SHIFT TO PATIENT-CENTERED CARE

“ What do you mean ‘ patient-centered care ‘ (PCC)? Isn’t that what we have been doing ever since? Haven’t we been taking care of our patients day in and day out? Who else have we been centering on?”

Healthcare providers wonder what’s the big deal about “ shifting to PCC”. I, too, scratched my head about it, until I participated in a lengthy survey on attitudes and behaviors of doctors managing diabetes. I was surprised to find myself ticking the NEVER box on questions like;

1. Do you ask your patient what he wants to achieve from his treatment ?

2. Do you ask your patient how he feels about his diagnosis of diabetes?

3. Do you ask your patient how his diagnosis of diabetes will affect his work, his family and community?

Huh! Huh! Huh! to all three questions, and more. After I saw that I was ticking NEVER away much too often, I started to feel, “ Am I practicing medicine right? “ It dawned on me that I haven’t been truly attentive to my patients , seeing things from their perspective, forgetting that there was more to people than their bodies. All I had in mind was to get that sugar, blood pressure, cholesterol, and weight down. Get them to diet, stop smoking, and exercise. Keep them from the ICU, the dialysis and the morgue as long as I can. I’m the one who is supposed to know about their health.

Picker Institute and Harvard Medical School did a study on what patients seek more in their healthcare providers. The results were not the expected answers from health providers point of view.

Being respected and treated as an individual .

How come patients were mum on facilities and equipment, highly skilled nurses and doctors , excellent clinical outcomes? Aren’t they important? Absolutely, but they are assumed to be givens . Those are matters expected of us. That’s our job but we are being asked to deliver more because our business is life.

Hence, we are now shifting our attention to the significant aspect of patients’ wanting to be heard, wanting to understand the nuances of their care, their viewpoints explored, their life circumstances including their finances considered - making them feel secure and comfortable in the strange and threatening hospital environment.

If a patient cannot read our directional signs and gets lost while looking for the OPD, we are not being patient-centered.

If a patient cannot read small letters of our forms, instructions, bills, etc., we are not being patientcentered.

If doctors and nurses do not explain procedures to be done , preparations needed, risks and costs articulated, we are not being patient-centered.

If patients are discharged, not knowing what to do when they get home, we are not being patientcentered.

If our comfort rooms smell, our roads rocky , our corridors uneven, fire exits impassable, processes tedious, personnel impatient, rained-on in-between buildings , and a multitude of other practical needs unmet, we are not being patient-centered.

Patient-centered care is not merely a sentimental, philosophical and abstract issue. It is all that and how it is translated into the practical day to day way we care.

Let’s look at our ways from the patient’s perspective. Maybe, even try being one!

4 Cover Story | TLC is more than just an ancroynm

DMIT it, health care in the Philippines is such a luxury such that there is a common thread that is running in the thoughts of people who do land in the hospital: Big trouble. There is one feeling that wraps every soul who are brought in: Dread.

Very few, if none at all, are the patients who do not have that feeling of dread – whether it be because of the mere sight of people in white seeming always to be in a hurry, because of not knowing what they are suffering from and wishing that it will not be something that will cost an arm and a leg, and yes, the fear of having to pay an arm and a leg…

Thus, a short shout out to friends on social network on what they wish hospitals will have was met with a lot of TLC’s as answer.

Tender, loving care.

Prodded to describe or define TLC further squeezed out this wish list:

• For doctors or at least the most knowledgeable person to be there to appraise you of your ailment.

• For emergency staff to attend to patients without imparting the feeling that the patients are such an inconvenience.

• For the reassuring presence of a doctor right within the emergency room to check out each patient.

• For patients and kin to be thoroughly briefed on why they have to wait for this much length of time instead of being left to wonder all by themselves when the next nurse, orderly, or doctor will look in on them.

• For doctors to allot more time discussing with the patients and kin about the patient’s status. One particularly wrote a detailed description that earned the likes of many: that of doctors just dropping by maybe once, usually late in the evening at that, and just going through the motions of looking through a patient’s chart and asking, how are you and basic questions that do not even establish rapport.

The point here is that scared patients want some reassuring presence. This is the very fact that many welcome a lot of visitors, because they try to draw reassurance from friends and kin that are otherwise denied them by the sterile environment of a hospital. But even that reassurance is limited because if there is anything that hospitals are strict about, it’s visiting hours and how huge a crowd can cram into one tiny room. Maybe, that friend is right. Maybe if the whole atmosphere exudes that feeling of wellness instead of dread, then less patient will try to draw the strength from as many kin as he can gather.

