EDITORIAL A

Small Wonder I n t h e a f t e r m a t h . . . sounds l i k e a d i s a s t e r r e p o r t , b u t no - J i m Green d i d a n exemplary job.

Anyone r e a d i n g t h e " p o l i t i c a l analyses" t h a t ran rampant f o l l o w i n g t h e C i v i c E l e c t i o n g e t s a l o t of numbers and p e r c e n t a g e s and comparisons and on and on and on.

The t r a d i t i o n a l b o r d e r l i n e between e a s t & w e s t seemed t o be c l e a r c u t w i t h t h e " r i c h " on t h e west and t h e rest of u s on t h e e a s t s i d e of t h e c i t y . J i m g a t h e r e d s u p p o r t i n every neighbourhood, from p e o p l e who had never v o t e d a n y t h i n g b u t NPA s i n c e t h e y were o l d enough t o v o t e .

There w e r e a few t h i n g s t h a t k e p t Jim from g e t t i n g t h e m a j o r i t y o f v o t e s , i n c l u d i n g , a s u s u a l , t h e lower t u r n o u t o f people who g i v e l i t t l e more t h a n v e r b a l s u p p o r t , and a l i t t l e number t h a t t h e N.P. A. c a n d i d a t e s d i d on t h e morning o f t h e e l e c t i o n .

I n S t r a t h c o n a , between 10 & 1 2 on Saturday morning, e v e r y household and apartment i n t h e e n t i r e a r e a had a s i n g l e p i e c e of p a p e r hand-delivered t h a t p r a i s e d t h e v i r t u e s of Campbell and h i s s o - c a l l e d " e f f o r t s " t o make Chinese-Canadians e q u a l s , whatever t h a t means.

It was t h e same kind of NPA propaganda t h a t f l o o d e d mailboxes and TV s t a t i o n s and r a d i o f o r weeks b e f o r e t h e 1 7 t h . It ended w i t h a c l e a r demand t h a t p e o p l e had t o v o t e

f o r Campbell i f they wanted t o be a 3 . p a r t o f t h e f u t u r e of Vancouver.

Thousands o f e l d e r l y Chinese c i t i z e n s went t o t h e p o l l s . The v o t e w a s almost e n t i r e l y uniform: COPE f o r aldermanic c a n d i d a t e s , Campbell f o r Mayor.

COUNCIL: Campbell a s Mayor (NPA) 5 Aldermen (COPE) 5 Aldermen (NPA) PARKS : 4 Commissioners (NPA) 2 Commissioners (COPE) 1 Commissioner ( C i v i c NDP) SCHOOL: 5 T r u s t e e s (NPA) 2 T r u s t e e s (COPE) 2 T r u s t e e s (Civic NDP) c a m p b e l l ' s a c c e p t a n c e speech was a s t r a n g e echo of t h e i s s u e s t h a t COPE had r a i s e d and campaigned on - neighbourhood involvement i n p l a n n i n g , a c o m p a t i b l e working r e l a t i o n s h i p would be developed between l o c a l i d e a s and d e v e l o p e r s ' p l a n s , making Council more open and a c c e s s i b l e t o p e o p l e P a r t of t h e d i f f i c u l t y i s t o t r y a full-blown campaign on i s s u e s t h a t have grown o v e r t h e y e a r s . What must be c a r r i e d on now i s t o keep people a s informed as p o s s i b l e on each p o l i c y & d e c i s i o n t h a t Campbell and t h e NPA t a k e on each i s s u e . I t was f r u s t r a t i n g t o have him run on a t t h e mouth a t a l l - c a n d i d a t e s ' meetings w i t h p a t ans w e r s t o p o i n t e d q u e s t i o n s and n o t have t h e d e t a i l s t o blow h o l e s i n h i s rhetoric.

It w a s c o n f u s i n g enough t h a t o n l y on b l a t a n t l i e s could h i s u n d e r l y i n g agenda be p e r c e i v e d .

I ' d c a l l on COPE t o keep every backt r a c k i n g move by t h e NPA i n t h e p u b l i c eye, e s p e c i a l l y on housing and t h e i r " a f f o r d a b i l i t y " schemes. More and more p e o p l e w i l l b e a f f e c t e d i n t h e n e x t 3 y e a r s w i t h t h e e l i m i n a t i o n of t h e i r hoped f o r ' s a f e t y ' from b e i n g p a r t of any d e v e l o p e r ' s nightmares.

Some s i c k people must t a k e medicine t o b a l a n c e t h e chemical imbalance.

I f they a r e homeless - abandoned and n o t t a k i n g t h e i r m e d i c a t i o n , some w i l l a c t out..sometimes t o t h e p o i n t of v i o l e n c e o r s u i c i d e .

These people need housing t h a t has some medical c a r e b u i l t i n . There i s n o t enough of t h i s type of housing.

There i s some b u t much more i s needed.

These people a r e given t h e "hard t o house" l a b e l . They a r e o f t e n p u t i n s m a l l rooms over b a r s . . a l l t h a t most people on w e l f a r e can a f f o r d . When t h e s t r e s s of t h e h o t e l g e t s t o them and they go o f f t h e i r medicine, they a c t o u t , a r e e v i c t e d , and f i n i s h up l i v i n g on t h e s t r e e t .

