2021 Benefits AllianceRx Walgreens Prime strives to provide a comprehensive and competitive benefits package that aims to satisfy the health and wellness, financial wellbeing and work-life balance values and needs of our team members, so we may provide hope and care for better tomorrows.

Eligibility Team members who are regularly scheduled to work 30 or more hours per week are eligible for:

• Paid time off • Employee assistance program¹ • 401(k) retirement plan² Eligibility begins the first of the month following or coincident with your hire date. For example, if you are hired on April 1, your date of eligibility is April 1. If you are hired on April 15, your date of eligibility will be May 1. ¹Team members automatically participate in these benefits. ²Team members 18 years or older are automatically enrolled in the 401(k) retirement plan with a contribution rate of 3% of eligible pay. Team members can opt out, change the contribution rate or change investment elections at any time online with Fidelity.


Eligible Dependents The following definitions apply to participation in our medical, dental, vision, spouse life, and child life:

• Your spouse to whom you are legally married or joined in a civil union in a jurisdiction where recognized

• Your children under 26 years of age³, including

your natural children, stepchildren, children legally placed with you for adoption, legally adopted children, children for whom you or your spouse are the legal guardian, foster children and children awarded coverage pursuant to a court order

• Your children over 25 years of age who are

unable to support themselves due to a physical or mental disability

³A dependent child’s coverage ends at the end of the month in which they reach age 26.

AllianceRx Walgreens Prime will ask for verification that covered family members are eligible dependents. Team members will be asked to provide documents like birth certificates and marriage certificates. 3

Medical and Prescription Drug Plan Health Savings Account (HSA) or Preferred Provider Organization (PPO): We offer two medical plan options through Blue Cross Blue Shield (BCBS) of Florida. Both plans use the same national provider network. When you use providers in this network, both plans cover preventive care at 100% and pay a greater share of your health care costs than if you get care from outside the network. The main difference between these two plans is how costs are structured. The HSA Medical Plan has a higher deductible and out-of-pocket maximum with lower per paycheck premiums. The PPO Medical Plan has a lower deductible and out-of-pocket maximum with higher per paycheck premiums.

Prescription Drugs Both medical plans use the same prescription drug formulary list. For all categories of drug coverage, an in-network retail pharmacy through Walgreens or mailorder service through AllianceRx Walgreens Prime must be used in order to receive a benefit. There are no out-ofnetwork benefits.

Dental Plan

Dental coverage is provided through Delta Dental of Florida. The plan covers diagnostic and preventive services or orthodontics without requiring an annual deductible first. The plan offers a choice of dental providers, but using Delta Dental providers can keep expenses lower compared to using out-of-network providers.

Vision Plan Vision coverage is provided by VSP Choice Plan. The plan covers an annual eye exam after a $15 copay, as well as prescription lens, frame and contact lens allowances after a copay. 4



Financial Wellbeing 401(k) Retirement Plan Our 401(k) plan, offered through Fidelity, can help team members achieve a financially secure future. The plan’s streamlined investment options make it easy to choose. Eligibility begins 30 days after date of hire. Team members are automatically enrolled to contribute 3% of eligible pay pre‑tax, and are enrolled in an automatic increase program that increases the contribution 1% each year, on Nov. 1 until the contribution reaches 6%. Team members can change contribution rates at any time on a pre-tax basis or on an after-tax Roth basis. Team member contributions are immediately vested. AllianceRx Walgreens Prime will match team member contributions dollar for dollar up to the first 4% of eligible pay (up to the IRS limit). The company’s matching contributions will become 100% vested after two years of service.

Disability Insurance Basic Short Term Disability For team members unable to work for more than seven consecutive days due to a covered disability, short-term disability (STD) pays a percentage of their basic weekly earnings. Coverage may continue up to a maximum of 25 weeks, for a total of 26 weeks (180 days) of disability. Eligible team members are automatically enrolled in this coverage and the company pays the cost.

