The President’s Thoughts Dear Friends, As you can see by the front cover, Concordia’s Mission has again been blessed with growth. Two great organizations – Concordia Village of Tampa and Concordia at Villa St. Joseph – with their own impressive mission histories are now a part of the Concordia family. I am so thankful to God for these opportunities to serve Him in new ways.

Thanks also to Concordia’s Board of Directors for trusting management to handle these new challenges. Concordia’s Board has been working with management over the past 10 months on Concordia Village of Tampa in Florida and six months on Concordia at Villa St. Joseph in Baden, PA. Just envision…all of the Concordia departments that have been involved and will continue to help transition these new facilities into our Concordia family. Imagine all of the complexity with more than 40 Concordia leaders behind the scenes and on deck from nursing, personal care, therapy, activities, food service, maintenance, laundry, housekeeping, information technology, public relations, accounting, legal, payroll, human resources and even website specialists who have “stepped up” to ensure the smooth conversion for the future viability of these two new Concordia gems.

We welcome the more than 600 new employee team members: 408 at Concordia Village of Tampa and 203 at Concordia at Villa St. Joseph. I am personally excited to have these new employees as part of our family and wish them many years of success and satisfaction as they continue their careers at Concordia.

Welcome to the Family Concordia Adds Communities in Baden, PA and Tampa, FL In 2003, Concordia Lutheran Ministries acquired our first location outside of our original Cabot, PA campus to better serve our aging community with the highest quality of services. In the 15 years that have followed, Concordia and our mission have been expanding, adding 11 facilities, building two new retirement living communities and acquiring numerous parcels of land for further expansion – not to mention adding new office locations for Concordia Visiting Nurses and Good Samaritan Hospice (GSH) and GSH inpatient units.

Continuing in this tradition of mission-driven growth, Concordia is pleased to announce the two newest additions to our family of locations: Concordia at Villa St. Joseph in Baden, PA and Concordia Village of Tampa in Tampa, FL. With these two new locations, Concordia now operates 940 independent living units, 1,153 personal care or assisted living beds (including memory care), 663 skilled nursing beds and 34 hospice beds. In addition, Concordia has extensive home and community services, including home health care, private duty home care, durable medical equipment, hospice and palliative care, and pharmacy services for residents, allowing us to serve more people in more ways than ever before. System-wide, Concordia now employs more than 3,000.

Concordia at Villa St. Joseph After months of planning and preparing, Concordia welcomed Concordia at Villa St. Joseph to our family of locations on April 26. This new blessing has a faithful tradition of caring, comfort and healing and has sustained an outstanding reputation in the Beaver Valley.

Concordia at Villa St. Joseph is located off Route 65 in Beaver County – just a two-minute drive from the Concordia Visiting Nurses Baden Office. Situated amidst the beautiful backdrop of nature, those at Concordia at Villa St. Joseph enjoy tranquil surroundings.

The facility, formerly called Villa St. Joseph, is a CARF-accredited, 120-bed skilled nursing and shortterm rehabilitation facility that holds a 5-star rating from Medicare. It was founded in 1997 as a ministry of the Sisters of St. Joseph, an international religious community of over 500 vowed Catholic women and 500 lay associates whose origins date back to 1650.

Concordia at Villa St. Joseph will continue its faithbased mission of serving others in offering short-term rehabilitation, long-term nursing care, memory care and outpatient rehabilitation. It is our hope that God will continue to use Concordia to protect and preserve

The Difference Between Palliative Care and Hospice Care In this day and age, most of us know the importance of constantly educating ourselves about healthcare, but sometimes that’s easier said than done – especially when the meanings of many terms change over time. Two types of care that often get confused for one another are palliative care and hospice care, services that Concordia Lutheran Ministries offers through Concordia Community Support Services and Good Samaritan Hospice (GSH). Roni Lucas, a Registered Nurse Educator at GSH who has 20 years of experience with Concordia, shared her knowledge on palliative care and hospice care and explained how they are provided at Concordia.

