EXECUTIVE DIRECTOR’S

REPORT It seems like we blinked and 2016 was gone.

So much has happened over the past 12 months – in the diabetes industry, the active diabetes community, and with Connected in Motion.

In August, CIM welcomed new Team Member, Heather Gomez, on board as our Community Outreach Coordinator. We were so excited to have her energy, enthusiasm, and experience in helping to bring more programs to more people throughout North America. Last summer, we brought a team of 12 together to take on the challenging and formidable Long Range Traverse in Gros Morne, Newfoundland. The team raised almost $25,000 to support CIM’s programming. We also ran our first Teen Canoe Trip.

We went on the road with the diabetes education game, Diabetium. And in between all of that, we climbed, paddled, hiked, ran, portaged, swam, and skied our way through an amazing year.

The year was themed with big accomplishments (including several people’s first running race as part of Team CIM), beautiful scenery (if you haven’t had the chance, check out the video from our Gros Morne Trek), and endless generosity. For that generosity, be it from the diabetes community as they helped us spread the word about all we do, from the partners who supported our dreams and adventures, or from the participants who shared their time with us, we are so grateful. You are the reason that we are able to continue to redefine life with Type 1 diabetes.

REPORT With unwavering dedication Connected in Motion builds active communities focused on experiential, peer based Type 1 diabetes education. With the support from our corporate partners and individual philanthropic donors, to whom I express our sincere gratitude, it is extraordinary how far this vision has been able to extend across Canada and into the United States.

We are incredibly proud of the support our team and ambassadors have provided to teenagers and adults living with Type 1 diabetes and their families over the past eight years. I also record the appreciation of our Board to our Executive Director, Jen Hanson, and her dynamic team – all of whom work tirelessly in the pursuit of our building a more knowledgeable Type 1 diabetes community.

MISSION

Connected in Motion

There are over 1.5 people throughout North America living with Type 1 diabetes. Of that, 85% are adults. CIM is dedicated to providing programming and support for this demographic. Without the Connected in Motion community, many people would be left to face Type 1 diabetes alone.

To foster a community of people living with Type 1 diabetes through peer-based experiential diabetes education, physical activity and outdoor adventure pursuits so that they may inspire one another to live life without limits.

VISION

A community of healthy, active, and educated adults living with Type 1 diabetes who are suppor ted and engaged in diabetes self-management.

We want to connect the community with others they can relate to. People who live with diabetes every single hour of every single day. And we want to get people outside, moving, and on adventures.

We aim to inspire the community to foster relationships and healthy lifestyles. Since the CIM Team lives with Tyoe 1 diabetes too, they know that when someone is down in the diabetes dumps, a little inspiration can get you back on track.

We want to motivate individuals to take full responsibility of their personal health and well-being. In our opinion, to succeed means grabbing Type 1 diabetes by the horns. We can help people set some goals right off the bat.

Connecting allows individuals to share their experiences, challenges, techniques, strategies, and successes with each other.

We think that people living with diabetes can be the best resource for people living with diabetes.

We want to help the community find that something. Whether it’s an introduction to someone with an incredible story, a connection with someone who lives a similar lifestyle, or a chance to push limits and comfort zones, we’ll find a way.

New ideas? New tools? New technology? Inspiration? Or just a kick in the pants?

Whatever the definition of success, we’re here to help out in every way we can. You’ll hear us cheering the community on, every step of the way.

ABOUT

Experiential Education “In its simplest form, experiential learning means learning from experience or learning by doing. Experiential education first immerses learners in an experience and then encourages reflection about the experience to develop new skills, new attitudes, or new ways of thinking.” Lewis & Williams, 1994 Connected in Motion provides opportunities for individuals to put into practice the skills learned in a traditional healthcare setting and to learn from the experience of ‘doing’.

We build and support communities of healthy, active, and educated adults engaged in diabetes self-management by connecting lessons learned about Type 1 with real life experiences.

About 85% of people living with T1D are adults, but there are few services available for this population.

In 2016, CIM hired a Community Outreach Coordinator to manage clinic communication and day programming across the country. This added team member has helped CIM expand our reach across Canada and help grow our participant base.