L e t ' s s t o p blaming people f o r b e i n g sick.

S h r i n k s , medical p r o f e s s i o n a l s and s o c i a l workers make a good l i v i n g o f f t h e s e s i c k people y e t t h e problem of homelessness and mental s i c k n e s s i s s t i l l w i t h us..and growing

I n r e t u r n , c l o s i n g f a c t o r i e s and manufacturing, induced unemployment, a t t a c k s on s o c i a l and h e a l t h c a r e programs and t h e poor; an i n c r e a s i n g b r a i n d r a i n under f o r e i g n ownership; t h e ongoing " p r i v a t i z a t i o n " of a n y t h i n g t h a t moves and makes prof i t s i n c l u d i n g PetroCan and o u r n a t i o n a l a i r l i n e s and a i r p o r t s ; a c o n s t i t u t i o n a l c r i s i s and h i a t u s ; a n unmana g a b l e " d e f i c i t " and leap-frogging n a t i o n a l d e b t w i t h no r e a l i n t e n t t o b r i n g i t t o manageable l e v e l s . The indigenous p e o p l e s ( i n c l u d i n g women and c h i l d r e n ) a r e t r e a t e d t o t h e b u s i n e s s end of army bayonets and f i r e p o w e r o v e r c e n t u r i e s old land grievances. F i n a l l y , but f a r from l e a s t , Canadian l i v e s a r e p l a c e d i n jeopardy w i t h o u t c o n s u l t a t i o n w i t h t h e e l e c t e d Members of Parl i a m e n t . They w i l l be s a c r i f i c i e d t~ "defend" t h e h o l d i n g s of i n t e r e s t s of Exxon, Texaco, Aramco, Dutch S h e l l e t a l , i n an " o i l war" t o s u b j u g a t e t h e Arabic peoples. Countless people t h i n k Christmas is a s p e c i a l holiday.

It's celebrated t h r o u g h o u t t h e world b e c a u s e on t h i s s p e c i a l day C h r i s t was born. What I f i n d s o f a s c i n a t i n g w i t h Christmas is t h e p r e p a r a t i o n t h a t e v e r y o n e goes t h r o u g h t o make i t s p e c i a l f o r t h e i r families.

When I l i v e d a t home w i t h my pare n t s , we w e r e n ' t w e l l o f f . U s u a l l y t h e younger k i d s would g e t a t o y o r h a n d k n i t g l o v e s and a h a t . Sometimes w e would go t o o u r community c e n t r e b e c a u s e S a n t a would come w i t h p r e s e n t s f o r e v e r y l i t t l e fis;L and boy.

Somehow Dad would manage t o buy a t u r k e y (and a ham f o r t h o s e who c o u l d n ' t have t u r k e y ) , w i t h a l l t h e trimmings and homemade b r e a d .

On C h r i s t m a s Day t h e f i r s t o n e s up a r e u s u a l l y t h e c h i l d r e n , because t h e y a r e t o l d t h a t i f t h e y d o n ' t go t o bed e a r l y S a n t a won'^ come w i t h t h e i r presents.

Before t h e y go t o bed t h e y l e a v e m i l k and c o o k i e s f o r S a n t a a n d , f i r s t t h i n g i n t h e morning, t h e y check t o s e e i f S a n t a d r a n k t h e m i l k and had h i s treat. Then t h e y r u n t o wake up Mom & Dad. Mom & Dad always s a y , "Give u s a few m i n u t e s and w e ' l l be t h e r e . " Sometimes t h e k i d s go t o t h e l i v i n g room and w a i t f o r t h e r e s t of t h e f a m i l y . But i f you are C a t h o l i c s you g e t d r e s s e d t o go t o c h u r c h i f you d i d n ' t go t h e n i g h t b e f o r e . I n s t e a d of c h a s i n g a f t e r mega proj e c t s , Toronto should i n v e s t p u b l i c d o l l a r s i n p u b l i c needs. Editor: An important conference on Native self-government in Toronto a couple of months ago received virtually no local news coverage despite having a very original concept of native selfgovernment presented. Thomas Courchene, director of the School of Policy Studies at Queen's University, presented a paper recommending that the self-government issue be resolved by creating an eleventh province a native province.

The land for this province would include all existing reserve land, including the northern territories.

This would create a significant geographic base even though the land will be spread out across Canada.

If one includes Metis and Inuit and non-status natives in the population you would have roughly 5% of Canada; a significant people base.

Natives would elect their own gov-

ernment; have their own legislature;15have direct representation in Parliament and the Senate; attend with a voice and a vote at all First Ministers' Conferences; negotiate deals with other provinces; accept federal transfer payments to establish their own health, social service and education systems; form their own justice system and police force and have their own form of taxation. Quite simply, Natives would have their own form of government while remaining an integral part of Canada.

The more I read and ponder this proposal the more excited I become.

I think this could be a starting point to actually resolving many dif- .. ferent and difficult issues - from poverty to self-government and economic opportunities. Best of all, difficulties would be solved by native peoples themselves.

