n reading the company profiles in this issue, I was impressed by the efforts of the system vendors to help pharmacists initiate programs that will encourage adherence to a person’s medication regimen and improve outcomes. This is being done in a number of ways. Using IVR systems to remind people of refills coming due is one approach, and reminding people via IVR that their prescriptions are ready for pickup is another. There is now access to software to implement synchronization programs so all refills come due on the same day. There are packaging systems designed to improve adherence, and pharmacists are finding that they can charge for this service. Even apps for mobile devices have been introduced to promote adherence.

There is plenty of evidence that pharmacist intervention can make a difference. And with the decline in the number of primary-care physicians, pharmacists have an opportunity to pick up the slack. Pharmacists certainly have the technology tools and knowledge to do so. Adherence programs fit nicely with medication therapy management (MTM).

That said, the American Society for Automation in Pharmacy (ASAP) developed a standard for MTM information exchange a few years ago. The purpose was to allow pharmacists to receive alerts directly into their pharmacy management systems for patients who qualify for MTM interventions. OutcomesMTM is now supporting this ASAP standard, and is reporting growing participation in MTM programs as a result.

This is good example of how pharmacy management systems can be used to make a process more efficient.

In addition to receiving the opportunity alerts in the pharmacy management system, we are seeing functionality built into the systems to document and bill for this type of service.

System vendors respond to what their users are asking for. Apparently, the interest is there for MTM functionality. I see this as a positive development for pharmacy in positioning the profession to engage in health information exchanges. Will the next step be a pharmacy-specific electronic health record (EHR) that will interface with the pharmacy management system? Perhaps, but don’t expect incentive payments from the government to install such a system.

On the topic of incentive payments: There was a front-page article in The New York Times on Feb. 20 about the money being raked in by the companies selling electronic health record (EHR) systems. It appears that these companies did a full-court press on the White House and legislators in Washington to include incentive payments in the 2009 economic stimulus bill to encourage physicians and hospitals to install EHR systems. The pitch was how these systems could lead to more efficiency in healthcare delivery and better patient care through interoperability. As of February, CMS has given out over $12 billion in incentive payments to providers complying with the meaningful-use criteria established by the government. This is a significant price to pay to gain adoption of these systems when EHRs are getting mixed reviews by the users. What I have read is that EHRs add five to 10 minutes to a patient encounter, interoperability with other systems isn’t always the case, and they are difficult to use.

Buyer Beware, Seller Prepare

E

very year ComputerTalk publishes its buyers guide issue. It’s a good idea. (Notice how many more advertising pages are in this issue.) There are lots of comparison tables with lots of data to study and analyze.

My comments are about the decision-making process and some things you should think about as you make a buying decision.

Buying decisions are decisions about the future. Huge changes are about to occur in healthcare delivery and the systems that will be supporting it. Understand as much as you can about what is coming and know that any decisions that you make need to be compatible with that future and your role in that future.

Buyers: Remember that the sellers are probably also reading this.

First Things First ■ Make a list of things you like about the system you are

now using.

■ Make a list of things you don’t like about that system.

■ Make a list of features that you need in a new system. (I want a Ferrari, but I don’t need one to get to the grocery store.)

Give each item on the list a value: A 10 for the absolute must-haves, a 1 for the nice/fun-to-haves. Do not do this alone. Involve the people who will be using it all of their working hours, as well as the managers and executives who want reports to analyze. The best way is to do it in a group meeting where everybody can contribute to the list and its items’ values. In a large organization this can be difficult

because of the risk of the word getting out that you are thinking about changing. Protect against that by requiring confidentiality and having all participants sign a confidentiality document. I firmly believe that it is that important to include the folks who will be using the system on a day-to-day basis.

Remember, if the users don’t like the new system, it won’t work. For starters, they will spend too much time telling each other how bad it is.

Compare your list to the lists of features found on the other pages of this issue of ComputerTalk. Go to the conventions and visit all the booths that are showing systems that are supposed to do the job. There are two things you need to learn at the booths. First, do you like the way the system works? Will it work in your pharmacy or pharmacies? Second, and most important, do you like the people?

