I’m Dr Anita Hart, your Medicaid Medical Director. You can call me Dr Anita. Welcome to our first issue of SWHP 360 º !

I’m a doctor who sees patients when they are sick enough to be in the hospital, however, a big part of my job here at the health plan is to find ways to keep you and your family safely at home living a healthy life. A lot of people don’t ever think about their health coverage until they need care. We at RightCare are here for you when that happens. We’d also like to help you keep from getting sick in the first place.

The more you know about how to use your benefits, the easier it will be for you to stay well. This newsletter can help you with that. You’ll find information on different services we offer inside. You may not need all of them right now, but I’m pretty sure you’ll find something you can use.

You can always call us if you have questions at 855-TX-RIGHT (855.897.4448). Plus, we have a 24-hour Nurse Line you can call anytime day or night if you want to talk to a nurse. You’ll find the number on the back of your ID card.

Know Your Care Professionals

Your care team can include different healthcare professionals. They can be a traditional doctor, or someone from your health insurance plan. Read about the different types below.

Primary Care Physician

can help refer you to other specialty doctors when needed. You should see your PCP at least once a year, depending on your health needs.

Specialty Doctor: This is a doctor that has studied a specific area of medicine. They often manage specific health issues. For example, a cardiologist would manage heart disease.

Case Manager: This is often a nurse or social worker. Typically, they work for your health insurance plan. Case managers can help you find resources and programs to manage your health. They often monitor your health to help you understand if your health needs are being met. If you have questions about your health or next steps, your case manager could be a good first point of contact.

To opt in to Case Management, call the Customer service number on the back of your member ID card OR email

Please include your name, preferred phone number, RightCare member number, and a brief description of your needs.

Nurse Line: On the back of your health insurance card, you will find a phone number for our 24-Hour Nurse Line. This is another resource you can use when you are not sure how to handle health issues you may be having. It is a quick way to get health advice and determine if you should see your PCP or go to the hospital. Your PCP’s office may have a similar service as well. It may be called the “After Hours” line where you can reach a nurse or doctor.

Where to go for care

VIRTUAL CARE

Using your mobile device or computer

For conditions like acne, allergies, bladder infection, cold, flu, pink eye, quitting tobacco, sinus infection, stomach problems or yeast infections.

PRIMARY CARE DOCTOR

Another choice for care when it’s not an emergency

For conditions like asthma, diabetes management, earache, high blood pressure, headaches, preventive health, sprains, etc.

WALK-IN CLINICS

Same-day appointments when your doctor is not available; includes select primary care clinics and some pharmacy locations

For conditions like asthma, bladder infection, ear or sinus pain, flu, sore throat or sprains.

URGENT CARE

Needs immediate attention but is not life-threatening, or an appointment is not available with your doctor

For conditions like back pain, bladder infection, earache, minor burns, minor eye injuries, minor cuts that may need stitches, sore throat or sprains

EMERGENCY ROOM

Any condition you believe to be life-threatening

For conditions like chest pain, deep cuts or wounds, difficulty breathing, poisoning, overdoses and suicidal behavior, abdominal pain, coughing or vomiting blood, severe burns, severe head injuries, sudden loss of balance, vision change, facial droop, arm or leg weakness.

Need a provider?

You’ll find a list of all in-network Primary Care Providers (PCPs) in the provider directory on RightCare.SWHP.org.

You can choose or change a primary care provider or OB/GYN two ways.

1. In your member portal:

▪ Click View/Edit My Info

▪ Click My Providers

▪ Choose your new doctor and effective date OR

2. By calling RightCare Member Services

Referrals for specialty providers

Find a provider

A referral is an approval from your PCP for you to get specialty care and follow-up treatment. If you receive services from a specialist without your PCP's referral—or if the specialist is not a RightCare provider—you might be responsible for the bill.

You can get some services without going to your PCP first, such as:

▪ 24-hour emergency care

▪ Routine vision care

▪ Dental services (for children)

▪ OB/GYN care

▪ Family planning services and supplies

▪ Behavioral (mental) health and substance abuse services

▪ Texas Health Steps

Your total health is important. That includes physical, emotional and mental (behavioral) health. So, if you…

Our Behavioral Health Care Managers will work with you to find providers, help make sure your prescriptions get filled easily and on time and connect you with local resources.

