People may remain anonymous when logging on to Zoom by creating any name and email of their choosing. In a meeting, public comment will be received, either or both, orally (raise hand in Zoom or use *9 on phone) and by writing. Note: Written comments may be sent to the Board at any time. For oral presentations, the Board may set a time for comments and speakers.

All matters listed with the Consent Agenda were previously distributed to each Board Member for reading and study, were available for public viewing, are considered items of regular County business, and will be approved, without separate discussion, by one motion of the Board of County Commissioners. An item may be removed from the Consent Agenda and placed on the Regular Agenda for separate discussion and voting at the request of any Board Member.

PROCLAMATION BEFORE THE BOARD OF COMMISSIONERS OF COWLITZ COUNTY, WA

In the Matter of declaring September) 17-23, 2025 as Constitution Week )

Resolution:______________

WHEREAS, September 17, 2025, marks the two hundred and thirty-eighth anniversary of drafting the Constitution of the United States of America by the Constitutional Convention; and

WHEREAS, It is fitting and proper to accord official recognition to this magnificent document and its memorably anniversary; and to the patriotic celebrations which will commemorate the occasion; and

WHEREAS, Public Law 915 guarantees the issuing of a proclamation each year by the President of the United States of America designating September 17 through 23 as Constitution Week.

THEREFORE, the Cowlitz County Board of Commissioners hereby proclaim the week of September 17 –23, 2025 as Constitution Week and urge all citizens to study the Constitution and reflect on the privilege of being an American with all the rights and responsibilities which that privilege involves.

FOURTH AMENDMENT TO THE PERSONAL SERVICES AGREEMENT

This Fourth Amendment (“Amendment”) is made and entered into as of this 5th day of September, 2025, by and between Cowlitz County, a political subdivision of the State of Washington, (“County”), and Everhealth, LLC (“Contractor”)

WHEREAS, County and NaphCare, Inc. (“NaphCare”) entered into a Personal Services Agreement, Contract Number 03012020 on February 25, 2020 (“Agreement”), wherein NaphCare provided inmate health care services at the Cowlitz County Corrections Department facilities;

WHEREAS, County and NaphCare entered into an amendment on December 1, 2020, which amended staffing, and compensation;

WHEREAS, County and NaphCare entered into an amendment on March 21, 2022, related to the 2022 MOUD Grant;

WHEREAS, County and NaphCare entered into an amendment on January 1, 2024, which assigned the Agreement to Contractor and amended staffing and compensation;

WHEREAS, pursuant to Article 8 “Modifications” of Attachment C to the Agreement, the parties wish to amend the Agreement to modify the term of the Agreement and compensation payable to Contractor;

NOW THEREFORE, County and Contractor agree to amend the Agreement as follows:

I. The parties hereby agree that the Agreement will terminate on October 30, 2025

II. The parties hereby agree to amend the current compensation payable to Contractor as follows effective October 1, 2025:

*Revised Year 6 includes an increase of 17% to Jail Medical

IN WITNESS WHEREOF, the parties have executed this Amendment in their official capacities with legal authorityto do so.
