Welcome On behalf of the Medicines Management 2011 Organising Committee, welcome to Hobart!

It is an honour to welcome you to SHPA’s 37th National Conference, especially as this year marks the seventieth anniversary of SHPA and fifty years since becoming a national organisation.

The conference theme is ‘Make Your Move’. We chose this theme as it represents a call to action for all delegates wishing to further their career, patient care, the profession and our role in the wider health system.

We have endeavoured to create a conference program that allows Australian pharmacists and pharmacy technicians access to a fantastic array of luminaries in the field of healthcare from around the world.

We are delighted to bring you plenary sessions from Professor Miles Shore, Professor of Psychiatry and Director of Mental Health Systems at Harvard Medical School. Professor Shore will also be joined by his wife Dr Eleanor Shore, Consultant to Harvard Medical School and founder of the Harvard Fiftieth Anniversary Fellowship Program for Scholars in Medicine.

In addition we have a video presentation by Dr David Bates developed exclusively for the conference. Dr Bates holds a number of positions at Harvard and at Brigham and Women’s Hospital, including Head of General Internal Medicine, Professor of Medicine, Professor of Health Policy and Management, and Executive Director of the Center for Patient Safety Research and Practice. Many delegates will recognise Dr Bates as the author of hundreds of publications relating to e-health and e-prescribing.

We are also honoured to have Professor Cynthia Raehl from Texas join us to share the outcomes of her follow up research on hospital pharmacy and the impact on hospital mortality rates.

cornerstone of the major SHPA study of the importance of clinical pharmacists in Australian hospitals. Professor Raehl will provide us with an update on her internationally-renowned study.

And finally Professor Imti Choonara, one of the world’s leading paediatric clinical pharmacologists, will close the conference with a challenging discussion about paediatric care in hospitals.

In addition to our plenary speakers we have a diverse and exciting range of invited speakers from across the country, covering topics from pharmacists in outpatient clinics, through to some fascinating new developments in virotherapy in the ‘wildcard’ stream of the conference.

We would like to also draw your attention to the contributed papers and posters submitted by delegates. This year a record number of abstracts were submitted and as a result the quality of presented work is exceptionally high. We believe that all delegates will find inspiration from the ideas and initiatives presented by our colleagues, and will take these experiences back to their workplace to further improve patient care.

To those presenting a paper or poster, we thank you for your contribution to this event, and we strongly encourage you to have your work recognised further by submitting it to the Journal of Pharmacy Practice and Research for publication.

Introduction to SHPA This year, 2011, marks fifty years of The Society of Hospital Pharmacists of Australia (SHPA) as a national organisation. SHPA was established in 1941 following the pioneering efforts of 25 public hospital pharmacists in Victoria.

From 1947 to 1964 other branches were developed. The inaugural meeting of the SHPA Federal Council and the first SHPA national conference were held in Adelaide in 1961.

SHPA is the professional body which represents around 3000 pharmacists, pharmacy technicians and associates practising in all parts of the Australian health system.

SHPA has a long-standing commitment to the provision of pharmacy services in hospitals and to the profession’s role in ensuring optimal health outcomes for Australian consumers by the safe and effective use of medicines.

Safe and effective medication use is the core business of pharmacists, especially in hospitals.

SHPA is the only professional pharmacy organisation with an especially strong base of members practising in hospitals and other facilities. SHPA aims to improve health outcomes by supporting its members’ roles in optimising medicines use.

What were once “hospital services” are now being delivered in a range of other settings and the changing nature of healthcare delivery is now also reflected in the diversity of SHPA membership.

Whilst maintaining a strong public hospital base, SHPA draws members from a diverse range of pharmacy and health practice settings including public and private hospitals, community pharmacy, academia, research, industry, government, consultant pharmacy and a range of quality use of medicines projects, clinical governance and medicines management programs.

Fundamental to the success of SHPA is the culture of cooperation and contribution that is reflected in the high level of membership involvement. A core value held by SHPA is the recognition of the commitment of the many volunteers throughout our membership whose efforts are noteworthy in so many national and branch level SHPA activities and member services.

SHPA CPD shpacpd activities are designed to keep members current, informed and connected. A range of educational services are provided to support a wide range of pharmacist CPD plans.

Delegates at Medicines Management 2011, the 37th SHPA National Conference, will find many opportunities for unplanned learning, as well as topics that they may have identified already as being relevant to their own CPD plan. Catering and Dietary Requirements Lunches, morning and afternoon teas on Friday and Saturday will be held in the Federation Ballroom. Lunches will be served as an informal stand-up buffet. Dietary requirements noted on your registration form have been passed on to the catering staff.