The operative word that can be gathered from the long discussion the shout out generated is that feeling of wellness and assurance imparted by each and every health professional and institution to those who need to give them a visit.

In short, real TLC.

5 MEET THE EXPERTS

Nelson Caroran Villano, MM, MD, FPSGS, FPCS General, Minimally Invasive and Bariatric Surgeon

If you talk surgery, business, or golf… then, you’ve got this person’s undivided attention.

Dr. Nelson Caroran Villano, known to friends simply as “Nelson”, was born and raised in Davao City. He is the second of four children of Dr. Herminio A. Villano, Sr. and Mrs. Aurora C. Villano.

Having been born to a father who is a surgeon and a mother who is a nurse, it came as no surprise when he gravitated towards becoming a medical professional himself, just like his three other siblings.

He graduated with a degree in Chemistry at the Far Eastern University in Manila after which he pursued his Doctor of Medicine degree at the Davao Medical School Foundation. Soon after passing the board exam, he underwent Residency Training in Surgery at the Davao Doctors Hospital. He then went on to become a fellow of the Philippine Society of General Surgeons and the Philippine College of Surgeons. He was one of the pioneers in the practice of Minimally Invasive Surgery in Mindanao and underwent training in advanced laparoscopy of gastrointestinal cancer at the Cho Ray Hospital in Ho Chi Minh City, Vietnam. In his constant quest to explore new frontiers in his profession, he recently found a new passion in Bariatric Surgery for which he trained in minimally invasive bariatric surgery at the IRCAD Asian Institute of Telesurgery as well as in other bariatric surgery centers in Taiwan and Singapore. He is now an active member of the Endoscopic and Laparoscopic Surgeons of Asia (ELSA).

His other passion which is business and management prodded him to earn a Master in Management degree from the Liceo de Cagayan University where he graduated at the top of his class. He put his learning to good use to set up the family business, The Metropolis Suites Davao.

Dr. Villano is currently the Chairperson of the Department of Surgery of Davao Doctors Hospital where he also heads the Bariatrics and Metabolic Surgery Unit. He is a former member of the Board of Governors of the National Chapter of the Philippine College of Surgeons.

He is a Past President of the Davao Doctors Hospital Staff Organization. He is also a Past President of the Philippine Society of General Surgeons, Davao City Chapter and a former member of the Board of Directors of its National Chapter. He was also a former member of the Board of Trustees of the Davao Doctors College. He had a stint in the academe as part-time instructor at the Davao Doctors College and the Davao Medical School Foundation. Just recently, he was appointed by the National Chapter of the Philippine Society of General Surgeons as member of the AdHoc Committee on the Revision of the Surgical Curriculum. Dr. Villano is a champion golfer who regularly plays golf for exercise and relaxation.

Bonding time with the family is either spent at home just hanging-out with his wife and kids or traveling on vacation together. If there is one thing that not a lot of people know about Dr. Villano, it is the fact that he actually loves to cook for his family and that they love his cooking!

His principle in life is encapsulated in this quote by E. Joseph Cossman: “Our business in life is not to get ahead of others but to get ahead of ourselves”.

Serves 4 Preparation time: 15 minutes No cooking required (if rice is pre-cooked)

Always take the stairs It’s time to let elevators and escalators rest. Commit to carrying yourself from floor to floor using your own two feet.

There’s one exception — if you work or live in a high-rise, no one expects you to traipse up and down 30 flights of stairs every time you head out for coffee with friends. Decide in advance how many flights you’ll walk — three, five or 10 — and then catch the elevator for the rest.

Your kids will go gaga over this Hawaiian –inspired dish, perfect for Sunday lunch dates!

Ingredients 1/2 carrot 1/2 celery stick 1/2 small red or green capsicum 2 spring onions 1 can pineapple rings in natural or unsweetened juice 2 slices reduced fat and reduced salt ham* 4 cups cooked rice* 4 tbs sweet corn kernels Cracked black pepper, to season 2 tbs reduced fat salad dressing (made from a healthier oil such as canola, sunflower or olive oil*) Instructions 1. Wash the carrot, celery, capsicum and spring onion and pat dry with paper towels.

