hat’s the number of years we have been publishing this buyers guide issue. And what changes have taken place over the years. The amount of functionality featured in the systems back then pales in comparison to what’s offered today. Pharmacy has changed over the years, and so has the technology being offered.

ComputerTalk (ISSN 07363893) is published bimonthly by ComputerTalk Associates, Inc. Please address all correspondence to ComputerTalk Associates, Inc., 492 Norristown Road, Suite 160, Blue Bell, PA 19422-2339. Phone: 610/825-7686. Fax: 610/825-7641.

Because pharmacy is a dynamic profession, and because our federal government continues to pass legislation that requires adaptation to new rules, the companies behind the technology are always busy keeping up. One example is having their e-prescribing software audited for compliance with the DEA requirements for electronic prescribing of controlled substances. With front-store POS systems it is SIGIS, IIAS, and PCI compliance that comes into play. Another example would be 340B programs and the special inventory management requirements. Next up is the conversion to ICD-10 from ICD-9 codes, come October of next year. And in addition to that will be the new track-and-trace requirements in order to keep the supply chain free of bogus drugs.

Then there are the biotech drugs to contend with. The emergence of these drugs has spawned a new type of pharmacy called specialty pharmacy, where there’s particular emphasis on ensuring that the patients receiving these drugs are taking them as prescribed and getting the expected outcomes. Because of the expensive nature of biotech drugs, this places new demands on the inventory management of these drugs, and requires more rigorous documentation and reporting by the pharmacies managing the patients on these drugs. Biotech drugs will eventually eclipse the drug spend on traditional drugs by plan sponsors.

I see specialty pharmacy as providing more evidence of the importance of pharmacists as therapy management specialists. Slowly but surely pharmacy will be a recognized healthcare provider under Medicare. There has been a push for this recognition for a number of years now, but recent events are showing how pharmacists need to be recognized. To this point, there was an interesting article in the Sunday, Feb. 16, issue of The New York Times Magazine, where a woman was in and out the E.R. several times with acute diarrhea. The doctors were having a hard time pinpointing a diagnosis. Numerous tests were done, as well as a CT scan of her abdomen and pelvis. They thought she had celiac disease. It was a pharmacist, on the team making the rounds, who suggested that her condition might be tied to Benicar, the medication she was taking for her high blood pressure. In researching the drug the pharmacist found an F.D.A. warning that described patients who had the same symptom that looked like celiac disease. The patient was taken off the drug and her condition has since improved — end of story. CT

The most important thing about your DATA is the story it tells. Are you listening?

Liberty Software RXQ — A Next-Generation Pharmacy System Have you outgrown your pharmacy system? That’s what many community pharmacies are asking themselves as they experience “growing pains.” While your pharmacy staff fights insurance claims, error messages, uncooperative doctor offices, and a computer system that seems slower than it used to be, your customers still expect the same fast service they have always received from you. Many pharmacies are realizing that their computer system cannot keep up with their growing business. That is why so many pharmacies are making the switch to Liberty Software. Next-Generation Pharmacy Software Here at Liberty, we believe that RXQ is a next-generation pharmacy management system. Why do we say that? It’s not just the technology, although RXQ is built on Microsoft’s newest technology platform. It’s not just the feature list, although it is comprehensive. More than anything, we believe that it is the user experience. Prescriptions can be filled quickly and accurately. Many pharmacists have told us it is the easiest system they have ever used. Point of Sale Designed for Pharmacy For your pharmacy, point-of-sale software needs to be more than cash drawer management for your front end. For your patients, it should enable your staff to provide better service. For you, it should result in better accountability, assistance with HIPAA compliance, and improved pharmacy management. These are values that not just any point-of-sale package can deliver. That’s why we developed the BZQ pharmacy pointof-sale software from the ground up: designed especially for community pharmacies’ needs. BZQ is not simply an add-on to your pharmacy system, but a completely integrated solution

that works hand-in-hand with the RXQ pharmacy system. Simple and Easy How long does it take to teach new employees how to operate your existing pharmacy software? Pharmacies using RXQ have found that new employees can often begin filling scripts immediately, with little to no training. It’s just that simple. A welldesigned pharmacy system should make your employees more productive and happier. When you do need help, it’s only a phone call or mouse click away.

