visually, a new kerb featuring the aesthetics of granite was created especially for certain conservation areas which demanded enhanced visual appeal. The bespoke design features a bull-nose edge running along the carriageway, capable of withstanding vehicular impact, while a full batter profile on the cycleway side significantly reduces the chance of cyclists catching a pedal. A frog in the base of the unit improves the key into the bedding material, facilitating a swift installation as the units can simply be dropped into place – an attractive design feature for contractors under pressure to minimise impact to traffic, residents and businesses while works are ongoing.

Striking a balance between security, aesthetics and budget Jacksons Fencing Research Highlights Challenges Facing Architects When Installing School Fencing • A third of architects say that schools have weak points in their perimeters and entrances • Multiple entry points are the biggest access control challenge affecting schools (90%) • Nearly all architects are familiar with Secured by Design standards (95%) • 71% of refurbishment specifications are identical to the original ones Jacksons Fencing, the UK perimeter security solution provider, recently released insights into architects’ views on school safety from its research report ‘Protecting the Future’. Taken alongside teachers’ perspectives, the report highlights significant differences between the views of architects and school management as well as some reassuring common ground. On average, almost two thirds of architects know of schools being affected by fencing and gate issues (60%). While a similar amount of head teachers have

commissioned new access solutions at least once (63%) and over three quarters have had their perimeter inspected within the past five years (80%), these problems are clearly not being resolved. The majority of architects (71%) find that school security refurbishments use the original specifications with some value engineering while a significant number know of schools down specifying to save cost (17%). This is despite head teachers’ greatest concerns being unauthorised exits and entrances (30% and 25%), both of which can be prevented with well-designed and appropriately specified fencing and gates. Specification evasion Architects identify budget as the greatest challenge when installing school fencing (84%), followed by restricted installation times (48%) and working where children are present (39%). The high number of schools keeping the same specifications or down specifying may be a reflection of this concern for cost and minimum disruption. A perimeter’s price tag is not the most significant concern for schools, however. Performance is the highest priority (73%) for teachers, followed closely by security ratings and accredited products (68%). These come ahead of both lifetime and initial costs (64% and 59% respectively). Setting the standards Recent bomb threats sent to schools across the UK reinforced the crucial role that school staff play in protecting pupils from harm. Head teachers identify that creating a positive learning environment (86%) and keeping the school community safe (82%) as the most important parts of their positions. Ensuring their security solutions meet industry standards is one way of fulfilling both these aims. A third of architects are seeing Secured by Design (SBD) and LPS 1175 standards increasingly specified (31%). This is complemented with 95% being familiar with SBD. While change
is on the horizon, there is work to be done. Currently under half of architects know about LPS 1175 (47%). While head teachers are concerned about the safety of their pupils, 39% don’t know where to go to find out more about school security and access control. They do, however, have some awareness of safety guidelines, with half being familiar with the recommended height for school fences. A similar number know of the Royal Society for the Prevention of Accidents (RoSPA) guidelines, which apply primarily to playgrounds. An inside perspective Overall, architects’ views on school security are in line with teachers, with 32% and 29% respectively acknowledge weak points in the perimeter. There is also agreement between both parties about which safety issues are afflicting schools the most: multiple entrances, having difficult to see areas and having more than one building. Architects, however, are more aware of these problems than teachers, with a 40% average difference between their responses. This disparity was especially marked where 72% of architects said that aging fencing is an issue, compared to 16% of teachers.

The study which examines how tiredness and feeling fatigued impacts on performance at work, also shows 88% of workers in the industry don’t feel confident in speaking with their line manager about how it is impacting on their performance.

