Employee Benefits Guide

PLAN YEAR: January 1 – December 31, 2026

Benefit Plan Highlights

Employees who are eligible for benefits can choose to enroll in the following benefits. You must enroll within 30 days of your hire date.

Benefit Plan Highlights

Medical/Prescription

We offer the choice of two different plan options for medical and prescription drug coverage. Both plans allow you to choose between in-network and out-of-network providers, however, you will save money when you choose in-network providers.

Dental

We offer comprehensive dental coverage through two different plan offerings, one of which includes coverage for orthodontia for adults and children.

Vision

For employees that need eyeglasses and/or contact lenses, there are two vision plan options which both offer in – and – out-of-network coverage.

Life Insurance

Disability

We provide an employer-paid life insurance benefit of 1x salary to $300,000 to protect our employees’ families. Employees also have the option to purchase up to $500,000 of voluntary life insurance for themselves, or other coverage amounts for a spouse or child(ren).

For short-term disability, employees have the option to purchase this coverage through Reliance. For long-term disability, we provide employer-paid coverage of 60% to $15,000 monthly maximum.

Wellness Program

Our wellness program allows you to save money on your medical benefit contributions – up to $120 per year!

Retirement Plan

We offer a 401(k) retirement plan that allows employees to save for their retirement and receive a contribution from us as well. The Company will match 100% of the first 3% and 50% of the next 2% of your contributions.

Benefits Eligibility

Who is eligible?

• Regular, full-time employees working 30 hours or more per week are eligible to enroll in the benefits outlined in this guide.

• Eligible dependents include your legal spouse and dependent children or stepchildren up to age 26.

When do benefits begin?*

• New Hire Enrollment: 1st of the month following thirty (30) days of continued employment

• Open Enrollment: January 1st of the new plan year

• 401(k) Retirement Plan: 1st of the month following ninety (90) days of continued employment

* Voluntary Life and Voluntary Short Term Disability may require evidence of Insurability (EOI) in certain instances, and approval from Reliance will be required before coverage becomes effective.

When do benefits end?

There are several reasons for which you may not meet eligibility requirements anymore, including a change in work hours, or a change in employment status. It’s important to be aware of this.

• Medical, dental and vision plans: coverage will end on the last day of the month in which you become ineligible.

• All other benefits: coverage will end on the day you become ineligible.

You and your eligible covered dependents may be eligible to elect COBRA continuation coverage for certain benefits that are active at the time of your termination of employment or change to an ineligible status. You may also be eligible to convert your Life/AD&D coverage, if you wish to continue it on a direct basis after employment ends.

Enrolling & Making Plan Changes

Steps to Enroll

Are you ready to enroll? Follow the steps below to enroll in benefits.

1. Review your choices! Learn about your options in this benefits guide and by visiting the benefits website at prgbenefits.com. If you need help understanding your options, call the Benefits MAC team at 800-563-9929 for support!

2. Gather your personal information. To enroll in benefits, you’ll need the names, dates of birth, and personal details for your dependents and beneficiaries.

3. Log in to ADP Workforce Now to complete your enrollment. Make sure to review the confirmation page for accuracy before submitting your elections.

Qualifying Life Events (QLEs)

Remember: It’s important that you choose your benefits carefully.

Unless you experience a Qualifying Life Event, you cannot make changes to your benefits until the next Open Enrollment period. This means that your benefits elections made as a new hire and during Open Enrollment are effective through December 31, 2026, unless you experience one of the below events.

A qualifying life event can include (but is not limited to):

• Marriage/Divorce

• Birth/adoption

• Death

• Change in dependent eligibility

• Change in employment status

• Change in coverage under another employer sponsored plan, including loss of coverage

Making Plan Changes

If you need to make changes to your benefits outside of Open Enrollment, you must do so within 30 days of the qualifying life event effective date. Submit your request through ADP Workforce Now, and email prghr@prgrealestate.com with your necessary documentation within 30 days.

Independence Administrators (IA) Medical Benefits

Plan details

Choosing the right medical plan for you and your family is important. At PRG Real Estate, we offer two medical plan options. When you enroll in a medical plan, you are automatically enrolled in a prescription drug plan through Express Scripts (see the next page for more info).