Medicine-On-Time Compliance is Our Only Business Medicine-On-Time Compliance. Adherence. Persistence. The terminology may vary, but the concept is the same. The right pill in the right patient at the right time — every time — over the full course of treatment. As you well know, high levels of compliance are associated with better health outcomes and lower overall healthcare costs; low levels of compliance are associated with increased morbidity and mortality and much higher overall healthcare costs. Compliance Has Value Compliance has value. And as a result, compliance is a natural business opportunity for the community pharmacy. But how do you go about establishing an effective and profitable compliance program in your pharmacy? The answer is Medicine-On-Time. Medicine-On-Time’s specific brand of compliance provides proven protection against nursing home admissions, hospitalizations, and emergency room visits. Medicine-OnTime prolongs independence and provides convenience and peace of mind to your retail ambulatory customers. Turnkey Program For 26 years we’ve been teaching pharmacists how to make compliance a profit center in their pharmacies — generating fees separate and apart from the reimbursement on drugs. Medicine-On-Time is often compared to compounding. It’s neither traditional retail dispensing, nor is it traditional institutional dispensing. We provide the system — computer hardware and software, efficiency tools, comprehensive training, and marketing support — in a complete turnkey package. Only $395 up-front gets you started. Less than 25 patients will get you past break-even. Ahead of the Curve Medicine-On-Time pharmacies have been multidosing since the USP created standards back in the 1980s. Medicine-OnTime pharmacies have been synchronizing refills for decades. Medicine-On-Time pharmacies have been distributing charting forms via secure email for years. And now, Medicine-On-Time pharmacies can send text-message reminders to patients and caregivers via their cell phones. Join our pharmacy network, and your pharmacy will be ahead of the curve, too.

With Medicine-On-Time your pharmacy won’t be tied to one dispensing format like other systems. You have the option to dispense in a weekly format with up to five administration times in a single card. Or in a monthly format with up to 35 days in a single card. There’s even a sequential format for single drug PRN or Stat therapies. Medicine-On-Time software can accommodate even the most challenging and complex dose schedules. Do you need a cycle anchored to a specific day of the month? Or a specific day of the week? Medicine-On-Time has you covered. Cycle length can be fixed or variable — specific to a patient, a group of patients, or universal throughout the system.

When your customer says, “Can you…?” Say “Yes!” with Medicine-On-Time. The Opportunity Of course, the largest market for Medicine-On-Time is the independent ambulatory population — seniors living at home, at risk for institutionalization. Many of these folks are already customers of your pharmacy. But Medicine-On-Time is welcomed in many other populations, too, helping you capture the most heavily medicated segments of your market and turning them into loyal pharmacy customers. Other targets include assisted-living facilities and small group homes for the elderly; group homes for adolescents; group homes or community training homes for the mentally disabled; private home care agencies that oversee medication administration; HIV/AIDS clinics; local hospital discharge planners (to reduce readmission rates for their patients); adult daycare centers; adult foster care agencies; therapeutic foster care agencies (for children and adolescents); school districts; summer camp programs; PACE programs; and veterinarians. The Bottom Line The bottom line: For $395 per month (plus tax) and about 50 cents per prescription (disposable packaging cost), Medicine-On-Time can be the cornerstone of a successful compliance program in your pharmacy. What are you waiting for? Visit medicine-on-time.com today. Join our growing network of proud progressive independent community pharmacies.

Powerful and Infinitely Flexible Medicine-On-Time is powered by robust workflow software that integrates seamlessly with the top pharmacy management systems (PharmaServe, QS/1, HBS, Lagniappe Pharmacy Services, Rx30, etc.), or it can be used independently, as a stand-alone system. 24

QS/1t Multi-Site Management (MSM™) — Centralized Management for Multiple Locations

The software for up-to-the-minute management of multiple locations. Built on the Microsoft® SQL Server®, QS/1’s MSM offers real-time, systemwide control. Perform administrative tasks from one location and gain actionable data from one or all locations. Make MSM, paired with a QS/1 Pharmacy Management System, your strategy for highly effective, organization-wide operational management.

Central Management MSM helps overcome today’s economic challenges and the ongoing need to do more with less. Central management eliminates redundancies and streamlines distribution of pricing, third-party plans, drugs, clinical updates, employee access, staffing, drug price updates, frequent-shopper reward programs, and more. MSM provides flexible control on a global or per-site basis to help your business maximize resources. Share Databases For pharmacies using QS/1’s NRx® or PrimeCare® for LTC, MSM’s optional Central Database module securely shares key databases, including drug, pricing, patient, prescriber, employee, gift card, coupon cards, POS item file, and frequent shopper. Enhance customer service and patient safety with corporate-wide clinical checks of patient profiles during dispensing. The Central Prescriber database prevents duplicating doctors already in the system. Flexible Reporting MSM compiles data into one database so you can easily generate a range of pharmacy, point-of-sale, financial, and business reports. Automate report generation and save reports to file formats including Microsoft® Word, Excel, or Adobe® PDF. Utilize MSM’s standard reports or create customized reports through the SQL database using off-the-shelf software.

“QS/1 gives us the opportunity to compete with the big guys.” Chad Corwell, M.B.A. Director of Operations The PharmaCare Network

Interactive Voice Response System (IVR)

Seamless integration maximizes productivity. QS/1’s integrated IVR can save time and limit disruptions in the pharmacy each day. IVR’s management of inbound and outbound communication is one reason IVR continues to lead the list of productivity-enhancing pharmacy tools with an excellent ROI. Combine it with the RxMedic ADS ® robot and arrive each morning to refills already processed, labeled, and capped.

