Innovation Through Collaboration Dear friends, Our academic medical community is made up of an extraordinary combination of people: We are helpers, thinkers, and doers. We see challenges as opportunities for change, and we work together to create solutions that improve people’s lives. Innovation is in our nature, and collaboration is the key to our successes.

At the Geisel School of Medicine and Dartmouth-Hitchcock Health (D-HH), we share a common mission to build a healthier future for people everywhere—down the road and around the world. While the COVID-19 pandemic has forced us

to make adjustments for the safety of our students, our patients, and our employees, it has never deterred us from our important work.

This issue of Giving in Action celebrates some of our accomplishments of the past year, achievements made all the more special because of the unprecedented circumstances under which they occurred. Notably, we started to see the impacts of the Susan & Richard Levy Health Care Delivery Incubator and the Dartmouth Innovations Accelerator for Cancer—two programs launched in 2020 that spark innovation through collaborations between our scientists, clinicians, and students.

Also in 2021, gifts to Geisel in support of Dartmouth College’s Call to Lead campaign brought the medical school’s total to-date to a milestone $180 million. The 40th Prouty raised a record $4 million toward patient care and research at Dartmouth’s and Dartmouth-Hitchcock’s Norris Cotton Cancer Center. We appointed a new director to The Dartmouth Institute for Health Policy & Clinical Practice and welcomed four new faculty members, all of whom are dedicated to advancing health equity research. And we’ve made significant progress on the Patient Pavilion at Dartmouth-Hitchcock Medical Center in Lebanon, which will allow us to care for more people closer to home.

In the year ahead, Geisel will embark on a strategic planning process to refine our educational priorities and develop new programs to support our students.

D-HH continues to move forward with the planned GraniteOne Health combination, expand the Children’s Hospital at Dartmouth-Hitchcock in Manchester, increase the availability of clinical trials, and strengthen healthcare worker pipeline programs with Colby-Sawyer College. These steps ensure that more residents of Northern New England—from rural villages to small cities—have access to the best-inclass expertise of an academic medical network.

None of this would be possible without the support of generous donors like you. Thanks to you, we raised a record $60.8 million in the last fiscal year—during the most difficult time in modern history. More than 3,700 alumni, parents, and friends have answered Dartmouth’s ongoing Call to Lead campaign with gifts to the Geisel School of Medicine.

Inspired by Geisel’s vision for a healthy, resilient, and equitable future, donors have pledged more than $135 million toward Geisel’s top campaign priorities and an additional $45 million to other programs, bringing the school within reach of its $207-million goal. Their generosity is fueling vital student initiatives, research and innovation, and advances in healthcare delivery.

“Our faculty, alumni, and students are improving the health and healthcare of millions of people worldwide, extending the school’s impact farther than ever before,” says Duane Compton, PhD, dean of Geisel.

Scientific discoveries made at Geisel led to the first successful cancer immunotherapies, helped transform care for people with cystic fibrosis, and, more recently, laid the foundation for the first COVID-19 vaccine— to name just a few examples.

“The generosity of alumni and friends is allowing our faculty and students to pursue bold new ideas and become the healthcare leaders our world needs,” adds Compton.

Todd Sisitsky, a member of the Dartmouth College Class of 1993 and chair of the all-volunteer Geisel Board of Advisors, is one of many College alumni to make gifts to Geisel during The Call to Lead campaign.

“As a donor and a volunteer, I’ve been able to marry my love of Dartmouth with my love for biomedical research and innovation,” says Sisitsky, who is also managing partner at TPG Capital. “For anyone who wants to make a positive impact on the world, Geisel is a great place to start.”

Every big idea is a seed of change, but it takes support to help them grow. Philanthropy is crucial to give investigators the time and resources they need to develop their ideas, collect data, and make life-changing discoveries. Without it, potential treatments never make it out of the laboratory, and promising theories about improving healthcare delivery never get tested.

In 2020, generous, visionary donors helped establish two programs that allow scientists and clinicians at the Geisel School of Medicine and DartmouthHitchcock (D-H) to pursue innovative ideas: the Susan & Richard Levy Health Care Delivery Incubator and the Dartmouth Innovations Accelerator for Cancer. The Incubator promotes the development of pioneering solutions that reimagine care and improve outcomes for people experiencing illness or other medical events. The Accelerator provides researchers with the support, entrepreneurial guidance, and infrastructure needed to bring new cancer treatments to patients more quickly.

At the heart of both these programs are collaborations—between scholars, between institutions, and between donors and investigators equally committed to improving the quality of human life.

Health Care Delivery Incubator

Tyler Hartman, MD, and Kathryn Richards, MSN, APRN, envisioned a better way to care for stable preterm infants.

T

yler Hartman, MD, a neonatologist at Children’s Hospital at DartmouthHitchcock, tells a tale of two babies.

Both born weeks early, Owen was on a ventilator to help him breathe as he fought a brutal infection, and Libby was being fed through a nasogastric (NG) tube but was otherwise stable and healthy. So why was Owen at home, and Libby in the Neonatal Intensive Care Unit (NICU)?