Both plans allow you to choose between in-network and out-of-network care, however, it is important to remember that you will pay less when you choose to go to in-network providers and facilities. In both plans, you are not required to elect a Primary Care Physician, nor do you have to get referrals for specialist care.

Covered infertility services are subject to a $10,000 lifetime maximum for all employees enrolled in a PRG medical plan. Covered services include diagnosis & testing, artificial insemination, egg freezing, IVF, GIFT and ZIFT.

Express Scripts

With each PRG medical plan, comprehensive prescription drug coverage is included through Express Scripts. The amount you pay out-of-pocket depends on the medication and coverage tier. Certain medications may require prior authorization, step therapy, and quantity limits.

Filling Specialty Medications

Specialty medications are limited to a 30-day supply and must be obtained through Accredo Specialty Pharmacy. If you take a specialty medication, SaveOnSP will work with you and the drug manufacturer to get copay card assistance. Participation lowers your outof-pocket cost to $0 for covered specialty medications under the program.

Members enrolled in a PRG medical plan filling Specialty Rx for the first time will be asked to enroll in the SaveOn program.

Members that enroll and have eligible specialty medications will receive their medications for a $0 copay. Your medication must be included on the SaveOn Drug List to be eligible for the $0 copay.

HelpScript

HelpScript helps reduce the cost of specialty medications used to treat chronic or genetic conditions. If you’re enrolled in a PRG medical plan through Independence Administrators, eligible medications may be covered at a $0 copay.

Which medications are covered?

Specialty medications used for complex conditions like cancer, rheumatoid arthritis, or multiple sclerosis are included. These drugs may require administration by a healthcare professional and are covered under the medical benefit.

Do I need to enroll?

No enrollment is required. If you’ve been prescribed an eligible medication in the past six months, you’ll receive a letter with details about HelpScript. From there, you can connect with a HelpScript liaison to opt in.

Care when you need it most Virtual and Urgent Care Options

Teladoc Virtual Care

If you are enrolled in a PRG medical plan through Independence Administrators, you and your dependents have access to Teladoc for 24/7, 365 virtual care with U.S. board certified doctors for a variety of health needs. You can access Teladoc for telemedicine services, telebehavioral health, and teledermatology!

Teladoc provides convenient and affordable care through virtual phone or video visits, so you can skip the doctor’s office and avoid a commute.

Use Telemedicine services for non-urgent health issues, including colds, the flu, sore throat, sinus infections, pink eye, and more!

With Telebehavioral Health, speak with a licensed therapist to help you with a broad range of mental health needs, all done virtually.

Virtual dermatologists through Teledermatology can help treat a wide range of skin conditions - all from photos you upload from the privacy of your own home.

Care options and when to use them

While we recommend that you seek routine medical care from your primary care physician whenever possible, there are alternatives available. Services may vary, so it’s a good idea to visit the care provider’s website.

Wellness 2026 Wellness Program

If you participate in the wellness incentive program in 2026, you will be eligible for a $5 per pay discount beginning January 1, 2027!

Getting Started

• To participate, you must go for your annual Preventive Care Visit with your Primary Care doctor anytime between January 1 and December 31, 2026.

• Complete the Preventive Care Discount Affidavit form after your visit.

• You do not need to wait a full calendar year form your last preventive care visit.

Once your visit is complete, make sure to send the completed “Preventive Care Visit Discount Affidavit” to prghr@prgrealestate.com. You can find this form at prgbenefits.com.

Delta Dental

Dental Benefits

At PRG Real Estate, you can choose between two dental plan options through Delta Dental – both of which include out-of-network coverage, and coverage for dependent children up to age 26. Orthodontia benefits are available to adults and children in the Buy-Up plan, with a lifetime maximum of $1,500 per member.

Note:

You will not receive a physical ID card for either plan – only electronic ID cards are provided.