Talk to more than one of them. Do you like their attitude, their goals, and their philosophy regarding pharmacy services? Remember that you are buying people and services as well as hardware and software.

Narrow it down to two or three (maximum four) likely competitors. Then invite them to visit you to show you their systems. When they visit, include your actual users in the demonstrations. Remember, they are the ones who have to live with your decision. Let them test the system under circumstances as real as you can create. Look out for the brief waits while the system is working on something.

Our Recommended Approach to Buying Technology

I

f you are like us, you have a stack of journals (paper and electronic) that you have good intentions of reading and others that you open immediately upon receipt and find a way to look at every page and read every article.

We are unashamedly tech guys, and ComputerTalk falls into the “read now” category for us. We thought we would cover some of our recommendations for getting the most out of the ComputerTalk 2013 Retail Buyers Guide while you are finding a quiet place to look over the product offerings found herein.

When we talk in our live presentations and seminars or in our columns about trying to acquire, upgrade, or jump to new capabilities in your practice, we always try to approach the process systematically. While it is frequently true that gadget lovers have little difficulty in justifying their next purchase, mainstream pharmacists usually have to prioritize their investments and make sure that a measurable return on investment occurs. A simple “coolness factor” purchase criterion for technology should be balanced with how well a new technology will ultimately perform in your operations and what the investment is for your new purchase, not only in acquisition costs, but also in learning curves and practice disruption when a change takes place.

Consider our colleague’s approach to technology. We both had a department head, years ago, who was asked why he didn’t have a computer in his office.

He finally met with us and said he was getting one of those PCs people keep asking about. Unfortunately, it was only a prop. In fact, when the printer on one of our computers went down, we exchanged our dead one for his unused one. We went months before he mentioned his printer was not working. His approach to his computer changed, however, when he learned that certain people could best be contacted by email. He also realized that using 2x2 slides to talk about the future of pharmacy was not as effective as using presentation software like Microsoft’s PowerPoint. Achieving Recognition and Payment for Services

I

t seems a fitting topic for this column in the annual ComputerTalk Retail Buyers Guide to focus on a number of recent announcements within the pharmacy profession to move forward in advancing initiatives that will allow pharmacists to be recognized and receive payment for their services.

Within the pharmacy community, the language used is often “provider status.”

While technology has long supported more efficient and safe dispensing, more recently, technology has been used to advance patient care services, including immunizations, adherence programs, and medication therapy management services, among others. Continued integration of pharmacy systems into the broader electronic health record and health information exchange infrastructure will be crucial to support the profession’s push for recognition.

The American Pharmacists Association (APhA) announced a major initiative to obtain recognition of pharmacists as providers in the healthcare system in January. So did the American Society of Health-System Pharmacists’ (ASHP) CEO Paul Abramowitz, who issued a statement saying provider status is a “top priority” for the organization. In the statement, Abramowitz noted, “It is essential to recognize pharmacists for the patient-care providers that they already are, along with other formally recognized providers, such as nurse practitioners, dietitians, psychologists, social workers, optometrists, nurse-midwives, dentists, and others. The data are conclusive: Pharmacists improve medication-use outcomes for patients when they are included on the patient-care team.” That announcement followed one by the American College of Clinical Pharmacy (ACCP) in November that their board of directors authorized an initiative to seek provider status for clinical pharmacists working in all practice settings. The Academy of Managed Care Pharmacy’s (AMCP) board of directors approved a position statement on “Non-Physician Provider Status for Pharmacists” in June 2012.

by the Affordable Care Act, through eight additional related educational sessions on integrated care models/ pharmacist opportunities and in remarks during the second general session by former Utah Governor and Secretary of Health and Human Services Michael Leavitt. Leavitt’s theme was contrasting compassion as a driver of policy decisions with the concept of global economic dispassion and how they are intersecting now.