There will be a table dedicated for special dietary requirements which will include vegetarian options. Please ask the catering staff to assist if needed. At the Gala Dinner, vegetarian meals will only be available for those who have previously advised of this requirement.

Professor David Bates is an internationally renowned expert in using information technology to improve clinical decision-making, patient safety, quality-of-care, cost-effectiveness, and outcomes assessment in medical practice. A practicing general internist, he is chief of the Division of General Internal Medicine at Brigham and Women’s Hospital in Boston, a professor of medicine at Harvard Medical School, and a professor of health policy and management at the Harvard School of Public Health, where he co-directs the Program in Clinical Effectiveness. He also serves as medical director of clinical and quality analysis for Partner’s Healthcare Systems.

At a time when patient safety has become a key driver for focusing national attention on health care quality, his work has not only shown the magnitude of the problem but also provided a blueprint for helping solve it. He led a seminal study on the epidemiology of drug-related injuries, demonstrating that the most effective way to prevent serious medication errors is to focus on improving the systems. He has also performed many studies on how computerised, evidence-based guidelines can improve quality and efficiency. He has been recognised for several years by “Modern Healthcare” magazine as one of the “100 most powerful” individuals in U.S. health care. He is a graduate of Stanford University, and the Johns Hopkins School of Medicine.

He began his fellowship in general internal medicine at Brigham and Women’s Hospital in 1988, and he received a M.Sc. in Health Policy and Management from the Harvard School of Public Health in 1990. He has been elected to the Institute of Medicine, the American Society for Clinical Investigation, the Association of American Physicians and the American College of Medical Informatics, and was chairman of the board of the American Medical Informatics Association. He serves as external program lead for research in the World Health Organization’s Global Alliance for Patient Safety. He is the editor of the “Journal of Clinical Outcomes Management,” and the associate editor of the “Journal of Patient Safety.”

His special research interests include clinical decisionmaking and affecting physician decision-making, particularly using computerised interventions; quality of care and costeffectiveness and medical practice; and outcomes assessment.

He has published on medication errors and injuries due to drugs, and the use of information systems to improve medication safety and the use of ancillary tests, as well as on predictors of bacteremia and evaluation of patients with suspected sepsis. He has written over 400 peer-reviewed publications.

Professor Imti Choonara is one of the world’s leading paediatric clinical pharmacologists. He has been personally involved in ensuring that paediatric clinical pharmacology is an accredited subspecialty of paediatrics in the UK. He is currently SecretaryGeneral of the European Society for Developmental, Perinatal and Paediatric Pharmacology which is the leading international organisation of paediatric clinical pharmacologists. He was the Foundation Editor of Paediatric and Perinatal Drug Therapy which is now incorporated within the leading paediatric journal Archives of Disease in Childhood as a regular Drug Therapy section.

His research interests have been in drug toxicity, clinical trials and epidemiology of the use of medicines as well as in the management of pain and sedation in children. He is currently the Chair of the NIHR HTA Pharmaceutical Panel which is responsible for commissioning research of clinical trials of medicines in adults and children in the UK.

Professor Cynthia Raehl Chair and Professor, Department of Pharmacy Practice, Texas Tech University Sciences Centre at Abilene, USA

Professor Cynthia Raehl is a founding faculty member and department chair for Texas Tech University Health Sciences Center School of Pharmacy located in four Texas cities. She is one of a select few who have been elected president of two national organisations. In 1994-95 she served as President for the American Society of Health-System Pharmacists (ASHP), an organisation that represents more than 30,000 pharmacists practicing in integrated health systems. In 2009, she completed a three-year term as President of the American Association of Colleges of Pharmacy (AACP), the organisation that represents the 120 US pharmacy schools and colleges, including their faculty, staff and students.

During her career she has procured more than $2.7 million in grants and contracts, garnered 7 national research awards and received several national practice awards and teaching awards. In 2011, she received the TTUHSC President’s Outstanding Professor Award. She currently practices as a consultant pharmacist for a faith-based teaching nursing home. She acknowledges her life partner, the late Dr. C. A. Bond, a TTUHSC University Distinguished Professor who passed away in June 2009.

Dr Eleanor Shore, now retired, was a primary care physician at Harvard University Health Services and Dean for Faculty Affairs at Harvard Medical School where she had responsibility for the academic activities of a faculty of 9,000. Issues of leadership, career advancement, governance, and scientific integrity were of particular concern. She is Consultant to the Office for Academic and Clinical Programs at Harvard Medical School, HMS Dean for Faculty Affairs (Retired), and Primary Care Physician at Harvard University Health Services (Retired).

