The Smartwatch

find the amount of press Apple has been getting with the introduction of the Apple Watch to be interesting. This smartwatch will have a base price of $349 and go up from there to $17,000 for the gold edition. I wonder how many of these they will sell, when we all know how quickly current-day technology becomes obsolete? While the Apple Watch isn’t the first smartwatch to come to market, it is more expensive than other offerings. And from what I have read, it requires, at a minimum, owning an iPhone 5. Then of course the watch has to be recharged at least once a day. Interestingly, it comes with a health-monitoring feature.

I am sure Apple will sell millions of these watches because of the loyal customer following the company has developed over the years and the cachet that will go along with having one on your wrist. But will it be the demise of the market for a Rolex or other luxury watches that are handed down, generation to generation? I doubt it.

No question, Apple is just an amazing company. It currently has the highest market capitalization of any publicly traded company. And it was recently added to the list of companies in the Dow Jones Industrial Average. That by itself is testimony that the company has arrived. Apple has come a long way since the introduction of the Macintosh computer. The graphical user interface (GUI) introduced with the Mac changed how we interact with computers. As an aside, we have been a Mac shop here at ComputerTalk since the beginning. Early on this put us at a disadvantage when Microsoft began to control the desktop market. Times have changed, however.

Times have also changed in the pharmacy market with the use of computer technology. Many companies have found a specialized niche where they can offer add-on services that improve one of the three areas where technology applications have been so instrumental in bringing about change. These areas are clinical management, financial management, and operational management of a pharmacy.

So will the smartwatch be another way for pharmacies to stay connected with the consumer? Will this change the way we use healthcare-specific apps that the vast majority of system vendors support? The smartphone was a driver of these apps. I would imagine that the screen size of a watch will place limits on what will work and what will not. But I am certain we will see advances made here.

In the years that I have been involved with this market, the innovation that has taken place and the positive impact it has had on pharmacy have been impressive.

As you read the company profiles in this year’s Buyers Guide, you’ll see that what’s being offered continues to position pharmacies to have a profitable presence in the community, with a more satisfying customer experience. CT

Interesting Books and More

I

read a lot. Everything from emails and text messages to thick hardbound biographies. No novels. My life is already too full of interesting things that haven’t been dreamt up by someone else’s imagination.

Recently, I came across a couple of books that are very interesting and well written. One is about the people who “invented” computers. The other is about the people who “created” Obamacare. Both examine how things get done and how people interact in the process.

I think if you ask the general public, “Who invented the computer?” most of the answers would be Bill Gates or Steve Jobs. Or, “Who created Obamacare?” Obama, of course. Both are wrong answers. Read these books to understand why.

The Innovators, by Walter Isaacson, is about the people whose individual efforts and, more importantly, collaborations have brought us to where we are and are pointing us to technology’s future. Isaacson wrote the authorized Steve Jobs biography. The Innovators is a collection of brief but thorough biographies of the primary contributors to computer development. It starts with Ada, Countess of Lovelace (1815–1852), and Charles Babbage (1791– 1871), and their collaboration working through the logical and mathematical ideas that are the essence of computers.

Ada was able to envision much of what we are experiencing today.

America’s Bitter Pill, by Steven Brill, chronicles the complicated tangle of forces, intrigue, and big-time political maneuvering that gave birth to the Affordable Care Act, aka Obamacare.

manufacturers, and hospitals) got most of what they wanted, and the healthcare reformers got a national health plan. The reformers backed off on cost savings because they could not get system reforms if it cost the “powers” too much money. This book details all of the inner machinations of what it now takes to get something this big off the ground. Who said what to whom in the local watering hole or wherever else. How much it cost (money) to get agreement.

Different books ­— Common Focus There is a characteristic that these books share. Both gather and analyze events, seeking a deeper understanding of the creative processes. Inventions are not “light bulb” ideas that occur all of a sudden. They are cooperative developments that grow over many years and involve many people, each contributing his or her own thoughts and perspectives. We need cooperative development in order to move forward in the provision of healthcare. We need it at all levels. From the highest levels of government policies, to what happens during individual patient encounters, collaborative development is sorely needed.

Healthcare delivery is changing. The focus is changing to knowing and treating the patient, instead of focusing on the disease. “Precision care” is the new buzzword. We will see healthcare providers talking about “John Smith” instead of “the colostomy in 316B.” Healthcare providers are transitioning from being small business owners to being salaried employees of larger organizations. In recent years physicians have moved from

being employers to being employees. Some because they would rather take care of patients instead of employees, others because they could not deal with all of the complicated insurance paperwork. Hospitals also are going through a huge change. Charging for each aspirin tablet is going away. “Does the patient come back too soon?” is the new measure of poor care. (In the prison system it’s called recidivism — aka patient not cured.) Hospitals are also going through a lot of organizational restructuring in an attempt to find the best mix. Outpatient as well as inpatient? General and/or which specialties to emphasize? Be part of an insurance program or deal with all of them? A Checklist for Patient Engagement

M

ost of you reading this column probably have a website for your pharmacy and a Facebook page, and your pharmacy management system allows you to send out an occasional email. If this describes your operation, and you feel that these activities put you into an elite group of practitioners who are way ahead of your competition, we’re here to tell you that your competition, especially the large chains, has gone so far beyond you in the world of connectivity that it’s time for you to consider moving to a level that improves your patient communication, engagement, and participation activities significantly. We suggest that you work through the following checklist and consider your priorities for adopting our suggested changes.