Productivity Enhancing QS/1’s IVR utilizes QS/1 Pharmacy Management Systems’ unique InstantFill® feature to automatically queue and process refill requests and print labels, so all you have to do is dispense the medications. Advanced Security Features IVR’s advanced security features help prevent refill fraud and dataentry errors. QS/1’s IVR prompts customers to enter one of three security options if they are calling to request a refill. The IVR system authenticates the request by comparing and validating the prescription number and security item. Multiple IVR Options QS/1’s IVR is available for a single location, or manage multiple locations with one phone number with our Central IVR. With Central IVR, the caller chooses a specific location after the initial greeting. With QS/1’s MultiScript IVR, each location has a unique telephone number and greeting. Compliance-Boosting Tools Use IVR’s outbound calling features to notify customers that prescriptions are ready for pickup. When paired with QS/1’s Health-Minder®, IVR can notify patients of prescriptions overdue for a refill with an option to request a refill on the same call. Notify Patients by Text, Email, or Phone With QS/1’s IVR you can provide outbound notifications through text, email, or phone. „„ Phone: Call customers via their home or mobile phones, and leave a voicemail if necessary. „„ Text message: Text customers on their mobile phones. „„ Email message: Contact customers via their preferred email address.

“The great thing about QS/1 is the seamless integration of its products.” Jeff Harrell, Pharm.D. Peninsula Pharmacies

IVR Benefits

„„Integrate seamlessly with QS/1’s NRx® Pharmacy Management System and SystemOne® for HME for enhanced productivity. „„Provide customers 24/7 access to prescription refill requests. „„Decrease pharmacist-answered phone calls by up to 85%. „„Save customers’ voice messages with their refill requests. „„Play, store, and retrieve doctor and patient messages without accessing a separate voicemail system. „„Utilize QS/1’s InstantFill to automatically move prescriptions through workflow with minimal intervention. „„Optional after-hours-only mode handles requests when the pharmacy is closed. „„Inform customers of pickup times based on your pharmacy hours. „„Automatically notify customers of prescriptions awaiting pickup. „„Notify customers of any prescriptions overdue for a refill. „„Send an outbound phone, text, and/or email notification. „„Customize voice scripts in a variety of languages and dialects. „„Route refill requests directly to the prescriptionprocessing queue. „„Make one call to QS/1 for service and support for pharmacy management and IVR.

RxMedic ADS® Robot

RxMedic ACS® Counting Solution

Fully robotic dispensing of up to 256 different oral solid medications, with built-in drug verification photos, cap application, and cells that automatically calibrate, makes it easy for a pharmacy to change out drugs on-site in minutes. ADS selects the appropriatesized vial, prints and applies the label, then counts and dispenses pills. Before capping, it stores a digital image of vial contents. A large, configurable 200+ prescription collating area supports overnight operations. Each ADS cell counts and dispenses drugs directly into the vial, eliminating the risk of cross-contamination. ADS uses a unique vacuum technology and HEPA filtration to provide unprecedented air quality.

Simultaneously counts multiple medications in scalable, space-saving designs that support 12 to 96 oral solid medications. ACS can precount minimum quantities and offers a fast, fully documented return-to-stock feature, as well as storing digital images of vial contents. Dispense filled prescriptions in any order by simply scanning the prescription label. ACS dispenses the drug directly into the labeled vial. Choose an ACS that fits your workflow: countertop, endcap, or freestanding. ACS supports barcode scanning and has lockable cells for additional security.

Epicor Software INSPIRING RETAIL EXCELLENCE IN INDEPENDENT PHARMACIES Pharmacy Retail. It’s a Whole New World.

In Retail, It’s All About the Customer.

The pharmacy business is no longer just about selling prescriptions. Today’s pharmacies are increasingly dependent on their front-of-store retail operations to maintain margins, fuel growth, and — of perhaps greatest importance — meet the growing expectations of their customers.

Today’s customers expect more. They shop where they find the products they want, when they want them. Where they receive fast, knowledgeable, and courteous service. Where they find competitive prices. And where they feel their patronage is valued and rewarded. Exceptional customer experience begins at the point-of-sale, but extends throughout your operation. State-of-the-art POS solutions provided by Epicor offer high-speed creditcard processing, FSA and EBT processing, line-busting, and electronic signature capture to reduce lines and provide fast, efficient, and secure checkout. Epicor also gives you new ways to keep your customers coming back. Eagle Loyalty rivals the programs offered by the big chains, except with Eagle Loyalty, you can choose how you want to reward your loyal customers with tiered loyalty discounts, print loyalty coupon on the receipt, designate specific items to give extra points, provide rewards if customers purchase a set number of certain products, and give a reward on a gift card. Eagle Gift Card solution helps increase sales and create new customers, all without third-party fees.