She received her Bachelor’s Degree from Radcliffe College, her M.D. from Harvard Medical School, and her MPH from the Harvard School of Public Health. She is Board Certified in Preventive Medicine.

For two years she was a primary care physician at Brooke Army Hospital in San Antonio, Texas before returning to Boston where for 35 years she was a primary care physician at Harvard University Health Services in Cambridge, Massachusetts. During that time, she chaired the Communicable Disease Prevention Committee and organised a course for travellers going to work in remote areas where no medical care was available. She also served on the University’s Benefits Committee, Health Benefits Subcommittee, and Retirement Subcommittee.

For nine years she was Harvard University President’s Assistant for Health Affairs in parallel with her primary care responsibilities.

Later, for 15 years, she was Dean for Faculty Affairs at Harvard Medical School, responsible for overseeing faculty governance, the appointments process, and compliance with federal guidelines concerning scientific misconduct and conflict of interest.

Concurrently she was Deputy Director and Chair of the Academic Leadership Committee of the Harvard Medical School Center of Excellence in Women’s Health funded by the U.S. Department of Health and Human Services.

In the above positions, she organised meetings for women regarding academic leadership in China, Australia, Spain, and Dubai. In 1995 she also initiated a fellowship program to assist junior faculty at the point in their careers when in addition to academic and clinical practice demands, they may be faced with increased family or other responsibilities. In the past 15 years it has funded more than 700 fellowships, with another 91 to be awarded this coming academic year. Support comes from the Medical and Dental Schools, hospitals, departments, divisions, and friends. She is currently a member of the Academic Council of the Real Colegio Complutense (a joint venture between Harvard University and Complutense University in Madrid to foster scholarly and scientific exchanges), the Board of Trustees at the Schepens Eye Research Institute, and the Archives for Women in Medicine Committee (Chair) at the Countway Medical Library at Harvard Medical School.

Professor Miles F. Shore, M.D. is Bullard Professor of Psychiatry, Emeritus, Harvard Medical School, and an affiliate of the Center for Public Leadership at the Kennedy School of Government. A graduate of Harvard College and Harvard Medical School, he trained in psychiatry at the Massachusetts Mental Health Center and Beth Israel Hospital in Boston.

From 1965-1975 he was on the faculty of Tufts University School of Medicine in Boston where he directed the Tufts Community Mental Health Center, was Associate Director for Ambulatory and Community Programs at New England Medical Center Hospitals and Associate Dean for Community Affairs at Tufts Medical School. In 1975 he was appointed Bullard Professor of Psychiatry at Harvard and Superintendent of the Massachusetts Mental Health Center, a major Harvard teaching and research program in psychiatry. In 1983 he served as interim Commissioner of Mental Health in the Massachusetts state government. From 1985-91 he was Director of the National Program for Mental Illness for the Robert Wood Johnson Foundation. In 1996-97 he was President of the American College of Psychiatrists. He has been a consultant and invited speaker on leadership, change management, and health policy in China, India, Spain, Portugal, Denmark, Iceland, Canada, and Australia.

In 1993 he was appointed Visiting Scholar at the Kennedy School of Government where he directed, with Joseph P. Newhouse, Ph.D., an executive program on health policy that covered health economics; health care quality; the political system and the media related to health care; international health care systems; and information technology. He developed and taught a course on leadership for Kennedy School graduate students. From 2005 to 2007 he co-directed the summer European Health Policy Seminar of the Real Colegio, Complutense University in Madrid.

Dr Manya Angley graduated as a pharmacist in 1989 and was awarded a PhD in Health Sciences in 1996. She held academic appointments (teaching and research) in the School of Pharmacy and Medical Sciences, University of South Australia for 20 years from 1990 to 2010. She has also worked extensively as a hospital and as a community pharmacist. In 2004 she established the Sansom Institute-based Autism Research Group (ARG) which is a multidisciplinary team of researchers involved in a range of activities aiming to better understand autism and help develop more effective diagnostic techniques and treatments. Her other research interests include supporting the patient along the continuum of care with respect to medication management. Early in 2010 she established Manya Angley Research and Consulting (MARAC) which is an independent organisation that provides expertise in the design, implementation and evaluation of health and academic programs and services.

Roseline Antoni Ward Technician, Repatriation Campus, Austin Health, VIC Rosaline Antoni trained as a pharmacy technician in Malaysia, in a comprehensive three year full time program. The course was taught through lectures and skill lab experience. During the three year course she also undertook placements in metropolitan and rural settings.

Since arriving in Australia 23 years ago she has worked for Austin Health in Victoria. For the past eight years she has worked as a ward based technician at the repatriation centre, and as such she has extensive experience in ward based technician roles in the aged care setting.

