The deaths of at least 30 people who drowned while attempting to cross the English Channel in 2021 could have been avoided, an inquiry has found.

The dinghy they were travelling on became swamped and capsized in the early hours of 24 November 2021, in what became the deadliest Channel small-boat incident on record.

Only two people survived, and were found almost 12 hours after they called for help.

The inquiry chair Sir Ross Cranston pointed to a "significant, systemic failure on the part of government", and said that several of the casualties were preventable. Twenty-seven bodies were found after the boat sank, and another four were confirmed missing.

assets were not briefed on the possibility that a dinghy was sinking or that there were people in the water.

There was also a widely held belief that migrants calling from small boats exaggerated the distress, meaning the Coastguard underestimated the emergency.

The inquiry said that the deaths could have been prevented, and pointed to three fatal errors: people smugglers overloading the boats, the slow response of French Naval vessel, Flamant, and flaws in the Coastguard's search and rescue operations.

The victims set off from a beach near Dunkirk in France and came from a number of countries, including Iraq, Somalia, Ethiopia, Egypt and Afghanistan.

Thirty-six infants in the UK have had suspected food poisoning from contaminated baby formula. It comes after specific batches made by Nestle and Danone were recalled because of contamination with the toxin, cereulide.

The UK Health Security Agency (UKHSA) says it has received clinical notifications of young children developing symptoms, including vomiting and diarrhoea, consistent with cereulide poisoning.

None of the infants – all around or under one – are reported to be gravely ill, the BBC understands.

Gauri Godbole, from UKHSA, said it was not unexpected, given "the widespread availability" of the affected products prior to the recall and "subsequent testing".

Godbole said there did not seem to be any signs that huge numbers of children had been affected so far.

"Current surveillance indicators do not show unusual increases in reports of vomiting in children under the age of one for this time of year," she added.

Sir Ross's report said HM Coastguard in Dover was placed in an "intolerable

position", with chronic staff shortages and limited capacity leaving them unable to rescue victims.

A surveillance aircraft that should have provided critical intelligence on the number of crossings never launched

due to poor weather conditions - and there was no contingency plan, the inquiry found.

It was also found that calls and messages from the vessel were missed or not followed up, and search

But the agency said it would continue monitoring the situation and providing updates.

The UKHSA has not been able to confirm categorically that the infants had been poisoned but it has been confirmed they consumed the specific brands of infant formula affected.

The Danone batch affected is the 800g pack coded EXP 31-10-2026. Meanwhile, for Nestle it relates to a number of products, including SMA infant formula and followon formula. The batches affected are listed on the company's website, external. Of the 36 cases, 24 were in England, seven in Scotland,

three in Wales, one in Northern Ireland and the final case reported to be in one of the Crown Dependencies, which includes the Isle of Man, Jersey and Guernsey. The UK is not the only country affected with both European nations and those from other parts of the world reporting products have been contaminated.

Survivors, and the victims' families gave testimonies in the inquiry having waited more than two years for the proceedings to start in March 2024. The 454-page report , externalidentifies the 27 men, women, and children who died, describing them as "a stark reminder of the human cost behind the statistics".

Cereulide is a toxin which is unlikely to be destroyed by cooking, or when making the infant milk.

If it is consumed it can lead to symptoms like nausea, vomiting and abdominal cramps developing quickly.

The Food Standards Agency (FSA) has said the toxin was present in a specific ingredient identified as arachidonic acid oil. The oil is added to infant formula to give it some of the important growth properties contained in breast milk.

The FSA has urged parents who had any of the affected products left, to stop using them and switch to an alternative formula.

If you have fed it to your baby and are concerned, contact your GP or NHS 111 for advice, it added.

The FSA is working with manufacturers to trace all products that may have used the affected ingredient and ensure all affected products are removed from sale.

The Bank of England has held interest rates at 3.75% after a knife-edge vote but has opened the door to cuts later this year. Borrowing costs were widely expected to be unchanged after the Bank reduced them from 4% in December.

Bank of England governor Andrew Bailey reiterated his forecast that inflation –which measures the pace of price growth - would fall close to the Bank's 2% target from April onwards, against a previous expectation it would hit that level in 2027.

"That's good news," said Bailey. "We need to make sure that inflation stays there. All going well, there should be scope for some further reduction in [the] Bank Rate this year."

The most recent inflation data showed the rate increased to 3.4% in December, above the Bank's target.

However, the Bank said policies announced in the Budget, such as cuts to household energy bills, as well as lower wholesale gas prices were expected to help cut the rate.

Despite this, the Bank cut its forecast for UK economic growth in 2026 – from the 1.2% growth it forecast last November to 0.9%. The

unemployment rate is also expected to tick higher this year, according to the Bank, up from an initial forecast of 5% to 5.3%.

The decision to hold interest rates this month was a close call, with four of the Bank's nine-member monetary policy committee (MPC) voting for a quarter point cut which would have taken borrowing costs to 3.5%.