Doctors, nurses, and other health professionals see discrepancies and weaknesses like these in healthcare delivery, and they envision solutions.

But there’s little opportunity for them to be proactive when they spend their busy days reacting. Time outside of a system is essential to making changes within it.

A gift to the Geisel School of Medicine from Susan and Richard Levy, PhD, D’60 launched the Susan & Richard Levy Health Care Delivery Incubator, a joint initiative between Dartmouth College and Dartmouth-Hitchcock (D-H) and based at The Dartmouth Institute for Health Policy & Clinical Practice. The Incubator supports projects focused on improving patient outcomes, prioritizing patient- and family-centered care, reducing spending, and promoting scholarly work. Importantly, it gives project leaders protected time in which to further their ideas. It was co-founded

Deep Understanding, Successful Solutions “The COVID-19 pandemic has magnified how resourceful and innovative our clinical teams are. They often have tremendous ideas about how to improve care for patients and families,” says Joanne M. Conroy, MD, CEO and President of D-H and Dartmouth-Hitchcock Health. “The Levy Health Care Delivery Incubator gives our care teams the time and resources to develop those ideas and, ultimately, implement the best ones.”

Teams are selected through a competitive pitch process, then spend 12 months developing and testing their healthcare delivery interventions at D-H. Scholars from throughout Dartmouth College with expertise in fields like engineering, business, and study design collaborate with clinicians to bring their solutions to fruition. In addition to funding that protects clinicians’ time, team members have access to financial and material support from the Incubator, including project management, analytics, and information technology, and mentoring from a network of Dartmouth College faculty and D-H senior leadership.

Born six weeks early and weighing just under 4 pounds, Libby Hale spent her first days in Dartmouth-Hitchcock’s Neonatal Intensive Care Unit. Though she was small and needed to be fed through a nasogastric tube, Libby was otherwise healthy—making her an ideal candidate for the Hope Grows at Home program.

Abbie and Matt Hale were overjoyed that they could bring their daughter home in days instead of weeks, and they felt comfortable managing Libby’s complex nutritional needs because of the exceptional support they received from the Hope Grows at Home team. Now just over a year old, Libby is thriving.

“Clinicians have a deep understanding of systemic problems that they’ve been seeing throughout their careers,” says Nirav Kapadia, MD, MS, medical director of the Incubator, medical director of radiation oncology services at Dartmouth’s and Dartmouth-Hitchcock’s Norris Cotton Cancer Center North, and an assistant professor of medicine at Geisel and of The Dartmouth Institute. “Part of our mission is to help them find experts from the College who can help them think through their problem differently, and solve it successfully.”

A Better Way of Doing Medicine Among the first cohort of funded projects was Hope Grows at Home, which redesigned care for stable preterm infants by transitioning their feeding from the hospital to the home with extensive medical support and monitoring. Many preterm babies like Libby remain in the NICU just to feed and grow. They are otherwise healthy, no longer in need of specialized medical care, and filling beds that could be used by critically ill newborns. Data suggest that extended NICU stays can affect infant development, cause emotional distress for families, and are a financial burden on hospitals.

Led by Hartman, who’s also an assistant professor at Geisel, and nurse practitioner Kathryn Richards, MSN, APRN, the project gave families the tools and education they needed to care for their growing babies at home. They were taught how to replace an NG tube; were provided with a continuously recorded monitoring system that sends data securely to D-H; and received daily check-ins with D-H caregivers via telemedicine, along with home visits.

To date, 31 babies have been through the program. There has not been a single adverse event and parents have reported 100% satisfaction. And

the project generated innovations with far-reaching impact: Collaborating with engineers from Thayer, Hartman and Richards designed a new NG tube that’s easier to place and are now working on patenting their invention; physicians throughout D-H are considering ways the continuous monitoring system may allow them to safely send their patients home sooner; enhanced feeding strategies developed for the program have been implemented in D-H’s NICU; and Hartman and Richards have developed a collaborative nationally with centers running similar programs to share data.

Fruits of This Labor With the third round of projects about to get underway, Kapadia notes that the Incubator is meeting its mission. “We’re engaging our clinical and non-clinical staff, providing better care for patients, and demonstrating that the improved care can be delivered at a lower cost.” Teams have started applying for external funding based on their results, and sharing their learnings through publications and presentations.

The Incubator is serving as a model for a similar program at Stanford University as well, where Dick Levy sits on the Board of Advisors. The former CEO and chairman of Palo Alto-based Varian Medical Systems, Levy is passionate about lowering costs and improving access, convenience, and outcomes for patients.

“I chose to fund the Incubator at Dartmouth because of its close-knit academic medical community and multidisciplinary approach to problem solving,” Levy says. “The first cohort of projects simultaneously addressed cost, outcomes, and access—the three major goals of healthcare reform—in a very realistic way, proving that we can accomplish them all.”

Accelerator for Cancer

Y

olanda Sanchez, PhD, uses an automotive analogy to describe some types of cancer cells: They’re like cars with two gas pedals and no brakes. As a cancer researcher, she looks for ways to crash that out-of-control car without harming the normal cars around it.