When a Concordia resident or patient is facing a serious illness and frequent hospitalizations, the first step to palliative care or hospice care is a Reflections of Care conversation with a GSH staff member.

The Reflections of Care program was created to Roni Lucas help residents, patients and their families learn about healthcare options, reflect on their wishes and address any concerns they might have.

“When we have a Reflections of Care conversation with our patients, we want to know their story,” Roni said. “We want to know what’s going on, what the doctors have said, what changes are happening, if there’s anything they are struggling with and how we can help. We’re there to explain and provide care options, no matter what the diagnosis looks like.”

Anyone can recommend a free, no-obligation Reflections of Care conversation. One of the main goals for the GSH staff is to help the person decide what his or her goals of care are. Staff members learn more about the resident or patient, discuss what he or she wants, what is realistic and what will benefit him or her the most in order to set up a care plan to work toward these goals. It is at this point that Roni notes she sometimes has to debunk popular misconceptions about palliative care and hospice care.

“The one thing I tell every single person that I come into contact with is that we never make blanket statements in palliative care or hospice care,” Roni said. “Each case and each person is different, and we are there to talk through it all.”

In palliative care, patients continue to receive treatment for their health conditions, such as chemotherapy or dialysis. The primary focus of palliative care is to ease the suffering that results from advanced illness, which often means providing treatment for symptoms even when the underlying illness cannot be cured.

Palliative care differs from hospice in that the patient doesn’t always have a terminal diagnosis. Palliative care is offered at Concordia through Concordia Visiting Nurses in the home or to those already in personal care.

“Here our palliative care team is part of Concordia Visiting Nurses, and they are the best – nothing gets past them,” Roni said. “The team is made up of RNs (registered nurses), LPNs (licensed practical nurses) and nurse aides, and GSH works very closely with them.”

Payment for palliative care, including whether medical equipment is covered or not, depends on what type of insurance the person has. Staff work with every resident or patient to determine what services

and equipment will be covered under his or her insurance plan, ensuring that everyone stays mindful of the financial side of care.

Whenever patients transition from palliative care to hospice care, or start hospice care, they or their families sometimes think that all care will stop.

“The care doesn’t stop; it changes,” Roni explained.

“The focus changes.”

Hospice care is generally for people who become more symptomatic and have exhausted or elected to stop all treatment options. While the resident or patient still receives comfort care to alleviate the symptoms of a disease or condition, the focus of care at Concordia shifts to offer even more emotional, social and spiritual support for the person and family - individually and as a unit.

GSH is made up of community educators, social workers, RN case managers, LPNs, home health aides, physical and occupational therapists, a pastoral care team and volunteers who offer in-home hospice care as well as respite care at three inpatient locations: Cabot, Heritage Valley Beaver and Wexford.

Unlike palliative care, hospice care is paid per day and care includes the hospice team, supplies and equipment that are related to the patient or resident's comfort. Equipment for hospice care is covered under most insurance plans.

“Ultimately, everyone’s goals are the same: keep you at home, keep you comfortable and make sure you have what you need,” Roni said. “Through this process, no matter what it is, we will care for you. The care could change, but we will care for you.”

Researching palliative care and hospice care can be overwhelming, but there is no reason to do it alone.

Nursing Students from Concordia University Ann Arbor Complete Rotations at Cabot Concordia Lutheran Ministries frequently welcomes students from local technical schools and colleges to our communities to complete their rotations, but we recently hosted nursing students from a location that’s a little farther away: Concordia University Ann Arbor (CUAA) in Michigan.

CUAA, a private liberal arts university, is a member of the Lutheran Church-Missouri Synod’s Concordia University System. Its new, accredited School of Nursing will graduate about 40 students in December. Of this group, eight were chosen to come to Concordia at Cabot from March 9-17 based on their ethics, grades and integrity: Shalena Blocker, Tiere Emerson, Ann Kossey, Kennedi Long, Becca Maccani, Jared McDaniel, Rebekah Olson and Samantha Southwell.

