Fall Festival 2019 Vendor Application

Dobson Ranch Recreation Department is pleased to announce our 6th Annual Fall Festival on Saturday, November 16th from 12:00 pm until 3:00 pm at Laguna Park, located at 2455 W. Laguna Azul, Mesa, AZ 85202.

We are inviting local businesses and organizations to be part of our event. Fill out the information below to participate in this family-friendly event. You will be contacted within one week of returning this form for confirmation. General Guidelines for vendors in attendance:

· A 10’ x 10’ space will be provided to each vendor. 1 table and 2 chairs will be provided (limited availability). Pop-up tents are allowed. · All vendors must have a representative on site at all times. · Vendors need to be set up and ready for participants at 12:00 pm and are expected to stay until 3:00 pm. · A copy of your City of Mesa Tax License is required, If you will be selling any products. · If applicable, the vendor fee is due in full two weeks prior to event date: November 4th. · A Maricopa County Health Permit is required, if a food product is being served to the public.