Basic Long Term Disability For team members that remain unable to work for longer than 180 days due to a covered disability, the long-term disability (LTD) benefit begins and pays 60% of their basic monthly salary, with a combined maximum monthly payout of $15,000 across Basic and Supplemental LTD. Eligible team members are automatically enrolled in this coverage and the company pays the cost.

Supplemental Long Term Disability Eligible team members may choose to purchase additional LTD coverage to increase the LTD benefit to 66.7% of their basic monthly salary ($15,000 combined monthly maximum across Basic and Supplemental LTD). 6


When you first become eligible for Supplemental Long Term Disability plan, you can elect coverage without having to provide evidence of insurability (EOI). EOI will be required for any coverage purchased outside of your initial eligibility period.

Life and Accidental Death and Dismemberment Insurance (AD&D) Company-Paid Basic Life and AD&D The basic life insurance benefit is 1.5 times annual salary, with a minimum of $50,000 and up to a maximum of $750,000. If death is a result of an accident, the accidental death and dismemberment policy pays an additional benefit of 1.5 times annual salary, up to a maximum of $750,000. Eligible team members are automatically enrolled in this coverage and the company pays the cost.

Team Member-Paid Supplemental Life Team members may choose to purchase supplemental life insurance for themselves, eligible spouses and eligible child(ren) in the following amounts:

• Team member: $10,000 increments up to $750,000 • Spouse: $10,000 increments up to $500,000 (cannot exceed amount of team member coverage)

• Child(ren): $2,500 increments up to $25,000 When you first become eligible for life insurance, you can purchase up to $300,000 in coverage for yourself and up to $50,000 in coverage for your spouse without having to provide evidence of insurability (EOI). EOI will be required for coverage over these amounts, for any subsequent increases in coverage and for any coverage purchased outside of your initial eligibility period. EOI is not required for child life insurance coverage.

Work Life Balance Employee Assistance Program The employee assistance program (EAP) is a confidential resource for you and your family members that is provided to you at no cost. You can get information and referrals to counseling for personal issues like relationship or family conflicts, job-related stress, money problems and more. The EAP can also help with childcare, college payment options, retirement planning and career development. It’s available 24 hours a day, 7 days a week, at no cost to you.

Adoption Assistance Team members who are scheduled to work 20 or more hours per week are eligible to participate in the adoption assistance program. Through this program, AllianceRx Walgreens Prime will reimburse up to $5,000 of adoption expenses you incur while you’re employed by the company. Eligible expenses include necessary adoption fees, attorney fees, court costs and other expenses directly related to the adoption of a child.

Education Reimbursement Team members who are scheduled to work 20 or more hours per week are eligible to participate in the education reimbursement program. Through this program, the company will reimburse up to $4,000 per calendar year for undergraduate classes or up to $5,250 per calendar year for graduate-level classes. To be reimbursed, courses must be part of a degree program and directly related to your position or our business. A grade of C is required for undergraduate courses. A grade of B- is required for graduate classes.

Holiday Pay AllianceRx Walgreens Prime recognizes the following paid holidays in 2021:

• New Year’s Day • Memorial Day • Thanksgiving Day

• Independence Day • Labor Day • Christmas Day

Additionally, team members will have two floating holidays to use within the calendar year. These holidays will allow you to celebrate the days that are important to you and your family.

Paid Time Off (PTO) Our time off programs let you manage your time off for vacation, illness and occasional absences from the scheduled work day. PTO is available if you are a non-exempt team member. Your PTO will accrue each pay period based on hours worked. The maximum PTO you can accrue is listed below. Once you hit the maximum, you will not accrue more until you use some. The actual PTO accrual benefit may vary. PTO calculates by applying the accrual rate to the actual hours worked. In addition to regular hours worked, PTO accrues on other company-paid time off including but not limited to company-paid holidays, paid bereavement leave, jury duty and overtime hours worked.

Flexible Time Off (FTO) Our FTO is available if you are an exempt team member. Exempt team members may request flexible time off as they see fit. That means nothing accrues and nothing expires. FTO is not an unlimited amount of time off. Everyone must ensure that the needs of our patients and the business are being met when requesting and using FTO.