Over o n e - t h i r d o f o u r app r o x i m a t e l y 4500 members a r e Chinese-Canadian. Accordingly, t h e meetings a r e i n E n g l i s h and Cantonese. 3. THE TYPICAL RESIDENT OF THE DOWNTOWN EASTSIDE:

1. INTRODUCTION: T h i s Commission h a s been a s k e d t o "make recommendat i o n s t o t h e p r o v i n c i a l government f o r t h e improvement o f h e a l t h and o u r h e a l t h c a r e system." T h i s is a lauda b l e endeavor. However, t h e t i t l e of t h e Commission i s "The Royal Commission on H e a l t h Care and Costs." The word " c o s t s " , by i m p l i c a t i o n a t l e a s t t o u s - i n t h e tit l e o f any Commission, board o r committee's mandate means t h a t t h i s Commission w i l l be looking t o cut c o s t s i n our h e a l t h c a r e system. We s t a t e t h i s , n o t i n a mood of cynif o r we hope o u r p r e cism s e n t a t i o n w i l l show t h a t we but i n response are sincere t o impending l e g i s l a t i o n : t h e F e d e r a l B i l l C-69.

B i l l C-69 i s now i n i t s f i r s t r e a d i n g . However, i f e n a c t e d , B i l l C-69 would acc e l e r a t e funding cutbacks f o t medicare, s o c i a l s e r v i c e s anc p o s t s e c o n d a r y e d u c a t i o n .

F e c e r a l money p a i d t o t h e p r c v i n c e s f o r t h e s e programs w o ~ l dbe reduced e a c h y e a r , from i t s 1989190 l e v e l of by about $9 b i l l i o n t o t h e y e a r 2004.

B i l l C-69 would a l s o , f o r t h e f i r s t time, place a l i m i t on f e d e r a l money f o r s o c i a l a s s i s t a n c e payments and soci a l services i n the three 'rich' provinces - Ontario, A l b e r t a and B r i t i s h Columbia.

I f f e d e r a l f u n d i n g i s reduced t h e p r o v i n c e s w i l l b e a r an i n c r e a s e d f i n a n c i a l burden which, i n t u r n , c o u l d jeop a r d i z e t h e a v a i l a b i l i t y and q u a l i t y of t h e s e programs and l i m i t the help available to t h o s e most i n need. I n t h e a r e a of h e a l t h c a r e , t h e ins t i t u t i o n of extra-billing o r h o s p i t a l u s e r - f e e s (now proh i b i t e d under t h e Canada H e a l t h Act) a r e v e r y l i k e l y .

H e a l t h c a r e i s a n investment.

Keeping o u r n a t i o n h e a l t h y should b e a p r i o r i t y . Maintaining health, especially i n t h e l a t e r y e a r s of l i f e and w i t h a n a g i n g p o p u l a t i o n w i l l become one of t h e most c r u c i a l challenges facing t h i s province.

2. WHAT DERA DOES: Before g i v i n g o u r p r e s e n t a tion, allow us t o explain the n a t u r e of our organizat i o n , i t s g o a l s and success e s , and o u r s t a n d i ~ gi n o u r community.

DERA (The Downtown E a s t s i d e R e s i d e n t s ' A s s o c i a t i o n ) was formed i n 1973. It was formed by r e s i d e n t s who were disgusted with t h e indifference, the years of neglect and t h e e x p l o i t a t i o n which i n c r e a s e d t h e i r p o v e r t y and l a b e l l e d t h e i r community "Skid Road". R e s i d e n t s orga n i z e d i n o r d e r t o improve t h e i r neighbourhood and r e named i t " t h e Downtown Eastside".

Over t h e y e a r s t h e r e s i d e n t s have f o u g h t l o n g and hard f o r d e c e n t and a f f o r d a b l e housing, jobs, l i v e a b l e incomes, community and r e c r e a t i o n a l f a c i l i t i e s , oark s p a c e , s a f e r s t r e e t s (curb-

t h e Downtown E a s t s i d e , we have added a p r o f i l e o f t h e a v e r a g e r e s i d e n t from o u r r e s i d e n t surveys.

The Downtown E a s t s i d e i s popularly referred t o a s "Skid Road", and o f t e n s e r v e s a s a catchment a r e a f o r many of t h e C i t y ' s s o c i a l problems.

What i s n o t w i d e l y known i s t h a t t h e Downtown E a s t s i d e is one o f t h e most s t a b l e commu n i t i e s i n Vancouver, w i t h a s t r o n g community i d e n t i t y , a v i b r a n t mix o f c u l t u r e s , a g e s and economic backgrounds and an e s t a b l i s h e d group o f r e s i d e n t s who a r e committed t o m a i n t a i n i n g and improving t h e comnunity.

The t y p i c a l r e s i d e n t of t h e Downtown E a s t s i d e i s a Caucas i a n male who l i v e s a l o n e and i s p o s s i b l y d i s a b l e d . He i s 45 y e a r s o r o l d e r and a r e c i p i e n t of s o c i a l a s s i s t a n c e , w i t h a monthly income of l e s s t h a n $580, t h i s b e i n g 66.15%

of t h e p o v e r t y l i n e income. ~n a v e r a g e r e s f d e n t i n 1990: - 89% a r e males - 93% a r e l i v e - a l o n e s a v e r a g e a g e i s 47 y e a r s - 46% were born i n B.C. 49% from e l s e w h e r e i n Can. 5% from o u t s i d e Canada - monthly income o f $500 - monthly r e n t $300 o r l e s s .