Dr Gough Au Research Fellow, School of Biomedical Sciences and Pharmacy, University of New South Wales, NSW Dr Gough Au is a conjoint post-doctoral research fellow with the University of Newcastle and industry partner Viralytics Ltd. He completed his NHMRC (National Health and Medical Research Council) Industry Research fellowship this year in April (2007-2011). During this four-year period, his main research interest was in the development of naturally occurring viruses with selective anti-cancer properties, for the treatment of melanoma, multiple myeloma and malignant glioma.

He began working in the field of viral oncolysis in the year 2000.

He completed his PhD in 2005, during which he contributed to the development of treatment for melanoma and multiple myeloma.

Currently he works with a team of scientists who are developing new viruses for the treatment of a wide range of cancers including breast, prostate, ovarian and colon cancer.

Professor Thomas Borody is a graduate of the University of New South Wales from which he holds a doctorate in medicine. He is the founder and current Medical Director of the Centre for Digestive Diseases. He has been a recipient of the Winthrop Travelling Fellowship, the Neil Hamilton Fairly Fellowship and the Marshall & Warren Prize, and was a Clinical Fellow in Gastroenterology at the Mayo Clinic in Rochester. He is a member of the Australian Medical Association, the Gastroenterological Society of Australia, the European Gastroenterology Society, the Functional Brain-Gut Research Group and Fellow of the American College of Gastroenterology and the American College of Physicians.

He supervises a number of major research programs as well as being a reviewer for a number of scientific journals. He has published in excess of 120 scientific papers. In 2004 he was appointed an Adjunct Professor of the Faculty of Science at the University of Technology, Sydney. In July 2005, he was awarded his PhD from the University of Newcastle and MD by thesis from the UNSW. He has successfully negotiated six international licensing deals for commercialisation of a number of inventions in his patent portfolio including a cure for duodenal ulcers (Helidac and Pylera), Moviprep (bowel prep), treatment for Crohn’s Disease and treatments for parasites Dientamoeba fragilis and Blastocystis hominis.

Professor Simon Brown Centre of Clinical Research in Emergency Medicine, Western Australian Institute for Medical Research, Royal Perth Hospital and the University of Western Australia, WA Professor Simon Brown is an academic emergency physician based at Royal Perth Hospital, WA. His current research areas are immunological mechanisms and management of serious illness and injury and the conduct of clinical trials in the ED setting.

Between 1997-2003 as a staff specialist at the Royal Hobart Hospital, he and Michael Wiese (then a clinical pharmacist) developed and tested a new immunotherapy to prevent jack jumper ant venom anaphylaxis. This launched them both into PhDs and research careers that have continued to focus on allergic and immunological disease.

Dr Daniel Christ completed undergraduate studies at the Swiss Federal Institute of Technology (ETH) and then joined Sir Gregory Winter’s laboratory at Cambridge University for his PhD. Winter had shown that in-vitro evolution technology (such as phage display) can be used for the generation of therapeutic antibodies, initiating a multi-billion dollar biotechnology industry. At Cambridge, Christ and Winter further developed monoclonal antibody technology, particularly in the field of domain antibodies. He was elected a Fellow of Trinity College (Cambridge University) at age 29. He maintains close links with industry and was involved the foundation of Domantis Ltd, for which he worked as a consultant until its sale to GSK in 2006. He joined the Garvan in 2007 as Director of the Institute’s translational Antibody Development Program.

Dr Ian Coombes is the Director of Pharmacy Royal Brisbane and Women’s Hospital and a clinical lecturer at the University of Queensland. He completed his basic training in the NHS, and undertook his Masters at the University of Brighton followed by a PhD in Queensland. He has a major interest in research and implementation of practitioner development and medication safety. He is a member of the Federal Council of the Society of Hospital Pharmacists of Australia, the Australian Pharmacy Council and in 2010 was awarded the SHPA Medal of Merit for contribution to hospital pharmacy and medication safety.

Rohan Elliott Clinical Senior Lecturer, Monash University; Clinical Pharmacist, Heidelberg Repatriation Hospital, VIC Rohan Elliott is a senior pharmacist at Austin Health and clinical senior lecturer at Monash University. He has practiced in various aged care settings over many years, and has led several major research projects addressing medication use and safety in older people. He has published more than 20 peer reviewed papers and a book chapter on geriatric pharmacy practice, and is an editor of the Geriatric Therapeutics Section of the Journal of Pharmacy Practice & Research. He has developed and coordinates two postgraduate units in geriatric pharmacy practice at Monash University. He is a Certified Geriatric Pharmacist and a Fellow of the Society of Hospital Pharmacists of Australia and the American Society of Consultant Pharmacists.