2. Grate the carrot and put into a mixing bowl.

3. Slice the celery finely and add to the bowl.

4. Remove the seeds from the capsicum. Cut into small squares and add to the bowl.

5. Cut the root end off the spring onion. Slice finely, then add to the bowl.

6. Cut the pineapple into small squares and add to the bowl.

7. Slice the ham into small squares and add to the bowl with the cooked rice, sweet corn, pepper and dressing.

8. Using a spoon, mix thoroughly.

9. If not using at once, cover with cling wrap (or transfer to a storage container) and refrigerate until needed.

DDH Balik Alay Foundation Extends Hand to Yolanda Survivor

On January 22, 2014, Davao Doctors Hospital Balik Alay Foundation turned over its donation of 220,000 pesos to the City Government of Davao for children who survived the devastation of Typhoon Yolanda. The donation is from the generous hearts of DDH management, doctors and employees as part of its Corporate Social Responsibility.

Assistant City Administrator Atty. Jhopee S. Avanceùa – Agustin received the donation from Balik Alay Foundation President Dr. Rizalina M. Pangan, Executive Vice President Elizabeth H. Anzures and Human Resource Director of DDH Ms. Mirasol B. Tiu. The donation will help fund the education of children of more than 200 families who are now currently residing here in the City. The beneficiaries were identified by the City Social Service Development Office (CSSDO).

The City Government of Davao has been active in reaching out to the survivors of the most destructive typhoon ever recorded in the history. Davao Doctors Hospital Balik Alay Foundation took part in giving hope to the families who are now taking their refuge in the City.

Patient Satisfaction : DDH Client-Centered Direction

Robert Wood Johnson Foundation defined Patient Satisfaction as a measurement designed to obtain reports or ratings from patients about services received from an organization, hospital, physician or health care provider.

Patient Satisfaction Survey (PSS) plays a vital role in determining the level of gratification that our clients receive. The measurement of patient delight through PSS has helped organizational leaders incorporate patient views and feedback in decision-making . This activity also creates a culture of patientcentered care within the institution.

Being truthful to the Davao Doctors Hospital Vision, “The Unparalleled Health Institution Beyond Borders”, Customer Care Department, together with Dr. Celia Castillo of Corporate Research and Development, Dr. Elsie Tee of Quality Improvement, and Mr. Paul Omar Dandoy of Information T e c h n o l o g y Department, has come up with Actual PSS using an electronic tablet with the revised format. Some major revisions were made to shortened and render concise the questionnaires used in the face to face interview and those issued to sampled in-patients and outpatients. We also experimented on utilizing the Net Promoter Score (NPS) as a quick gauge of patient satisfaction and delight. All the feedback, comments and inputs from patients are collated, graphed and trended. These data are emailed to the senior managers weekly to help them in analyzing the quality of the services we are giving so they can decide rationally and quickly

. This way our leaders get the feedback of the clients when they need to.

On July 3, 2013, face to face PSS interview was pilot tested. Proven effective, the tool was upgraded. The use of electronic tablet commenced last December 1, 2013. The process can be followed thru this cartoon. We continue to improve our process to make our data even more timely.

Through PSS, both commendations and complaints to our personnel as well as our facilities and services are easily heard and directly answered. This innovation was made possible because we set our clients’ delight and experience as the heart of all our endeavors.

Evidence from the literature suggests the potential role of PRF in periodontal regeneration and tissue engineering.

We also use PRF in dental implants placement to ensure optimum healing and tissue regeneration. The department’s oral surgical specialist is the first specialist in Visayas and Mindanao to use PRF routinely, with wonderful results.

Reuben Higino Cabrera Board of Director January 11, 1970 marked a milestone for Davao Doctors Hospital as this was the day they opened the Delivery Room of the fledgling hospital. For Dominador and Rosa Cabrera, new additions to the Medical Staff of DDH, it was the birth of their youngest child, Reuben Higino. Reuben, fondly called “Ludoy or Luds”, had shown at an early age, a fondness for animals particularly dogs and would constantly accompany his parents every time they would visit the family farm during weekends. These undoubtedly were very influential in his decision to pursue Veterinary Medicine in college despite the determined effort of his mother to convince him otherwise.

Having graduated in 1992 and taking the board exams thereafter, he went back to Davao and joined Nestfarms, Inc. the piggery operation of the ANFLO Group of Companies. In the middle of 1993, with the recruitment of new veterinarians, he was transferred to Empire Joy Stud Farm in Marapangi, Toril, Davao City to take care of the company patriarch’s prized possessions, Thoroughbred race horses.

