and better sleep, among others — but also improves psychosocial well-being. Aesthetic (cosmetic) surgery can have a positive effect on individuals with anxiety and depression, according to a study in Clinical Psychological Science.

After performing plastic surgery in Santa Fe for more than 25 years, CHRISTUS St. Vincent board-certified plastic surgeon James Green, M.D., has seen plenty of examples of plastic surgery’s benefits for emotional health, especially among women who have reconstructive breast surgery following lumpectomy or mastectomy.

“Reconstructive surgery has a tremendous psychological benefit,” Dr. Green says. “Losing a portion or all of a breast can leave women with significant asymmetry, which may affect how they view themselves. Breast reconstruction can restore a more natural feeling and appearance, and that can reduce the sense of loss they may experience. For these women, breast reconstruction can be an important part of moving forward after cancer.”

Plastic surgery does more than improve physical appearance. Dr. Green and the practice he established joined CHRISTUS St. Vincent last summer. He brought with him a leading-edge technique for breast reconstruction following mastectomy: prepectoral reconstruction. Historically, implant-based reconstruction involved placing an implant beneath the pectoralis major muscle in the chest, but in prepectoral reconstruction, the new component goes above the muscle with no need to disturb its position or function. A 2017 study in Plastic and Reconstructive Surgery found that for women who undergo nipple-sparing mastectomy with immediate reconstruction, prepectoral reconstruction produces aesthetic results similar to submuscular reconstruction but with less postoperative pain.

“Prepectoral reconstruction is incredible because we don’t put implants below the muscle,” Dr. Green says. “I work closely with the breast surgeon at CHRISTUS St. Vincent on reconstruction cases. Patients can often go home the same day after having mastectomy with bilateral reconstruction. In addition to prepectoral reconstruction, I perform other breast restoration procedures, including types that use patients’ own tissue with flaps and expanders.”

a variety of factors, including each person’s goals, age and the three broad options available: skin care, injectables, such as muscle relaxers and fillers, and surgery. Generally speaking, skin care is often a good option for younger patients, he says, whereas middle-aged patients may be candidates for injectables and older individuals may need an approach that incorporates all three options.

Eat, Sleep and Console Transforming care for infants and families affected by neonatal abstinence syndrome (NAS) About 4 percent of babies born at CHRISTUS St. Vincent Regional Medical Center are in opiate withdrawal. Though it could be easy to judge the parents of these children, the hospital's approach to NAS treatment leaves judgment at the door.

“Every mother has a story,” says Jennifer Castaneda-Lovato, RN, clinician-directed performance improvement (CDPI) pediatric nurse at CHRISTUS St. Vincent. “We need to have the heart to help them.”

CHANGING CARE After hearing these stories firsthand, Jennifer wanted to empower families to take care of their children while decreasing the length of their hospital stays and reliance on withdrawal medications.

“Babies used to stay in the hospital for an average of 18 days while undergoing a methadone weaning program,” Jennifer says.

To achieve her goal, Jennifer turned to others for help.

“I found a webinar from a Yale doctor who invented a treatment plan that shortened the length of stay without complications,” says Jennifer Achilles, M.D., pediatric hospitalist at CHRISTUS St. Vincent. “I sent the webinar to Jennifer, and we knew we wanted to follow that model.”

Jennifer and Dr. Achilles led the development of the NAS project.

They brought the doctor from Yale to CHRISTUS St. Vincent for a visit to explain the new care approach to nurses and physicians.

LEADING WITH LOVE Instead of relying on a time-consuming scoring system to determine if babies need withdrawal medication, the new approach stvin.org | Winter 2019

uses a simple three-part system: eat, sleep and console. Withdrawal medication is replaced by cuddling whenever possible.

“If babies can eat and sleep properly and have their symptoms soothed by their parents, they don’t need the withdrawal medication,” Jennifer says. “Instead, the parents are the treatment, which can be continued indefinitely.”

The NAS project has been a tremendous success. In the past, infants who required withdrawal medication received an average of 39 doses during their stay. This number has fallen to seven doses. The average length of stay has also fallen from an average of 18 days to 11 days.

“We’re sharing our success story with other hospitals,” Dr. Achilles says. “I truly believe this will become the national standard of care.”

"The purpose of our Clinician-Directed Performance Improvement program is to give practicing, front-line clinicians the protected time, support and training to do performance improvement projects of their choosing,” says CDPI program director Lara Goitein, M.D. “It empowers them to design our hospital systems to support gold-standard care. The neonatal abstinence program is a great example of the successes we are seeing when we give our physicians and nurses the space to improve our hospital processes."

Award-Winning Care Last September, CHRISTUS St. Vincent accepted the 2018 Quest for Excellence Award from the New Mexico Hospital Association (NMHA) — one of only three hospitals in the state of New Mexico to receive the NMHA’s highest given honor for quality and performance improvement. The hospital was recognized for its Clinician Directed Performance Improvement (CDPI) program — a novel hospital quality program designed to give practicing front-line clinicians protected time, support and training to conduct quality improvement projects of their choosing. The quality improvement project featured by the award was initiated by a pediatrician and pediatric nurse to improve the care of newborns born with dependence on opioids because their mothers’ use of opioids during pregnancy.

“This award is a direct reflection of our team’s determination to improve lives in Santa Fe and Northern New Mexico,” says CHRISTUS St. Vincent President and CEO Lillian Montoya. "We’ve worked hard to empower our physicians, nurses and Associates to be the change they want to see in our hospital, and they have wholeheartedly embraced that mission.”

MRIs for Patients With Pacemakers In October of 2018, CHRISTUS St. Vincent became the first hospital in northern New Mexico to perform MRI testing on a patient with a pacemaker.

In the past, MRIs have been a medical taboo for those with implanted cardiac devices due to concerns that the strong magnetic and radiofrequency fields might damage the device and/or harm patients. By implementing a newly developed protocol for safely mitigating these risks, CHRISTUS St. Vincent has made this valuable diagnostic tool accessible to these individuals.

“This is groundbreaking,” said Yvonne Bieg-Cordova, CHRISTUS St. Vincent director of radiology, “With the ability to perform MRI testing, we are opening up a whole new world of diagnostic and treatment options for these patients. It’s a highly involved process requiring coordination between our cardiology team, MRI team and product representatives, but it’s worth it. We couldn’t be more thrilled.”
