*TERMS & CONDITIONS: DC Dollars Rewards will be sent to the customer together with each order placed. Actual DC Dollars Rewards must be used to redeem the prizes. Prizes will not be rewarded until the Dollars are sent back for redemption. Dollars may not be used to pay a DC Dental balance without first redeeming it with the DC Redemption Form. Dollars will be distributed to the nearest $100 mark. For example,if a customer spends $467, he or she will receive five DC Dollars Rewards–equivalent to the amount rewarded for spending $500. To redeem your DC Dollars Rewards,call and ask your DC Dental representative to send you a redemption form. This program is limited to dental offices purchasing dental supplies ONLY. Equipment purchases are excluded from this program. Vendors, suppliers, or those buying wholesale are excluded as well. This is a trial program and is therefore subject to termination at any time. If the program is terminated, prizes may still be redeemed for a one month period following termination. DC Dollars are redeemable by the dentist or current employees of the redeeming account. Dollars are not redeemable by accounts not in good financial standing with DC Dental Supplies, LLC.