The a v e r a g e r e s i d e n t h a s l i v e d i n t h e Downtown Easts i d e f o r 1 3 y e a r s . Most l i v e i n a housekeeping o r s l e e p i n g room. have some s o r t of h e a l t h problem which makes i t d i f f i c u l t f o r them t o g e t around. The s l e e p i n g and housekeeping u n i t s i n which most of them l i v e a r e t y p i c a l l y 100 s q u a r e f e e t of l i v i n g s p a c e w i t h i n which t h e r e is a bed, a d r e s s e r , a c h a i r and a s i n k . A growing percent a g e o f u n i t s have a h o t p l a t e and a f r i d g e . R a r e l y do s u c h u n i t s c o n t a i n waehroom f a c i l i t i e s o r a phone.

The above i n f o r m a t i o n is ext r a c t e d from t h e r e s u l t s of o u r tow housing s u r v e y s (1988 and 1990) done f o r Canada Mortgage and Housing Corporat i o n (CMHC). The 1990 s u r v e y has not y e t received approval f o r p u b l i c a t i o n from CMHC, a l t h o u g h we do a n t i c i p a t e app r o v a l l a t e r t h i s month (Nov).

The l a t e s t s t u d y found t h a t , w h i l e changes i n r e s i d e n t types a r e occurring, the trad i t i o n a l Downtown E a s t s i d e r e s i d e n t type, a s outlined above, i s remaining i n t h e neighbourhood. The t y p i c a l r e s i d e n t p r o f i l e h a s changed m a r g i n a l l y due t o a n i n f l u x o f 'younger' p e o p l e , i n c l u d i n g women and c h i l d r e n , i n t o t h e a r e a . T h i s was caused by t h e city-wide r e n t a l c r i s i s of t h e p a s t two y e a r s .

I n o u r 1988 s t u d y t h e r e was a vacancy r a t e o f between 1326% in t h e ' r e s i d e n t i a l ' hot e l s which form t h e m a j o r i t y of o u r h o u s i n g s t o c k . I n o u r l a t e s t s t u d y , t h e vacancy r a t e f l u c t u a t e s between 2 6 3%. T h i s ' s l a c k ' i n t h e vacancy r a t e was t a k e n up by younger p e o p l e , f o r c e d o u t of a f f o r d a b l e accomodation by r i s i n g costa. As w e l l , a n in-

c r e a s i n g number o f d e - i n s t i tutionalized mental p a t i e n t s have g r a v i t a t e d t o t h e a r e a , a l s o f o r c e d o u t by r e n t i n creases i n the surrounding areas.

Until l a t e l y , relatively l i t t l e was known a b o u t t h e h e a l t h s t a t u s and p a t t e r n s of,health service utilizat i o n o f a r e a r e s i d e n t s . Nearl y one h a l f o f t h e r e s i d e n t s q u e s t i o n e d in o u r s u r v e y r e p o r t e d some form o f d i s a b i l i ty. I n o u r 1988 s u r v e y , s e r i ous d i s a b i l i t i e s were r e p o r t ed by about 15% o f t h e people responding. We have a l s o i d e n t i f i e d s u b s t a n c e abuse, v l i l e n c e and v i c e a s major prcblems i n t h e a r e a .

W i l e t h e average resident i s a Caucasian male o v e r 45, more f a m i l i e s a r e moving i n t o t h e a r e a , b o t h o u t o f necessi t y and by c h o i c e . While t h e r e s i d e n t s f o u g h t l o n g and hard f o r a w a t e r f r o n t p a r k i n the area, recreational facili t i e s a r e still severely l a c k i n g . The Carnegie Communi t y Centre provides recreat i o n a l f a c i l i t i e s , but its limited space even though i t i s expanding restricts t h e amount o f s e r v i c e i t can provide.

4. HEALTH STUDY DONE

RESULTS DISTURBING:

I n J u l y of 1990, a "Prelirnin a r y Study o f S e l e c t e d Morb i d i t y and M o r t a l i t y I n d i c a t o r s " i n t h e Downtown East- ' s i d e was done by D r . J a n ).

Tollefson, a Resident i n Community Medicine a t t h e U n i v e r s i t y o f B r i t i s h Columb i a . It must be n o t e d t h a t t h i s is a preliminary study. but i t provides an i n d i c a t i o n of t h e h e a l t h and r e l a t e d problems we f a c e i n t h e Downtown E a s t s i d e . T h i s s t u d y uses eight determinants o r i n d i c a t o r s of h e a l t h s t a t u s f o r assessment purposes...

1 ) Income and employment 2) Tuberculosis 3) Human immunodeficiency virus 4 ) Alcohol withdrawal r e q u i r ing c a r e a t a detox unit

5) c h i l d r e n - i n - c a r e c a s e l o a d s 6) Mental h e a l t h c a s e l o a d s 7) a o m i c i d e s and o t h e r 'crimes against person' 8) M o r t a l i t y , i n c l u d i n g premature l o s s of l i f e .

The r e s u l t s of t h i s s t u d y , w h i l e shocking, were n o t s u r p r i s i n g t o DERA. We had a l ready s e n s e d t h a t we had a n i n c r e a s i n g number of s o c i a l problems and t h e a t t e n d a n t h e a l t h problems. We a t DERA e n t i r e l y concur w i t h t h e recommendations of t h e r e p o r t .

