During the Open Enrollment period, you must login to make your 2026 benefit elections through the bSwift portal at https://FDM.bswift.com.
Elections made during this period will be effective from January 1, 2026 - December 31, 2026.
PLEASE NOTE: All benefits, aside from medical, dental, and vision REQUIRE you make elections in the bSwift portal as they DO NOT rollover to the upcoming plan year from January 1, 2026 - December 31, 2026.
If you miss the enrollment deadline, you will not be able to make changes/enroll until the next Open Enrollment period unless you experience a Qualifying Life Event during the year.

• FDM will continue to offer a choice of the following three medical plans through Anthem BCBS:
EPO with HSA High Deductible Plan (In-Network only)
EPO (In-Network only)
EPO Buy-Up (In-Network only)
• UPDATE! Due to IRS requirements, the deductible on the High Deductible Plan will increase to $1,700 for individual and $3,400 for family. All other medical benefits will remain the same.

There will be no changes to the current Cigna Dental plan for the 2026 plan year.
There will be no changes to the current EyeMed Vision plan for the 2026 plan year.
There will be no changes to the ComPysch Employee Assistance Program for the 2026 plan year.

The 2026 FSA is administered by MEDSURETY. The contribution limits are:
• Healthcare FSA: $3,400
• Dependent Care Assistance Program DCAP:
$3,750 per year if you file your tax return as married filing separately, and $7,500 for joint tax returns
• Transit Pre-Tax Accounts: $340/month
IMPORTANT NOTE: If you have a balance in your current Healthcare FSA account, we encourage you to use those funds before the end of the plan year. Any funds exceeding $660 at the end of 2025 will be forfeited. You can rollover up to $680 of unused 2026 funds into 2027. The rollover funds will not be available until after the runout period ends. Remaining funds will be reflected in your account on April 1st, once the 90-day runout period has expired.
There will be no changes to the current Prudential Life and Disability plans for the 2026 plan year.
There will be no changes to the existing Anthem voluntary plans: Critical Illness, Hospital Indemnity and Accident. Voluntary benefits are 100% employee paid.