Parallel to these events, San Miguel Foods Inc. started their broiler contract– grower operations here in Davao City. Familiar with this business opportunity during his college days, he was able to convince his family to engage in the business with a farm having an initial eighteen thousand head capacity. In 1995, after the birth of his first child, he left his job and decided to join the family business which, by then, has reached a capacity of thirty thousand heads. In 2004, with the poultry houses nearing the end of their serviceable life, he decided to start anew but this time, the farm would incorporate new technology needed by the chickens to reach their full genetic potential. By 2007, RER Cabrera Poultry Farms, Inc. was formed with a capacity of ninety six thousand heads.

Realizing that the business was not “chicken feed” anymore, he saw the need for a new set of skills to ensure the success of the family business which led him to finish with a Master in Management degree from UP Mindanao in 2009. While taking up this course, he helped his spouse establish their private practice, Central Park Veterinary Clinic.

Professionally, he was also very active with the Philippine Veterinary Medical Association – Southern Mindanao Chapter, taking on various roles as an officer or director culminating with the presidency in 2004. In 2006, he was among the chosen few to be trained on Good Agricultural Practices (GAP) under the sponsorship of the Philippines-Australia Human Resource Development Facility (PAHRDF) for the Davao City Chamber of Commerce and Industry, Inc. (DCCCII). Together with Dr. Francisco Vilela, he authored the GAP Manual for Broiler, Layer, Breeder, and Game fowl Production for the Small and Medium Enterprises Center of DCCCII. He has coached teams, from Dole Phils. and Dole – Stanfilco, as a member of the UP Strategic Research and Management Foundation, Inc. (UPStream). In addition, he was also coach for a RD Group of Companies unit under the ADDU Center for Business Research and Extension.

In 2012, he joined the Ateneo de Davao University – School of Business and Governance as part-time faculty handling courses in Strategic Management and Human Behavior in Organizations (HBO). In Davao Doctors College, he has handled HBO for the Master of Arts in Nursing and Medical Entrepreneurship classes for undergraduates.

Being the newest member in the Board of Directors, he seeks to provide a view of “an outsider looking in” and is determined to decide in the best interest of the DDC shareholders in particular and the people of Davao City in general. And to fulfill this, he sees Davao Doctors Hospital sustaining its leadership through its people, technology and innovations. And someday, he wishes for DDH to spread its wings further to address the healthcare needs of this rapidly growing city and be the Unparalleled Health Institution Beyond Borders.

Joyce Sta. Ana- Custodio MSO President

Meet the new Davao Doctors Hospital (DDH) Medical Staff Organization (MSO) President, DR. JOYCE STA. ANA CUSTODIO. She has been with DDH since 1984 to present as an active consultant in the Department of Obstetrics & Gynecology. She is a member of the Philippine Board of Obstetrics and Gynecology (2002-2004) and a Fellow of the Philippine Obstetrical and Gynecological Society, and the Philippine College of Surgeons and the Philippine Society of Gynecologic Endoscopy. She served as Department Chair from 1995 to 2000.

She has been a member of the Davao Doctors Hospital Institutional Ethics Review Committee (DDH-IERC) since the year 2000 and is DDHaffiliated. She is the current Chair of the IERC and has been in this position since January 2007.

At the helm of the MSO together with the Vice-President Dr. Oscar M. Escudero; Secretary Dr. Elizabeth B. Villano; Treasurer Dr. Robert B. Bandolon; Auditor Dr. Jeanmarie C. A Month for Women's Health

In every third month of the year, the whole world celebrates the Women’s Wellness Month. A Month wherein we create awareness to all medical conditions that affects women's well By Dr. Noel Villanueva being because every woman deserves to have a healthy way of life.

In 2014 a significant shift in mindset is taking shape in Davao Doctors Last March 24 to 28, 2014, Davao Doctors Department of Hospital. The Board of Directors has approved recommendations of the Obstetrics and Gynecology in partnership with Ancillary Medical Staff regarding changes in the classification and requirements and Marketing Department hosted a five day Celebration on of Physician staff.