We would a l s o encourage f u r t h e r s t u d i e s . As i s e v i d e n t from t h e s t u d y f i n d i n g s , t h e h e a l t h problems i n t h e Downtown E a s t s i d e a r e worse t h a n i n any o t h e r a r e a o f t h e C i t y o f Vancouver.

5. IN-DEPTH DOWNTOWN EASTSIDE HEALTH STUDY NEEDED: '

DERA r e c e n t l y appl!ed to s e v e r a l s o u r c e s f o r funding t o do a more comprehensive s u r v e y of h e a l t h n e e d s i n t h e Downtown E a s t s i d e . The proposed o b j e c t i v e s a r e : 1) t o derive estimates of t h e burden of i l l n e s s i n t h i s population; 2) t o d e t e r m i n e t h e p a t t e r n of health service u t i l i z a t i o n .

Aside from t h e s t u d y done by Dr. Tollefson, r e l a t i v e l y l i t t l e i s known about t h e h e a l t h s t a t u s and p a t t e r n s of health service u t i l i z a t i o n o f Downtown E a s t s i d e r e s i d e n t s . Research elsewhere ind i c a t e s t h a t r e l a t i v e depriva t i o n , and t h i s i s t h e c a s e i n t h e Downtown E a s t s i d e , c o r r e l a t e s s t r o n g l y and posit i v e l y t o both higher r a t e s of m o r t a l i t y and p o o r e r h e a l t h s t a t u s , and i t i s t h u s anticipated t h a t the study w i l l uncover g e n e r a l l y poor health status.

Other p o p u l a t i o n s w i t h simil a r characteristics to the Downtown E a s t s i d e tend t o u s e fewer h e a l t h c a r e s e r v i c e s r e l a t i v e t o overall l e v e l s of need, and o f t e n do n o t s e e k c a r e u n t i l c o n d i t i o n s have become r e l a t i v e l y s e v e r e .

One i n t e r e s t i n g f o c u s of o u r proposed s u r v e y would b e t o

compare t h e h e a l t h s t a t u s of s i n g l e l i v e - a l o n e s , who compr i s e t h e m a j o r i t y of o u r popu l a t i o n , w i t h t h o s e i n coo p e r a t i v e and s o c i a l housing p r o j e c t s . We a r e c e r t a i n t h a t t h i s would p o i n t t o a s t r o n g c o r r e l a t i o n between p r o p e r housing and h e a l t h .

We would a l s o s t u d y t h e h e a l t h e x p e r i e n c e of a subsample of a r e a r e s i d e n t s ; how t h e y cope when ill, and who t h e y t u r n t o f o r help.

We would examine t h e s u p p o r t and m o n i t o r i n g systems, and how people l i v i n g w e l l below t h e poverty l i n e n o u r i s h themselves d u r i n g p e r i o d s of i l l n e s s , when t h e y a r e conf i n e d t o t h e i r rooms.

We would a l s o e x p l o r e t h e l e v e l o f s u b s t a n c e abuse especially alcohol ( i n the Downtown E a s t s i d e we have n e a r l y 6,000 pub s e a t s , o r n e a r l y one f o r e v e r y two residents).

While t h e funds f o r t h i s s t u d y do n o t appear t o b e forthcoming, e s p e c i a l l y i n t h e s h o r t term, we f e e l t h a t d o i n g t h i s t y p e of s t u d y i s i m p e r a t i v e . To d e a l e f f e c t i v e l y w i t h h e a l t h c a r e , we must b e aware o f t h e t o t a l i t y of t h e problem. Chronic d i s e a s e and s u b s t a n c e abuse require constant attention i f we a r e t o d e a l e f f e c t i v e l y w i t h t h e c h a l l e n g e of p r o v i d i n g p r o p e r c a r e , prev e n t a t i v e measures and b e t t e r a l l o c a t i o n o f medical and s o c i a l s e r v i c e s .

6. HOW DERA APPROACHES HEALTH:

I n 1989 DERA was given a S e n i o r s H e a l t h Independence Program g r a n t by H e a l t h 6 Welfare Canada. The purpose o f t h i s g r a n t was t o e n a b l e us t o improve t h e independence, s e l f esteem, and g e n e r a i h e a l t h of s e n i o r s (45 y e a r s and o l d e r o u r t y p i c a l r e s i d e n t ) . We view h e a l t h a s a t o t a l i t y , i n c l u d i n g p h y s i c a l , mental, s o c i a l and communnl h e a l t h .

T h i s s u c c e s s f u l program is now i n i t s second year.

ALICE: Under t h e a e g i s o f SHIP SHAPE, DERA received a start-up grant from t h e Vancouver Foundat i o n t o i n i t i a t e a program c a l l e d A.L.I.C.E. (A Low Income C o o p e r a t i v e Enterp r i s e ) . I n December 1990 we hope t o open ALICE, a low-cost g e n e r a l s t o r e f o r s e n i o r s and o t h e r r e s i d e n t s of t h e Downtown E a s t s i d e .

We w i l l b e endeavoring t o keep t h e p r i c e o f b a s i c f o o d s t u f f s and c l e a n i n g s u p p l i e s a s low a s p o s q i b l e f o r p e o p l e on f i x e d incomes.