Dentists

With Delta Dental, you don’t need to worry about carrying an ID card when visiting the dentist—your coverage can be verified using your personal information. To make the most of your benefits, we encourage you to register at www1.deltadentalins. com/members.html. Once registered, you can easily find a dentist, access a digital ID card, review your plan details, and explore other helpful resources.

National Vision Administrators (NVA)

Vision Benefits

PRG offers two vision plans through National Vision Administrators (NVA) for you and your eligible dependents. Employees pay the full cost of vision. Both plan options provide in- and out-of-network coverage, however, you will save money when you choose in-network vision providers. Dependents are covered up to age 26.

Reliance Standard Life and AD&D Insurance

Basic Life and AD&D

Basic life and AD&D insurance is provided to all benefit eligible employees through Reliance Standard. The benefit provides one times your annual base salary with a minimum of $50,000 up to a maximum of $300,000. The AD&D benefit is equal to the basic life amount. The full cost of this benefit is covered by PRG.

Note: In order for this benefit to be tax free in the event of your death, the premium value of basic life coverage over $50,000 must be included in your taxable income.

Supplemental Life and AD&D

In addition to the coverage provided by PRG, you can purchase supplemental life and AD&D Insurance for you, your spouse, and your dependent children up to age 26. You will pay the full cost of this coverage through payroll deductions.

If you enroll during your initial benefit eligibility period as a new hire (or during special enrollment events), you are eligible to enroll in supplemental life and AD&D coverage for yourself or your spouse up to guaranteed issue amounts without answering medical questions. If you do not enroll during your initial benefit eligibility period, or choose to elect coverage above guaranteed issue amounts, you will be required to provide evidence of insurability (EOI). Note: EOI is not required for supplemental child life.

Beginning at age 65, Supplemental Life and AD&D coverage reduces.

Reliance Standard Disability Insurance

Long-Term Disability

PRG provides long-term disability (LTD) benefits to all benefit eligible employees through Reliance Standard. This coverage offers income protection in the event that you become disabled for an extended period of time.

You must submit proof of disability for approval to Reliance, and there may be pre-existing condition limitations.

Voluntary Short-Term Disability

Employees have the option to purchase short-term disability (STD) coverage through Reliance Standard. This coverage offers income protection in the event that you become disabled for an extended period of time. This benefit is 100% employee-paid.

You must submit proof of disability for approval to Reliance, and there may be pre-existing condition limitations.

* Offset by other sources of income, including but not limited to Social Security Disability and Workers Compensation.

Imputed Income Option

Our Long-Term Disability (LTD) plan includes an Imputed Income (Tax Choice) option, designed to give you flexibility in how your benefits are taxed. PRG fully covers the cost of LTD premiums.

With the Imputed Income option, the value of these premiums will be added to your taxable income each pay period. While this means you’ll pay taxes on the premium amount now, it allows you to receive your LTD benefits tax-free if you experience an approved disability.

All employees are automatically enrolled in the LTD Imputed Income option. If you prefer not to participate in this option, please reach out to Human Resources.

* If you work in a state or territory that provides statutory disability benefits (NY, NJ, CA, HI, RI, or Puerto Rico), you may be eligible for benefits through the state. The Voluntary STD benefit will be offset by any state disability payments.

Additional Benefit Programs

Identity Theft Protection

Identity Theft Protection is provided as part of the company-paid Life insurance benefits through Reliance Standard. Safeguard yourself against digital fraud with features such as dark web monitoring, lost wallet assistance, reimbursement for fraud-related losses up to $25,000, and comprehensive case management and resolution support. Travel Assistance Protection

Traveling in unfamiliar locations can often feel overwhelming and stressful. To help ease these concerns, you and your dependents have access to a comprehensive range of services designed to provide protection and support while you are away from home. Click here for the list of covered services.

LifeMart Discount Program (ADP)

LifeMart, available to all PRG employees through ADP, is one of the world’s largest discount shopping platforms, offering savings on a wide variety of products and services including wellness, family care, travel, electronics, apparel, event tickets, restaurants, and more. To access these discounts, log in to ADP, search for “Discount,” and select “Employee Discounts – LifeMart.”