CIM worked with other non-profit organizations, including Riding on Insulin, Beyond Type 1 and Children with Diabetes – all other non-profit organization with similiar missions.

Together we continue to seek to build a network of like-minded community members in the Type 1 diabetes space.

RAISED OVER $60,000 IN FUNDING

We worked tirelessly to expand our individual donor base, bringing in over $36,000 from 420 generous donors. CIM also succeeded in building an Adventure Team who raised $25,000 for programming while empowering team members as advocates in their own communities.

STRIVING TOWARDS EXCELLENCE

Throughout 2016, CIM strove to continue to build our reputation within the outdoor adventure industry, working with mentors across the diabetes, experiential education, and adventure therapy industries to ensure CIM is employing best practices and exceeding industry standards.

PLATNIUM CLUB PILOT

In 2017, CIM will be expanding our teen programming throughout Ontario to better serve youth as they are transitioning into young adulthood. We will also be extending our Slipstream weekends and day programming throughout Atlantic Canada, further expanding our particpant base.

CIM aims to pilot Type 1 seniors program in Toronto with goals of expanding our portfolio to serve an aging population.

UTILIZING DATA

DIVERSIFY REVENUE

Harnessing the power of CIM’s framework and CRM systems, we aim to make data-informed and -driven decisions about our programming and expansions.

CIM will work to diversify revenue by developing relationships with new corporate partners, building foundation support, and increasing individual donor contributions and major gifts.

THE CIM TEAM EXECUTIVE DIRECTOR

JEN HANSON

Jen has lived with Type 1 diabetes since the age of three. She is a Registered Kinesiologist, a member of the Ontario College of Teachers and completed her Master of Education degree at Brock University, with research focused on experiential diabetes education. She grew up camping and adventuring with her family, something that remains her passion today. Jen spent 18 summers at a camp for children with Type 1 diabetes and attributes much of her character and success to those experiences.

Today, Jen seeks out opportunities to engage in many aspects of the Type 1 diabetes community. She is the Elementary Program Coordinator with Children with Diabetes, a Global Ambassador for Beyond Type 1, and a Faculty Member with the Young Leaders in Diabetes program through the International Diabetes Federation.

JULIE DE VOS

PROGRAM DIRECTOR

A Certified Health Coach and Registered Kinesiologist, Julie oversees the educational components of CIM’s Slipstream Programs. Julie draws on her own experiences growing up with Type 1 diabetes to develop and facilitate many of CIM’s experiential education workshop. Julie also works as the International Program Director of Riding on Insulin, one of CIM’s non-profit partners, helping to build synergy within the Type 1 diabetes community.

COMMUNITY ENGAGEMENT COORDINATOR

HEATHER GOMEZ

Heather joined the CIM team in 2016. Using her skills in project management and event coordination, Heather works to connect and engage the community across Canada and the United States through day programming and engagement intitiatives. Heather has worked as a personal trainer and has a passion for healthy, active living. Heather grew up with Type 1 diabetes in Halifax, Nova Scotia.

ADVENTURE PROGRAM COORDINATOR

PETER VOOYS

Peter is currently pursuing his Master of Education degree at Queens University and is a member of the Ontario College of Teachers. He took his first canoe trip when he was six years old and hasn’t stopped since, to date logging 8000km of paddling and hiking, including a 120 day cross-Canada paddling expedition. Peter is sharing his love for the wilderness with CIM by coordinating and guiding CIM’s Adventure Programming.

HANK DEVOS

FILM AND CREATIVE

Hank has been a part of Connected in Motion since its inception in 2008, helping to build CIM’s story through his short films and media. He has travelled with CIM throughout Canada and the United States, exploring unique and remote places, collecting the communities stories and helping to show the world how to live without limits with T1D. Hank grew up with Type 1 and can be found with his skis in the mountains through the winter months.

MIKE LAST

Mike has been traveling and enjoying the outdoors his whole life. After finishing a 4 year Managerial and Organizational Studies degree at the University of Western Ontario, Mike moved to Toronto where he is currently working in marketing and photography. Mike helps to share CIM’s stories through web and photography. He has been living with Type 1 since his teens.

