Fire Service College launches new breathing apparatus training school in partnership with Dräger

T

he Fire Service College has opened a new state-of-the-art breathing apparatus (BA) training school at its incident training ground in Moreton-in-Marsh. The facility will provide training on the full range of compressed air breathing apparatus (CABA) and will be runin partnership with leading technology provider Dräger. This will ensure the College’s training curriculum will be delivered using the most up-todate equipment available. Up to 200 delegates will be able to undergo training at any one time, expanding the College’s ability to deliver assessed, accredited and assuredtraining to fire and rescue services across the UK and internationally.

The new facility houses four networked briefing rooms, in which delegates will learn how to service and prepare BA equipment. Improved delegate facilities include three new drying rooms andan expanded provision of showers and changing spaces, incorporatingan innovative sealed ‘dirty room’. The roomhas been specially designed to minimise the spread of contaminants during the removal and changing of personal protective equipment.

Association and Chief Fire Officer of Staffordshire Fire and Rescue Service, opened the facility on Thursday 8 January. Speaking at the launch, Peter said, “The opening of this facility is a fantastic step forward in the transformationof the Fire Service College and the investment in state-of-art equipment and facilities is another tangible demonstration of the progress that is being made to develop a true centre of excellence of firefighter training. The Chief Fire Officers Association is fully supportive of the transformation that is taking place at the College and to our partnership which is delivering benefits to firefighter training and safety across the UK and beyond.”

Jez Smith, managing director of the Fire Service College, commented: “This new facility is testament to the investment we are making in building a true centre of excellence for fire service training in the UK. It will allow us to train more delegates to a higher standard – ensuring that the public and firefighters across the UK are kept as safe as possible.”

Castell launches new heavy-duty access interlocks to protect workers in heavy industries AI-HD and AIE-HD withstand high potential loads and high frequency of use

I

ndustrial safety specialist Castell has launched heavy-duty access interlocksfor use in industries such as waste and recycling, aggregates, steel and chemical processing. The AI-HD & AIE-HD have been designed to deliver robust performance where there are high potential loads and a high frequency of use.

The risks to personnel safety in harsh environments can be increased due to the heavy wear placed on safety components. The UK’s HSE recently reported that the construction industry in particular, while accounting for only aroundfive per cent of Britain’s employees, still accounted for 31 per cent of fatal injuries to employees and 10 per cent of reported major or specified injuries.

Recognising these issues, Castell examined how it could develop

its durable AI and AIE access interlocks to deliver enhanced products that would meet the day-to-day operational cycles encountered in heavy industry.

The AI-HD and AIE-HD will deliver years of performance in the harshest environments. The products use a heavy-duty stainless steel support mechanism, designed to take up to 1.5 times the load of current access interlocks, which reinforces the locking action. The design also provides additional protection for applications with high levels of vibration, such as mixers and shredders.

oE is becoming an increasingly popular way of powering devices through an Ethernet cable/port rather than using a separate mains supply. In networked world every device is connected on IP whether it’s wired or wireless. Wireless connectivity is equally important as wired connectivity since is provides mobility and more flexibility of deployments.

For most office networks the infrastructure are more like Wireless for LAN and Wired for HPC datacenters. Wireless Access Points are integral part of any office LAN or hotel infrastructure today and aesthetics

The only challenge faced earlier was power socket provisioning at each install points.

Hikvision and forward-thinking protect India’s Uka Tarsadia University

ecently, Hikvision’s assistance was enlisted to secure Uka Tarsadia University’s newer Maliba Campus in India. This solution was notable for a number of reasons. However, first and foremost, providing a secure environment for the 4200 students and 200 faculty members on this campus was paramount.

Uka Tarsadia University is located in India’s northwestern coastal state of Gujarat. Due to the location, historical footnotes, and other environmental factors, Gujarat is one of India’s primary economic hubs. Since Uka Tarsadia University is considered one of this state’s elite centers of learning, it was vital for this facility to be safe.

In this vein, Indian-based installer Master’s Mind Technologies was contacted to help the university: • Build a state-of-the-art CCTV system • Future-proof the solution • Provide an easy learning curve for university IT administrators

Starting from Scratch Ketan Patel, owner of Master’s Mind Technologies noted, “This solution was unique in the sense that the Maliba Campus was a blank slate the campus did not have any existing CCTV solution when we began.”

Mr. Patel explained that generally in large-scale projects such as this, it is often a case of upgrading an existing solution - not building one from scratch. “This presented both opportunities in the caliber of solution we could offer, as well as potential drawbacks. Namely, the costs associated with building a solution from the ground-up and cost of hardware provided,” furthered Mr. Patel.

High-Tech Components and Performance Due to the massive distances between academic departments and outdoor surveillance points, fiberoptics were run between these locations and connected to each other using PoE switches. While fiber-optics are expensive, multiple PoE switches helped reduce the costs associated with installation. Importantly, this setup allows state-of-theart video transmission, as well as providing an easy platform for any future upgrades that the university may request.

