from the comfort of your home or provider’s office COPC’s Comprehensive Care team provides medical care and support to patients experiencing advancing illness and/or loss of mobility.

Patients Who May Benefit From This Service Patients with any late or advanced illness including: ● Heart failure ● COPD (Chronic Obstructive Pulmonary Disease) ● ESRD (End-Stage Renal Disease) ● Cancer ● Dementia ● Other progressive neurological diseases Patients who frequently visit the ER or become hospitalized due to illness or progressive decline.

Patients who are home-limited with difficulty getting to their doctor appointments.