“A lot of cancer drugs harm normal cells almost as much as tumor cells,” says Sanchez, a professor of molecular and systems biology at the Geisel School of Medicine and associate director for basic sciences at Dartmouth’s and Dartmouth-Hitchcock’s Norris Cotton Cancer Center. “My lab focuses on finding drugs that can kill a tumor cell and not harm the person.”

Scientists like Sanchez who study cancer share a common goal: to make life better for patients. Dartmouth researchers are dedicated to improving outcomes for the estimated 1.9 million men, women, and children newly diagnosed with cancer in the United States every year. Unfortunately, it takes about 20 years for a discovery to progress from the laboratory to the marketplace, and just 1% of promising ideas complete the journey successfully. The Dartmouth Innovations Accelerator for Cancer aims to change that.

The Accelerator has a goal of raising a total of $15 million in philanthropy and is a fundraising priority within Dartmouth’s Call to Lead campaign.

Reaching Patients Sooner The Accelerator was launched in 2020 to help scientists like Sanchez bring new cancer treatments to patients more quickly. A joint initiative between the Cancer Center, Geisel, and Dartmouth’s Magnuson Center for Entrepreneurship—and supported by philanthropy—the program provides researchers from across Dartmouth with the entrepreneurial guidance and infrastructure needed to translate innovations into diagnostics, devices, and treatments for patients through commercialization.

“When you look at the number of biotech start-ups that have spun out of our Cancer Center, it’s astounding,” says Steven Leach, MD, director of the Cancer Center and the Preston T. and Virginia R. Kelsey Chair in Cancer at Geisel. “The Accelerator builds on our history and culture of collaboration and innovation.”

In its first year, the Accelerator attracted 17 research teams—one of them led by Sanchez—including 41 faculty and students, all eager to turn discoveries into life-changing treatments for patients with cancer. In a 10-week course taught by instructors from Simbex and Celdara Medical, two local biomedical companies, Accelerator participants learned about biomedical entrepreneurship, pitching to potential investors, regulatory requirements, and how to generate new invention disclosures and patents, and they created step-by-step, multiyear plans for the commercialization of their innovations. Although not typically the realm of biomedical researchers, these are all skills needed to ensure that a drug or device reaches its end goal of impacting the lives of patients. Participants also have access to a network of Dartmouth alumni who serve as industry guides and mentors throughout the program and beyond.

At the conclusion of the first year of the program, the teams participated in a two-day pitch competition and an external review panel of successful biomedical entrepreneurs and investors selected three winning teams to receive awards of $300,000, $100,000, and $50,000. That money will fund critical next steps in the development of high-potential cancer treatments.

A cancer-driving gene known as MYC plays a central role in tumor growth in breast, colon, and lung cancers as well as in leukemia, lymphoma, and melanoma. Yet because MYC is also essential to normal cell growth and to human life, it has long been considered off limits for drug therapy. Accelerator award-winners Michael D. Awards and Rewards This year’s top prize was awarded to a team led by Arti Gaur, PhD, assistant professor of neurology at Geisel, for its project on a new potential therapy for high-grade gliomas, a kind of brain tumor for which the median survival is only 14 months.

“For three and half years, I have pieced together funding for this project,” says Gaur. “This award will help us get to the next phase, where we can potentially attract the money that will be needed for clinical trials”—the next step in making this treatment available to the patients who need it most.

Though only three teams were awarded funding, all participants reaped the benefits of the program.

Sanchez was especially grateful to learn the steps involved in getting a drug to human trials and then into the market. She also appreciated the opportunity to meet people on the business side of drug development. “A CEO on the review panel was interested in our project, so our initial 10-minute pitch turned into an hour-long pitch later,” she says.

“We received a lot of feedback from people who can help move our project forward.”

The studies are necessary to enter clinical trials.

Members of Sanchez’s team included Dartmouth College undergraduates Cindy Takagawa D’21 and Albert Wang D’21, and Bernice Leung, a PhD student in Dartmouth’s Molecular and Cellular Biology program. Sanchez notes, “It was empowering to see so many women participate in the Accelerator both as leads and as team members because the world of entrepreneurship is not, for the most part, a comfortable platform for women. And I think one of the things that Dartmouth does really well is include students in programs like this that give them such invaluable experience.”

Strong Roots, Growing Branches The next cohort of Accelerator teams will be welcomed into the program in late 2021 and will compete for the second annual awards in spring 2022. New offerings to this second round of participants include fast-track awards totaling $100,000, a more robust pitch preparation program, and new educational topics.

None of this would be possible without philanthropy. The Accelerator took off in 2020 when five Dartmouth alumni contributed $1.4 million. Since then, more alumni, parents, and friends of Dartmouth and its academic medical community have made gifts, bringing the total to $3.5 million. The Accelerator has a goal of raising a total of $15 million in philanthropy and is a fundraising priority within Dartmouth’s Call to Lead campaign.

“There were so many deserving teams just in our first year.

With greater philanthropic support, the Accelerator will be able to make more awards and enable more Dartmouth faculty and students to advance their innovations for the benefit of cancer patients everywhere,” says Jamie Coughlin, director of the Magnuson Center. “That’s the power of philanthropy and the Dartmouth entrepreneurial community.”