However, the lynchpin of the solution is a pair of Hikvision camera models: the DS-2CD2112-I 1.3MP IP66 Network Mini Dome Camera and the DS-2CD2012-I 1.3MP IR Mini Bullet Camera.

The DS-2CD2112-I IP66 Network Mini Dome Camera was primarily tasked with the interior of the campus. Mr. Patel stated, “This camera’s 720p real-time video is much better than analog, which obviously serves the school’s security goals to a higher degree. Furthermore, Hikvision’s support really allowed us to customize this camera to our

specific needs.”

Mr. Patel was referring to Hikvision’s ability to help modify the camera’s lens. Out-of-the-box, the DS-2CD2112-I is equipped with a 4mm standard lens. This 4mm lens provides 75° of coverage. However, due to the specific dynamics of many of the school’s classrooms, a wider field of coverage was needed.

In conjunction with Hikvision engineers, Mr. Patel instead placed a 2.8 lens on the DS-2CD2112 to provide 90° of coverage - enough for the entire classroom.

Furthermore, while some indoor facilities are lighted overnight, the majority of classrooms are not.

In this instance, “the up-to 30m of IR visibility is invaluable for proper surveillance in off-hours,” added Mr. Patel.

Ironically, outdoors, the vast majority of the environment is lighted, thus reducing the need for the DS-2CD2012-I 1.3MP IR Mini Bullet Camera’s identical 30m IR range. However, the unit’s 1.3 megapixel high resolution is vital for university IT personnel to quickly identify a possible threat, regardless of time of day. Additionally, the DS2CD2012-I’s IP66 rating is critical to withstand India’s notoriously punishing weather conditions.

Master’s Mind Technologies Promising Future Citing a need to both reduce the learning curve for the university’s IT department (the operators of the CCTV system), as well as reduce costs, Mr. Patel used Hikvision’s iVMS software to create a PC-based solution. Instead of NVRs, IT administrators are able to view up-to 135 cameras per

PC, allowing the entire network to be fully viewed on 4 PCs. Mr. Patel again cited Hikvision’s exemplary product support as a boon for the growth of his company. In addition to the SMS alerts, Email alerts are sent to the emergency contacts along with your location details on a map. An automated voice call is made to the emergency contacts alerting them about the incident.

Emergency responder contact information (i.e. Police, Ambulance Services and Hospitals) in the vicinity of the user’s current location is sent via SMS to the emergency contacts so that they can take further action effectively. In addition, user’s mobile phone automatically sends a video clip, audio clip and photographs of immediate surroundings to Zicom’s Cloud server.

The answer to this question is to first understand the requirements of the customer. It is important to determine what the surveillance system is expected to achieve. The operational requirements have to be understood.

Is it mere detection or detection and observation or detection, observation and recognition or detection, observation, recognition and identification?

• Identification: identification of an unknown individual or object • Recognition: recognition of a known individual or object • Observation: being able to interpret what is going on • Detection: detection of activity within a scene It is these operational requirements that will determine the ‘resolution’ or the ‘pixel requirement’.

As yet there is no industry standard for pixel density requirements. However it is accepted that

mere detection could be achieved at 0.5 pixels per inch while recognition would require closer to 3 pixels per inch. Identification is the most demanding and would require close to 13 pixels per inch.

Proper combination of camera selection, lens pairing and installation location can achieve the requirements of a customer. Standard resolution cameras can either provide details or situational awareness by setting a wide or narrow field of view. Higher resolution cameras can deliver both details and situational awareness.

Specifying the level of quality.

Two common measures of specifying surveillance video quality are: • Resolution • Pixel Density Resolution : Historically this has been the way of specifying surveillance cameras. This is easy to specify and verify as resolution is prominently stated by most manufacturers. However it is extremely inexact and may, at times, be over specified for some situations. While in some situations it may not provide adequate coverage. A better approach is to specify higher resolution cameras for situations covering large areas (e.g., only 4CIF for entrance cameras but 3MP for parking lots).

of specifying surveillance cameras. Specific levels of pixel density are specified (e.g. 40 pixels per foot for entrance cameras but 15 pixels per foot for the auditorium camera).

This is more exact than simply specifying resolution.

It is therefore important to understand how pixel density is to be specified. Merely specifying so many pixels per foot or pixels per meter misses out on crucial details that are vital to accurately achieving the desired video quality.

In order to accurately specify and obtain the desired video quality levels it is important to specify the pixel density along with the following attributes: • • • •

Lighting Conditions in Scene Horizontal Width of field of view (FoV) required Distance from Camera Vertical Range of Coverage

Verifying pixel requirement A) Let us take a case of the desired target pixels to be 45 pixels per foot.

Let us assume that the field of view (F.O.V.) is 35 feet.

Pixel density = Pixels/F.O.V. Therefore Pixels = FOV x Pixel density; 35 x 45 = 1575 pixels A 3 mega pixel camera, which has a horizontal pixel count of 2048 pixels, will meet the desired quality.

