Clinical Laboratory Reference (CLR), Medical Laboratory Observer’s annual reference guide, provides a guide to tests, equipment, and services for the clinical lab market, in addition to tables of critical limits, reference intervals, and critical values. It is the industry’s only complete resource guide.

The tables of critical limits and reference intervals help laboratorians interpret test results. Critical limits establish the upper and lower values that indicate when a patient’s medical condition may be precarious, requiring the immediate attention of the provider who ordered the test. Reference intervals are the most common decision support tool for interpreting pathology reports, as patients’ test results are compared to these ranges to indicate normal results or those that may require clinical follow-up. These decision tools assist in one of the top goals in healthcare: timely communication.

When to panic over an abnormal value

Sometimes, a very high or low abnormal laboratory value indicates that the patient must be treated immediately or he may either die or suffer irreparable physical harm. In this author’s hospital, laboratory personnel know when to drop everything and alert the clinician.

ONE EVENING in 1969, an unaccompanied young man was admitted to the Los Angeles County-University of Southern California Medical Center in coma of unknown etiology after having been found unconscious in a downtown hallway. When a physical examination disclosed a laceration of the scalp, he was admitted to the neurosurgical service. Shortly thereafter, CBC, urinalysis, and serum electrolytes were ordered and the proper specimens were sent to the laboratory. Deep coma persisted. All laboratory results were normal except for a serum glucose of 6 mg.%. The hard-copy laboratory results were returned by telecopier

to the ward of origin within two hours of receipt of the specimens in the laboratory. However, the results were not noticed by the house officers, who were busy with several other seriously ill patients. Ward personnel also failed to communicate the lab results to the responsible physicians. The following morning, when an intern noticed the laboratory value, he administered glucose immediately, but there was no response. Irreversible brain damage had occurred; the patient died soon afterwards.

This tragedy prompted us to organize a special procedure for reporting out “Critical Laboratory Values,” so that appropriate

clinical personnel can take responsible, timely action to save a patient’s life.

We began in 1969 by asking members of the laboratory utilization committee to determine those laboratory values which reflect pathophysiologic derangements at such variance with normal as to be life-threatening if therapy is not instituted immediately. The table on page 7 is the updated version of their original compilation, and is posted at strategic points around the clinical laboratory.

This compilation has been deliberately limited to a crucial small variety of tests with undeniably critical limits so as not to dilute the effectiveness of, and respect for, this notification. Many other tests and less extreme results could be included in this system. However, if we included them we would run the risk of reducing the urgency of the critical (panic) value, and in a large hospital the“Wolf, wolf!”cry would fall upon deaf ears in short order. Vital tests that merely confirm usually obvious clinical observations (e.g., neonatal jaundice) have been excluded. The tests listed, therefore, often represent states in which the clinician may not have clear clinical suggestions that these life-threatening events may be occurring.

Now, when a technologist gets one of these abnormal values—and verifies it—he immediately telephones the responsible physician and reports it. If he can’t reach the doctor whose name is on the request slip, he’ll go after another physician who holds responsibility for that patient. If neither physician can be reached by telephone within a few minutes, the appropriate ward “charge nurse” is notified, and she immediately alerts some physician who may be responsible for that patient. In the case of an outpatient, if the laboratory cannot reach the responsible physician, the appropriate administrative offices are notified. As a last resort, if there is no responsible answer at any of the above locations, the administrative nursing office is notified for appropriate emergency action.

It is up to the unit head (pathologist, clinical biochemist, medical microbiologist) to decide who in the laboratory actually makes that phone call. The basic considerations are, first, whether the test result must be verified and, second, how to call through the smallest possible number of intermediaries, so that we can diminish delay, bolster essential trust in the validity of the result, and quickly pinpoint responsibility. Since we try to keep the responsibility for critical value notification as close as possible to the person who first discovered the abnormality, the

technologist makes the call in most instances. In other circumstances, a laboratory assistant, supervising or senior clinical laboratory technologist, intermediate clerk, pulmonary function technician, or medical microbiologist calls to alert the physician. Never do we involve personnel from our data and systems control group, even though this group is responsible for most other data handling and delivery: We want to avoid the delay which often occurs when laboratory data cross administrative boundaries.