Nutritional counselling 6 education a r e another goal of t h i s p r o j e c t . Those on f i x e d incomes, e s p e c i a l l y when ill o r n e a r t h e end of t h e i r monthly a s s i s t a n c e money, tend t o e a t p o o r l y ( t h e t o a s t h t e a syndrorne).

Proper d i e t is important i n m a i n t a i n i n g good h e a l t h , and a s most oi o u r r e s i d ents live i n residential h o t e l s , cooking and food storage f a c i l i t i e s a r e oft' en lacking.

Corner convenience s t o r e s tend t o have h i g h e r p r i c e s f o r canned goods and t o i l e t and c l e a n i n g s u p p l i e s . Bulk buying c o s t s a v i n g s a r e nearly impossible f o r those on f i x e d incomes. We w i l l a l s o be looking at a delivery service f o r shut-.L~G. Health c a r e does n o t , a s t h e saying goes, 'stop a t t h e s k i n ' . Environmental, s o c i a l and community f a c t o r s a r e a l s o i m p o r t a n t . These a r c t h e f a c t o r s upon which DERA h a s h i s t o r i c a l l y , and continues t o , cmcentcate.

Housing: Over t h e p a s t s i x y e a r s , DERA h a s sponsored t n e b u i l d i n g of n e a r l y 500 u n i t s of s o c i a l and c o o p e r a t i v e hous Lr.2, t h e r e b y l o d g i n g approximatel y 800 people. We a r e e x t r e mely proud o f t h i s accompl i s l u n e n t ; good, a f f o r d a b l e housing is i n c o n ! ~ t a n t damand in t h e Downtown Easts i d s . We have n e a r l y 3000 p e o p l e on o u r a c t i v e housing w a i t i n g l i s t , and o f t h e s e

approximately 550 a r e c h i l d ren. T h i s r e p r e s e n t s a s t a r t l i n g 80% i n c r e a s e i n appli-. c a t i o n s from f a m i l i e s w i t h c h i l d r e n s i n c e 1986.

When long-term r e s i d e n t s of r e s i d e n t i a l h o t e l s and o t h e r t y p e s o f s u x i ~ a n d a r daccomodation a r e f i n a l l y allocated d e c e n t and a f f o r d a b l e housi n g , s e c u r i t y o f t e n u r e , and a s a y in t h e c a r e and maintenance o f t h e i r d o m i c i l e , s e l f esteem, independence and h e a l t h (physicill and emotional) improve n o t i c e a b l y . T h i s program must a l l o w a minimum t a r g e t of 50% low income members i n t h e co-operative.

2) Proper housing f o r p e o p l e w i t h mental d i s o r d e r s . The G r e a t e r Vancouver Mental Health Service estimates t h a t i n Vancouver and Richmond a l o n e , t h e r e a r e o v e r 200 people w i t h m e n t a l d i s o r d e r s on w a i t i n g lists f o r b o a r d i n g homes. The S t r a t h c o n a Communi t y Care Team, which t r e a t s many p e o p l e w i t h m e n t a l d i s o r d e r s i n t h e Downtown Easts i d e , s t a t e s t h a t i t now t a k e s a s l o n g a s f o u r months f o r these people t o g e t i n t o a b o a r d i n g home, compared t o a w a i t o f t h r e e weeks o n l y two y e a r s ago. As a r e s u l t , many a r e f o r c e d t o t a k e inapp r o p r i a t e and s u b s t a n d a r d housing i n t h e Downtown Eastside, i f they can g e t i t , and e x p e r i e n c e d i f f i c u l t y i n s t a y i n g i n one p l a c e .

There a r e two emergency s h e l t e r s f o r p e o p l e w i t h ment a l h e a l t h problems i n Vancouver T r i a g e and Lookout.

They a r e b o t h i n t h e Downtown E a s t s i d e . Both a r e b e i n g f o r ced t o t u r n away p r o s p e c t i v e c l i e n t s because o f t h e p r e s s u r e t o accomodate t h o s e who have no s h e l t e r a t a l l , b u t

don't necessarily f i t t h e i r mandate. More s h e l t e r s a r e needed, b u t l o c a t i o n s i n o t h e r a r e a s of t h e c i t y should a l s o be c o n s i d e r e d .

L o c a t i n g more r e s i d e n t i a l f a c i l i t i e s i n t h e Downtown Eastside f u r t h e r complicates t h e a r e a ' s problems, c e r t a i n l y d o e s n o t a i d r e c o v e r y and r e s u l t s i n many b e i n g v i c timized.

The l a c k o f p r o p e r mental h e a l t h f o l l o w up, m o n i t o r i n g and p r o p e r accommodation res u l t s i n many p e o p l e w i t h m e n t a l d i s o r d e r s becoming v i c t i m s o f crime: a s s a u l t , robbery, s e x u a l abuse, e t c .

T h i s c a u s e s them t o b e a f u r t h e r s t r a i n on t h e h e a l t h c a r e system, o f t e n r e q u i r i n g r e i n s t i t u t i a l i z a t i o n and/or hospitalization.

C. RECREATIONAL FACILITIES: 1) More p h y s i c a l r e c r e a t i o n a l f a c i l i t i e s a r e needed s u c h a s a swimming p;ool, more gymnasium s p a c e , a n o t h e r community c e n t r e and more p a r k space.