B) What would be the FOV if a 5 mega pixel camera were to be used?

FOV = Pixels / Pixel density.

For a 5 mega pixel camera the horizontal pixel count is 2592 pixels.

Therefore the FOV = 2592/45 = 57.6 feet.

The biggest challenge for the brand has been building the home security category in India. Executive Summary The intention of this paper is to examine how success and failure in safety is currently measured in industries. Safety is defined as “the state of being “safe,” the condition of being protected against physical, social, spiritual, financial, political, emotional, occupational, psychological, educational or any other event which could be considered non-desirable” (definition from Wikipedia). In other words, it is how well protected personnel are and not otherwise. In spite of this, the success in safety is measured in terms of failures, i.e. the number of persons injured or nearly injured. Instead of discussing how protected are we, or what are we doing to protect ourselves the board rooms world-wide discuss the injury rates with no information on how to bring it down.

The corporate Key Performance Indicators (KPIs) are set in terms of injury rates, however no clear cut guidelines are there to the operatives on what must be done in order to accomplish them.

This results in a variety of safety programmes based on the injuries, which is like trying to gauge the performance of a machine by the

trouble it gives. This also results in under-reporting of incidents or a culture, where injuries or incidents are not reported to achieve the no injury state. This leads to a sense of false security, which is often broken when a serious injury happens.

This paper examines the existing parameters used in the industry to define the success or failure in safety. This is followed by a brief discussion on the pitfalls of these parameters and why these are not suitable to measure the success or failure in safety. Further, based on the classical definition of safety, a safety measurement model has been developed. This model throws light on what is safety, where it exists in an industrial set up and what exactly needs to be measured to define the success and failure in safety. The subsequent section then details these parameters and how these can be applied practically.

The summary of the paper is that action-based parameters (i.e. what one does to make his work place safe) must be used to define success or failure in safety and not result-based parameters (i.e. how many persons got injured or nearly injured). The incentives and success celebrations must be based on the team achieving action-based parameters. This drives the personnel towards a preventive regime rather than reactive regime. The focus shifts to safety management system performance and issues can be identified much before they result in an unsafe act, condition or an injury.

Background on measurement of safety The evolution of industrial safety

as a science goes beyond a century.

There were two laws in 1847 and 1897 that were introduced in the UK. These laws were around the stipulation of minimum age and hours of work in industry. After these, there were many laws that were introduced in the industry as the safety science developed. The organisations are now migrating towards safety management by objectives, away from a prescriptive legislation driven regime. This means that each leaders of the industry believe that as part of their moral responsibility (more than legal or economical) they are required to put systems in place that would protect their workers, community and environment from any harm or damage.

Today, most of the organisations with developed and matured health and safety management system follow the OSHA’s recordkeeping guidelines to measure the safety performance. This system of measuring the safety performance was originally developed in 1970, for giving the US govt. authorities a flavour of occupational injuries in the work place. The intention was only to provide a starting point of measuring the safety performance and not to give an overall evaluation of the safety performance.

The organisations world-wide use these parameters to measure and report the health and safety performance. Worldwide boardrooms where safety presentations are delivered start with a list of how many personnel were injured in terms of rates, which most of the others present in the room term as failure. Most of the annual/monthly/weekly reports show these rates in various forms as bar charts or line charts. Although the ways in which this information is presented to the management has changed over the years with use of sophisticated software. However, the gist of it remains the same, it describes how many people got actually or nearly injured. In the last decade, the organisations have started recording the unsafe acts or behaviours and driving the behavioural safety programmes.

Along with this, the near miss incidents (i.e. accidents where no damage incurred) is also utilised to measure and report the safety performance. Injury rates are of higher interest to the management and often used to define the success or failure of the safety management system. It often demotivates the personnel who put lot of hard work to make the work place safe and prevent incidents, when after project completion, is not acknowledged as successful because during the project execution injuries and near misses happened.

Existing parameters used to measure safety & define success Before going further into the problem with measurement, let us examine which are the parameters that are currently considered as barometers to success or failure in safety. Most of the organisations like to benchmark their safety performance. There are various agencies that provide this benchmarking service, that compare similar industries on their health

and safety statistics and give them a relative position. The benchmarking is based on the safety statistics, predominantly the trailing or reactive safety (lagging) indicators.

Some of the parameters used in the industry are as follows • Work Related Fatalities • Lost Time Incidents – LTI (injured person doesn’t return to work on same day) • Medical Treatment / Restricted Work Injuries • Occupational Illnesses • First-Aid Cases • Near Miss or No Damage Events.

• Safety Observations or Behaviours Thus organisations end up making a typical safety pyramid with all these

indicators placed from top to bottom and track their safety performances.

Although these pyramids are used in reporting safety, the management focus is always on the top 3 sections i.e. Fatalities, LTI and recordable injuries. A spike in these three areas is portrayed as bad safety performance. Near Misses are events where no injury or damage happens. Often, these are dismissed by organisations rather than considering them as gifts or early warnings/alarms of something dangerous to follow. Organisations often celebrate days without LTI or recordable injuries thus linking the top three sections of the pyramid with their safety performances.