Retail is a highly specialized discipline that’s significantly different from your prescription business. Pharmacy chains and big boxes have spent millions perfecting retail methods and practices that allow them to operate efficiently, market effectively, manage their assets, and create a very positive customer experience. Fortunately for you, so has Epicor®. Epicor Eagle® is a leading point-ofsale and retail business management solution proven to give independent pharmacies the specialized capabilities and advantages needed to compete, thrive, and grow, and includes integration with all major pharmacy management systems and wholesalers.

QS/1t Point-of-Sale

Boost profitability with greater front-end control. QS/1’s Point-of-Sale (POS) provides tighter control of front-end operations and profits. Your customers’ credit cards are secure, and they can use their flexible spending account (FSA) cards without sending in receipts. Boost profitability through better inventory management and electronic price updates. Pair QS/1’s POS with a QS/1 Pharmacy Management System for an integrated, store-wide management solution. Integrated Pharmacy and Retail Operations

QS/1’s POS integrates seamlessly with QS/1’s NRx® Pharmacy Management System. View prescription status at the register and alert cashiers to additional prescriptions awaiting pickup. Cashiers can charge prescriptions to pharmacy accounts receivable from within POS. Remote Checkout®

QS/1’s Remote Checkout offers a solution for many pharmacy needs. Whether it’s a hospital pharmacy offering bedside checkout to its patients or a retail pharmacy offering drive-thru service or delivery, Remote Checkout can scan prescriptions, process credit and FSA cards, capture signatures, and print a receipt. This solution can also be used as an additional POS system during peak business hours. Customer Loyalty Programs

QS/1’s POS makes managing a customer loyalty program easy. Track preferred-customer purchases, assign reward points, and manage special discounts and sales. POS also allows you to sell and manage your own gift cards as well as in-house charge accounts. Efficient Front-End Operations

QS/1’s POS can save time on transactions and protect front-end profitability. You can suspend a transaction and then start a new one without interruption. Ensure up-to-date pricing and use barcode scanning to reduce cashier errors. POS maintains a bad-check file and prompts cashiers to collect purchaser contact information. Enhanced Inventory Management

Use POS to streamline inventory management: order and receive electronically, establish automatic reordering levels, and check in inventory. POS compares incoming inventory to the order and displays any exceptions. POS helps maximize cash flow by increasing inventory turns and reducing overstock. Identify fast- or slowmoving items and forecast seasonal and other high-volume trends to maximize supplier discounts. Up-to-Date Pricing

Electronic price updates keep pricing current, but POS allows for the flexibility of manual pricing to reduce slow-moving inventory.

“My POS integrates with my pharmacy system, which integrates with my IVR, and they both integrate with my website. It all works very smoothly.” Jeff Harrell, Pharm.D. Peninsula Pharmacies

POS Benefits „„ Improve inventory management with forecasting, electronic ordering, and price updates. „„Retain an electronic log for the pickup of controlled substance prescriptions (Class 2, 3, 4, or 5 drugs). „„Increase profits with price updates from Hamacher Resource Group and/or your wholesaler. „„Print shelf labels. „„Utilize electronic signature capture at the POS register or with a wireless tablet for PSE log, HIPAA, and HME documents, as well as credit and FSA cards. „„Use a single electronic signature for prescriptions, credit cards, FSA cards, PSE logs, and HME documents. „„Keep an electronic log and capture signatures for purchases of items containing PSE and ephedrine with the Pseudoephedrine Signature Log. „„Provide fast, accurate checkout via touch-screen or keyboard operation. „„Reduce the potential for employee theft with barcode scanning. „„Offer customers a variety of payment options: credit or debit cards, gift cards, and in-store charge accounts. „„Easily manage customer loyalty and promotional programs. „„Add new inventory items with Quick Add. „„Reduce store loss with multilevel security and set limits on individual check cashing, voids, and refunds. „„View prescriptions’ status within pharmacy workflow. „„Remote checkout for hospital bedside prescription delivery or at drive-thru. „„Provide FSA/HRA adjudication with SIGIScertified, IIAS-compliant POS. „„PA-DSS certified with a credit storage option. www.qs1.com 866/761-2201 Fax 800/231-7783 P.O. Rx.com Community Healthcare Network Outcomes-Based Healthcare Rx.com Community Healthcare Network, LLC (CHN), is a newly formed Texas company chartered by Rx.com, Partners, L.P., and affiliates PDX, Inc., and National Health Information Network, Inc. (NHIN). By licensing and enhancing derivative products of the PDX intrapharmacy electronic pharmacy record (EPR) and the PDX Care Rx clinical services platform — as well as NHIN’s standardized data files and reconciliation services — Rx.com CHN has launched the largest patient-centric, portable, interoperable electronic pharmacy record in the world. Today, the EPR contains well over 97 million uniquely identified patient records and in excess of two billion prescription transactions attached to the patients’ clinical and billing profiles in 54 chain and independent pharmacy databases in the PDX Intra-Pharmacy Electronic Pharmacy Record Data Center — housed in two mirrored, active-active data centers in Dallas and Forth Worth, Texas. Contracts are pending for participating pharmacies to also aggregate all their data into a single Interpharmacy, patient-centric, electronic pharmacy record housed in these two data centers. The inter-pharmacy effort allows patients to review their filtered electronic pharmacy record and authorize participating healthcare providers to see all the information aggregated from other participating providers. This authorization allows those providers to add information to the data and store the EPR in HIPAA-secure data centers for patients and providers to continue to review and expand the patient’s healthcare data. Rx.com Community Healthcare Network was chartered to be governed by the participating pharmacies housing their patients’ healthcare information in the Rx.com data centers and the technology companies participating in the integration of their software and services for use by patients and their participating healthcare providers. Rx.com CHN is the end game for a strategy that Ken Hill, Sr., R.Ph., began to develop with PDX customers in the late 1980s. NHIH was chartered as a Texas company in 1991 to standardize data files for all PDX pharmacies so that billing information and PBM remittance advices could reconcile prescription transactions to the penny. PDX and NHIN would remain active in the worldwide effort to create a portable, interoperable electronic pharmacy record—a small but important part of the larger electronic healthcare record—in the ensuing decades.