Helen Dixon Pharmacist in Charge, Manufacturing Services, Melbourne Health, Royal Melbourne Hospital, VIC Helen Dixon is the pharmacist in charge of manufacturing services at Melbourne Health.

After graduating with first class honours she worked as a clinical pharmacist for five years before finding an interest in oncology and haematology. She has spent five years working in oncology and haematology as a clinical and manufacturing pharmacist. She is a member of the International Society of Oncology Pharmacy Practitioners (ISOPP) standards committee that is currently reviewing the ISOPP Standards of Practice for the Safe Handling of Cytotoxics. She is the Lead of the review of the SHPA Standards of Practice for the Safe Handling of Cytotoxic Drugs in Pharmacy Departments.

Professor Simon Foote Director, Menzies Research Institute, TAS Professor Simon Foote is Director of the Menzies Research Institute, University of Tasmania, and his prime research interests are in the areas of infectious disease, specifically malaria, and the genetics of numerous diseases including multiple sclerosis and cancer. Professor Foote has been a Howard Hughes International Scholar since 2000.

He obtained his medical degree in 1984 at the University of Melbourne, Australia and in 1989 completed his PhD in Molecular Genetics studying the genetic basis of drug resistance of the malarial parasite. He worked at the Genome Center at the Whitehead Institute, Massachusetts Institute of Technology where he produced the first physical map of a human chromosome and then a map of the entire human genome.

He returned to Australia where he headed the Genetics and Bioinformatics Division at the Walter & Eliza Hall Institute of Medical Research, Melbourne, Australia.

His work in murine genetics has identified several loci contributing to the host’s response to malaria and leishmaniasis. This has resulted in tissue repair being recognised as a major host response mechanism to leishmaniasis. His work in human

genetics has led to the identification of loci responsible for glaucoma and in the identification of a susceptibility gene for prostate cancer. Recent work from his laboratory has created excitement within the scientific community when he and Dr Brendan McMorran discovered that platelets may play a critical part in the early defense mechanisms to a malarial infection. He and his team noted that the ability of platelets to kill parasites in infected red blood cells, a mechanism that the platelets lose when aspirin is added to the blood.

Christopher Freeman Consultant Pharmacist and PhD Candidate, University of Queensland, QLD Chris Freeman obtained his Bachelor of Pharmacy from James Cook University in 2003. Since then, he has had experience in many sectors of pharmacy including hospital, community, and academia. In 2006, he gained accreditation to conduct medication reviews and in 2008 completed a Graduate Diploma in Clinical Pharmacy at the University of Queensland.

In 2009 he commenced practice as a Consultant Pharmacist within the Camp Hill Healthcare medical centre located in Brisbane. In the same year, he commenced his PhD in which he is investigating the integration of pharmacists into the General Practice environment. He is also involved in a large primary care research project describing how multidisciplinary care planning works in practice.

Sharon Gordon-Croal Assistant Manager, Clinical Pharmacy, Royal Hobart Hospital, TAS Sharon Gordon-Croal is the Assistant Manager, Clinical Pharmacy at the Royal Hobart Hospital. She has six years’ experience as a clinical pharmacist working mainly with medical units and rheumatology.

She has acted in a number of pharmacy management positions, including Manager of Clinical Pharmacy, Dispensary Manager, and currently Manager of the Pharmacy Manufacturing Service.

She is a committee member of the Tasmanian branch of the Society of Hospital Pharmacists of Australia, and is a member of the SHPA Clinical Pharmacy COSP.

Having received his Fellowship of the Australasian College for Emergency Medicine in 2003, he was appointed as Director of the Emergency Department at the Royal Hobart Hospital in 2006. As Director, he oversaw the move to a new facility, the opening of dedicated paediatric and short stay areas, and the exploration of new models of delivering emergency care.

He also has also served in a number of roles with the Australasian College for Emergency Medicine. He is a past Chair of the Tasmanian Faculty, former Tasmanian Deputy Censor and has been the Tasmanian Councilor of the College since 2005, serving as College Secretary from 2008-2010. He also currently serves as a College Adjudicator and College Examiner.

He is currently Clinical Leader of the Tasmanian Emergency Care Network.

He has particular interests in flow and process redesign, professional development for clinician managers, and information support, data collection and performance measurement in emergency care provision.

Associate Professor Tony Lawler is currently Director of Medical Education and Training at the Royal Hobart Hospital and the Southern Tasmania Area Health Service, and Associate Professor of Health Services at the University of Tasmania.