The next section examines the issue or concern around the focusing on these pitfalls in linking the safety performance with injury rates.

The problem is not that these areas are measured and reported but the issue is that they often receive utmost attention or focus of the management and are often used to define success or failures in health and safety management.

Issues with the current system of defining success with safety The current system of measuring the health of safety management system focuses on the symptoms of a disease and not on what is being done to prevent the disease. Following are the issues that are highly likely to happen when management focuses on the injuries and near misses to define success in safety » Firstly, when the employees know that management is focusing on the injuries to define their success or failure in safety, they will have a tendency to manipulate or conceal the incident reporting.

If injuries are considered bad and receive utmost scrutiny of management no one would like to be part of it. Even if the safety culture of the organisation is good, until the management focuses on injuries as measurement of their safety performance this problem cannot be solved. » These parameters are applied mostly after the event has happened, even a near miss or behavioural violation is only observed after the fact » These incidents are mostly measurement of luck and not performance. The injury rates are outcome of functioning of a safety management system.

Continued from page no.80 Failure in examining its function will not solve the problem and lead to recurrence. The incident investigation process reveals the gaps in the safety management system, but do not indicate other systems that may also have gaps.

Essentially, we need to wait for another incident investigation to know what is wrong with the system » These parameters are not diagnostic in nature, there is a lot of analysis and evaluation needed i.e. incident investigation or analysis to find the problem.

Again the analysis will only show the problem when the damage is already done thus not proactive or preventative in nature. The measures also don’t tell an organisation what needs to be done to fix the issues » These measures are not driven by a measurable objective that staff can achieve, organisation often sets targets in terms of LTI or Recordable injuries, however this doesn’t mean anything to staff who keeps wondering, how they could achieve this target. The only way that is in their control to achieve this objective is to stop reporting incidents. » In organisations where there are different projects (with same safety systems and safety culture) being undertaken those projects where the incident doesn’t happen is often termed as successful and other is not.

This may motivate the personnel working on no injury project but is definite demotivator for others who put in same amount of effort to maintain the safe work place but were not lucky to get away without any injuries

Thus the current system of focusing on the injury rates or near misses is one of the ways of measuring the health of safety management system, but must not be used as the barometer to define the success or failure of the system.

Safety definition & measurement model Industrial safety is all about protecting the personnel, environment, assets and reputation (P.E.A.R) from the hazards of the business. Protecting the people from the hazards has moral, economical and legal benefits to the organisation. Safety is defined as a “condition of being protected“ or “something that would prevent the injury or damage”. For example, a safety boot is meant to prevent a likely injury to the wearer or if injury happens it would reduce the consequence. The above model

called “Safety Measurement Model” is a diagrammatic representation of how the safety management system works as a barrier between the people/ asset and the industrial hazards.

Therefore, essentially measurement of safety must be measurement of how this barrier works and what must be done to identify any concerns with the barrier as soon as it happens so the hazards do not reach the other end and cause an injury.

There are four sections in this model and each of them are briefly described here.

Section I This is the starting point of the model. Generally, this is the hazards in the business that can cause harm to the P.E.A.R in organisation.

Section II This is the most important section of the model as this is where the safety resides. This section includes the safety measures that are in place to prevent these hazards from reaching the P.E.A.R. The measurement of the functioning of section II must be the measurement of safety performance in an organisation. The more robust this section is the better safety outcome will be observed.

The section contains following steps to ensure the robustness of safety management system.

• The hazard analysis and risk assessment undertaken to understand all the hazards in the business and get an understanding on what can be the extent of harm.

This is a very important beginning point to managing the risk in the business.

• Engineering measures i.e. building a facility in place that is inherently safe by design. These measures are not dependent on the operators or people but would help them work safely and efficiently. These are the most effective controls that could be in place to prevent injury.

• To support the Engineering controls, administrative controls also needs to be exercised. These controls are put in place so that the engineering controls are correctly implemented. These include but not limited to preparing policies and procedures for safe way of working, providing safe working tools and essentially training the personnel.

• The last line of defence is Personnel Protective Equipment

(PPE), this is an additional and least effective barrier in the safety management system as it doesn’t eliminate the risk of an event, but it reduces the exposure thus reducing the consequence to the person. It is very important that there are administrative controls around the usage of the PPE. • Leadership Commitment is the foundation that supports the above four controls, thus has been shown on the base of the above controls. The leadership commitment sets an organisational climate and safety culture where all these processes can work efficiently. In the absence of leadership commitment and presence, even if an excellent safety management system is put in place, it is bound to fail. The leadership commitment to safety is the fuel to the safety management system efficiency.

Any organisation that focuses on this section i.e. working of the safety management system will be able to prevent the incidents or near misses from occurring thus have a good safety outcome in terms of injuries.