Obviously, this urgency of reporting is critical if the lab is to get these values to the clinician in time for him to treat his patient successfully. But it’s also critical for us to be absolutely sure that the panic value we’re reporting is valid. So we frequently will verify the result, though the procedure for doing this varies from department to department. laboratory technician reports the critical value to the physician in charge of the laboratory prior to notification of the clinician.

If the laboratory physician is not present, the tech reports the result to the appropriate clinician, and repeats the test on his demand if there is any question raised by the clinical circumstances. In

microbiology, results are verified by the medical microbiologist or the senior technologist prior to notification. In chemistry, the results are taken at face value by the clinical laboratory technologist if they lie within the assured range of the instrument and method, so long as tight quality control is present. Otherwise, the specimen may be diluted and the test

repeated or the analysis rechecked by another instrument or method appropriate for that range of value.

In general, we delegate a great deal of responsibility to our technologists in this, as well as most other areas of their employment. All of our technologists are college graduates who have served an approved year of internship and are licensed by the State of California. Most have been selected from a large pool of applicants, carefully screened, thoroughly trained in a continuing way, and very experienced, usually in a specialty. Most are truly “Super Technologists” who deserve the great trust we give them.

The critical value reporting system went into effect on January 8, 1970. It is widely believed by clinical, laboratory, nursing, and administrative personnel to be an invaluable aid to quality care of seriously ill patients in this medical center. There were problems, however, in implementing the system. For one thing, there have been differences in view as to what constitutes a critical or panic value. To this end, committee action resulted in a first list which was used for about 12 months. This list was then altered slightly to the form that accompanies this article. Also, communications were occasionally faulty, especially before the majority of hospital staffers learned the importance of this notification procedure. Then, too, we encounter some clinicians even now who are not surprised at the value reported; they become angry at being disturbed to be told something they already know. Most physicians, however, are quite grateful that the system exists as a “patient care quality control” measure.

There’s still one flaw: The problem of how to report a critical value on an outpatient who has already left the clinic has not been completely solved. Generally, we send telegrams to these patients asking them to quickly report to the emergency department.

In an average week in 1971, we phoned between 180 and 250 critical values out of a total weekly workload of approximately 120,000 laboratory procedures. This amounted to 1 critical value per 480 to 660 procedures. In a recent one-week period when we came up with 225 critical values, it took an average of 3.1 minutes to inform a responsible person, with a range of 0 to 103 minutes. Of the 225 total reports, 213 were delivered by telephone within 10 minutes from the time of verification.

is to be preferred over telephone communications. The obvious possibility of errors in transscription and/or misunderstanding of verbal messages strongly suggests hard-copy records whenever possible. But in most hospitals it is unrealistic to try to deliver test results quickly enough. Then, too, clinicians would have to ‘bird-dog’ their patients’ charts for

updated data. The telephone approach is an effective alternative.

Mechanisms of transportation and communication in many busy hospitals are such that the usual routine communication of laboratory results may lag hours or even days behind the generation of those results. instead is attended by many different physicians in various clinics or wards. It is possible that a laboratory result, which is of crucial importance to a patient may be reported, filed, and charted but still not noticed or acted upon—to the detriment of that patient.

In view of this, we believe that a laboratory must assume the responsibility of ensuring that a chain of clinical events begins soon after a critical value is found. Shouldn’t laboratory personnel play a role in assuring that the patient who has a positive culture for Mycobacteria tuberculosis, or a positive cytologic cervical smear, or hypercalcemia—or any one of dozens of other disease states uncovered by the lab—receives appropriate follow-up tests and/or therapy? Must we not develop “fail-safe” systems of clinical follow-through on clinically crucial laboratory determinations? It’s part of our professional responsibility toward the patients we serve.

