Dr. Daniel Fortino is a passionate Periodontist who takes great pride in his work. Over the past decade, he has had the opportunity to perform extensive oral surgery and periodontal treatments in both Ontario and the United States which has given him a well-rounded perspective in dentistry. This is how the vision and the goal came to fruition. By bringing together different specialities and focussed care dentists under one roof, this type of centre can foster more convenient collaboration and communication among patients, referring offices and dental care providers alike. This can lead to improved outcomes for patients and a better overall experience.

DOC+ stands for Dedicated Oral Care Plus. We are committed to creating a team of dental specialists and focused care dentists to provide patients with personalized and dedicated services. We want to ensure that every patient receives the highest quality of care possible in a comfortable and welcoming environment.

As the Georgian Bay and surrounding regions continue to expand, there is a growing need for a specialty centre that can cater to the area's specific needs. Such a centre would offer patients access to specialized and focused care services much closer to their homes. With this in mind, I am proud to introduce DOC+ (Dedicated Oral Care Plus), a Specialized and Focused Dental Centre dedicated to providing exceptional oral care services to the Georgian Bay and surrounding areas.

Dr. Daniel Fortino is a Periodontist dually licensed in the United States and Canada. Dr. Fortino grew up in Hamilton, Ontario and completed his undergraduate education at McMaster University, graduating with a Bachelor of Health Sciences (Honours) Degree with distinction.

He graduated from the University of Western - Schulich School of Medicine & Dentistry in London, Ontario. He then went on to pursue Specialty and Master's degrees in Periodontics from the Centre for Advanced Dental Education in St Louis, Missouri. He successfully achieved Diplomate status from the American Board of Periodontology after his residency. To further his expertise, he undertook a fellowship in Advanced Oral Surgery and Implantology at the School of Dental Medicine at the State University of New York at Buffalo (SUNY-Buffalo). Over the past decade, he has had the opportunity to perform extensive oral surgery and periodontal treatment across Ontario and the United States which has given him a well-rounded perspective in dentistry.

His focus is on periodontics, dental implant surgery and oral surgery procedures. His expertise extends to the removal of wisdom teeth with or without anesthesia, management of periodontal disease, advanced implant surgery and bone grafting, full mouth implant rehabilitation (all-on-x treatment), and soft tissue grafting.

Dr. Fortino has been published in the oral and maxillofacial pathology and the oral health preventive dentistry journals.

Dr. Daniel Fortino is not only dedicated to his professional life, but also enjoys pursuing his hobbies and interests outside of work. He is an enthusiastic outdoorsman who finds joy in spending time with his loved ones while engaging in outdoor activities. Dr. Fortino is particularly passionate about promoting a healthy lifestyle and enjoys encouraging others to do the same.In addition to his love for an active lifestyle, Dr. Fortino is also an avid sports fan and relishes the opportunity to travel and experience different cultures. He finds fulfillment in learning about the world and discovering new things.

Despite his busy schedule, Dr. Fortino always prioritizes spending quality time with his loved ones. He recognizes the value of cherishing moments spent with family and friends and makes it a point to create lasting memories with them.

PERIODONTIST

Dr. Perlus has been practicing periodontics since 1975 and implant surgery since 1982. A graduate of McGill University Faculty of Dentistry in 1973, he specialized in periodontology at the University of Toronto. During the early 1980s, he studied implantology and implant surgery in Sweden, the United States and Canada, and is now recognized for his proven methodology and successful cases. Over the past 30 years, Dr. Perlus has placed thousands of implants for patients of all ages.

A frequent lecturer to dental students and practicing dentists, Dr. Perlus has published articles on techniques and considerations necessary to achieve a high level of success. For his contributions to this innovative new field of dental surgery, Dr. Perlus was awarded the Gold Medal by the American Society of Osseointegration and a fellowship by the International Congress of Oral Implantologists.

/ Clear policies, clear expectations OUR POLICIES

At DOC+, we are committed to providing quality care to our patients. We have established the following policies and procedures so that each patient may receive consistent expert service:

1. All appointments must be made in advance by phone or online booking system.

2. For all oral surgery procedures, a deposit of the total cost is required prior to the scheduled appointment date in order to secure treatment. The remainder of the balance will be processed after the procedure. All deposits are non-refundable.

3. All patients must arrive at least 10 minutes prior to their appointment time or the appointment will be cancelled and a new appointment must be scheduled.

4. We kindly request that all cancellations be done 48 hours prior, or a cancellation fee may apply.

5. If there is a “No Show” or the appointment is cancelled without proper and advanced notice, a fee, a percentage of your services will apply. When a patient fails to cancel appropriately, it prevents other patients the opportunity to receive treatment sooner.

6. Payment is due at the time of service unless other financial arrangements have been made in advance with our office manager/billing specialist. Our staff will provide the patients with receipts.

7. Your insurance policy is a private contract between you and your insurance company; we cannot be responsible if there is a change, decline or delay in your benefit. We will help you understand your insurance and provide you with the appropriate documents you may need.

8. For your safety, it is essential that you inform us of any medical conditions or allergies that could affect your treatment plan or care during your visit.

9. Respectful language, attitude and behaviour towards staff members is expected at all times in order for us to provide excellent customer service to you, our valued patient.

What to expect on your first visit with DOC+

On your first visit to DOC + Specialized & Focused Dental Centre, you can expect a comprehensive evaluation of your oral health. Our experienced Specialists and hygienists will ask about your medical and dental history and assess for bone loss, loose teeth, bite alignment, oral cancer screening and more.

We may use x-rays or other tests to get a more accurate understanding of your condition. Depending on your individual needs, we may make recommendations for treatment. 3

For the safety of our patients and staff, please arrive at DOC + with a referral slip from your dentist and a list of any medications you’re taking. You should also have insurancepaperworkifapplicable.

Contact

The referring partner fills out a Referral form to initiate the referral process. The form can then be sent by email or through our website.

Verification

The referring partner may need to provide additional information, such as the medical history, to verify proper scheduling and treatment at DOC+.

Communication

Throughout the referral process, DOC+ maintains open communication with the referring partner to ensure that they are aware of the patient's progress and any updates regarding their care.

Follow-Up Care

DOC+ provides comprehensive followup care to ensure that the patient is on track to achieving their treatment goals and that any issues or concerns are addressed in a timely manner.

Patient Information

The referring partner provides information about the patient, including their name, contact information, and reason for referral.

Appointment Scheduling

Once verification is complete, DOC+ will contact the patient to schedule an appointment at a time that is convenient for them.

Treatment Plan

DOC+ conducts an examination, diagnoses, and creates a personalized treatment plan for each patient. The Treatment coordinator discusses the plan and costs with the patient. The referring dentist receives a report and updates on the treatment.

Feedback

DOC+ provides feedback to the referring partner on the patient's progress and treatment outcomes to ensure that the referring partner is kept informed and up-to-date.