The program, a first-ever for both Concordias, was developed by the two for about seven months ahead of the time the students were set to arrive. It was decided that while in Cabot, the CUAA students would complete clinical rotations in Concordia’s inpatient hospice unit and skilled nursing floor as well as with Concordia Visiting Nurses.

“The students completed all of their clinical hours for the semester while they were here,” said Kathleen Sheehan, CUAA’s Skills Lab and Clinical Liaison who accompanied the students from Michigan. “They did

what normally takes about 14 weeks during a semester in just one week.”

Kathleen has been working at CUAA since August 2017 and stayed with the eight students at Concordia at Cabot’s model apartments and guest rooms for the week.

“We ended up calling the program a nursing immersion, since the students were here living, training and working on campus” said Karen McCreary, Concordia Visiting Nurses’ Chief Operating Officer, who played an integral role in planning the program. “Most nursing schools are more focused on hospitals, so this was a unique opportunity for CUAA and Concordia.”

Once the students arrived, they were assigned to a preceptor/nurse and had a complete orientation just like a new Concordia employee would. In addition to completing their clinical hours they were also given educational presentations by Concordia staff.

“This has been a wonderful week,” Kathleen said.

“This experience was an exact fit for the courses the students have taken.”

They specifically learned about transitions of care, such as patients moving from retirement living to personal care or from palliative care to hospice care, as well as getting a feel for nursing outside of a hospital setting. Ultimately, they will need to decide what branch of nursing to enter, and experiences like this can help them expand their knowledge base.

“It was really beneficial for us to come here to learn and see nursing in a different state,” said Jared McDaniel, one of the CUAA nursing students. “Everyone here was really friendly and welcoming, and I was really impressed.”

The students were all appreciative for their opportunity to train at Concordia.

“Thank you for allowing us to complete our clinical experience at your beautiful facility,” said Tiere Emerson, who looks forward to pursuing a Master’s of Science degree in Nursing and becoming a Nurse

Practitioner after she graduates. “This was a memorable opportunity for all of us.”

Rebekah Olson, who works as a nurse aide at the University of Michigan hospital in Ann Arbor when not in class, was also thankful.

“I have had such a good experience at Concordia this week, and I cannot wait to be a nurse,” Rebekah said. “Thank you!”

In addition to helping the students, the program is also a benefit for the Concordia family.

“The nursing immersion program was a great way for Concordia to give back to the nursing community and expand our horizons as well,” Karen McCreary said. “Our staff enjoyed working with the CUAA students immensely and found it very exciting to see young people learning and wanting to get into nursing careers. I’m not sure that either Concordia or CUAA knew what to expect from this program, but it has just been amazing. We hope to have more opportunities like this in the future.”

Horst & Eleanor Schwalm Faith in Caring Award Winner Announced Each year, Concordia honors volunteers across the organization for their dedicated service. Beginning in 2018, we will annually present the Horst & Eleanor Schwalm Faith In Caring Award to a single volunteer (or group of volunteers) who truly goes above and beyond in service to others. The award is named in honor of longtime administrator Horst Schwalm and his widow Eleanor. Eleanor remains active at Concordia as a faithful volunteer and resident. The Faith In Caring Award is regarded as Concordia’s highest volunteer honor.

At the end of April, we announced this year’s Horst & Eleanor Schwalm Faith In Caring Volunteer Award winner: Valerie McMahon! Valerie has been volunteering twice a week at Concordia of Cranberry with arts, crafts and cooking club for almost two years. She was described by the Cranberry Activities Department as caring, devoted and – most of all – inspiring. Valerie is living with autism, and truly gives all of herself every week to impact the lives of those in our care. The employees and residents have watched her grow over the past two years and she has become an inspiration to everyone she comes in contact with her. Thank you, Val, for your loving heart and dedication to our residents. This award carries a $2,000 stipend - $1,000 to benefit Concordia of Cranberry’s Activity Department and $1,000 to benefit all of Concordia’s volunteer programs to honor Valerie as an outstanding volunteer.