1. Pharmacy Website. We anticipate that most of you have a website that could best be described as a placeholder, because the content rarely changes and it serves to tell people your location and hours and perhaps a telephone number or email address. You may even have a picture of your smiling employees. The old Internet website for pharmacists was primarily a place for people to read static information before having to take a brick-and-mortar journey to visit your pharmacy and conduct a transaction. Current pharmacy websites should support transactions such as e-commerce and allow patients to have a place to document their medication profiles to include legend, overthe-counter, and supplemental products. Patients should also be able to record their experiences regarding the medication you provide them. Your homework assignment is to visit the website of your closest chain competitor and create a listing of all of the features provided on that website, including the download of mobile apps and convenience support. Pick from this list the things that would make the greatest difference in your operations and price what it would take to add these features.

2. Social Media Presence. A recent study found that the first thing that one-third of all women do in the morning upon rising is to check their social media sites, such as Facebook, Twitter, Pinterest, Instagram, LinkedIn,

YouTube, or Foursquare. If you don’t know what every last one of these social media sites represents, we suggest you get busy with Google and find out. Every one of them represents an opportunity for you to connect and promote relationships and business with your patients. Katy Perry has 60 million followers on Twitter and is now commercializing her presence online. Don’t be confused by your desire to avoid “friending” your patients. Facebook can be made into a pharmacy-level connection by having a separate presence for your business and your personal life. Every posting can remind your patients that you are not only there, but also wanting to connect with them. It costs you nothing to post a video describing what compounding can do. It costs you nothing to create posts of interesting photographs that depict your art of practicing.

Electronic Distribution of Professional Labeling Coming

T

he Food and Drug Administration (FDA) issued its proposed rule for the electronic distribution of prescribing information on Dec. 18, 2014. The rule is entitled, “Electronic Distribution of Prescribing Information for Human Prescription Drugs, Including Biological Products,” (see link at the end of this column). The FDA says the purpose of the proposed rule is to be sure that health professionals have access to the most current prescribing information for prescription drugs at the time of clinical decision-making and drug dispensing. Further, they note the action is in keeping with other FDA initiatives since 2003 that require various electronic submissions for drug approvals and listings, and Department of Health and Human Services actions to provide electronic health information to healthcare professionals, patients and the public.

The major provisions of the proposed rule include: Requiring

manufacturers to submit prescribing information to the FDA for posting on the agency’s publicly available “labeling repository” website, labels.fda.gov. For new drugs, the information would be submitted prior to the product being sold. For existing drugs, any changes to current labeling would trigger a resubmission within two days of FDA approval of the labeling change.

The agency is allowed to create exemptions to the electronic labeling requirement in situations where it could adversely affect the drug’s safety, effectiveness, or purity; if it is not technologically feasible; or is otherwise inappropriate. The example the FDA gives is for a product that is intended to be used in an emergency room or is stockpiled for emergency use. In these cases the prescribing information may be distributed in paper form with the product or attached to the product’s packaging.

In accompanying discussion, the agency clarifies a number of issues related to the rule. First, prescribing information may contain instructions for the pharmacist for administering, reconstituting, mixing, diluting, or other product preparation. The FDA says that if the information is not concise enough to be printed on the product’s packaging, then often the pharmacist is referred to the product’s labeling.

In the proposed rule, the FDA provides a summary of comments from an April 2, 2007, hearing they held on electronic prescribing information, noting that these comments were taken into account during the rules draft. Of note, with regard to cost/benefit, the agency anticipates annual cost savings to industry of $52 million to $164 million from

Eighty-one percent of the pharmacists noted they use the prescribing information when counseling patients or verifying product dosages. Chain pharmacists said they primarily use corporately provided prescribing information rather than accessing the information directly at manufacturer websites, as other pharmacists reported.

Of those surveyed, only 6% said they use paper labeling as a resource exclusively, and 4% said they did not have Internet access. Second, the agency said their analysis of safety labeling changes showed there are approximately 500 made annually, making immediate access to current product labeling very important. E-Prescriptions Leading to Uncharted Waters

E

lectronic computer-to-computer transactions have impacted the way pharmacies conduct their business. The biggest impact was from the introduction of online claims adjudication 25 years ago, which replaced universal claim forms. Remember them?

Now there is the use of e-prescribing for new prescriptions, including controlled substances. The main benefit of using this technology is patient safety. However, as technology helps solve problems, it may also create new problems.

One specific example is from New York state and will likely be a model for other states in the future.

New York In New York, legislation known as I-STOP (Internet System for Tracking Over-Prescribing) required all prescriptions, including those for controlled substances, to be transmitted electronically beginning March 27, 2015.

However, the Medical Society of the State of New York asked the state legislature for a delay to allow all prescribers more time to have the required software for controlled substances installed. Two weeks before the March 27 deadline, the governor signed a bill to delay the electronic prescribing mandate for one year.

Challenges One concern aired by New York pharmacists is how e-prescribing systems used by prescribers will allow the two parties to communicate outside of the relatively simple process of transmitting a new prescription to the pharmacy.

For example, what happens when a pharmacy receives an electronic prescription for a controlled substance and is unable to fill that prescription for any reason? The pharmacy is unlikely to print out the prescription for the patient to take to another pharmacy, and if it did, could another pharmacy accept it? The pharmacy, in this case, wants to let the

prescriber know promptly about this, but does the communication back to the prescriber need to use the e-prescribing route in reverse? While it is probably not required that the return communication be electronic, the processes pharmacists and prescribers are asked to use should be consistent.

The NCPDP SCRIPT standard allows for over 20 different types of transactions between prescribers and pharmacies. Transaction types include allowing the pharmacy to send a message to the prescriber requesting additional refills for an existing prescription, or a new prescription, for a patient. Another example is a transaction that can be used when the pharmacy needs to request a change in the original prescription, such as allowing for generic substitution. However, if the systems used in the prescriberâ&#x20AC;&#x2122;s office or pharmacy are not set up to create and process these transaction types, then this leads to the old-fashioned method of communicating by phone.