You can also call the Behavioral Health Crisis Line 24/7 at 800.327.6943. If it’s an emergency, call 911.

Meet mindoula®

Mindoula is a behavioral health program. It provides 24/7 care management and psychiatric support programs to members.

Mindoula offers programs on:

▪ Reducing Violence (IVRP)

▪ Preventing Suicide (SPP)

▪ Substance Exposed Living (SELP)

▪ Substance Exposed Pregnancy (SEP)

▪ Med-Psych Management (MPMP)

These services are provided at no cost to you.

Expecting the Best®

A program to help mothers enjoy a healthy pregnancy

Expecting a little one? You can receive helpful tips and assistance during your pregnancy and for one year after your baby’s birth.

During pregnancy and delivery

▪ Planning for delivery, including one-on-one support and education during your pregnancy and for one year after your child’s birth.

▪ Coordinated efforts to promote full-term birth in women who previously delivered pre-term.

▪ In-home help for high-risk conditions such as diabetes, high-blood pres sure, severe nausea and vomiting.

▪ Early enrollment in our service coordination program

▪ Support for postpartum depression and maternal mental health during and after pregnancy. This includes screening, working with your care team, resources and individual help based on your needs.

After the baby’s birth

▪ Screening for postpartum depression, with continued support and resources

▪ Education for parents on keeping your baby healthy in their first year

▪ Immunization and well-child reminders.

▪ Planning for returning to work

Come to a RightCare STAR Baby

You’ll learn more about health facts and community resources that can help you and your little one. Plus, you’ll get a diaper bag and other gifts for coming to the shower.

Showers are held in person or online. Watch for an invitation with details on how to register. You may qualify for a $20 gift card for attending!

Keeping your child healthy

Taking your child to their pediatrician or primary care physician is important even if your child is healthy. Texas Health Steps (THSteps) well child visits are scheduled around certain critical months, years, and ages in your child’s life. These visits help detect conditions early and provide the opportunity to diagnosis, treat, manage or prevent health problems. Check out the Texas Health Steps Medical and Dental Checkups Chart.

THSteps appointments focus on your child’s:

1. Development—motor skills, language development, and social interactions.

2. Immunizations and vaccines they need as they grow

3. Overall health, including, height, weight and vital signs, like their heart rate

4. Nutrition—the importance of a healthy and balanced diet

Before you go:

▪ Take a look at the milestones for your child’s age

▪ Write down any concerns or questions to take to your appointment

Did you know?

▪ Your provider may be able to do well child visits for all of your children at the same appointment! Call the clinic and ask.

▪ You can get help with transportation to appointments. Find out more. You may qualify for A GIFT CARD for taking your child to these appointments.*

Help for diabetes

In 2021, about 38.4 million Americans had diabetes. Diabetes is a chronic condition that affects how the body makes and uses glucose (aka – sugar).

There are three types of Diabetes:

1. Type 1 Diabetes causes the body to NOT make ANY insulin.

2. Type 2 Diabetes causes the body to NOT make ENOUGH insulin or to use it inefficiently.

3. Gestational Diabetes develops during pregnancy.

Type 1 and Type 2 are the most common. Type 2 Diabetes makes up 90-95% of all cases. Type 1 makes up only 5-10% of all cases.

If you have diabetes, RightCare offers help from nurse care managers. Your care manager will coordinate your care between your doctors and benefits. You can also get help with your medication. This program helps parents and caregivers, too.

Our diabetes coaching program will help you learn about things like nutritional information and how diabetes affects you.

Your voice matters!

RightCare holds quarterly Member Advisory Group (MAG) meetings with members and member advocates who live in our service area. Why? We want to help you understand your Medicaid benefits and improve your member experience.

At each meeting, we:

▪ Review your benefits.

▪ Share upcoming events and programs.

▪ Give you a chance to ask questions.

▪ Listen to your healthcare experiences.

▪ Ask for areas we can improve.

We use the feedback we receive at these meeting and in the post-meeting survey to improve our services. Register for upcoming meetings to make your voice heard!