Joni Leslie is a Senior Pharmacy Technician specializing in Acute Post Acute Care (APAC or Hospital in the Home) services at Gosford Hospital Pharmacy Department on the Central Coast of New South Wales. She first started her training with Royal North Shore Hospital Pharmacy Department in 2003 where she developed a strong interest in aseptic manufacture whilst working in the production department.

Since moving to Gosford Hospital Pharmacy Department in 2004 she has spent time managing and guiding the technician team as senior technician and then as the sterile and cytotoxic manufacture trainer.

In April 2010 she was appointed to her current position of Senior APAC Technician. This new role was created due to the expanding APAC service on the Central Coast and as such, the growing need for a dedicated APAC pharmacy service.

Since this role was introduced, she has utilised her extensive organisational and production experience to implement strategies to reduce costs within APAC and improve bed block across the Central Coast.

Michelle Lockwood was born in Launceston, Tasmania and raised on the family dairy farm at Meander.

She started her career in community pharmacy in Deloraine in 1982, before moving to Launceston to work in a busy suburban pharmacy as the Dispensary Manager. Twenty one years later she moved to her current position as Regional Health Technician in 2009 at the Launceston General Hospital. Working with the Regional Health Pharmacist; her objective is to provide efficient and reliable hands-on service to the state Regional Hospitals and Multi-Purpose Centres.

Outside of pharmacy she enjoys the great Tasmanian outdoors with her husband and daughter.

Professor Ross McKinnon is a Professor of Cancer Research at Flinders University.

He was previously the inaugural Director of the Sansom Institute (2005-2009) and holds Adjunct Professorships in Pharmacy at Monash University and the University of South Australia. His research interests are predominantly in the molecular pharmacology area including pharmacogenomics and molecular oncology. He also has a research interest in cancer chemoprevention related to skin cancers. This work led to the founding of PharmaQest. His current roles include Chair of the SA Tall Poppy Campaign, Member of the TGA Pharmaceutical Science Subcommittee, Member of the Australian Institute of Policy and Science (AIPS) Board and Member of the Federation of International Pharmacists (FIP) Council. He is also immediate past-President of the Australasian Pharmaceutical Science Association and a member of the NHMRC Academy. He was recently appointed to the FIP Education Steering Committee, which will oversee all educational activities undertaken by FIP. He recently served as National Facilitator for the Federal Government’s Translating Health Discovery Super Science Project.

He has published over 140 research and education manuscripts since 1991.

Karen O’Leary started her career as a pharmacist at The Royal Melbourne Hospital where she became the senior pharmacist for clinical pharmacy services. She was then the deputy director of pharmacy at the Peter MacCallum Cancer Institute. During her time in hospitals, she was heavily involved in the education of intern pharmacists, the quality of clinical pharmacy services, casemix funding systems and expanding the role of pharmacy technicians.

She then left pharmacy and became a health consultant at KPMG and then moved to the health insurer MBF as the Funding Methods Manager with the core responsibility of the development of strategies for the purchase of health services by MBF at a reasonable and sustainable price.

She has worked at SHPA for the last eight years. She is and has been involved in all of the major projects undertaken by SHPA during this time including the revamp of CPD for pharmacists, the expansion of CPD services offered by SHPA, both reviews of Australia’s pharmacy technician qualifications and development of competency standards and support tools such as shpaclinCAT.

Dr Lisa Nissen Associate Professor (QUM), School of Pharmacy; Deputy Director, Centre for Safe and Effective Prescribing (CSEP), University of Queensland, QLD Dr Lisa Nissen is an Associate Professor at the University of Queensland, School of Pharmacy. She has over 17 years experience, having worked in hospital and community pharmacy in both rural and metropolitan settings. Her research focuses on the Quality Use of Medicines in the wider community and the expansion of roles for pharmacists, including development of pharmacy practice into other cognitive service delivery areas including sleep, pain and pharmacist prescribing. Her specific clinical interest is in the management of chronic pain.

She was the 2002 PSA Young Pharmacist of the Year, and the 2008 PSA Pharmacist of the Year. She also received a 2008 Australian Learning and Teaching Council National University excellence in teaching award. She is a past chairman of SHPA (QLD) Branch, a Fellow of SHPA, a Honorary Fellow of the HKAPh, vice-president of PSA (QLD) Branch and PSA national board member.

Debbie Rigby DR Pharmacy Consulting, Brisbane, QLD Debbie Rigby is a consultant clinical pharmacist from Brisbane. Jim Ryan Specialist Pharmacist Mental Health, Royal Hobart Hospital, TAS Jim Ryan is a graduate of the University of Queensland and a long-term staff member of the pharmacy department at Royal Hobart Hospital. He has worked in a variety of general and specialist practice settings and is currently specialist pharmacist for Mental Health Services South.

