Voluntary and Community Sector Multi-Speciality Community Provider (MCP) Engagement Session Notes from session Date of session: Tuesday 7th November 2017 Time: 1-3pm Venue: DY1 Building, Stafford Street, Dudley, DY1 1RT Purpose of session Following the launch of the procurement process for Dudley’s Multispecialty Community Provider (MCP) in June, Dudley CCG recently announced the bidder that has made it through to the next stage of the procurement process. Since September 2017, Dudley CCG has been going through a dialogue process with a consortium involving four local NHS Trusts and local GPs. The four trusts are – Birmingham Community Healthcare NHS Foundation Trust, The Dudley Group NHS Foundation Trust, Dudley and Walsall Mental Health Partnership NHS Trust and Black Country Partnership NHS Foundation Trust. The new Multispecialty Community Provider (MCP) will bring together services in an integrated manner and will:   

hold a contract of up to 15 years’ duration; manage a single, whole-population budget; transform the access to and delivery of community health and care services with Primary Care at the centre, and meet a defined set of outcome and performance measures.

As part of the dialogue phase, Dudley CVS hosted in partnership with Dudley CCG this engagement event for the voluntary sector with the potential consortia. This event enabled the voluntary sector to learn about proposed plans for the MCP and an opportunity to observe and engage in discussions on pre-set questions about how the potential bidder intends to engage with the sector over 10 – 15 years to meet the outcomes of the MCP. Presentation about the voluntary and community sector offer During the presentation it was emphasised that the voluntary sector is integral to the delivery of the MCP outcomes, in particular delivery of non-clinical outcomes. The voluntary sector feels that the MCP does need to actively engage and work with the sector. The presentation initially covered some basics about the makeup of the voluntary sector, the contribution made to society. Following this the benefits and approach about way of working in the voluntary sector was covered including areas such as:  Facilitating self-help and mutual support  The sector can communicate with people in a way that makes sense to them, we ask the right questions Notes produced by Dudley CV – November 2017

As we are not statutory services, we can engage with people in grassroots communities. The sector does not solely deliver peer support and social activities but we deliver on professional services such as palliative, respite, sheltered housing etc. The voluntary sector has been around a long time, we have a lot of knowledge to offer and bring to the table. The sector can often get initiatives going quickly with flexibility and with less red tape.

Presentation on the children’s work stream Doug Simkiss – Birmingham Community Healthcare NHS Foundation Trust Doug Simkiss provided information at the meeting about the potential focus of work around children and young people going forward if the bidders win the MCP contract. Focus will be on the following:  Looked after children  Child protection  Long term conditions  Acute illness  Complex health  Severe acute illness  Early Help  Safeguarding  Connected Care for children  Coproducing a children’s model  Children’s Multi-disciplinary team meetings Mental Health Focus Jacky O’Sullivan – Clinical Development Director – Dudley and Walsall Mental Health NHS Foundation Trust Jacky provided some information at the meeting about the potential focus of work around mental health is the consortia wins the MCP contract. Focus will be on the following:  Mental health assessments/entry into services across 7 days  Psychotherapies Hub – where more psychotherapies could be developed and offered  Alcohol/substance misuse and the effects of this on mental health  Mental health Multi-disciplinary team meetings  Re-enablement – preventing people needing to come back into primary care eg supporting with housing issues, employment, tackling isolation etc Q&A – outcomes from discussions Notes produced by Dudley CV – November 2017

Panel responding to questions: Steph Cartwright, Gillian Love, Mickey Griffiths, Dr Richard Bramble (The Links) 1. The MCP model will be about doing things differently and will have a strong focus on prevention. It is widely acknowledged that the voluntary sector delivers on prevention and early intervention. If you win the contract, what commitment or strategy will you have for working with and supporting the voluntary sector to help deliver on the MCP outcomes? Arwyn Jones (Beacon Centre for the Blind) Response NHS is under strain. Recruiting and retaining GPs is a challenge across the country. The MCP does require a different kind of workforce. The MCP will need to deliver services and respond to communities differently. Prevention and early intervention is key and we recognise this as a strength of the voluntary and community sector. We do want to engage with and work with the sector. Over the last 3 years we have developed our knowledge about how diverse the voluntary sector is through the Integrated Plus service. We would welcome ideas from the sector about how you would like to work with us going forward. If the bidders win the MCP contract there will be various work streams established focusing on mental health, older people, children’s, end of life, respiratory etc. We would welcome voluntary sector representation on these work streams. 2. What reassurance can you give us that residents of Dudley Borough won’t get just more of the same services? Jayne Sargeant (Phase Trust) Response The MCP will need to do things differently and we need to be innovative. We can’t have just more of the same but we need a change of culture. We would like to work collaboratively with the sector to help us engage with people and communities to help ensure people’s views are heard and people can influence a new way of working. 3. What would you like the voluntary sector to do more of to help achieve the MCP outcomes such as reducing isolation, enable people to better self-manage conditions? Caroline Webb (White House Cancer Support) Response We recognise that the voluntary sector provides a wide range of services and not just services that tackle isolation and loneliness. Going forward the MCP will need to deliver on helping people to better self-manage long term conditions, promote education, help improve life expectancy, enable the enhancement of peer support, encourage healthier behaviours and healthier lifestyles. We would like more befriending on a larger scale, for example, befrienders/peer support in say Alcohol Anonymous etc. We would welcome your ideas about how we could enable this to happen. We would encourage your ideas about what the sector can do and what it can’t do and about what capacity the sector has.

Szygowski (AgeUK Dudley) Response As mentioned above the potential bidders are proposing to set up a number of new work streams to help redesign and explore patient pathways. If the bidders win the MCP contract we would welcome involvement and representation from the voluntary sector at these. 5. How will the VCS current contracts with Dudley CCG be managed through the MCP? Julie Duffy (the What? Centre) Response Current contracts held between Dudley CCG and voluntary sector organisations will move into the MCP. These contracts will be rolling forward for up to 2 years, initially. Dudley CCG and the bidders would welcome discussions with the voluntary sector organisations who hold contracts about how these contracts could possibly be redesigned going forward. Request from the voluntary sector on collaboration going forward In light of the discussions held during the session it was emphasised by the voluntary and community sector that it would welcome the following:    

Regular formal engagement with the MCP over the next 10-15 years To be part of any new work streams established To be part of discussions which help to redesign patient pathways eg mental health, frailty, end of life etc A possible ring-fenced budget to develop services and activities that help to meet the MCP outcomes eg peer support, befrienders, self-management programmes etc.

Whole population management and prevention

As the name suggests this is taking the medium to long term view of the health of the Dudley population and includes both the analysis of trends and the interventions to promote a healthier population

Single Point of Access/Frailty

This is about streamlining access and the mechanics of people and staff contacting the services within the MCP. It also includes all services and support for the frail elderly (incl dementia)

Long Term As the name suggests this cover long term conditions such as Conditions respiratory (COPD), diabetes, MSK, CKD etc. It’s focus will be those Management/Primary services that could/should be seen as community delivered. This is the redesign of both district/community nursing and the medium term opportunities for primary care at scale.

Everything to do with children and Young people prior to adulthood (which will vary between 19 and 25 yrs depending on individual’s needs.

This ensures adult LD needs are covered and is linked to all age LD through links to the CYP workstream above.