Changing Institutional Paradigms

We started the week-long activity with a Lay Forum on Healthy Pregnancy, in the panel were our OB Specialists: Dr. Sigrid Barinaga, Dr, Irene Uy and Dr. Geoffrey Que. Dr. Junette Grace Ko, also facilitated the question-and-answer portion of the talk.

In attendance, were women of all ages who gamely engaged in sharing thier own beliefs on common pregnancy myths. The Department also offered a five-day free Prenatal Screening from 8 am to 12 noon and free screening for No Balance Billing procedures from 1pm to 5pm at the Out-Patient Department.

First, Visiting Physicians are no longer required to own shares of stock in the Corporation. This makes it easier for all recent graduates of Accredited Residency Training Programs to attend to their patients in Davao Doctors Hospital.

Second, Associate Active Medical Staff are now allowed to hold Clinic elsewhere provided they spend twenty hours a week in their Davao Doctors Hospital clinic. However, they can enjoy more hospital benefits if they choose to become Full time Associates within two years and even more benefits when they become Active Rotating members of the Staff.

A mini-exhibit was also installed at the lobby featuring Facts about Prenatal Care, Pap smear, Women’s Vaccines, 4D Ultrasound, Mammography and Menopause. To add fun to the celebration the OB group invited seasoned fitness instructors to a 5- day session Open to All Zumba at AVR 3, 4th Floor, Oncology Building every 6pm.

These changes are meant to encourage the Best and the Brightest to join the Davao Doctors Hospital Medical Staff. This will mean more and better services that can be offered to patients. We also expect to have more Residency Training Programs to become Accredited. With the infusion of new and qualified Specialists, we also expect the current Accredited Programs to become even better.

Davao Doctors Hospital is also increasing the number of Service Beds for our Philhealth Case Rate cases. This is in line with our Vision of being the “The Unparalleled Health Institution Beyond Borders”.

To ensure success of this program, we are strengthening our Social Work Services as well as formulating more Clinical Pathways to improve efficiency of medical care.

Congratulations to the Department of Surgery for successfully implementing their Service Case Program. Since November of 2013 they have operated on seventeen Service cases. Not to be outdone, the Department of OBGYN has admitted two patients into the Service ward since late February of this year. Plans are underway to utilize the Engineering Building to house our Philhealth Beds and we expect implementation in 2015.

In this Year of the Wooden Horse, places great emphasis on hard work and tenacity. You’ll fare better by working on improving your health and wellbeing than trying to maintain the status quo. Also, according to the Chinese zodiac, the year 2014 promises fantastic opportunities for business, travel and adventure. I believe that we have ushered in enough good luck for Davao Doctors Hospital to soar higher this 2014.

We are witnessing great progress in our strategy to change the image of Davao Doctors Hospital as we aggressively change the landscape of the healthcare industry by future marking our organization.

Our face lifting of the façade, our ground floor renovation, our new well-appointed patient rooms, and our new hotellike lobby – all these create a more welcoming environment for our patients. Together with these, DDH has opened its doors to new and talented doctors to practice in our hospital through amendments in the requirements for our incoming medical staff.

We also welcome the New Medical Staff Organization (MSO) president, Dr. Joyce Sta. Ana Custodio and thank the out-going MSO president, Dr Parkash Mansukhani for his invaluable contribution to our organization.

For 2014, our thrust is to engage more fully our patients and clients through more effective communication, externally and internally. We will raise the DDH value curve by communicating our Clinical outcomes, Safety and quality programs and intensify our customer service programs.

We have embarked on the following initiatives: • The FGD Project – aims to catch positive narratives, “good news” and happy stories from our patients, business partners, employees and stakeholders.

• Digital Information – it guarantees instant and global reach all the time. We will use social media to share our culture and values along with the promotion of our services. Also, this year we aim to launch the Web-based Digital Information Kiosk system, where all information (doctors, business hours and location maps) will be provided to our patients in real time.

• Lay Fora and monthly activities – we would like to reach out and educate our patients and clients on how to better care for their health. We provide a venue for them to interact with our doctors. Just recently, we have started doing on-site lay fora for companies engaged with us.

• Digitalized queuing system – we have started the second phase of our Queuing System, where we have additional kiosks located in our billing section and pharmacy.

Finally, I want to express how proud I am of this organization.

We have a really great team, doing great work that focuses on engaging our patients and other stakeholders through various Care-Beyond-Compare projects. I would like to thank the whole organization for their continued support and effort in making DDH, the Unparalleled Health Institution Beyond Borders!