However, most of the organisations focus on the incidents and near miss statistics to find out what is wrong with the safety management system, which is not preventative in nature.

Further sections in this paper will indicate the parameters that can be used by an organisation to focus on the Section II of the model, thus prevent accidents.

Section III This section is where the disease in the safety management system starts manifesting in terms of visible symptoms. The hazards from section I, when they breach the barriers in section II i.e. safety management

These are regarded as proactive safety measurement, however, these are observed after the event has happened thus cannot be regarded as preventative safety parameters.

Normally, these parameters form the base of a safety pyramid, but are hardly used for any purpose other than statistics. These incidents are an early warning to the management that there are issues in the section II and that needs identification and an action plan for correction. If these areas are not identified by the management these will eventually lead to more serious incidents.

Section IV This section often makes it to the headlines of a safety report. This is the most discussed part of the safety management system. The injuries and illnesses are generally used to make a health check of the safety management system. Organisations have legal obligations along with procedural requirements to record this data. When the hazards in section I pass through the barriers in Section II and come in contact with the person injuries and illnesses happen.

This data is often used for benchmarking and comparing the safety performance with other organisations in similar businesses.

The only concern is that this data is considered as the only reflection of the safety performance and used to decide the success or failure of the safety management system. If there is high number of injuries this is considered as poor system and if there are no injuries in a stipulated period it is considered as matter of Continue on page no.86

Continued from page no.84 celebrations. However, we are aware that the joy from such celebrations is short lived and an injury follows.

The purpose of this paper is to establish that although it is very important reflection of the safety performance, the organisation which wants to prevent injuries must go beyond injuries and look more into the functioning of the safety management system.

This section thus shows a safety management cycle, and that where currently the management focuses to make a health check of the system. The problem is that currently the success and failure in safety is defined by sections IV and III, hence these sections are most reported and focused within the management circles. The Section II which is the actual safety in action, or controls or management barriers, which prevent incidents from happening is not evaluated or used for system health check. The result of this is that most of the reporting is based on injury statistics and organisations lack direction on what engineering controls must be put in place to prevent these incidents from occuring.

Degree of Management Control The triangle shown at the bottom of the safety measurement model shows the degree of freedom that management has on each sections.

As we move from the left to right of the safety measurement model i.e. from activity-based regime to the outcome-based regime the degree of direct control goes down.

For example, it is in control of management to undertake a hazard analysis and put control measures in place, however, the actual outcome is very difficult to control. In fact, the outcome-based safety measurement is the result of the activities undertaken to prevent the incidents. Thus,

the focus needs to move back to the activity-based measures, whereas the outcome-based measures need to take a back seat.

In the subsequent sections of this paper, it will be established what could be the possible measurement parameters that would best reflect the performance of the section II of the safety performance model and how the organisation can migrate to preventative ways of measuring safety.

What Is Best Reflection Of Safety In terms of system model of an organisation, there are three main components i.e. input, processing and output. Performance measurement is undertaken to measure the effectiveness and efficiency of the performance. Through measurement, the systems and individuals within the organisation are held accountable. Thus, there can be corrections made within systems if inefficiency is identified. The process of continually measuring performance helps in evolving, eliminating the deficiencies and reaching the goals.

In order to measure safety, the focus needs to be shifted to the process or the activities undertaken towards having desired output that is no incidents. The section II of the previous section is what needs to be measured and focused.

To analyse safety performance (1984), Petersen (write full name) describes the following elements of a Safety Management system: 1. Management safety policy 2. Hazard Identification process 3. Leading towards safety, direction and supervision 4. A monitoring system that can point out deviation from plan 5. Consultation and Participation of

personnel in implementing safety These elements form the basic framework, which needs to be measured in order to know the safety performance.

The lagging indicators are the outputs of the system and focusing on these indicators as measurement of safety system efficiency is like trying to measure efficiency of an engine by the type of failure that happens in it. Lagging indicators are just one area of the safety performance but not all of it.

Setting Safety Measurement Parameters Having concluded that safety is a state of being protected from harm or it is something done to prevent incidents from happening, this section focuses on how it can be measured and what can be success and failure in terms of safety performance.

From the previous section, it is clear that safety is more or less an action undertaken to prevent an incident. The words leading and lagging is often used in health and safety when it comes to monitoring safety.

The parameters like injury rates or numbers are generally considered a lagging indicator as this is after the fact, whereas the parameters like unsafe actions or Near Misses (incidents without injury or damage) are considered leading. However, these words are relative and it is often difficult to define what is leading and what is lagging.

An unsafe act is also observed after the fact has happened, but it is still referred to as a leading indicator. In safety, there are basically two types of parameters. One is Actionbased parameter that refers to a specific activity performed to design a system and prevent an incident and Result-based parameter, which

is the outcome of the activities undertaken. Often, management has higher degree of control on what they do, and less degree of control on what would be the result. So, for clarity, we must migrate from use of the leading/lagging and use the terms Action-based indicators and Result-based indicators.