Gillian Sharratt has been the Executive Officer at NSW TAG since July 2010. She graduated with a Bachelor of Pharmacy in the UK in 2000 and spent a number of years working in hospitals as a clinical pharmacist completing her Post Graduate Certificate and Diploma in Pharmacy Practice at the London School of Pharmacy. She specialised in the areas of mental health and neurology before taking on the role of an Education and Training Pharmacist at Southampton Hospitals NHS Trust. This role involved local coordination of post graduate lectures for the London School of Pharmacy Post Graduate Certificate Course and Portsmouth University Clinical Pharmacy Diploma. In 2006 she moved to Australia to take on the position as Drug Use Evaluation Pharmacist at St Vincent’s Hospital in Sydney. For the next four years she undertook and supervised numerous DUE and QUM projects and was an active member of the hospital Drug and Therapeutics Committee. Her interests include education, decision making around the use of medicines, and strategies to support safe use of medicines.

Dr Peter Vervaart Principal Scientist, Pathology Royal Hobart Hospital, TAS Dr Peter Vervaart is Principal Scientist in Pathology Services at the Royal Hobart Hospital and has appointments with the University of New South Wales, where he convenes the Molecular Basis of Disease Modules of the Masters in Drug Development, and University of Tasmania. Prior to moving back to Tasmania in 2006 he was National Education Manager with the Association of Regulatory and Clinical Scientists (ARCS). He has a PhD from the University of Melbourne and a Diploma in Frontline Management and Graduate Certificate in Public Sector Management. He is a Fellow of the Australasian Association of Clinical Biochemists (AACB) of which he is also Vice President-Education and Training and representative on the Medical Testing Accreditation Advisory Committee (MTAAC) of the National Association of Testing Authorities (NATA) and is a Foundation Fellow of the Faculty of Science of the Royal College of Pathologists of Australasia (RCPA).

She commenced her pharmacy career in retail pharmacy in 1990 and has over 15 years’ experience as a hospital pharmacy technician. She is committed to technician education and training and is a mentor for technicians undertaking Certificates III and IV in Hospital/Health Services Pharmacy Support. She was also the principal trainer for the department’s aseptic and cytotoxic manufacturing unit for eight years.

Dr Michael Wiese Lecturer in Pharmacotherapeutics, Division of Health Services, School of Pharmacy and Medical Sciences University of South Australia, SA Dr Michael Wiese graduated with a Bachelor of Pharmacy degree from the University of South Australia in 1996, upon which he entered clinical practice, working in various hospitals in Adelaide and Hobart, prior to his appointment as a Senior Lecturer in Pharmacotherapeutics at the University of South Australia in 2008. He primarily teaches into the 3rd and 4th year of the Bachelor of Pharmacy program. He has also obtained a Master of Clinical Pharmacy in 2002 and PhD in 2008 (both via University of South Australia). During his time at the Royal Hobart Hospital, he co-authored a manuscript describing the successful implementation of ant venom immunotherapy, which was published in ‘The Lancet’ in 2003 - this was the first ever randomised, double blind trial of venom immunotherapy.

He has subsequently published a number of articles regarding venom immunotherapy, and his current research interests include drug and venom allergy, and establishing a role for personalised medicine in the treatment of various conditions.

Pharmacist Leadership Program Speakers Peter Custance Peter Custance is a graduate from the School of Pharmacy, University of Sydney. Following short stints working as a pharmacist at Nepean District Hospital, Sydney Hospital and in community pharmacy he headed ‘west’ where he has worked exclusively in public and private hospital pharmacy. Initially working at Royal Perth Hospital, he was appointed to his current role of Director of Pharmacy at St John of God Hospital Subiaco in 1993.

He completed his MBA at the University of WA whilst working at RPH. He has a particular interest in the application of business principles in the development of sustainable pharmacy services in a ‘not for profit’ private hospital environment. He also has a special interest in human resource management and meeting the challenges of recruitment and retention in an increasingly dynamic working environment.

He is a Fellow of SHPA and has been a member since initially registering as a pharmacist.

David McKnight David McKnight worked as a Community Pharmacist in Ireland and Australia before commencing a career in hospital practice in the mid eighties in Perth metropolitan hospitals. Following nine years at Princess Margaret Hospital for Children, he took up the position of Coordinator of Clinical Pharmacy Services and Deputy Chief Pharmacist at St John of God Hospital Subiaco in 1997, a Catholic private not for profit hospital of 550 beds. He has a very keen interest in all aspects of medication safety and was an invited speaker on the topic at the Qatar Health 2010 Conference in Doha, Qatar.

