Passive Open Enrollment
For the 2025 plan year, Gateway will be conducting a PASSIVE Open Enrollment. If you are currently enrolled in benefits, and wish to retain your current benefit options for the 2025 plan year, you will need to go into your Paycom ESS to review, add or decline current benefits and finalize. If you wish to make any changes to current benefits, the change must be made in Paycom ESS.
If you enroll in any Guardian Benefits that require Evidence of Insurability (EOI), Prepaid Legal and Identity Theft through Countrywide ID or in the Flexible Spending Accounts, you will be required to fill out a paper form. Copies of these forms can be found on our BenePortal at www.gatewaybenefits.net
To enroll in any of the other voluntary products offered, you must access the PayCom system during the scheduled Open Enrollment period.

MEDICAL & PRESCRIPTION: AMERIHEALTH ADMINISTRATORS
There will be no changes to the medical and prescription drug benefits for the 2025 plan year.
REMINDER: Urgent Care Centers are a convenient, cost-effective alternative to Emergency Rooms. In-Network urgent care visits will be subject to a $50 copay per visit.
TELEMEDICINE: TELADOC
As part of your current Amerihealth Administrators medical insurance, you are covered for telemedicine visits. For a $20 copay, you can speak with a board certified doctor by phone, secure video, or mobile app at any time.

DENTAL: GUARDIAN
There will be no changes to the Guardian dental plans for the 2025 plan year.
REMINDER: Employees enrolled in the Guardian dental plan may be eligible to rollover unused annual maximum amounts, as part of the Maximum Rollover benefit. Members can visit www.guardiananytime.com to register and check their availability.
VISION: VBA
There will be no changes to the VBA vision plan for the 2025 plan year.
REMINDERS:
Contact lens exams continue to be fully covered under the vision exam portion of the benefit, with a 15% discount available for evaluation and fitting fees from in-network providers.
The materials allowance for contact lenses continues to be $120 for both in– and out-ofnetwork services. This benefit may be used for evaluations, fittings, and materials.
Photo Sensitive, Transition Lenses, and Tints are fully covered at VBA in-network providers only.
VOLUNTARY PET INSURANCE: NATIONWIDE
All eligible employees have the option to enroll in the Nationwide My Pet Protection plan which offers you cash back on eligible vet bills for your covered pets.
Choose from two different levels of reimbursement: 70% or 50% after satisfying a $250 deductible.

VOLUNTARY LIFE INSURANCE: GUARDIAN
SPECIAL OPPORTUNITY! There will be no changes to the Life and Disability benefits offered through Guardian; however, as a one-time exception, Guardian is offering a special opportunity to enroll in or increase your voluntary life coverage.
During this Open Enrollment period, employees and spouses are eligible to increase their current Voluntary Life elections or submit a new election up to the Guaranteed Issue amount, without having to submit Evidence of Insurability.
Guaranteed Issue:
Employee: $200,000
Spouse: $10,000
VOLUNTARY GROUP ACCIDENT: GUARDIAN
All eligible employees have the option to enroll in Guardian’s Voluntary Group Accident Plan. This plan pays a lump sum benefit based on the type of injury (or covered accident) you sustain, or the type of treatment you need as a result of an accident. Benefits are paid on an indemnity basis per a pre-determined schedule, regardless of other coverage, including medical. This benefit includes coverage for on and off the job accidents, as well as accidental death.

HEALTHCARE & DEPENDENT CARE FSA: WEX
The Healthcare FSA is used to reimburse out-of-pocket medical expenses incurred by you and your eligible dependents. The maximum you can contribute to the Healthcare FSA in 2025 is $3,300.
The Dependent Care FSA is used to reimburse expenses related to the care of eligible dependents. The maximum that you can contribute to the Dependent Care FSA is $5,000 if you are a single employee or married filing jointly. If you are a married employee filing separately the maximum you can contribute is $2,500.

COMMUTER BENEFITS: WEX
Employees can enroll in a spending account specific to work related transit expenses for the 2025 plan year.
Employees are eligible to make a monthly pre-tax deduction of up to $325 into their commuter spending account.
∗ Once you make your election, you will receive a debit card if you are newly enrolling that can only be used to pay for work related transit expenses. Your debit card is loaded with your pre-tax deductions each time a deduction is taken from your paycheck. Each time you use your debit card to pay for transit purchases, the funds are automatically debited from your transit account.
Eligible work related expenses include vouchers, passes, tokens for buses, trains, rail, subways, ferries and vanpooling costs.