CHRIS CARMICHAEL

Chris Carmichael is a Certified General Accountant and the president of CRIS Inc., a company that provides accounting, financial reporting, and advisory services to Canadian publicly listed companies. Chris Carmichael is a father of two children and has Type 1 diabetes.

IAN BULMER

DIRECTOR

Ian Bulmer joined the CIM Board in 2017 after having an exhilarating team experience on the Gros Morne Adventure Challenge in 2016. Ian is a Toronto lawyer with a close family connection to Type 1 diabetes.

DON CAMERON

Don Cameron is a retired educator having worked over thirty years for the Toronto District School Board as a Secondary Teacher, Secondary Vice Principal, Adult Education Vice Principal, Secondary Principal, and Adult Education Principal. Don is married with three children (one of whom has Type 1 diabetes) and one grandchild. He currently lives in Ottawa.

LUKE CAMERON

DIRECTOR

Luke joined the CIM board in 2016. After attending Winter Slipstream 2012, Luke has consistently been involved with CIM events, either participating or volunteering. Having Type 1 diabetes since 1990, Luke wants to help CIM reach other Type 1s that haven’t had the opportunity to experience CIM. Luke has worked in the water/wastewater field since 2004.

JOANNA NICHOLSON

Joanna joined the CIM board in 2014. Having lived with Type 1 diabetes since 1998, an active, healthy lifestyle has always been an important part of her life and she does whatever she can to educate those around her. In her free time, Joanna loves cooking, cycling, triathlons, running, dancing, yoga and volunteering. Joanna spends her days as a Communications Manager at OMERS.

ABOUT CIM’S FOUNDER

A catalyst and creator, Chloe’s passion lies in building resilient communities and guiding people of all ages through the experiential learning process. Be it working to connect and engage young adults with Type 1 diabetes through Connected in Motion, inspiring a local food movement through the creation of school gardens with Alpine Edible Schoolyards or supporting First Nation’s students on their educational journeys with the Canadian Rockies Public School Board, Chloe thrives on mobilizing people to tackle big global problems by making small personal changes.

Adventure-seeker and life-long learner, she has circumnavigated the South Island of New Zealand by bicycle, traveled the Yukon by dog sled, and guided expeditions on numerous Arctic and subArctic rivers.

Chloe lives in Canmore, Alberta with her husband Rob and their German Shepherd Chile. When she’s not playing in the mountains, experimenting in the kitchen or teaching her dog new tricks, she is the Aboriginal Success Coach for Canmore Collegiate High School.

Tammy Hand

Connected in Motion is driven by its community. Each and every one of our community members inspires us to grow–to push harder to redefine life with Type 1 diabetes and to breathe fresh air into diabetes education.

Awarded for the first time in 2012, the CIM Inspiration Award recognizes the individual whom has inspired us, and others living with type 1 diabetes, to live life without limits.

After a brief hiatus in 2015 (we weren’t able to host Slipstream in the City, where the annual award is giving out!), we had a pool of people TWICE as big as ever before to choose from. And still, the choice was unanimous and easy. This year’s Inspiration Award Winner has been an unstoppable force in the CIM community. Congratulations to the 2016 CIM Inspiration Award winner, Tammy Hand!

Tammy has been involved with Connected in Motion since we can remember. She is always the first to jump to her feet and volunteer for whatever it is that the community needs. Tammy shared her amazing 40 years of T1D experience this summer on CIM’s first Teen Canoe Trip. She was the first to step forward and join CIM in 2014 in achieving Wilderness First Responder certifications. As an outdoor educator, she is constantly looking for ways to bring her expertise to the CIM community, whether it be teaching cross country skiing or welcoming folks for a tour of the sugar bush (hello, lifetime of low supplies!) Tammy also hit the trails as part of CIM’s first ever Adventure Team, tackling Gros Morne’s Long Range Traverse with a pack that rivaled her in physical size (and helping the team to raise $25,000 along the way!) Next time you see Tammy, be sure to give her an extra high five.