CRITICAL LIMITS

Critical limits define the quantitative boundaries of life-threatening diagnostic test results. Critical results falling outside high and low critical limits must be reported to clinicians urgently, so the patient can be treated promptly if necessary. Critical value reporting was implemented by George Lundberg, MD, and published in MLO in 1972.1 Analysis of the impact of critical notifications on patient outcomes appeared in MLO in 1993 with follow-up articles calling for national harmonization and standards of care for critical notification practices. 2-4

These tables reflect recent research by Gerald Kost, MD, PhD, MS, FADLM of the POCT•CTR, School of Medicine, UC Davis, and his UC Honors Program student team. The research entailed a review of web-posted critical limits/values at 26 university hospitals5 with comparison to 1990-93 data compiled through the first three national surveys of critical

limits used by 92 responding U.S. medical centers, including 20 trauma centers and 39 children’s hospitals.6-8 Most hospital policies posted on the Web defined what a critical value is and stated that a licensed provider must be informed immediately.5 Statistically significant differences in 2023 versus 1990-93 critical limits were observed for blood gas (pO2, pCO2), chemistry (glucose, calcium, magnesium), and hematology/coagulation (hemoglobin, platelets, PTT, WBC) tests, and for newborn glucose, potassium, pO2, and hematocrit, as indicated in bold in the tables. Notable were INR and cardiac troponins, although not all hospitals listed the latter. Fourteen listed troponin (6), troponin I (3), hs-TnI (3), or troponin T (2).5 The number of qualitative critical values increased substantially, including bioterrorism threats and high-risk pathogens.5 While COVID-19 was not listed frequently, it should be in view of the risk of infected patients spreading the disease to care providers, families, and other patients.

The Joint Commission identifies timely reporting of critical tests results as a National Patient Safety Goal (NPSG.02.03.01) in its accreditation programs.9 Clinical laboratories can develop notification practices collaboratively with emergency physicians, hospitalists, and point-of-care specialists to achieve timely, effective, and focused patient care. For example, the statistically significant increase in the low critical limit for glucose, which increases the notification frequency, may reflect recognition of the level two threshold [54 mg/dL (3.0 mmol/L)] at which neuroglycopenic symptoms require immediate action.10

In the recent survey, the number of tests listed varied from 21 to as high as 116 with a median of 62.5 Hospitals are advised to review their notification practices, quantitative decision thresholds, and qualitative critical value notifications to assure that effective life-saving alerts improve patient outcomes without excessively burdening laboratory, bedside, critical care, or emergency staff.

This table was put together by MLO staff referring to federal regulations at 49 CFR part 40, Section 40.85 and information from Thermo Fisher. Section 40.85 states that:

1. As a laboratory, you must use the cutoff concentrations displayed in the following table for initial and confirmatory drug tests. All cutoff concentrations are expressed in nanograms per milliliter (ng/mL).

2. On an initial drug test, you must report a result below the cutoff concentration as negative. If the result is at or above the cutoff concentration, you must conduct a confirmation test.

3. On a confirmation drug test, you must report a result below the cutoff concentration as negative and a result at or above the cutoff concentration as confirmed positive.

4. You must report quantitative values for morphine or codeine at 15,000 ng/mL or above.1

A. For grouped analytes (i.e., two or more analytes that are in the same drug class and have the same initial test cutoff):

Immunoassay: The test must be calibrated with one analyte from the group identified as the target analyte. The cross-reactivity of the immunoassay to the other analyte(s) within the group must be 80 percent or greater; if not, separate immunoassays must be used for the analytes within the group.

Alternate technology: Either one analyte or all analytes from the group must be used for calibration, depending on the technology. At least one analyte within the group must have a concentration equal to or greater than the initial test cutoff or, alternatively, the sum of the analytes present (i.e., equal to or greater than the laboratory’s validated limit of quantification) must be equal to or greater than the initial test cutoff.

B. An immunoassay must be calibrated with the target analyte, Δ-9-tetrahydrocannabinol-9-carboxylic acid (THCA).

C. The concept of critical values for drug levels was originally developed by the late Daniel M. Baer, MD, and first published in the April 1982 issue of MLO. This table lists the critical values for common therapeutic drugs updated in 2024 by reputed laboratories in the US. Ranges are approximate and may vary with laboratory and/or assay. Proper interpretation of therapeutic drug concentrations requires that the specimen be drawn at an appropriate time in relation to drug administration.