Leaving a Legacy of Caring Concordia Lutheran Ministries Legacy Society members are individuals who decide to make a bequest in the name of Concordia. Bequests can be in the form of wills or living trust gifts, gift annuities and charitable remainder trusts, retirement plans or life insurance policies that name Concordia as beneficiary.

So by bequesting a gift to Concordia, members help ensure the future of the organization and the 40,000 people it serves every year.

Nearly 125 living individuals are members of Concordia’s Legacy Society at any one time, and growing this organization is a top priority of the Development team.

Acts 2:29-36 - “Brothers, I may say to you with confidence about the patriarch David that he both died and was buried, and his tomb is with us to this day. Being therefore a prophet, and knowing that God had sworn with an oath to him that he would set one of his descendants on his throne, he foresaw and spoke about the resurrection of the Christ, that he was not abandoned to Hades, nor did his flesh see corruption. This Jesus God raised up, and of that we all are witnesses. Being therefore exalted at the right hand of God, and having received from the Father the promise of the Holy Spirit, he has poured out this that you yourselves are seeing and hearing. For David did not ascend into the heavens, but he himself says, “‘The Lord said to my Lord, Sit at my right hand, until I make your enemies your footstool.’ Let all the house of Israel therefore know for certain that God has made him both Lord and Christ, this Jesus whom you crucified.”

Dear Brothers and Sisters in Christ, We are now in the season following Pentecost. Sunday, May 27 was Holy Trinity Sunday, a day in which we look at who our God is and all that He has done.

We look at how He set forth a plan to redeem His people from sin - a plan in which He swore an oath to Israel that He would set a descendent of David on the throne of Israel.

In the Book of Acts Peter tells us of what King David foresaw and spoke of as a prophet of Israel. David spoke of “the resurrection of the Christ, that he was not abandoned to Hades, nor did his flesh see corruption.”

God laid forth a plan to send His Son, Jesus, a descen-

dent and heir of David, who would sit on the throne of David. This descendent would be the Christ, the one to redeem the world. He would rise from the dead to bring new life for all who believe. The apostles and disciples were witness to the resurrection as Peter said.

This is written that you may have confidence that Jesus is the Christ and that He truly is risen from the grave, and believe that you are redeemed by your God, the Father, Son and Holy Ghost.

Because Jesus Christ is risen from the grave you may have confidence that: • you are forgiven of your sin, for His blood was shed to cover your sin • in Him you have Salvation, for the enemies of sin, death and Satan are defeated • in Him you have eternal life, for Jesus is risen from the grave and He promises that in Christ Jesus you too shall be resurrected, healed in body and soul and given new life in Him forever.

Your God is the Trinity, the Father, the Son and the Holy Spirit - one God in whom you have confidence that He loves you and has redeemed you, that He has done just as He promised in His oath to David, that there on David’s throne is the Son, Jesus, the Christ, the resurrected Lord who brings you hope and joy in His death and resurrection, because in Him you have been redeemed. In Him you live!

Let us pray: Almighty and ever-living God, You fulfilled Your promise by sending the gift of the Holy Spirit to unite disciples of all nations in the cross and resurrection of Your Son, Jesus Christ. By the preaching of the Gospel spread this gift to the ends of the earth; through the same Jesus Christ, our Lord, who lives and reigns with You and the Holy Spirit, one God, now and forever. Groundbreaking Announced for Retirement Living Expansion at Concordia at Sumner Concordia at Sumner will celebrate a Groundbreaking Ceremony on Friday, June 15 at 3:30 p.m. on the 64-acre property located at 970 Sumner Parkway in Copley, OH. Concordia’s Walking Through Grief Good Samaritan Hospice conducts grief support groups for anyone who has suffered the loss of a loved one. The weekly meeting will begin Monday evenings for six weeks from July 9 - August 13, 7 - 8:15 p.m. in the Bistro at Highpointe at Rebecca, 1871 Highpointe Lane, Allison Park, PA 15101.