by-product of clinical services — such as comprehensive medication reviews, disease management programs, and specialty pharmacy clinical documentation—provided to the patient. In other words, the retail clinic software has exactly the same workflow pattern as the prescription filling process. As pharmacy transitions from a fee-based reimbursement model to an outcomes-based model with the implementation of the Affordable Care Act, technology will play a more important role than ever before in the delivery of healthcare. To not just be reimbursed—but to participate in this new healthcare model—pharmacies in every market, of all sizes and volumes, must prove they are improving patient outcomes. And they must fully participate in the exchange of healthcare data with other healthcare providers. To meet this objective—and stay in business—any technology strategy for any pharmacy must include more than a management system. Pharmacies must also: Operate with standardized data files, including new data elements not currently available to the pharmacy industry. ■

Participate in the aggregation of pharmacy and clinical data. ■

Enhance the scope of healthcare provided at retail locations. ■

Provide their patients with 24/7/365 secure, online access to their healthcare records. ■

Submit and bill for the clinical services they provide.

These are the precise offerings available to any community pharmacy in the United States through the Rx.com Community Healthcare Network.

Today.

Recognizing that the workflow creating an electronic medical record (EMR) in a doctor’s office is dramatically different from creating an electronic pharmacy record behind a prescription case, Rx.com CHN is delivering an EMR-EHR system for retail pharmacy clinics that creates both a billing transaction and a clinical transaction for the patient profile and the retail clinic. VUCA Health

Let’s take a minute to talk about... VUCA Health has created the largest and most robust medication video library in the world that educates patients on medication safety. Despite the growing complexity of medications and the advancement of technology, most medication information is delivered today the way it was decades ago. VUCA Health is changing this paradigm by delivering a medication experience like none other: a digital experience to support patients and their families, when they need it and in the media format of their choice.

Introducing MedsOnCue: The majority of patients will discard medication information leaflets after leaving the pharmacy. This often leads to patients’ confusion about where to turn when they have questions regarding their prescription medication. MedsOnCue addresses this problem by providing on-demand and pharmacy-branded prescription medication counseling that answers questions such as correct dosage and side effects. In addition, MedsOnCue dosing alerts are prepopulated for patients and can be emailed or delivered via SMS text messaging, further supporting medication adherence.

1

ACCESS:

Patients can quickly and conveniently access thousands of medicationspecific videos in the MedsOnCue library by clicking on a link in an email or text message, or by scanning a QR code printed on prescription labels and patient information sheets.

2

WATCH:

Available in English and Spanish, videos provide comprehensive information on top prescribed medications, including proper usage, expected benefits, and potential side effects.

3

INTERACT:

One-click access to call the dispensing pharmacy, access medication reminders, and connect to other healthcare resources helps ensure that patients understand how to take their medications safely and limit any avoidable reactions.

The video library, available in both English and Spanish, provides optimal information on thousands of topprescribed medications across multiple conditions, including specialty medications and administration techniques. Videos can be accessed on any computer, tablet, or smartphone, or at designated kiosks or other locations inside the pharmacy or healthcare organization. Videos can also be posted to the pharmacy’s website or even run on a loop in the facility itself—ensuring that the right information reaches the right patient, regardless of technology preferences or existing infrastructures.