The Action-based indicators must be the one that tells staff where they must put efforts to prevent the incidents. The result-based indicators must only be used to identify if the action-based indicators chosen are correct or they need to be modified. The Action-based indicators must be the verification and measurements from the safety management system. Organisations often utilise various models for the safety management system. Some of the models used for benchmarking or certification purpose are OHSAS 18001 or ILO Model. Each organisations based on the nature of business must develop the list of the actionbased indicators, which would help them measure the preventive activities undertaken by their team for developing a safe work place. Some of the areas that must be covered in the action-based indicators must be: 1. Management presence, oversight and involvement 2. The hazard analysis and design process 3. The workplace inspections and safety sampling programmes 4. Training, Awareness and competencies management of the personnel 5. The Tools and Equipment Management system 6. The safety promotion and incentive programmes in place 7. Safety Committees and consultation opportunities 8. The Organisational factors

around work execution 9. The incident reporting and investigation 10. Corrective Action closure effectiveness and timeliness 11. The on and off job welfare arrangements 12. Procedures and Processes to assist and empower personnel to work safely and stop the job if they think it is not safe to do so 13. The system of managing work (permit to work) audit and compliance 14. The Legal requirement compliance 15. The change management process 16. The behaviour safety processes or unsafe act/condition reporting process 17. Near Miss reporting and investigation 18. The lessons learnt & continual improvement process These parameters need to be discussed with the teams and specific measurement criteria must be set. For example, to access the corrective action closure the company can measure how many corrective actions were overdue on weekly/monthly basis and give a measure of the percentage of compliance or Key Performance Indicators (KPI). Individual section percentages can then be added up to give a department level measure and finally organisation level percentage.

These parameters must be reported to the management, so they can understand which elements of the management system need assistance or reinforcement.

The incentive programmes must be linked to these KPIs rather than the lost time injury or recordable injury rates and those performing good must be awarded. This will not

only motivate them to keep working but also motivate others to deliver similar performance.

The result-based indicators must be used to investigate and find out gaps in the action-based indicators thus set better KPIs for the action-based indicators.

How to implement this in industries The process of moving from a result-based regime to an actionbased regime has to be a slow and gradual process. But, this can only be initiated if Management develops an understanding on what is safety and where they must look for safety. If they still consider injury rates as barometer for defining success and failure, people will keep focusing on these rates and not on the activities they do to prevent it.

This doesn’t mean that the result or outcome-based indicators should be overlooked.

These results will still give a good reflection on if the actionbased indicators are going on right direction and help fine tune these.

The result-based indicators will still be required for benchmarking or legal reporting purposes but the time has come that management recognises that these are just symptoms of the disease and the focus needs to be on the system performance.

To begin with there must be equal weighting to the outcomebased and action-based parameters to define the success and failures of safety. None of the incentives or project celebrations must be based on the injury numbers but based on the KPIs set for activities undertaken to prevent the incident. In this way, the personnel will understand where the management focus is and will not hide any incidents.

If the management focuses on the activities rather than the injuries they will gradually understand that it is not their fault if an event happens and they are successful if they played their part by participating in the activities that could prevent the incidents.

The result-based parameters must only be reported at the highest level in management so the causal factors from the investigation can be used to fine tune the actionbased parameters. All the incentives, schemes or celebrations must be based on the action-based parameters i.e. before start of any project all the individuals must have KPIs or targets to meet in terms of activity undertaken to prevent an incident and if the site achieves these KPIs they must be regarded as successful regardless of fact that someone gets injured. A no injury site can only be a reality if all the sections of safety management system are efficient. This can happen only if management shifts its focus from the result-based safety measurement to the action-based safety measurement index.

The safety performance can be represented like these: The charts signify the performance in each of the areas and where more focus is required. The Management can check which are the sections or departments that have contributed to lower compliance and then steward these areas. Also, incentives or bonuses can be linked to these parameters thus driving the personnel to accomplish good safety performance. As these are action driven parameters management has greater control over these and these can be transformed into an action plan with closure dates.

Conclusions Following conclusions can be drawn from the study undertaken in this paper. The injury rates or the leading indicators are only symptomatic indications of an ineffective system and must not be used to gauge the success or failure in safety. Safety is essentially the preventive controls that are put in place through the

Need for Genuine Safety

he Indian fire safety market is growing at a significant pace especially due to the strengthening of the compliance rules. A recent report titled “India Fire Safety Equipment Market Forecast & Opportunities, 2017” anticipates that the fire safety equipment market in India shall cross $4.26 billion. Given the vast potential, small companies are mushrooming to cater to the increased need for fire safety equipment’s thereby making it an unorganized sector. The buying decision of a fire fighting product is left to the decision of a third party due to low awareness about its selection as well as application and use.

Purchase of poor quality or spurious products compounded with improper maintenance is the leading cause of fire related disasters.