Mapping Out a New

Dartmouth Atlas of Health Care

The Dartmouth Institute for Health Policy & Clinical Practice is renowned for its Dartmouth Atlas of Health Care. Initiated in 1996 by John “Jack” Wennberg, MD, MPH, the Atlas transformed the world’s understanding of health systems by documenting variability in the cost and quality of healthcare nationwide. The Atlas and The Dartmouth Institute’s groundbreaking research on variations in healthcare costs and outcomes, its expertise in the science of quality improvement, and its pioneering role in shared decision-making have all helped shape federal policy and influenced how care is delivered.

But whether through pandemics or politics, the world is changing ever more rapidly, and the Dartmouth Atlas is changing with it. Led by new director Amber Barnato, MD, MPH, MS, The Dartmouth Institute is poised to create a Health Equity Atlas. This reimagined Atlas will help scholars investigate inequities in healthcare due to racial and economic injustices and to improve patient outcomes where they are most needed.

To begin this vital project, The Dartmouth Institute hired four new investigators who focus on health equity. They join a faculty of 150 researchers in teaching 200 full-time students in a range of master’s and doctoral degree programs. A New Dartmouth Atlas for a More Equitable World

mber Barnato, MD, MPH, MS, began her directorship of The Dartmouth Institute for Health Policy & Clinical Practice on July 1, 2021. A physician-scientist with a special interest in end-oflife decision-making, Barnato has been a researcher and clinician at the Geisel School of Medicine and Dartmouth-Hitchcock since 2017. She was also the inaugural Susan J. and Richard M. Levy 1960 Distinguished Professor in Health Care Delivery at Dartmouth.

What is the role of the Dartmouth Atlas of Health Care at The Dartmouth Institute today?

The discipline of health services research really started with Jack Wennberg’s small area variations studies in the 1970s.

His research acquired a national scale in the 1990s through the Dartmouth Atlas. The Atlas informed some of the most important healthcare legislation of our generation, including the Patient Protection and Accountable Care Act in 2010. But the most burning policy questions have evolved considerably since the 90s and 00s. The Atlas needs to help the United States meet today’s challenges.

Dartmouth, like the nation as a whole, is in the midst of a reckoning with institutionalized gender and racial inequality. I’ve been very interested in thinking about what our obligations are as a f ield—health services research—that has been shaped by a white racial lens. For example, with the Atlas, we have focused especially on overuse of medical care. Overuse is disproportionately a problem of insured people with access to healthcare. At The Dartmouth Institute, we’ve promoted shared decision-making as a strategy to “right size” medical care use. Yet shared decision-making is a privilege of people who engage in conversation with their clinicians and who have high levels of self-efficacy in what are predominantly white spaces.

We need scientists and practitioners who can help us center other voices in our work. To that end, we’ve hired four new health equity researchers.

How will this equity focus build on the legacy of the Dartmouth Atlas?

What inspired you to take on this new vision for The Dartmouth Institute?

We will use a truth and reconciliation framework to reflect upon the legacy of the Atlas and to create a new Health Equity Atlas. For a national Health Equity Atlas, I hope that we can use our large repository of Medicare claims data to revisit some of our past work. For example, given what we know about segregation of healthcare delivery in the U.S.—that 20% to 30% of providers care for 80% of Black people—has Medicare service delivery become more or less segregated over time? No one has ever used 30 years of claims data as a lens to examine the ways that Medicare policy affects systemic racism in healthcare. This framework will allow us to challenge our own assumptions and ask new research questions.

I love health services research and have a thriving lab devoted to studying end-of-life decision making. Providing palliative care to patients with serious illness and their families—many of whom are dying years younger than they would have if they’d been born into different social and economic conditions—keeps me connected to the broken realities of our system. I felt called to take on this new vision now through a recognition that my privilege as a white cisgender female with high educational attainment comes with responsibility. I hope that facilitating the research and educational missions of The Dartmouth Institute will have a greater impact on health policy and clinical practice than my individual lab ever could.

We also envision designing a local version of the Health Equity Atlas here in New Hampshire. Stakeholders will participate in deciding what questions are most important to them and their communities. We can look beyond just healthcare to include water quality, housing, and other factors to assess health outcomes. Our approach might enable others to create local atlases for their communities.

These are all ideas at this stage. The Health Equity Atlas will depend on philanthropic support and broad community engagement to come to fruition.

tenants, she documents how they view their lives and housing. Unlike quantitative work that captures trends and resource allocation, qualitative research uncovers people’s individuality, which can be just as important when designing healthy communities.

“We all experience the world uniquely,” says Lewinson, whose approach is informed by her training. She earned her doctorate at the University of Georgia School of Social Work. “My experience of joy will look very different from someone else’s experience of joy.”

One public housing complex might be well maintained, another allowed to deteriorate. Residents in one extended-stay motel might be able to walk easily to nearby shops, doctors’ offices, and green space. In another, residents might find themselves on an island, encircled by traffic-choked highways and without sidewalks or reliable bus service. These differences can have a dramatic impact on physical and psychological health, especially for low-income older people, explains Terri Lewinson, MSW, PhD. Previously at Georgia State University, Lewinson has found that factors less obvious than building design are crucial to quality of life, for better or worse. By conducting in-depth interviews with

Lewinson aims to expand her research to explore the intersection of housing and environmental issues—and ultimately inform better public policies that will enable more low-income people to live healthier lives.