Concordia Fashion Show Raises More than $20,000 The 13th annual Concordia Fashion Show, which had a “Peaches and Cream” theme, hosted residents, staff and family members as well as our church members and local community for a day of fun, fashions and fine food. The audience was entertained by 25 models – comprised of Concordia staff, residents and friends who walked the runway for five fashion houses, 55 auction baskets, 125 mystery bags and several vendors on Saturday, May 13 at Butler Country Club. Final figures have not yet been calculated, but preliminary reports appear to show the event generated more than $20,000 to benefit the Good Samaritan Endowment Fund for charity care. Thanks to all who supported our annual event.

NEWS / NOTES Concordia of the South Hills Schedules Annual Indoor Flea Market Retirement living residents at Concordia of the South Hills are hosting the Annual Indoor Flea Market on Monday and Tuesday, June 18 and 19 from 9 a.m. - 3 p.m.. The sale will be held in the Harmony Room on the ground floor at the 1300 Bower Hill Road location in Mt. Lebanon. Save the Date for the Third Annual Concordia at Sumner Fashion Show Concordia at Sumner’s 2018 Fashion Show will be held on Saturday, Sept. 8 at St. George’s Family Centre, 3204 Ridgewood Road in Fairlawn, OH from 10 a.m. - 2 p.m. The event will feature a hot lunch, valet parking, baskets for auction valued at $100 each, a silent auction, a vendor mall and a fashion show by Allie M Designs Boutique of Akron, OH. Tickets are $40 each. Proceeds will go toward the final building phase of Concordia at Sumner’s newly established garden adjacent to the Louise Sumner Pavilion. Advance payment is needed, and checks should be made payable to the CLM Foundation and sent to Rowena Wilkins, 970 Sumner Parkway, Copley, OH 44321.

New Administrator Named at Concordia of Monroeville and Harmony Physical Rehabilitation Melissa Sullivan was recently named Administrator of Concordia of Monroeville and Harmony Physical Rehabilitation. Sullivan, who holds a BSN from Duquesne University, is a licensed Nursing Home Administrator and Registered Nurse with 23 years of healthcare experience, having spent eight of those years as a Nursing Home Administrator in the area. She is responsible for the overall operations at both Concordia of Monroeville and Harmony Physical Rehabilitation, day-to-day 26 - Faith in Caring

patient/resident/family experiences and implementing a culture of success and fulfillment for employees.

She was drawn to Concordia’s reputation and stability in the post-acute healthcare arena. She lives in Murrysville with her husband, Chris, and their 5-year-old twins, Chad and Mikey.

Concordia to Host 18th Annual Dog Show August 16 Concordia at Cabot Lund Care Center Activities Department will host its 18th Annual Dog Show on Thursday, Aug. 16 at 6:30 p.m. in the Lund Care Center backyard, 134 Marwood Road, Cabot. “Dog Days of Summer” is free and open to the public and will feature a number of Concordia employee, resident and family-owned canines competing in Best Performer, Best Groomed, Most Obedient and Most Friendly categories. Several residents serve as judges for the event.

2018 Fishing Derby for Kids Draws 650 On Saturday, May 5, families descended upon Lake Saxony to reel in some bass, blue gill and catfish! It was a perfect day for fishing with the temperature around 70. The largest fish caught was 16½ inches, while the smallest was 3 inches. This year, an additional section of the lake allowed everyone more room.

Over 1,000 red worms, 800 nightcrawlers and 250 waxworms and the hot dogs and drinks were compliments of Concordia. Thanks to our prize sponsors and volunteers.

Most often, that service occurs in our senior care communities and in the homes of those we serve through our home and community services organizations. But there’s a lot of service and support that we provide others that many people

community servant events in the areas we serve. The first servant event was held in March when a number of employees spent an afternoon volunteering at The Lighthouse Foundation, a non-profit based in Butler, PA that provides, among other things, a food bank for those in need.