Some recent examples of such incidents that have taken place in major commercial complexes are a fire extinguisher exploding at the Goregaon Exhibition Centre gravely injuring two fire marshals. And just few months back a fire broke out at Naman Towers, Lower Parelthat started due to a short circuit on the 7th floor and spread swiftly up to the 13th floor. The building’s internal fire-fighting system was not functional and so the fire could not be controlled immediately.

Such fire incidents are strong signal on the need for genuine safety equipment’s & regular maintenance of the same. So far, most commercial complexes are putting in place fire safety mechanism as mere tick marks to satisfy compliance requirements in order to get approvals and clearances from government authorities; however they are completely clueless & sometimes callous about ensuring maintenance of these fire

systems. With commercial establishments increasingly getting more congested, it is absolutely imperative & crucial for industrial and commercial institutes to be equipped with the right fire apparatus.

The two critical issues concerning the fire security industry today are low awareness of adequate security measures and lack of understanding of standardization norms to gauge spurious or inferior products from the quality products.

The life of a stored pressure fire extinguisher is 10 years from the date of manufacture if maintained as per guidelines and refilling has to be done once in 3 years. Users should ensure that their service provide changes the medium (powder/foam) while refilling and not just tops it up or reuses the existing medium. In the case of hydrant systems it is mandatory that the fire pump is kept in operational condition. Poor maintenance of the hydrant equipment like valves and hose reel drums will result in nonfunctional hydrant system which has resulted in uncontrollable fires and loss of property and fire.

Customers should always give priority to selection of the right devices, choose reputed manufacturers and trusted service providers to ensure the safety of their life and property. The choice is yours.

3. Type of compressed gases

A compressed gas is a material which is a gas at normal room temperature and pressure, but is packaged as a pressurized gas, pressurized liquid or refrigerated liquid and is contained usually in a cylinder. The compressed gases may be toxic, flammable or explosive.

The compressed gas cylinders can present a variety of hazards due to their pressure and/or contents. It must be stored and handled very carefully. Regardless of whether a compressed gas is packaged in an aerosol can, a pressurized cylinder or a refrigerated container, it must be stored and handled very carefully. Puncturing or damaging the container or allowing the container to become hot may result in an explosion.

This article will focus on the type of compressed gases, its hazards, testing and inspection of cylinders, safe practices of handling the cylinders, etc.

The three main types of compressed gases that are stored in gas cylinders, which are as follows: 1. Liquefied Gas: The liquefied gases are gases that become liquids at room temperature when compressed at high pressure in a cylinder. e.g., Carbon Dioxide, Chlorine, LPG, Ammonia (See Label at Fig 1).

2. Compressed Gas - Definition As per the Gas Cylinder Rules 2004 of India, “compressed gas” means any permanent gas, liquefiable gas or gas dissolved in liquid under pressure or gas mixture which in a closed gas cylinder, exercises a pressure either exceeding 2.5 Kgf/cm2 abs. (1.5 Kgf /cm2 gauge) at 15 deg C or a pressure exceeding 3 Kgf/cm2 abs. (2 Kgf/cm2 gauge) at +50 deg C or both.

It may be stated here that, the pressure of most compressed gases will not exceed 2000 to 2640 psig (138 to 182 bar ), though some do go up to 6000 psig (414 bar).

3. Dissolved Gas: The dissolved gases are gases that are dissolved in a volatile solvent in order to stabilize them. e.g. Acetylene (See Label at Fig 3).

4. Nature of Hazards As per the nature of hazards, compressed gases are classified into four types which are as follows: 1. Flammable or combustible: Gases are flammable, if their flashpoints (temperature above which there is not sufficient vapours given off to ignite) are lower than room temperature (See Fig 4). In these situations there is an ever present danger of fire or explosion.

3. Poisonous: Exposure to poison gases and vapours can go unnoticed for long periods of time (See Fig 6).

4. Inert: An inert gas is non-reactive gas and is usually a member of noble gas family. It is a nonflammable gas (See Fig 7).

5. Main Hazards The main hazards associated with gas cylinders are:

2. Corrosive: Gases that causes visible destruction or permanent changes in the skin tissue at the site of contact (See Fig 5).

1. Asphyxiation caused by gas leaks.

2. Contact with the released gas or fluid (such as chlorine).

3. Fire resulting from the escape of flammable gases or fluids.

4. Impact from the blast of a gas cylinder explosion or rapid release of compressed gas.

5. Impact from parts of gas cylinders that fail, or any flying debris.

6. Impact from falling cylinders.

7. Manual handling injuries.

1. Inadequate training and supervision for handling the cylinders.

2. Poor installation or maintenance of cylinders.

3. Faulty equipment and/or design, e.g. badly fitting valves or regulators.

4. Poor handling or storage of cylinders.

5. Inadequately ventilated working conditions.

7. Testing and Inspection of Cylinders Manufacture, possession and use of any gas when contained in cylinders in a compressed or liquefied state is regulated under the Gas Cylinders Rules, 2004, of the Government of India as amended from time to time.

