Procedure for Ordering Flowers for CPUT Staff Members

PROCEDURE FOR ORDERING FLOWERS AND HAMPERS/FRUIT BASKETS FOR CPUT STAFF MEMBERS Flowers and Hampers/Fruit Baskets can be ordered for CPUT staff from MCD on behalf of the Vice Chancellor in the event of: • Births • Hospitalization - Serious illness, Operations/Procedures, Accidents • Death of immediate family – ONLY spouse, child, parents, siblings, mother- & father-in law.

Important notes: • Special Requests – only from Executive Management in a case of staff member or an immediate family member being seriously ill.

Process In the case of any of the abovementioned events, the following will apply: • Inform MCD via email to bassonk@cput.ac.za cc kochs@cput.ac.za with the following information: Hospitalization: Staff member name, surname and cell number/department Name of hospital and Ward. PS: If in ICU, no flowers will be accepted until the staff member is moved to a ward.

Home: Staff member name and surname/department, street address, cell numbers.

• Allow at least 48 hours for delivery - deliveries in the Western Cape Province only.