A particularly powerful research method Lewinson uses is Photovoice—giving tenants cameras to document their lives.

The photos then serve as springboards for conversation. One study participant photographed a busy highway next to their home and told Lewinson, “I can’t cross this street.” Another showed her a picture of an iron and said, “My husband hit me with this iron.” The first photo led to a conversation about neighborhood walkability, and the second, a discussion about domestic abuse.

Lewinson’s research interests continue to evolve. In 2017, she served as a Health and Aging Policy Fellow in the Washington, D.C., office of the late civil rights icon and Georgia Congressman John Lewis. Her portfolio included environmental issues.

“It opened my eyes to the inequities, not only arising from affordability and quality, but larger environmental issues,” she says.

The siting of low-income housing often exposes residents to industrial emissions, unclean water, and noise pollution.

Drawing on the expertise of her new colleagues at The Dartmouth Institute, Lewinson aims to expand her research to explore the intersection of housing and environmental issues—and ultimately inform better public policies and community programs that will enable more low-income people to live healthier lives.

Like Lewinson, Christine Marie Gunn, MA, PhD, works to include the voices and perceptions of individuals, but her focus is on cancer and the decisions people make. Physicians and patients tend to view decision-making differently, Gunn explains. Physicians look at likely benefits and harms, while patients focus on what’s important to them.

Gunn recalls a woman with kidney cancer who declined a drug because it would have made her hair turn white. In another study, she found that women chose not to take a medicine to prevent breast cancer despite being at high risk because they feared the rare side effect of stroke and were concerned about possible menopause-like side effects.

Gunn, who earned a doctorate in health services research at the Boston University School of Public Health and taught at Boston University School of Medicine, brings to Dartmouth her expertise in health literacy and working with ethnically and linguistically diverse patients. For instance, in a study completed in 2016, she analyzed breast density notifications (after mammograms) in 24 states and found that in most cases the notifications were difficult to understand and contained language whose complexity surpassed the average reading level of state residents.

At The Dartmouth Institute, Gunn will have access to unparalleled healthcare data sets and colleagues who are world leaders in patient decision making, health systems research, and in co-designing care with patients and families. With those resources, she plans to expand her work to include socioeconomic disparities.

Excited to be part of Dartmouth’s collaborative environment, Gunn is also a member of Dartmouth’s and Dartmouth-Hitchcock’s Norris Cotton Cancer Center, where she has clinical partners to support her direct work with patients. And at Dartmouth College’s Digital Applied Learning and Innovation Lab, students are assisting Gunn in the creation and building of a mobile application to help ensure that patients at all literacy levels can make educated decisions about their health.

Most importantly, Gunn will continue to center patients in her research, as she has done throughout her career. For a current study, she is hiring male prostate cancer survivors to interview men who had also completed prostate cancer treatment.

“Talking to someone who’s been through a similar experience and can relate in different ways to study participants can sometimes generate a more open conversation,” says Gunn. And that could make all the difference.

Alka Dev, MHS, DrPH

New Hampshire and Vermont may not seem to have much in common with Haiti, Indonesia, Kosovo, Mexico, Bangladesh, and India. But for Alka Dev, MHS, DrPH, who has worked to improve maternal and newborn health in all of those places, she sees a similar story of health inequity. People face precarious housing and homelessness; unemployment or low-wage jobs; distrust of the care system; difficulty in finding transportation to clinics or hospitals; the inability to take time off work; and lack of practical support from people around them during a health crisis.

Dev is always looking for ways to make care more patientcentered. “I’m really interested in redesigning maternal care from the perspective of the women who are using the system for several months during one of the most important times in their lives.”

Dev joined The Dartmouth Institute last year after four years as a maternal and newborn health researcher within Dartmouth-Hitchcock Health. She previously earned a master in health science at the Bloomberg School of Public Health at Johns Hopkins University and a doctorate at The Graduate and University Center of City University New York. Much of Dev’s recent research has focused on Haiti—including the effects of gang violence on women’s decision-making during pregnancy, the quality of prenatal and neonatal nursing care, and women’s choices about whether to give birth in a hospital or at home.

Dev is always looking for ways to make care more patient-centered. “I’m really interested in redesigning maternal care from the perspective of the women who are using the system for several months during one of the most important times in their lives,” she says.

In addition to conducting research, Dev teaches two health equity courses at The Dartmouth Institute and helped rethink the on-campus MPH program to implement a social justice thread across the curriculum.

Frustrated by what she sees as the failure of many healthcare systems to consider how factors outside the clinic affect patients’ health, Dev wants her students to do more than recognize inequities in health and healthcare. “Ultimately, rather than simply studying inequities, it’s important that we generate the necessary evidence on how to reduce them,” she says.

