A:Inadditiontothetherapyservices commitmentrequiredforyourchild’slevelof care,yourfamilywillmeetwithaCare Coordinatoroncepermonthforscreeningand assessmentappointments.Basedonthe assessmentyoumayberequiredtomeetwith theCareCoordinatortogetherasafamily, caregiveronly,ortheteenwithoutacaregiver present.Weofferspecializedassessments basedonyourchild'streatmentneeds,butall CarePathwayswillroutinelyofferthefollowing assessments:

TheStrengthandDifficultiesQuestionnaire (SDQ)isabriefbehavioralassessmenttool forassessingyourchild’shelp-seeking behavior,lookingatwhatisgoingwellina child’slifeandwhatchallengestheremay be.
ThePatientHealthQuestionnaire(PHQ-9)is usedtoscreenforandassesstheseverityof depressionoverthepast2weekstomonitor theseverityofdepressivesymptomsand trackapatient'sresponsetotreatment.