The only member to change their position from December's vote was Bailey. He voted to hold rates, but said he could "see scope for some further easing of policy".

Announcing its latest decision, the MPC said: "On the basis of the current evidence, [the] Bank Rate is likely to be reduced further.

"Judgements around further policy easing will become a closer call. The extent and timing of further easing in monetary policy will depend on the evolution of the outlook for inflation."

Analysts said the Bank's comments have brought forward expectations over when the next rate cut will be, with the focus now on the MPC's next two meetings in March and April.

"It seems it wouldn't take much to prompt a majority of MPC members to vote for another cut to 3.50% in the coming months," said Paul Dales, chief UK economist at Capital Economics.

"We've pencilled in the next cut for the meeting in late April, but wouldn't completely rule out March," he added.

Lindsay James, investment strategist at wealth management firm Quilter, said: "Markets had not been fully pricing in the first rate cut until June, but this has shifted to April following today's report."

Ex-Tory councillor unveiled by Nigel Farage as Reform's Welsh leader

Nigel Farage has named former Tory councillor Dan Thomas as Reform UK's Welsh leader.

At a rally in Newport on Thursday, Thomas told Reform supporters: "With you, the people's army, Reform will be the change that Wales desperately needs."

The ex-leader of Barnet council, who is originally from Blackwood, Caerphilly county, said he was now "back home, raising my two boys in the south Wales valleys - I've come back to where I belong".

The Conservatives said he appeared to have been "parachuted back to Wales" to take advantage of Senedd electoral changes, Labour dismissed Thomas as "another former Tory" leading Reform in Wales, while Plaid Cymru said Reform was "recycled, washed-out Tories".

Farage also confirmed on stage that former Conservative Senedd member James Evans had joined Reform.

Evans was expelled from the Tories last month after telling senior figures in the party he was talking to Reform about defecting.

Farage said he had chosen

Thomas because he was "battle hardened".

"I think we deserve Reform UK, here in Wales, to be led by somebody who's been battle hardened, who's been there before," he said.

Farage said Thomas would "keep a calm head through the good and bad, because you're always going to get both in a campaign". Thomas told Reform activists at the rally that the Senedd election on 7 May was Wales's "last chance".

"We have a once-in-a-

lifetime opportunity to smash Labour's failing grip in Wales and, as your new leader in Wales, I will pour my heart and soul into fighting for every vote," he said.

"We will have a positive, ambitious manifesto, we will have a team of talented, passionate candidates who know their communities and will be taking our message of change to the streets.

"We're fighting to win, fighting for every vote, because this is the last chance for Wales."

Thomas led Barnet council

in north London from 2019 to 2022, when Labour won the local election, before becoming opposition leader from 2022 until May 2024. He resigned at the end of last year after 19 years as a councillor.

Local media quoted him saying he moved away from Barnet in 2024 so that he and his wife could raise their young sons in the countryside and live closer to their families in south Wales. Thomas said at a press conference after his

unveiling that he had "kept abreast of Welsh politics" and was "very informed" about the country's political scene.

There were, he said, "very many overlaps" between England and Wales on policy.

Farage stressed Thomas and his party colleagues in Wales would set Welsh policy, saying he was "not a hard line dictator".

The Reform UK leader described Thomas as a "man who was born in the valleys who like so many, sadly, young people from Wales headed off to London for a career in which he did well".

He added he was "somebody who knows what it is to run big budgets, somebody who knows how difficult it is to be in government".

Reform has not had an official leader representing the party in Wales since Nathan Gill, who led it into the 2021 Senedd election and left the party shortly after.

Gill was sentenced to 10-and-a-half years in prison in November after admitting taking bribes for pro-Russia interviews and speeches.

Warning of long airport queues under new EU border control system

There are warnings of queues at passport control at airports in Europe over upcoming holiday periods, as the rollout of a new biometric border system continues.

Passengers have already reported hours-long waits at some destinations, at busy times, due to the new systems.

A cargo ship captain found guilty of gross negligence manslaughter has been jailed for six years.

Vladimir Motin, from St Petersburg, Russia, was on watch on the Solong when it collided with US tanker Stena Immaculate off the East Yorkshire coast - leaving 38-year-old Mark Angelo Pernia missing presumed dead.

A trial at the Old Bailey in London heard Motin, 59, did not keep a proper lookout on 10 March 2025, or use all available means to determine the risk of a collision or leave enough time to take evasive action.

Passing sentence, Mr Justice Andrew Baker told Motin he had fallen prey to his own "complacency and arrogance", adding: "You were a serious accident waiting to happen." The trial previously heard the tanker was visible on the Solong's radar display for 36 minutes

before impact, yet Motin failed to steer away from the collision course, sound the alarm, summon help or initiate a crash stop.

Defence barristers told the trial there was no doubt Motin

was at fault for the collision, but claimed his actions did not amount to gross negligence manslaughter. Prosecutor Julia FaureWalker said Motin had lied about what happened to "get back to his wife" in Russia.

