Ram Jack is pleased to submit this proposal based on this description and the attached terms and conditions for: SCOPE OF WORK The following scope of work was designed by Ron Bitler PE with RB Engineering. Building #6 (A-K) 1. Excavate rear corner as shown to expose previously installed helical piers. 2. Install 26 Ram Jack Triple Helical Piers to stabilize foundation settlement. ** The price will be adjusted downward if existing piers are found and can be used. 3. Repoint cracks in brick vaneer.

All proposed work has been designed by a structural engineer from Engineered Foundation Solutions. The Rules and Regulation Board of Professional Engineers and Land Surveyors require all foundation repairs and remediation to be undertaken in accordance with a plan prepared by or under the supervision of a board certified professional engineer. ( Paragraph c 3 of 21 NCAC 56.0701 Rules of Professional Conduct, Title 21 - Occupational Licensing Board and Commission, Chapter 56 - Professional Engineers and Surveyors. )

The above described scope of work is approved by NC Licensed Engineer.

Ram Jack proposes to furnish labor and materials to complete the work based on this scope and the attached Terms and Conditions: Proposal Price:

Terms and Conditions FOUNDATION REPAIR SYSTEMS

1. Payment schedule:

Payment due upon completion. Notes:

All Cash accounts are due at job completion Late fees of 1.5% monthly will accrue on all accounts after 30 days.

2. All material is guaranteed to be as specified. All work to be completed in a workmanlike manner according to standard practices.

3. Ram Jack provides up to 40 feet of pier material for each pier location. Site geological conditions determines the completed pier depth. Should site conditions require pier depths over 40 feet, the additional cumulative footage will be billed at $25.00 per lineal foot. If the pier depth exceeds 40 feet, the Owner will be notified of the potential additional charges. There is no deduction for pier depths less than 40 feet.

4. The Owner releases Ram Jack from all responsibility for damages caused by pre-existing defects. Such defects may include, but are not limited to, insufficient steel or cable reinforcement, insufficient or weak concrete, block or brick work, etc.

Disclaimer: Ram Jack uses the finest methods and materials in the industry to repair foundations. However the ability to [completely] level or close cracks varies from site to site. Generally foundation will not be lifted beyond a practical limit unless a waiver [for potential hidden] damages is signed by the Owner. Lifts may also be hindered by cosmetic repairs to previously damaged areas or inaccessible construction debris, distorted internal structures, etc Ram Jack exercises due diligence in performing all work. However, occasionally, cracks may develop in re-aligned concrete, brick, sheetrock, rock veneer or other rigid materials during the course of the work. Ram Jack is not liable for any such damages. Piers should not be exposed to the weather after installation. If any should become exposed, owner should contact Ram Jack immediately. Should rock, debris or over sized concrete footings be encountered during the course of the work, the Owner will be notified and given a rough estimate of the additional costs associated with the removal.

Dear Emily: At your request, RB Engineering, Inc. is pleased to provide the following summary engineering letter concerning our recent site observations at the subject residential building. We performed our site visit on August 26, 2011 and were asked to inspect the building in order to evaluate the general structural condition of the foundation system in regards to the exterior cracking that has occurred and to provide a foundation repair plan. FINDINGS For the purpose of this report, all directions are given from the perspective of an individual standing in front of and facing the front of the respective building. No material testing, excavation, soil testing or destructive inspection techniques were performed for the purpose of this report. A copy of the construction plans was available on site for our review. We did not have access for interior inspections for the purpose of this report. We understand that a Raleigh based foundation repair contractor installed underground helical piers approximately 8 years ago to stabilize a section of the back, left corner of the building which has continued to move. During our site inspection, we observed the following relevant information: •

The subject apartment building (building #6, Apartments A-K) is a two-story, timber-framed, multifamily residence with a building exterior comprised of both brick veneer and siding. The foundation system consists of a cast in place concrete floor slab with a continuous reinforced perimeter stem foundation wall. The existing lot slopes from front to back at the subject building with a significant drop off toward Big Tree Way at the rear of the building. No roof drainage was present along the rear of the building and the existing roof gutters and downspouts along the sides of the building outlet near the base of the building.

Moderate to severe differential settlement was observed toward the rear of the building. Exterior disturbances consisted of brick veneer cracking (both horizontal and vertical cracking), brick veneer separations at windows, and separations between the brick veneer and trim indicating that the brick veneer has rotated. Flashing has been temporarily installed at the second floor elevation to prevent water intrusion where the brick has reportedly separated between 1 and 2 inches. Using a surveyor's level, relative exterior elevations were measured along mortar joints in the exterior brick veneer and revealed that there was between !/2-inch and P/i-inch vertical misalignment from front to back with up to 2!/4-inch vertical misalignment measured along the rear exterior walls.

CONCLUSIONS The subject multi-family residential structure exhibits differential settlement related issues at the rear of the building. Inadequate site and roof drainage have contributed to the differential settlement of the foundation system. Structural repairs are suggested to stabilize the building foundation system and we recommend that the foundation repair contractor work closely with a structural engineer during the repair process to ensure that the implemented foundation repairs achieve their intended purpose. As requested, we have provided you with a proposed foundation repair plan for the subject building. •

Repair recommendation - Excavate at the back, left corner of the building to verify the location of any existing piers. Determine the existing pier locations and any warranty coverage for the existing piers as many foundation repair companies carry a life of structure warranty. Install (or adjust) 26 underground helical piers along the rear exterior to stabilize the foundation movement. Monitor the building on a routine basis. Improve roof and site drainage to provide positive drainage away from the foundation.

Prep includes acid washing pool and filling delaminated plaster areas with new plaster. Prime pool with Gunzite Epoxy Primer by Olympic Paints. Important Notes: 1. If more than 20% of the old plaster has delaminated and has to be removed, there will be additional charges for labor. 2. Spa equalizers (VGB approved and NC Health Dept approved) will be installed 3. Step & bench treads will have a black line painted on them similar to a tile line. *Option- Pressure wash and paint pool only >$3800.00 We do not recommend paint. It is bad to blister and peel if the water is not balanced. It also chalks badly after a short time making the maintenance a little harder. If you ever want to re-plaster, you have a more expensive prep after paint. You cannot plaster over paint without sand-blasting. Provide all materials and labor to complete all renovations per above scope of work as per manufacturer. Any hidden damage or work other than listed will be considered additional work. Allsouth will warranty all workmanship for a period of one year. The scope of work for this project is attached and an Architectural Drawing will be provided upon request from Owner at cost. All work will meet Industry standards and will be performed in accordance with the drawings and specifications submitted for the above work.