Gas cylinders which are in use are subject to wear and tear during handling. Thus with the passage of time, they may become unsafe for further use. In order that their safety might be ascertained, gas cylinders which have been in use are periodically inspected and tested at regular intervals.

The periodical inspection comprises visual examination, measurement of the wall thickness, internal cleaning, weighing and hydrostatic testing. Such periodical inspection is carried out in accordance with the instructions of the Chief Controller of Explosives of the Government of India. However, the cylinder is subjected to external visual examination as per IS 5845:1984, whenever it comes for refilling.

8. Relevant Standards Bureau of Indian Standards has issued a number of standards relating to the design, transportation and handling of compressed gases.

The specifications of colour codes for industrial gas cylinders are dealt in IS 4379:1981. The specifications for LPG are given in IS 4576:1999. The test methods of LPG are dealt in IS 1448:2007. The flameproof electrical fittings to be used in the filling area is

as per specification of IS 2148:2004.

The methods for Hydrostatic Stretch testing are dealt in IS 5844:1970.

Periodical inspection interval for gas cylinders in use are dealt in IS 8868:1998.

9. Safe Practices Observe the following practices while handling the compressed gas cylinders: 1. Contents of the gas cylinder should be clearly identified.

2. Read the MSDS and labels for all of the materials you work with.

3. Color coding is not a reliable means of identification. Cylinder colors vary from supplier to supplier.

4. Do not deface or remove any markings, tags or stencil marks used for identification of contents attached by the gas vendor.

5. Gas cylinders should always be used in a vertical position, unless specifically designed to be used otherwise.

6. Never roll, drag, or drop cylinders or permit them to strike each other.

7. Close all valves when cylinders are not in use.

8. Use cylinder trolley when handling gas cylinders.

9. Transport cylinders with valve caps. Do not lift cylinders by the cap.

10. The vehicles / containers used for transporting the compressed gases must use respective Hazchem Code, TREMcards & MSDS regardless of colour code used for cylinders (See Fig 8 & Fig 9).

11. Gas cylinders should not be stored for excessive periods of time. Only purchase sufficient quantities of gas to cover shortterm needs.

12. Store gas cylinders in cool, dry, well-ventilated areas, away from incompatible materials and

ignition sources.

13. Gas cylinders should be stored away from sources of ignition, other flammable materials or oxygen cylinders.

14. Cylinders (full or empty) shall be secured by chains, straps, or other sturdy tiedowns during storage and transport (See Fig 10 & 11).

15. Full cylinders shall be separated from empty cylinders within the storage area.

16. Flammable gases shall be separated from nonflammable gases.

17. Cylinders shall not be stored at temperatures above 125 oF (52ºC) or in direct sunlight, or outside of the temperature range specified by the manufacturer.

18. Cylinders shall be protected against tampering and damage.

19. Cylinders shall not be refilled except by authorized suppliers.

20. Open flames and smoking shall not be permitted in areas where oxygen is used or stored.

21. “No Smoking” & “No Open Flames” signs shall be conspicuously posted in these areas.

10. Safe Use of Regulators A regulator is a device that receives gas at a high pressure and reduces it to a much lower working pressure (See Fig 12). Regulators are gas specific. However, many a times adequate care is not taken to maintain and handle the regulators. Thus the following precautions may be taken while handling the regulators:

1. Be sure to use the proper regulator for the gas tank in the cylinder.

2. Always check the regulator before attaching it to a cylinder.

3. If the connections do not fit together readily, discard the wrong regulator.

4. Before a regulator is removed from a cylinder, close the cylinder valve and release all pressure from the regulator.

5. Regulators shall be removed from the cylinder during transport.

11. Use of personal protective equipment 1. Wear appropriate personal protective equipment such as safety shoes, safety glasses, aprons, gas masks for all work involving in compressed gas cylinders.

2. Poisonous and irritating gases, such as Chlorine should be handled only by well-informed operators with appropriate safety equipment.

3. Fire extinguishers and heat protecting gloves should be used with gas welding equipment.

4. Flame arrestors should be used in acetylene lines, where acetylene is used with compressed air.

12. Conclusion Personnel who handle compressed gases should be familiar with potential hazards before using the gas. In addition to the chemical hazards of compressed gases, hazards accompanying high pressure or low temperature may also be present due to the physical state of the gas, i.e., liquefied or non-liquefied. All compressed gases must be labelled packaged and transported in accordance with local and national requirements and industrial standards.

The vehicle /container used for transporting the compressed gases must use respective Hazchem code, TREMcards and MSDS, regardless of colour codes used for the cylinders.

TECH UPDATE

The supervisor or responsible person shall designate and train employees and workers who are required to handle and use compressed gases and ensure that compressed gases are handled in accordance with safe working practices.

It is the supervisor’s responsibility that employees and workers using compressed gases understand the proper procedures.

Address potential emergencies and corresponding measures necessary to safely avoid such emergencies.

Consider scenarios that could result in gas leaks or other emergencies in order to be fully prepared to react appropriately.