Health Systems for All Identities

"Ultimately, healthcare has been designed and delivered by heterosexual white men, studied by heterosexual white men, and, historically, those studied have been heterosexual white men,” says Ellesse Roselee L. Akré, MA, PhD. As a result, healthcare systems often don’t work well for diverse individuals.

No group is a monolith, she says. Within any group—even heterosexual white men—there are wide variations in social class, education, and circumstances. “That same thing applies to the LGBT community,” says Akré, whose research focuses on the health and healthcare of lesbian, gay, bisexual, and transgender (LGBT) people, and of people who identify as Black, indigenous, and people of color (BIPOC). She examines how healthcare policy and healthcare itself have ignored the multiplicity of human experiences, identities, and bodies.

As a postdoctoral researcher at Vanderbilt University, Akré looked at how macrolevel systems contribute to health inequities including racism, sexism, ageism, and heterosexism. For her doctoral research in health services at the University of

Akré's research focuses on the health and healthcare of lesbian, gay, bisexual, and transgender people, and of people who identify as Black, indigenous, and people of color. She examines how healthcare policy and healthcare itself have ignored the multiplicity of human experiences, identities, and bodies.

Maryland School of Public Health, she had studied differences in healthcare access in California among four groups: gay and lesbian people in cities and in rural areas, and bisexual people in cities and in rural areas. She found different problems with access in each of these groups; a study that grouped these identities together would likely have missed these important variations.

Through her research, Akré wants to give diverse individuals “information and tools they need to empower their communities, so they can advocate for themselves for better care and better policy.”

At The Dartmouth Institute, Akré will bring her expertise in LGBT and BIPOC health not just to the Institute’s research community but also into classrooms, from the undergraduate to the graduate level.

“We’re forming the minds of our future leaders, educators, practitioners,” says Akré. And those individuals will be the ones creating the more equitable health systems of the future.

Progress on the Patient Pavilion

Dartmouth-Hitchcock Health (D-HH) achieved a significant milestone in the construction of its 212,000-squarefoot, five-story Patient Pavilion at Dartmouth-Hitchcock Medical Center (DHMC) on Friday, June 18, 2021, when one of the final beams in the building’s steel structure was installed.

The occasion was marked with a traditional “topping off” ceremony, which is a long-standing tradition in the construction industry to mark the completion of a building’s structure— specifically the placement of the final steel beam. This beam was particularly

special: It had spent two weeks in DHMC and was signed by patients, volunteers, employees, and donors.

Construction on the Patient Pavilion began in July 2020, with an anticipated completion date in early 2023. It will include spacious, state-of-the-art private rooms to meet the needs of the most seriously ill patients in the region, as well as unfinished space to allow for future expansion. More than an entire floor will be occupied by the Heart & Vascular Center, with 16 beds dedicated to cardiac special care.

Hits the Road for Heart Health

If knowledge is power, then the Dartmouth-Hitchcock Heart & Vascular Center (HVC) brings power to the people throughout rural Northern New England. Using a dedicated Dodge Transit minivan and a red-and-white pop-up tent, the Community Awareness Research and Education (CARES) program brings free, no-hassle blood pressure, blood sugar, and cholesterol screening to people across New Hampshire and Vermont who might not be aware of their risk for heart disease.

The program launched two years ago, but paused during 2020 because of the pandemic. In August 2021, HVC CARES resumed and held events at a home game of the Upper Valley Nighthawks collegiate baseball team in Hartford, Vt., and at Dan & Whit’s General Store in Norwich, Vt. Clad in HVC polo shirts and hats, the team of nurses who staff the CARES van set up their tent, tables, and chairs wherever the people are—at county fairs, farmers markets, food pantries, sporting events—and screen anyone over the age of 18 for the risk factors for heart attack and stroke. They know that for many rural residents, the logistics of time off from work, travel, childcare, and insurance can put basic healthcare out of reach and raise the risk of a catastrophic heart event down the road.

HVC CARES nurses like Leane Matchem, BSN, RN, travel to farmers’ markets, athletic games, health fairs, and other public events offering free heart health screenings to the community.

The CARES team sees screening as just the first step in spinning a web of community connections to support heart-healthy living for the residents of rural New England, no matter how far they live from DartmouthHitchcock Medical Center in Lebanon, N.H. The CARES nurses welcome whole families under their tent—even kids too young for screening can squeeze a free HVC stress ball and chat about the benefits of fresh fruits and veggies while a relative gets their blood pressure checked. Folks with more risk factors—a history of smoking, for example, or family members who have suffered heart attack or stroke—receive referrals to local providers, while those interested in smoking cessation, healthy eating, and new fitness routines can take home pamphlets, as well as information about local resources to support lifestyle change.

“The purpose of HVC CARES is to raise awareness about heart and vascular health in our communities, and identify those at risk for heart disease by screening high blood pressure, high cholesterol, and diabetes.

We particularly wish to connect with those living in underserved rural areas,” says Heart & Vascular Center Director Mark Creager, MD, the Anna Gundlach Huber Professor in Medicine at the Geisel School of Medicine.

Leading the Way for Patients with Movement Disorders

I

magine trying to take a sip of your morning coffee and not being able to get the mug to your mouth. Or reading on your phone as it wobbles in your hand. Or going out to eat with your friends and the food tumbles off your fork.