Following the collision, Motin messaged his wife, saying he would be "guilty", the court was told. However, he denied he had been asleep, or had left his post. The judge said Motin's version of events

was "extremely implausible" and that Pernia's death was "wholly avoidable".

The basic facts of the collision "suggest a ship unaware of the ship ahead" and that was the "most likely" explanation, he added. Motin had led jurors on a "merry dance" in his evidence, which was an "exercise in inventive distraction", Mr Justice Baker said.

"His claim to be confused was a lie that unravelled when tested at trial." In a victim impact statement read to the court, Mr Pernia's widow, Leacel, said no amount of compensation made up for the pain of her loss and the impact on her young family.

Defence barrister James Leonard KC told the court Motin was remorseful about what happened and had vowed never to go to sea again.

Starmer grovels for his job

Desk report: Sir Keir Starmer frantically tried to spread the blame for his disastrous decision to make Peter Mandelson US ambassador today as he grovelled for his job.

morning, Sir Keir said he believes most people go into public life out of 'duty' and 'service' but 'that is not why Mandelson did it'.

'In recent days, serious allegations and serious evidence has emerged concerning Mandelson's conduct, including his relationship with Epstein, a convicted sex offender,' he said.

'It had been publicly known for some time that Mandelson knew Epstein, but none of us knew the depth and the darkness of that relationship.'

The PM said Lord Mandelson was asked directly about the nature of his relationship with Epstein, and it is 'clear that the answers he gave were lies'.

He insisted the peer had made out he 'barely knew' Epstein during his vetting.

He added that he wanted to release documents relating to Lord Mandelson's appointment as ambassador to Washington DC on Wednesday, but the police had 'advised that releasing certain information now could risk prejudicing a future investigation or legal process'.

Sir Keir went on: 'The victims of Epstein have lived with trauma that most of us can barely comprehend, and they've had to relive it again and again.

'I want to say this: I am sorry, sorry for what was done to you, sorry that so many people with power failed you, sorry for having believed Mandelson's lies and appointed him, and sorry that even now you're forced to watch this story unfold in public once again.

'But I also want to say this: in this country, we will not look away, we will not shrug our shoulders, and we will not allow the powerful to treat justice as optional.

'We will pursue the truth. We will uphold the integrity of public life, and we will do everything within our power and in the interests of justice

to ensure accountability is delivered. 'That is what the public expects. That is what the victims deserve, and it is what I will do.'

Sir Keir said after an initial 'due diligence' assessment of Mandelson there was 'security vetting carried out independently by the security services, which is an intensive exercise that gave him clearance for the role, and you have to go through that before you take up the post'.

'Clearly, both the due diligence and the security vetting need to be looked at again,' he said.

'I've already strengthened the due process. I think we need to look at the security vetting, because it now transpires that what was being said was not true. And had I known then, what I know now, I'd never have appointed him in the first place.'

Pushed on what he would say to Labour MPs who now believe his position is untenable, Sir Keir said: 'I understand their anger and frustration. I am angry and frustrated like them...

'We must remain focused on what we were elected into government for.'

Sir Keir was adamant he will not be resigning, saying: 'I was elected in on a mandate in 2024 to change the country for the better.'

Downing Street sources played down the prospects of papers being released today, although it is still possible. Sir Keir has been forced to commit to publishing a bewildering array of material, including all messages between Mandelson and No10 ministers and aides, and details of his severance when sacked as ambassador.

Even normally loyal Labour MPs have questioned how long Sir Keir can hang on with his authority in tatters.

Senior MPs warned the situation will 'not end well'

for Sir Keir and the 'clock is ticking', while ex-Cabinet minister Lord Hutton suggested his time in power is coming to a close.

Labour Jonathan Hinder said the PM 'struck the right tone' this morning but he had to go further.

'But what needs to be acknowledged is that this was a catastrophic error of political and moral judgment,' he told BBC Politics Live.

'I don't think that has been addressed yet. So what I am looking for is for the PM to come and speak to Labour MPs on Monday night at the PLP and acknowledge that.

'Because everyone can see that information was available to make a very clear decision, that this many should not just have not been our ambassador he shouldn't have been a borough council election candidate.

'It is an absolute disgrace. And let this be the moment that people like Mandelson are no longer representing a working class party, a socialist party when he is swimming around in those circles. I am absolutely raging about it.'

Pressed whether he meant the PM had made a catastrophic misjudgement, Mr Hinder said: 'Yes.'

He said he 'understood' Sir Keir's point that he was 'lied to'. But he added: 'I think with the information that was available... I don't think it is unreasonable to suggest it was a mistake even with that information.'

He added: 'I think Labour MPs would appreciate the PM saying ''I cocked up''.'

Mr Hinder said he did not believe Sir Keir should resign if he accepted his 'error of judgment'.

There are demands for chief aide Morgan McSweeney to be axed over the Mandelson debacle - which ignited again after the US release of millions of Epstein emails showing the depth of their relationship.