D. ALCOHOL AND DRUG ABUSE TREATMENT PROGRAMS: 1) More e x t e n s i v e community d i r e c t e d and c u l t u r a l l y appr o p r i a t e a l c o h o l and d r u g t r e a t m e n t pzograms ( e s p e c i a l l y i n t h e Downtown E a s t s i d e o f Vancouver). Without t h e n e c e s s a r y f a c i l i t i e s t o prov i d e ' i n house' t r e a t m e n t , t h e c y c l e s of dependency and abuse w i l l c o n t i n u e . T h i s dependency and a b u s e c a n cause physical d e t e r i o r a t i o n a s w e l l a s h i g h e r r a t e s of crime, ' d y s f u n c t i o n a l ' famil i e s , f a m i l y b r e a k up, more c h i l d r e n - i n - c a r e , and a h o s t o f o t h e r problems which, i f n o t d e a l t w i t h , become chron i c and l e a d t o a f u r t h e r d r a i n on olr a l r e a d y overtaxed h e a l t h and s o c i a l s e r v i c e s system.

2) Substance abuse e d u c a t i o n and awareness programs a r e needed.

F. OUTREACH PROGRAMS: 1 ) Increased outreach f o r the l o n e l y and ' s h u t - i n ' a d u l t s and s e n i o r s .

Because of i l l n e s s , a l c o , h o l / d r u g abuse, p h y s i c a l o r e m o t i o n a l impairment, economic d e p r i v a t i o n , a n d / o r l a c k of t r a d i t i o n a l s o c i a l t i e s a g r e a t number of o u r r e s i d e n t s become ' d i s c o n n e c t e d ' from a v a i l a b l e r e s o u r c e s .

According t o most s t u d i e s , t h e s e people become d i s e a s e d , p h y s i c a l l y and e m o t i o n a l l y impaired, and g e n e r a l l y s e e k t r e a t m e n t most o f t e n when t h e i r s i t u a t i o n reaches the c h r o n i c s t a g e . Outreach and q u a s i - c l i n i c a l programs such a s more community h e a l t h workers ( p h y s i c a l 6 mental) and c o r n u n i t y h e a l t h n u r s e s , s e n i o r s ' programs such a s DERA's SUIP SHAPE and neighbourhood h e l p e r s programs a r e n e c e s s a r y i f we a r e t o i n t e r c e d e and promote p r o p e r p h y s i c a l and m e n t a l h e a l t h .

G. CHILDREN'S SERVICES: 1) S t e p s s h o u l d be taken t o ensure t h a t children a r e s a f e from v e r b a l , p h y s i c a l and s e x u a l a b u s e b o t h i n s i d e and o u t s i d e t h e home.

2) That t h e r e be a f u r t h e r development o f s e r v i c e s f o r the children of battered women.

3) That c h i l d r e n i n t h e s c h o o l system be t a u g h t how t o deal w i t h v i o l e n c e and c o n f l i c t and t h a t neighbourhood programs be developed t o a s s i s t children i n learni n g s k i l l s r e g a r d i n g coping w i t h t h e i r own v i o l e n t beh a v i o u r o r t h a t of o t h e r s .

4 ) That more s h o r t and l o n g term c h i l d - c a r e and r e s p i t e c a f e s e r v i c e s be p r o v i d e d t o f a m i l i e s i n t h e Downtown Eas t s i d e .

8. SPOUSAL ASSAULT SERVICES: 1) That f u n d i n g f o r t r e a t ment programs f o r t h e perpet r a t o r s o f spouse a s s a u l t b e provided, in which i s s u e s o f s o c i a l i z a t i o n and a t t i t u d e s t o power and c o n t r o l a r e d e a l t with.

2) That more t r a i n e d and s e n s i t i v e v i c t h support s e r v i c e s a r e needed f o r b a t t e r e d women.

3) There is a need t o expand c o o r d i n a t i o n of spouse a s s a u l t s e r v i c e s across the P r o v i n c e and communities should be encouraged t o develop Spouse A s s a u l t Coo r d i n a t i n g Committees.

I. WELFARE CHANGES : 1) That t h e minimum wage and w e l f a r e r a t e be r a i s e d t o t h e Federally defined poverty l i n e ( a t l e a s t ) t o r e f l e c t the a c t u a l cost of l i v i n g i n Vancouver.

The l o n g term s a v i n g s r e s u l t i n g from s u c h a move would more t h a n pay f o r t h e short-term expense.

2) S e t up a system of r e n t reviews and remove t h e d i v i s i o n o f w e l f a r e money i n t o s u p p o r t and s h e l t e r compone n t s s o t h a t l a n d l o r d s do not automatically r a i s e r e n t s , t h e r e b y f o r c i n g welfare r e c i p i e n t s t o u s e food money f o r housing charges.

3) P r o v i d e m e d i c a l and d e n t a l coverage f o r a l l p e r s o n s on w e l f a r e t o i n c r e a s e acce s s i b i l i t y t o health care.

J. GENERAL RECOMMENDATIONS: I ) That t h e h e a l t h c a r e mand a t e be broadened t o provide more h e a l t h promotion a c t i v i t i e s ( p r e v e n t i o n and e a r l y intervention).