He cited many problems, including the national debt and growth in Medicare and Medicaid with an aging population, noting the need for change. He described the evolving health delivery model of accountable care organizations (ACOs). While community pharmacies are least frequently included in these risk-bearing payment arrangements, the ACO’s need to manage financial risk and meet quality requirements will drive ACO interest in pharmacy, he said.

He issued a call to action, noting this was a “golden period” for pharmacists, but only for those who can change.

ing a National Action Plan, held in conjunction with APhA2013. Nearly 200 pharmacy leaders from national organizations, state associations, and academia gathered to discuss how to achieve integration of pharmacists into the evolving patient-centered, team-based healthcare system.

The conference was led by APhA, the National Alliance of State Pharmacy Associations (NASPA), the University of Southern California (USC) School of Pharmacy, and the California Pharmacists Association (CPhA).

Dana Goldman, director, USC Leonard D. Schaeffer Center for Health Policy and Economics and adjunct professor at the USC School of Pharmacy and Price School of Public Policy, noted, “Prevention is key to controlling healthcare costs, and pharmacists can play an important role in managing this process.” Patients and healthcare providers often do not have access to the benefit of the patient care services pharmacists provide because the proper recognition and payment models are not in place. Studies and practice-based experience have shown that when pharmacists are involved in the provision of patient care services as members of the healthcare team, patient outcomes improve, patients report higher rates of satisfaction, and overall healthcare costs are reduced.

CPhA CEO Jon R. Roth noted, “With the Affordable Care Act and Medicaid expansion set for implementation nine short months from now, we must press forward with opportunities to ensure that the millions of people who will be receiving insurance coverage have avenues to achieve care. By affirming pharmacists as healthcare providers, it will ensure patients can receive a range of primary care services from highly trained and widely available pharmacists.”

The attending pharmacy leaders considered a slate of draft principles for the profession to seek recognition for pharmacists’ role as healthcare providers under one unified voice and message. These broad-based principles will serve to frame the profession’s efforts to seek increased access for patients to pharmacists’ patient care services.

APhA EVP and CEO Thomas E. Menighan said, “If we are successful [in achieving recognition], patients will be well served by pharmacists’ patient care services, and our services will be valued and covered. If we can speak with a common message and one voice as a profession, we will be much more effective at getting buy-in from policy makers and the public.” Becky Snead, EVP and CEO, NASPA, concluded the meeting by saying, “For success to 84

be achieved, pharmacy representatives from local, regional, and national pharmacy organizations and stakeholders need to help shape the discussion.”

APhA announced in late January that its board of trustees has allocated $1.5 million toward a multimillion dollar, multiorganizational, multifaceted, long-range effort by APhA and the profession to gain recognition for pharmacists’ role as healthcare providers. It was clear from the sessions at APhA2013 that there are a number of avenues and models where pharmacists are contributing to current and evolving health delivery models.

In an issue brief handed out at the meeting, APhA noted that it was “pleased so many national and state organizations are doing the same [working on the issue]” and that they would “work diligently to marshal our collective strength into one set of principles that everyone — pharmacy, medicine, consumers, and legislators — can support. Further, the brief says, “The ultimate goal is a consensus-based approach for advocacy and legislative efforts.” The profession’s ability to deliver an understandable, consistent, and coordinated message is crucial to achieving our overall objective — simply stated as “providing patients access to pharmacists’ clinical services and having pharmacists as recognized members of the healthcare team,” according to the brief.

APhA has created an issue-oriented space on its website at www.pharmacist.com/providerstatusrecognition. It also has the latest news on a petition that has collected over 35,000 signatures and that should prompt a White House response.

I encourage pharmacy technology vendors to get up to speed on the issue and brainstorm ways their offerings could support the profession’s efforts. It’s clear from perusing the annual ComputerTalk Retail Buyers Guide that there are numerous technologies that can be brought to bear to help.