Extreme tiredness happens as a result of a number of factors including mental health and diet.”
Fatigue, which is defined as extreme tiredness resulting from mental or physical exertion or illness, is an epidemic which is stretching beyond work for employees in the construction industry, with 56% of workers saying it is affecting them at home too.
So what’s the solution? 16% of employees in the industry believe their employer could do more to reduce the risk of accidents and mistakes at work due to stress. Currently 85% believe their colleagues, including management, do not understand the potential consequences of fatigue and lack of sleep and only 8% say their workplace would accept tiredness or fatigue as a genuine reason to call in sick.
More than a third (37%) say they have lost concentration at work due to fatigue and tiredness, and almost half (44%) of employees in the construction industry regularly turn up to their jobs feeling too tired to work.
Richard continues: “Fatigue and tiredness is an issue that employers and particularly line managers should not overlook. Employers in the construction industry need to ensure there are clear health and safety regulations in place around fatigue.
The effect of fatigue is broad with people admitting to making errors and being forgetful (37%), being short tempered with colleagues (34%), drifting to sleep (23%) and even drifting off whilst driving (18%).
“The importance of rest time both at work and at home should be taken seriously and encouraged by employers, and fatigue should be considered just as important as any other physical injury or illness.”

Almost 70% of people in the UK wear glasses or contact lenses some or all the time, the third highest proportion in Europe, and this number is steadily rising.
S
afety eyewear suppliers are increasingly providing prescription safety spectacles or goggles to safeguard people’s eyes in the workplace, since ordinary prescription glasses do not offer adequate protection. Amongst many features that must be considered when buying protective eyewear, such as fit, comfort, suitability and style, the correct lens material is one of the most important. It is crucial to match the correct lens material to the job in hand, understanding the requirements of the wearer and the hazards they may be exposed to.
CR39 For everyday glasses and users of visual display units (VDUs), CR39
(Columbia Resin 39) lenses made of plastic polymer are commonly used, replacing the glass lenses of old, which were thicker, heavier and shattered easily. CR39 lenses have some advantages, such as being lightweight and offering good chemical resistance. However, CR39 does not have the impact strength of new, polycarbonate lenses, and is therefore not suitable for hazardous workplace environments where impact may pose a risk, since it may break and shatter. In addition, a UV coating must be added to CR39 lenses to protect the eyes from the sun’s harmful rays that can cause cataracts, macular degeneration or other eye diseases. CR39 is also thicker than more recently-developed impact-resistant
lens materials, making spectacles heavier and distorting the appearance of the eyes, rendering them less appealing cosmetically.
Polycarbonate lenses Originally developed for the aerospace industry, polycarbonate is now the ‘go to’ material for safety eyewear. Made by injection moulding, polycarbonate lenses are up to 10 times more impact-resistant than plastic or glass lenses and give high optical quality. They naturally provide 100% protection from the sun’s harmful UV rays without needing a special coating to be added. Polycarbonate lenses have exceptional strength and resilience and will survive the impact of a steel ball travelling at 160 km/

of the Correct Lens Safety Eyewear hr (45 metres per second) without cracking, chipping or breaking. They are 25% thinner and 20% lighter than CR39 lenses, making them more comfortable and aesthetically pleasing for the wearer. Owing to its versatility and robustness, polycarbonate can be used for rimmed, rimless and half-rimmed spectacles, allowing manufacturers to offer a large choice of lens and frame options to wearers. Polycarbonate lenses are slightly more expensive than CR39 lenses, but they are still very affordable, and their benefits more than make up for this. Because polycarbonate is a relatively soft material, which is what makes it flexible and able to absorb energy without fracturing, so a hard, scratch-resistant coating must be applied to the lens to help protect it and prevent surface scratching before use.
Check your glasses Prescription lens wearers may not be aware that some high-street opticians will automatically supply eyewear with CR39 lenses unless polycarbonate is specifically requested, potentially leaving them at risk from impact injuries at work. You can check your glasses by looking at the lens markings, which will be either at the top or on the side of the lens. Look for the letters
‘S’ or ‘F’. If you can see an ‘S’, your glasses are likely to be CR39. In conclusion, it is important to ask the optician which lens material will be supplied in your safety eyewear and to ensure it is the right one for you. For example, when inspecting rooftop solar panels of large solar farms, being able to clearly view temperature anomalies allows problem panels to be quickly isolated and fixed keeping energy production at optimum levels.
