CHANGECONNECT AWARD 2017 APPLICATION FORM Sharing stories and acknowledging excellence is key to elevating the value and role of change management. With this in mind ChangeConnect celebrates the best change management project in South Australia with the biannual ChangeConnect Awards. All types of organisational change projects are encouraged to enter – whether social or organisational and transformational or incremental –- we are looking for projects that have responded directly to the context, showed creative and sound techniques, tackled challenges and influenced stakeholders. In other words, projects with a story to tell and measurables to prove it.

Nomination Thank you for entering the ChangeConnect award. We know you are busy. All good change managers are. So we have kept the application process lean and concise to ensure that the focus is on the situation, approach and outcomes of the change. You are restricted to the length of your responses, so be creative, make sure you show outcomes and most importantly tell a great story of why your project is distinctive and worthy of recognition. Applications over the word count limit will not be accepted. When completing the application, please ensure to: • • • •

Complete all questions to the best of your knowledge Use measurable, metrics and examples to demonstrate the outcomes of the change project Provide work sample(s) to help explain your work Tell the evaluators a great story

Recognise that the change project is the nominee, not the individual nominating. Of course, the change leader and team will be acknowledged for driving the change. Acknowledge that the change project nominated has been implemented (or is at least three-quarters of the way through) with clear measurable outcomes and additional metrics submitted upon request. Approve that the change story can be shared to the ChangeConnect community via social media events, conferences or other activities and communications. Rest assured that we will connect with you before we publish/promote any details about the change project. Consent to be contacted and interviewed to tell us more about the change story. Confirm that the information submitted is true and correct.

Submit the form To submit the form, email the Word document to info@changeconnect.org along with any change project samples (pdf and/or video). Forms must be submitted by close of business 6 October 2017.