Pet Insurance

Pet Insurance, offered through Nationwide, is a voluntary benefit designed to help you manage the financial aspects of your pet’s healthcare. Coverage includes veterinary care for accidents and illnesses, such as diagnostic tests, x-rays, hospital stays, surgeries, prescription medications, and more.

• Please note that pre-existing conditions are excluded; any illness or injury your pet experienced before the policy begins will not be covered.

• Premium rates are determined by your pet’s type and your state of residence, and payments are made via post-tax payroll deductions.*

• You can enroll in Pet Insurance online or by phone directly with Nationwide.

* Enrollments made during Open Enrollment will be effective on January 1. Enrollments made as a new hire between the 1st and 15th of the month will be effective the first day of the coming month. Enrollments processed between the 16th and the end of the month will be effective on the first of the next following month.

Holidays and Paid Time Off (PTO)

Paid Time Off (PTO)

Regular full-time employees are eligible to receive paid time off which is accrued monthly and determined on a calendar year basis. PTO can be used for vacation, illness, doctor’s appointments, or personal time. Accrual is determined by years of service, with increases effective the pay period following your service anniversary.

Floating Holidays

Benefits eligible employees may also be eligible to receive up to an additional two (2) floating holidays per year, based on employee tenure. One (1) floating holiday per calendar year upon hire and two (2) floating holidays after completing 3 years of service. Floating holidays may be used for religious or cultural holidays, employee birthdays, spouse or partner anniversaries, or other state or federal holidays during which PRG remains open, and more.

Wellbeing Benefits

Talkspace

Talkspace is a digital platform for behavioral health and emotional wellness that connects you and your eligible dependents with a dedicated therapist. This service is provided at no cost and is available to all employees and their dependents (age 13 and up), regardless of participation in other PRG benefits.

• Send your therapist text, voice or video messages— anytime, anywhere—including from the comfort of your home.

• You have access to 2 sessions of 45 minutes each per month at no cost to you. Additional sessions are $65 each.

• Services are confidential, and include a 24/7 secure “room” to communicate with your therapist.

• Post and reply to messages at your pace. Your dedicated therapist will engage daily, 5 days per week, often including weekends (depending on your therapist’s schedule).

Get Started: Download the Talkspace mobile app by scanning the QR code and create an account. Answer a handful of questions to help Talkspace understand what you are feeling and the type of support you need, then review recommended therapists to select one that is best for you.

HUSK Nutrition

HUSK Nutrition offers you and your dependents unlimited nutrition counseling services covered under the PRG medical plans. Schedule a complimentary, initial one-on-one consultation with one of HUSK’s Registered Dieticians and receive a customized nutrition program to support healthy eating habits, help you manage stress, and recipes to help you stay on the right track.

Employee Assistance Program

You and your eligible dependents have access to confidential, professional assessment and referral services at no cost through the Employee Assistance Program (EAP). The EAP is designed to support your overall health, wellbeing, and work-life balance, offering assistance with issues such as burnout, anxiety, depression, relationship concerns, and referrals for additional care.

Reliance takes a holistic approach to wellbeing by providing tools, resources, and access to qualified providers to help you live a healthier, happier life. With highly-trained coaches, therapists, and userfriendly digital lessons and resources, you and your dependents can receive support whenever it is needed.

Enrollment in a PRG medical plan is not required to use the EAP. You can begin accessing EAP and Work-Life Services today.

EAP and Work-Life Services are available 24/7/365, and include:

• Unlimited telephonic assessments and up to 3 faceto-face counseling sessions, per person, per year.

• Unlimited referrals for work/life needs, including: child care, elder care, pet care, education, and online resources for over 100 work/life topics.

• Unlimited legal consultations (with 25% discount for services beyond initial consultation) and an online resource center including document preparation.

The Benefits Member Advocacy Center (“Benefits MAC”), provided by Conner Strong & Buckelew, can help you and your covered family members navigate your benefits.

Contact the Benefits MAC to:

• Find answers to your benefits questions

• Search for participating network providers

• Clarify information received from a provider or your insurance company, such as a bill, claim, or explanation of benefits (EOB)

• Guide you through the enrollment process or how you can add or delete coverage for a dependent