For more than two decades, voiceTech has developed pharmacy growth solutions for retail pharmacies. Driven by a sophisticated two-way integration to over 70 pharmacy management systems, these solutions help pharmacies increase medication adherence, customer loyalty, pharmacy revenue and improve patient outcomes — all of which impact the pharmacy’s bottom line. This is accomplished by providing convenient technology-based methods for people to do business with the pharmacy and by letting voiceTech handle the day-to-day operation of connecting with customers of the pharmacy, whether through call, text, email, or mobile app integration.    With the goal of improving patient outcomes, voiceTech created FusionRx-Star AdherenceTM — a series of star services a pharmacy can choose from to create its custom adherence program — because one size does not fit all. When a pharmacy takes proactive steps to increase medication adherence, there are many trickle-down effects, including an increase in customer loyalty and pharmacy revenue. As a bonus, when patients are adherent they pick up their prescriptions in a more timely manner, resulting in a significant reduction in return to stock. Consistently, voiceTech customers report a 50% reduction in return to stock within 60 days, with a high of 84% reported. One of voiceTech’s qualified pharmacy growth advocates can help any pharmacy create its custom Star Adherence program, whether the pharmacy has inbound IVR or not. We can implement a Star Adherence program even if you have an IVR from another vendor. By using a certified interface to the pharmacy system, timely and meaningful messages are sent to patients via their preferred method (i.e., automated outbound call, email, text, or mobile app). Additionally, as you probably know, during voiceTech’s tenure in the pharmacy market its inbound IVR system has been the “best of breed.” Core modules include TeleFill® (integrated phone-in refill requests), ExpressCall-Rx® (automated refill and pickup reminders), WebFill-Rx® (online refill requests), FusionFax-Rx® (automated fax authorizations), and DirectLink-Rx® (expedient refill ordering for long-term care facilities). voiceTech’s pharmacy growth solutions and technology services are available individually or in just about any combination to meet specific pharmacy needs and budget.   Start taking steps today!

Historical Reflections

E

very now and then it is a good idea to stop and think about where we were and where we are. Thinking about how we got from there to here is worth pondering, if only to get a better understanding of ourselves and our surroundings. I hope the following reflections stir some memories of yours that need revisiting.

Electronics I have been reading a book about the

evolution of electronics over the last 50 years or so. It is by T. R. Reid and is titled The Chip: How Two Americans Invented the Microchip and Launched a Revolution. The two Americans in the title, Bob Noyce and Jack Kilby, both figured out how to make the microchip at about the same time — but went about it in opposite ways. The book also covers many side stories and has clear, layman-friendly explanations about how these things work. My interest in what can now only be called the history of computers is strong for two big reasons: 1) I was fortunate to be a friend of Bob Noyce for a couple of years, and he was an amazing man; 2) When my brother and I put together the first stand-alone pharmacy computer system in 1972, we created it using one of the first microchips. Just a few words about Bob Noyce. We both served on the University of California Board of Regents. He and I were the only members who had any science understanding. Things like light-years and nanoseconds flew right over the heads of some intelligent, in other disciplines, members. He was a kind, gentle, forceful, and brilliant gentleman. I was a fledgling pilot; he flew his own private jet. I was trying to keep financially solvent; he was a multimillionaire. We were good friends. If he had lived long

When I graduated from pharmacy school the most sophisticated calculating device was a slide rule. The primary manufacturer of slide rules gave the last one it made to the Smithsonian in 1976. Electronic calculators totally replaced slide rules and were faster, more accurate, and easier to use. My father was a gadget nut. In his preteens he was one of the first ham radio operators in California. My brother Chad and I inherited his love of gadgets. My brother patented several electronic devices, all of them using the cutting-edge technology of the time. I am not the inventive type. I just like to use the gadgets. We got together and decided that many pharmacy functions could be handled by a computer. Chad knew how to make the computer and how to program it. I knew what went on in a pharmacy. He proceeded to design and manufacture the computer components. I went to work charting out all of the things that happen between receiving the Rx and handing the completed prescription to the patient.

His tasks soon turned out to require much more time than he had, so we hired a programmer. I started out with an 8-1/2-by-11-inch sheet of paper and soon found that I needed long sheets of butcher paper to list and relate all of the things that go on in a pharmacy. And that was when the only third party was the Medicaid program. Things have changed.

know why he said 4096? We used to do a lot of extemporaneous compounding — ointments, creams, suppositories, liquids, and capsules. Not much of that anymore. I remember making capsules that had Dexamyl granules (the little beads emptied out of the manufactured capsule) mixed with thyroid powder and dextrose. We mixed the ingredients and punched out the