Millions of people in the U.S. live with Parkinson’s Disease, essential tremor, or dystonia, three conditions that cause involuntary or uncontrolled movements. For patients with these neurological disorders, everyday activities can evoke feelings of frustration, embarrassment, and hopelessness.

Dartmouth-Hitchcock (D-H) physicians are making life better for people with movement disorders through deep brain stimulation (DBS), and they’re doing it in pioneering ways. DBS involves surgically implanting electrodes into a specific part of the brain to provide highly focused electrical stimulation to that brain region. Stimulation is controlled by a small impulse generator battery (IPG) implanted in the chest wall, and connected to the electrodes by wires that run under the skin.

Joshua Aronson, MD, a D-H neurosurgeon and an assistant professor of surgery at the Geisel School of Medicine, led D-H’s efforts to make the procedure more comfortable and less frightening for patients. Awake for the four-hour operation, patients typically need to have their heads placed in a clamp attached to the table. This year, D-H became the first hospital in New England to offer robotic DBS surgery, which uses a small

robot to aid in the placement of the electrodes and allows patients to avoid the discomfort of the clamp. D-H is also the only hospital in Northern New England to offer completely asleep MRIguided DBS placement as an alternative to awake surgery.

“For some patients, just the thought of what they would endure during surgery was a barrier to them seeking treatment,” Aronson says. “I want people to know that there’s a better option.

Technology has changed the way we do surgery.”

New technology has also allowed Anas Hannoun, MD, to become the first neurologist in New England—and one of just a handful nationally—to calibrate a DBS device remotely.

People who have DBS devices can adjust them using a manual controller, but must also see their neurologists every three-to-six months to recalibrate the IPG and electrodes. When one of Hannoun’s patients experienced a return of some Parkinson’s symptoms, Hannoun was able to adjust her device from his laptop during a virtual appointment based on real-time feedback.

“If you had told me six months ago that I would have the capability to program a DBS device remotely, I’d have said, ‘No way’,” says Hannoun, a neurologist at D-H Manchester and a clinical assistant professor of neurology at Geisel. “Connecting to and adjusting the DBS device while our patient is at home is a huge breakthrough.”

From Plant Virus to Cancer Vaccine A team of researchers at Dartmouth’s and Dartmouth-Hitchcock’s Norris Cotton Cancer Center, led by Steven N. Fiering, PhD, has identified pathways through which cowpea mosaic virus (CPMV), a plant virus that does not infect mammals, is recognized by the immune system. This discovery opens the door for a new biological drug for the treatment of cancer.

CPMV is recognized by the immune system as a potential pathogen—any infectious agent that can cause disease. When tumors are injected with CPMV, molecules in the immune system send a warning signal of the invasion, which is heard by the “ears” of the immune system called toll-like receptors. The toll-like receptors then mobilize immune cells to attack the pathogen—and along with it, the tumor.

Commercial development of CPMV as a cancer treatment is in progress by Mosaic ImmunoEngineering Inc., a biotech company co-founded by Fiering and Nicole Steinmetz, PhD, of University of California San Diego, with a team of scientists and entrepreneurs. Early-phase trials of CPMV in humans are planned to start in early 2022.

New Ways to Understand Post-Traumatic Stress Disorder A study of brain tissue from patients afflicted with post-traumatic stress disorder (PTSD) has uncovered new information about the disorder, and is poised to change how PTSD is understood and guide future treatment development. In research spanning several years, Matthew J. Friedman, MD, PhD, and his colleagues found that the brains of people with PTSD had “extensive remodeling” of the normal operations by which genetic information is coded, impacting everything from neural transmission to metabolism to inflammation.

“This study is the first of many to come that are going to change the field of PTSD research,” Friedman said. “Unfortunately, our emerging understanding of the psychobiology of PTSD has not led to the development of effective medications for the disorder. We haven’t even seen a new drug developed for the treatment of PTSD in 20 years. Our team’s findings offer new clues about the causes of PTSD, the possibility of improved treatment, and most importantly, hope for people who live with this terrible disorder.”

Parkinson’s Risk Factors Among Rural New Englanders A study by researchers at the Geisel School of Medicine suggests ways that rural New Englanders can lower their risk of Parkinson’s Disease, a progressive, neurodegenerative condition whose debilitating symptoms include balance and mobility issues. The team, led by Angeline S. Andrew, PhD, found that among people living near the Vermont-New Hampshire border, head trauma or concussion was associated with a fourfold greater risk of Parkinson’s. Exposure to lead—whether at work or through such hobbies as bullet casting, fishing with lead weights or sinkers, or stained glass making—increased risk by 2.7-fold.

In their report in the journal Parkinson’s Disease, the investigators note that encouraging the use of lead substitutes and helmets are general health recommendations that may also reduce the risk of Parkinson’s. The study also highlights the importance of understanding regional variations in risk; while other studies have implicated pesticide exposure as a risk factor for Parkinson’s, few participants in the Geisel study reported such exposures.