2) The n a t u r e and d e s i g n of s e r v i c e s must be mixed t o r e f l e c t the multiple causes o f poor h e a l t h . We need t o e s t a b l i s h a system of communi c a t i o n t o e n s u r e t h a t separa t i o n of j u r i s d i c t i o n s between P r o v i n c i a l M i n i s t r i e s does n o t c o n t i n u e t o t h w a r t e f f o r t s t o promote h o l i s t i c c o n t i n u a of s e r v i c e s .

3) That community h e a l t h coo r d i n a t i o n committees be e s t ablished t o ensure t h a t deliv e r y systems a r e guided by and r e f l e c t t h e needs of h e a l t h c a r e u s e r s , and t h a t t h e y develop both s h o r t and l o n g term s t r a t e g i e s . They should be backed by and inc l u d e government r e p r e s e n t a t i v e s , and have enough power t o be a f o r c e f o r change.

4 ) It i s n e c e s s a r y t o coordi n a t e s e r v i c e s and r e s e a r c h i n a community t o determine

how t h e y can d e s i g n ' t h e i r own i n t e r v e n t i o n s . I n t h e absence of l o c a l l e a d e r s h i p o r resources the health agencies should a c t a s a c a t a l y s t f o r m o t i v a t i n g community l e a d e r s , v o l u n t a r y o r g a n i z a t i o n s and government s e r v i c e a g e n c i e s t o engage i n a n i n t e r - a g e n c y approach t o h e a l t h promotion.

5) That e d u c a t i o n a l m a t e r i a l s be produced i n p l a i n language i . e . i n a c l e a r and c o n c i s e manner t o e n s u r e t h a t t h o s e w i t h l i t e r a c y problems c a n f u l l y understand them.

6 ) A l l programs must i n c l u d e a n e v a l u a t i o n component t o ensure t h a t they serve a c t u a l community-identified needs.

7) That f u n d i n g b e provided for multicultural services to be expanded and f o r t h e prov i s i o n of m u l t i c u l t u r a l int e r p r e t a t i o n programs f o r use r s o f t h e h e a l t h c a r e system.

8) That governments and t h e i r departments s t o p taking s h o r t - t e r m and myopic views of problems and t h e i r p o s s i b l e s o l u t i o n s and work towards more comprehensive and longterm p l a n n i n g t h a t w i l l a f f e c t t h e root causes of the problems (no more p o l i t i c a l l y expedient s o l u t i o n s ) .

We g e t one day and they g e t 1,095 days (municipal) o r 1,460 days (prov. & f e d . ) P o l l s a r e u s u a l l y open f o r 12 hours.

M i l l i o n s of people g e t 1 2 hours t o go t o p o l l i n g p l a c e s f o r p r o v i n c i a l and f e d e r a l governments, who t h e n g e t 35,040 hours of power t o d e c i d e o u r fate.

You must have mobility ot labor. Any ~ i m eyou convert industry from peace to war, you have to move people from one place to another, from one job to another.

If you have large numbers of jobless, that's easy; they'll go to the jobs, on their own. But you don't have that today.

If you and draw people to new jobs with higher pay, that's easy too. But you couldn't do that again; as the chart shows on page 19, living standards would have to go down - not up.

The problem would be to find ways to control people, the way you control materials. After all, you never have to worry about the morale of an ingot of steel; it never gets mad at you.

War I1 ended before we had to get at all tough. So-thereare no f miliar techniques available. We wo Id have to work these out with a combination of fumbling and brilliant imaginat ion.

But you can see a few features of a labor control pattern: First of all, a national service system would have to be tied in with the draft of soldiers. It would be more than just offering a choice of

"work or fight" ; the government waul$ decide who works, who fights.

1Y The decision would be based on individual usefulness to the war effort,'" not on how many children you have. s9' the decision couldn't be made by a board of your neighbors; it would have Q to be made by experts, bureaucrats.

Another thing we'd have to do, that was not done in War 11, is flatly to outlaw strikes, treat incitement to strike as treason. !&?hat s t r i k e s there ;ere fii W i i r II grew out of job frictions, the ponde ousness of government strike-handlin machinery; the issues were essentia economic matters and were dealt with as such.

A strike during a war with Russia would be another breed of cat. Even if it were not Communist-started, the Reds would be attracted to it, keep it going, sharpen the bitterness - if they were allowed to. Such strikes, both in intent and in effect, would be sabotage. So we'd have to prevent them.

Could you even get grudging cooperation in all this from organized labor?

Labor leaders would have to be assured in more than mere words that this isn't a plot to end free labor in America.

Obviously, cooperation could be obtained only by putting union leaders directly in charge of the program giving them a responsibility and an authority in government never before approached in this country. hat's what Britain had to do last time. i

Civil rights of all kinds would take an awful beating of course. In a war with Russia, we would have a real fifth column, as we didn't when we fought Germany and Japan.

That means that, just as in the case of strikes

"The majority of male judges are ill-equipped to deal with cases of wife battery," she said. *Many ofthem will identify with themale perpetrat~rs~rather than with the victim."

Schirm believes .that many women break down in front of the court system and are not able to testify against their abusive partners because they are not prepared for the trial.

"The man is often better prepared to face the court than the woman is. He has been advised by his lawyer and told how to answer questions,"she said.'On the other hand, the woman who is standing infrontofa man who has hurt her has received no preparation."