Pharmacy When I became a pharmacist in 1960

things were a lot different. The name of the drug was never on the prescription container. No patient profiles. We could not refill an Rx without the patient giving us the Rx number. Manual typewriters. If you typed the wrong letter, you had to back up and hit the right one harder. When I paused to think about what I would type next, I always rested my hand on the keyboard, until I began to use an electric typewriter. Resting my hand resulted in the keys all flying at once and getting tangled up trying to reach the paper. Throughout life it seems little habits need to be modified to accommodate new technology. If there were any questions from the patient: “Ask your doctor.” “What’s it for?” “Ask your doctor.” Prices for prescriptions were chaotic. Cost of the drug, plus a percentage, plus a small fee. Prices were from $1.49 to $15.95. Notice they always ended in 9 or 5. If someone asked for a price quote, you would write your price quote on the prescription using the code PHARMOCIST. $1.49 was coded PRS. That way the next pharmacy could be friendly and quote the same price or be hungry and quote $1.39. One day our programmer asked if he would have to program the computer to price prescriptions that were over $40.96. I said: “No. If one does come up the pharmacist will want to handprice it instead of letting the computer do it.” Boy was I wrong. Do you

capsules. How many pharmacies even have the empty capsules on the shelf? We also dispensed a lot of placebos. You could do that because the name of the drug never appeared on the label. We had placebos for most of the Schedule II drugs. The physician would call us and ask if we had placebos for Percodan. We would say yes, and he would say, “Give her 20 tabs; 1 prn pain.” Can’t do that today. I think the current AMA position on placebos is that the prescriber needs to tell the patient that he or she is prescribing a placebo. That sounds self-defeating to me. There were no technicians. There were no computers. And most importantly, there were nowhere near as many drugs and therapeutic options. We have come a long way. Drug interactions were just beginning to be thought about. Changes in Healthcare Demand Different Thinking

T

here’s an old joke about an extension agent who approaches a farmer and tells him that a $10 book on how to farm better will make him twice the farmer he is now. The farmer scratches his head and says, “Heck, son, I don’t farm half as good as I know how to now.” Times are changing in healthcare, and it’s still early enough in the year to add a few resolutions. Think about each of these changes and challenge yourself as to whether you have aligned your practice accordingly.

Less Provider Centric and More Patient Centric In an

environment where better outcomes and cost containment are still dominant, healthcare is finally realizing that without patients who are engaged and participating in their own health, the whole thing goes thud. As a practitioner who has more contact with patients, this should give you an opportunity to be actively involved in participatory healthcare. You can identify, measure, and make interventions to help patients achieve healthcare goals that are important to them and to the healthcare system in which you practice. This isn’t to say that you will have to figure out a way to be competitive in the distribution aspect of your practice. Accountable care organizations (ACOs) are looking to partner with providers who can deliver efficient and effective transactions, but they really need to identify providers who can help manage patients between encounters. ACOs are predicted to double in number during 2014. Check the Internet to see if there is an emerging ACO in your area with which you can affiliate. We believe that pharmacists are uniquely positioned to help patients when they exit hospitals and doctor appointments, and when they begin new therapies. Adherence has been addressed by former Surgeon General C. Everett Koop, who said, “Drugs don’t work in patients who don’t take them.” Adherence is also closely associated with your medication therapy management service offering. The takeaway is whether or not your practice can legitimately claim that you can deliver a patient-centric, observable, and measurable outcome that engages patients and their nonprofessional caregivers whenever possible.

Brent I. Fox, Pharm.D., Ph.D.

Value-Based Reimbursement and Accountability

Accountability used to focus primarily on financial accountability. But clinical accountability and quality measures are receiving new attention, especially as CMS implements the Five-Star Quality Rating System for health plans. Do you believe that your practice is better in these measures than your competitor’s? Can you prove it? Are you tracking what value you bring to patient care? How many reports from your pharmacy management system do you regularly review that would provide you with evidence to make the claim that you bring a higher value with your efforts? How often do you perform follow-up with patients to make sure that the medication prescribed is achieving results? How frequently are you chasing down “no-show” patients on both acute and chronic medications?

Continuous and Coordinated Care How connected are

ONC and HITECH Anniversaries

2

014 marks the anniversary of two profound industry-changing events: the establishment of the Office of the National Coordinator for Health Information Technology (ONC) in 2004 and the passage of the Health Information Technology for Economic and Clinical Health (HITECH) Act in 2009 as part of the American Recovery and Revinvestment Act. ONC is the primary federal entity that coordinates the nation’s efforts to implement and use the most advanced health information technology (HIT) and the electronic exchange of health information. The position of national coordinator was created in 2004, through an executive order, and legislatively mandated in the HITECH Act. Physician and venture capitalist David Brailer served as the first national coordinator. In January 2014 the nation’s fifth national coordinator, Karen B. DeSalvo, M.D., M.P.H., M.Sc., was appointed. Other national coordinators have included Harvard Medical School professor David Blumenthal, psychiatrist Robert Kolodner, and physician and public health expert Farzad Mostashari. ONC is at the forefront of the administration’s health IT efforts and is a resource for the entire health system in supporting the adoption of health information technology and the promotion of nationwide health information exchanges to improve healthcare. ONC’s efforts received a great boost in 2009 with the passage of HITECH, when $49 billion was earmarked to expand the use of HIT. Money was set aside to promote adoption of electronic health records (EHRs) and their meaningful use, as well as for health information exchanges (HIEs). That investment has been successful in moving the nation’s HIT agenda forward, according to DeSalvo. In her recent blog post on HITECH’s anniversary (www.healthit.gov/buzz-blog/category/from-the-onc-desk), she notes these figures:

Marsha K. Millonig, R.Ph., M.B.A.