Telemedicine in Pharmacy

H

ow many friends do you have on Facebook? How many connections do you have on LinkedIn? How many people do you follow on Twitter? If you use social media, your answer is probably “a lot.” Now how many of those people do you actually see or talk to on the phone? If you’re like us, it’s probably not very many. In social media, online contact often doesn’t translate to “offline” interactions.

However, that is not true for online interactions between patients and healthcare professionals, which is a good thing. A JAMA study from November 2012 showed that online patient visits with physicians (a form of telemedicine) were associated with higher rates of phone calls and inperson visits with physicians than for patients who did not participate in online visits.

We view this increased online patient contact with healthcare professionals as a huge opportunity for pharmacists to expand their scope of supportive care via telemedicine. The American Telemedicine Association defines telemedicine as: “The use of medical information exchanged from one site to another via electronic communications to improve a patient’s clinical health status.

Telemedicine includes a growing variety of applications and services using two-way video, email, smartphones, wireless tools and other forms of telecommunications technology.”

With the development of new technologies for home testing with remote monitoring, pharmacists can extend clinical services to patients remotely.

itors blood pressure, blood oxygen saturation, pulse rate, weight, glucose level, prothrombin time and ratios, temperature, fluid status, and electrocardiogram data, which are then available to clinicians in real time via laptop or cell phone.

Some of these vital signs are not necessarily something that a pharmacist can act on, but the availability of this data could certainly make it easier for the pharmacist to help care for the patient.

■ Avaya telehealth and home-care delivery — pro-

vides voice and video conference functionality for patients to interact with healthcare professionals.

Patients using these products could benefit from interaction with a pharmacist. A study funded by the National Heart, Lung, and Blood Institute is looking at pharmacist telemonitoring of patients’ blood pressure compared to usual care by primary-care providers.

Six months into the study the researchers found that “home blood pressure telemonitoring with pharmacist case management was effective at improving blood pressure control and at lowering blood pressure over six months.” This study is still ongoing to determine longer-term efficacy as well as evaluation of costs of medications and physician services.

Retail pharmacies are also using technology to expand in-store clinic and general health services as another way to serve patients. Rite Aid recently began offering patients remote access to physicians and nurses through its NowClinic online care service, an Internetbased service of OptumHealth and American Well.

Patients visiting a participating Rite Aid location can use the NowClinic technology, with the pharmacist’s assistance, to discuss healthcare concerns and questions. Discussions with a nurse are free of charge for a limited time. Discussions with a physician are $45 for a 10-minute session and can be extended for an additional charge. Physicians may make simple diagnoses and prescribe noncontrolled medications if appropriate, which can then be dispensed at any pharmacy. An electronic record of the conversation can be sent to the patient’s primary-care physician, if he or she has one.

Are Pharmacists Healthcare Providers?

Most pharmacists see themselves as healthcare providers, but the Social Security Act does not. This has been cited as a barrier to pharmacists’ participation in new healthcare models, which may include telemedicine.

National pharmacy organizations, including NCPA and NACDS, have asked the Center for Medicare and Medicaid Innovation (CMMI) to use its authority under section 1115(A) of the Social Security Act to “expand the role of pharmacists in new ACO models, community-based transitions of care and bundled payment initiatives.”

However, the CMMI needs to hear from more of us to make this a reality.

These screenings are not directly tied to the pharmacy, but patients who receive the screenings could easily stop by to talk with the pharmacist about their results.

Critics of this type of kiosk argue that they are primarily used as an advertising vehicle, but they do provide some valuable health information and may cause the patient to engage with a healthcare provider. As the FDA considers additional products for OTC status, the availability of patient’s clinical data could help the pharmacist in appropriate product selection.

As with many pharmacist-delivered services, documentation, billing, and payment for services remains a challenge. Many pharmacy dispensing systems do not have the interfaces to store and track the clinical data from these remote monitoring technologies. Two states, Kansas and South Dakota, require Medicaid to cover telemedicine services, including those related to “pharmacological management services.” While these do not specifically call out pharmacists as the providers of these services, this represents one potential source of

payment for pharmacists.