He is a member of the Western Australian Medication Safety Group (WAMSG) and has been active in working parties on the development of a state wide Anticoagulation Chart with prescribing guidelines and Medication Reconciliation. He is a member of the St John of God Health Care National “Group Medication Reference Taskforce” and is a member of his hospital’s Medication Safety and Drug and Therapeutics Committees. He was the recipient of the Bayer/Society of Hospital Pharmacists of Australia (SHPA) WA Branch Achievement award in 2006 and was twice nominated for the GSK/SHPA National Medal of Merit Award in 2007 and 2008.

He has a keen interest in pharmacy student education and is an active Pharmacy Intern Examiner, Supervisor and Preceptor. He supports the Pharmacy School programs of Curtin University of Technology and the Graduate Masters program of the University of Western Australia where he is accorded the title of Adjunct Associate Professor. Recently he submitted his thesis for a Masters of Pharmacy by Research from Curtin University, which looked at medication errors in private hospital practice and is undertaking accreditation to be an AACP Consultant Pharmacist.

He is married with two children.

Michael Dooley Michael Dooley (BPharm Grad Dip Hosp Pharm FSHPA FISOPP) holds a joint appointment as Director of Pharmacy at Alfred Health and Professor of Clinical Pharmacy in the Department of Pharmacy Practice of the Faculty of Pharmacy and Pharmaceutical Sciences of Monash University.

He manages the pharmacy services to The Alfred, Caulfield and also the Sandringham hospitals and is co-director of the Monash University Centre for Medicine Use and Safety. The Alfred Health pharmacy services include comprehensive clinical pharmacy services, medication use evaluation programs and an active research program.

His contribution to pharmacy and improvements in medication use have been recognised by receiving the Australia Council of Health Standard (ACHS) 2005 Gold Medal, the 2004 Society of Hospital Pharmacists of Australia Clinical Pharmacy Award and the 2003 Medal of Merit.

He contributes to many national and local professional committees and working parties related to improving the delivery of quality heath care services and medication usage. He has been actively involved in a range of research activities from practice based undergraduate projects through to competitively funded National Health and Medical Research Council (NHMRC) and Australian Research Council (ARC) collaborative initiatives and post graduate programs. He has presented and published both locally and internationally on a wide range of topics.

Suzette Seaton Suzette Seaton has been employed as Manager of Pharmacy Services/Site Manager Pharmacy NWAHS since 2003.

After completing a pharmacy degree at the University of Tasmania in 1978, she practised as a pharmacist in community and hospital pharmacy, in Tasmania until 1993. After a two year stint in Central Queensland she returned to Tasmania in 1996. She was employed as a Senior Pharmacist at the North West Area Health Service, moving into her current position in 2003. She has a keen interest in improving medication safety, and has been involved in several medication safety related collaboratives over the years, including National Medication Safety Breakthrough Collaborative, Safer Systems Saving Lives and High 5s. She completed a Masters by Research degree in 2008, researching the excretion of oxycodone into human breast milk. Suzette is Chair/Convenor for the NWAHS Medication Management Committee.

Olimpia Nigro Olimpia Nigro is the Team Leader Pharmacist for Clinical Pharmacy & Medicines Information Services at the Royal Adelaide Hospital.

She graduated from the University of South Australia and has completed a Graduate Diploma in Clinical Pharmacy. She is an experienced clinical pharmacist with over 15 years practice in hospital pharmacy with a background in medicines information and internal medicine.

Her areas of interest include optimising medication management, medication safety and electronic health information systems. She has been actively involved in the implementation of electronic PBS discharge prescriptions and the introduction of the APAC Guiding Principles at the Royal Adelaide Hospital. She was also involved in the development of APAC Key Performance Indicators for South Australia and is a current member of the Quality Use of Medicines Working Group in SA.

Yvonne Allinson Yvonne Allinson is CEO of The Society of Hospital Pharmacists of Australia (SHPA), the peak professional membership organisation for continuing professional development and practice standards for Australian hospital pharmacists. She works at both national and state/territory levels to support efforts to improve health care and pharmacy services for individual consumers. She is a hospital pharmacist with extensive clinical pharmacy experience.

She has undertaken post graduate studies in hospital pharmacy, health education and health administration and she has worked at several Australian public hospitals and overseas in clinical pharmacy and management positions. Prior to joining SHPA, she worked in the Victorian Health Department on the change management aspects of the information technology and telecommunications strategy for public hospitals.