It will be interesting to see how the data-sharing agreement and consortium infrastructure develop and whether similar efforts will begin as a result. I’ll keep readers posted in my future columns. Eighty-three percent of the nation’s eligible professionals and 94% of eligible hospitals have signed up for the Medicare and Medicaid Electronic Health Records Incentive Programs. Slightly more than 435,000 professionals and 4,690 hospitals are registered for the programs. Over 335,000 professionals and 4,400 hospitals have received incentive payments for adopting certified EHRs or demonstrating meaningful use. In another recent blog post, DeSalvo continued on next page March/April 2014

highlighted key statistics from two recent surveys. They showed: Almost 80% of office-based physicians used some type of EHR system, an increase of 60 percentage points since 2001 and nearly double the percent in 2008 (42%). About half of office-based physicians surveyed said they use a system that qualifies as a “basic system,” up from just 11% in 2006. Almost 70% of office-based physicians noted their intent to participate in the EHR incentive program.

HITECH also set aside nearly $600 million to establish HIEs. I had the opportunity to give an update on HIE status to participants at the annual conference of the American Society for Automation in Pharmacy (ASAP) this past January. Progress continues, and in February plans were announced by 16 state and regional HIEs to collaborate in an effort to build trust and remove barriers to patient data sharing. The effort moves forward a 10-year-old plan to create a national HIE network. The group’s hope is to ensure that important patient information is available at the point of care. The consortium’s next step is to elect leaders and hammer out a data-sharing agreement. What’s Coming with Track and Trace: Part Two

T

he Drug Quality and Security Act (DQSA) was signed into law on Nov. 27, 2013, and in the last issue of ComputerTalk we discussed the requirements to be implemented under the act in the first three years. Within this time frame, guidance documents will be issued, standards for information exchange will be published, wholesalers will become licensed, and transaction information, history, and statements will be provided for each product transaction. However, the law is slated to take 10 years to fully implement. Here, we will discuss the longer-term conditions and implementation issues expected from 2017 through 2023, by which time the act will be fully operational. Starting Nov. 27, 2017, drug products must contain a product identifier — a two-dimensional data matrix barcode that contains information on the standard numerical identifier, lot number, and product expiration date. The standard numerical identifier will comprise the product’s NDC and a unique alphanumeric code of up to 20 characters. The product identifier should be both human and machine readable. The product identifier will facilitate product recalls because it will contain individual lot numbers of a drug. At this same time, manufacturers will be required to respond to verification requests regarding whether a particular product identifier corresponds to that manufacturer’s product. Manufacturers may satisfy this requirement through an electronic database that could be developed and operated either in-house or by another entity, the latter of which may be the most likely course of action. The development and operation of these manufacturers’ databases could potentially be done by one outside entity, but it remains to be seen if this will be the case. If so, this could add an additional security component, since all product information would be centralized. By November 2020, pharmacies will only be allowed to engage in product transactions if the product is encoded with a product identifier. At this same time, pharmacies must be able to check the lot numbers in their inventory to

Ann Johnson, Pharm.D.

determine if potentially counterfeit drugs are present. This will require pharmacies to keep track of the product identifiers in their inventory, and pharmacies may need internally developed electronic databases or have an outside firm design such a database. These requirements lend themselves to the adoption of barcode scanning by more pharmacies, which could be used to track product identifiers. Since many drug products have an expiration date that extends beyond three years, additional issues may be raised if older drugs are still left in the supply chain from before the November 2017 product identifier implementation deadline. Although the act states that exemptions may exist for these products, this introduces another layer of complexity to the act’s implementation. By Nov. 27, 2023, the act specifies that transaction information and statements will be exchanged in a secure, interoperable electronic manner. By this time, it should be possible to trace the transaction information for each continued on next page March/April 2014

drug package from the manufacturer to the dispenser. With this new technology, the entrance of counterfeit drugs into the supply chain will become much more difficult and likely require collusion. Wholesalers and third-party logistics providers will be required to obtain licenses through the government and be subjected to inspections, making it more difficult for questionable parties to enter and participate in the supply chain. Furthermore, a database of authorized wholesalers will be published by the FDA and will be available to the public on the Internet. Not only will the new track-and-trace legislation help to reduce the entrance of counterfeit drugs into the marketplace, it will also facilitate product recalls for contaminated drugs. The addition of the product identifier that contains lot numbers means that manufacturers and/or pharmacies will be able to directly identify patients who have received the recalled product. The new technology will make it easier to reach these patients quickly and efficiently. A rarity in today’s political climate, DQSA received bipartisan support in Congress, and it seems like most parties