Intervention Promotes Healthy Weight Behaviors Obesity affects more than 13 million youngsters, increasing their lifetime risk of diabetes and heart disease. Thus the American Academy of Pediatrics (AAP) standard of care for all pediatric patients includes measuring Body Mass Index (BMI) and laboratory screening for diabetes, high cholesterol, and liver function. Among patients with high BMI, the AAP recommends positive reinforcement for healthy behaviors.

Yet nationwide, screening is inconsistent and positive behavioral change can be difficult for families and providers alike. To promote better care for patients at the Children’s Hospital at Dartmouth-Hitchcock, Komal F. Satti, MD, MPH’19, and colleagues launched a quality improvement program to promote compliance with the AAP standard of care. The intervention— which included education sessions, the identification of “obesity champions” among providers and staff, and provider cues within the electronic medical record system—increased providers’ rates of laboratory screening, obesity documentation, referral to weight management programs, provision of lifestyle counseling, and weight-specific follow-up visits.

Cancer treatment is difficult enough without the extra burden of financial hardship. But some patients at Dartmouth’s and Dartmouth-Hitchcock’s Norris Cotton Cancer Center struggle to pay for the transportation that gets them to their treatments, or they go without food so they can afford their medications. When Helene Perrier’s husband passed away, she wanted to honor his memory by making a difference in the lives of these patients and their families. In 2019, she established the Marc Labelle Fund for Patient and Family Support at the Cancer Center, and this year she designated the fund as a beneficiary of her retirement assets.

Thanks to her generosity, people in need can tackle their diagnoses with less anxiety and more hope.

Alan I. Green, MD, served as chair of the Department of Psychiatry at the Geisel School of Medicine and Dartmouth-Hitchcock for nearly 18 years before his death in December 2020. Psychiatry and psychology teaching and training for medical students, residents, fellows, interns, and post-doctoral fellows flourished during his tenure and research initiatives blossomed. To honor his memory, Alan’s wife, Franny Cohen, and the Green family established the Alan I. Green M.D. Psychiatry Award Fund at Geisel. The award will be given to Geisel faculty within the Department of Psychiatry who demonstrate outstanding teaching, mentorship, and research.

Dartmouth College alumna and cardiologist Holly Andersen D’85 wants medical students to thrive and to experience the joy of being a physician. Her recent $2.5 million gift in support of student mental health at Geisel brings the total raised for the Healthy Students, Healthy Physicians program to $4.5 million. The program, launched in 2020 with an anonymous gift from a Dartmouth College alumnus and his family, benefits all Geisel students and provides access to free confidential counseling, mental health screening, resilience skills-training, and a variety of other supports and special programming. The school seeks to raise an additional $5.5 million to fully endow the program and ensure such supports are available to future generations of students.

A retired nurse practitioner, Diane Eleftherakis experienced life as a patient when she had a kidney transplant at Dartmouth-Hitchcock Medical Center (DHMC) in 2021. Even with health insurance and a supportive network of family and friends, her recovery was difficult. She understood that patients with fewer resources have an even harder time managing their care after a serious diagnosis or major medical event. In memory of her husband, Dartmouth-Hitchcock (D-H) oncologist Robert Jackson Forcier, Diane has made three generous donations to patient and family support services at D-H, benefiting people with cancer, Alzheimer’s disease, and her fellow kidney transplant patients.

For his 50th medical school reunion, David Zamierowski MED’66 and his classmates established the Six of DMS ’66 Fund in honor of the six pioneering women in their class. Now, for his 55th reunion, Zamierowski has established the Medical Education Innovation Fund for Bias Awareness at Geisel. With funding from Zamierowski’s latest gift, a nascent project on microaggressions, originally supported by the Six of DMS ’66 Fund, will develop into a pilot research program focused on incorporating bias awareness into the school’s MD curriculum.

for Hospitalized Kids Local children’s book author Clyde Watson has long supported Norris Cotton Cancer Center, the Children’s Hospital at Dartmouth-Hitchcock (CHaD), and creative arts programming at Dartmouth-Hitchcock. When her grandchild faced a serious illness and received exceptional care in CHaD’s pediatric intensive care unit (PICU), she was inspired to do even more. In tribute to the incredible nursing staff whose skill and caring she had witnessed firsthand, she made a generous donation to the PICU Fund. Her gift enabled the purchase of specialized tools and equipment that make certain Mr. and Mrs. Michael M. Schnitzer medical procedures more comfortable for David Schoetz children, and provided PICU nurses with Drs. 40TH PROUTY RAISES $4 MILLION

Together, our Prouty community raised more than $4 million in the 40th Annual Prouty to support lifesaving research and meaningful patient and family support services at Dartmouth’s and Dartmouth-Hitchcock’s Norris Cotton Cancer Center. This year’s record-breaking success would not have been possible without each and every participant, donor, volunteer, and corporate partner.

The Prouty is Northern New England’s largest family-friendly fundraising event combining cycling, walking, and more to raise funds and awareness for the Cancer Center. Participants got involved June 1 through July 10 both virtually and in person to help fuel groundbreaking research and provide services that enhance the lives of people living with cancer and those who love them.

Thank you to our supporters for making a difference and providing hope to all those touched by cancer.
