WELCOME TO THE 72nd ANNUAL CONVENTION & EXPOSITION

WELCOME It’s full speed ahead as we explore new ways to learn and network at our annual convention & exposition. As this is a new experience for all of us, we wanted to highlight a few key reasons why we are so excited to bring you this virtual event. First, we are making the convention & exposition extremely easy to “attend.” Our convention & exposition will be rolled out over a four-week period! This expanded time frame allows you to experience the convention when it is most convenient for you—while at the same time, continuing with options that make it feel like we are all together. During our first week—November 9-13—six new learning sessions will be rolled out each day—including real-time Q & A with the presenters. These sessions* are recorded so you can access them for weeks—just think, you don’t need to miss out on any of the sessions.

Think of it—four weeks of timely education, access to vendors and products, opportunities to network, three sensational and inspiring keynotes…and more. The second week—November 16–18—features our LIVE premium days—with three keynote speakers, roundtable discussion groups, special exposition-only time, plus two additional live learning sessions presented each day. Then, as an added bonus—November 19-December 4— convention attendees will have access to our entire* convention archived library. This means that if you missed a session or two, you will have access to every single one of our sessions, 24/7. But—what about the exhibitors? Well, with our virtual platform, it’s easy to visit exhibitors and to check out their new products and services. And during premium days, there will be designated times when vendors will be available to chat, as well as arrange for specific times to “meet” with a vendor. There will also be scavenger hunts (with four $100 prizes) that can be played weekly when you visit a certain number of exhibitors.

Virtual code of conduct Care Providers of Minnesota (CPM) is committed to providing a safe, productive, and welcoming environment for all program participants and CPM staff. This policy applies to all CPM meetingrelated events, including those sponsored by organizations other than CPM but held in conjunction with CPM events, on public or private platforms.

Disruption of presentations during sessions, in the exhibit hall, or at other events organized by CPM throughout the virtual meeting. All participants must comply with the instructions of the moderator and any CPM virtual event staff. Presentations, postings, and messages should not contain promotional materials, special offers, job offers, product announcements, or solicitation for services. CPM reserves the right to remove such messages and potentially ban sources of those solicitations. Participants must not copy or take screen shots of Q&A or any chat room activity that takes place in the virtual space.

CPM reserves the right to take any action deemed necessary and appropriate in response to any incident of unacceptable behavior, including immediate removal from the meeting without warning or refund. CPM reserves the right to prohibit attendance at any future meeting, virtually or in person.

GENERAL INFORMATION A virtual experience

While we wish we were gathering in person, we are very excited to bring this virtual experience to Care Providers of Minnesota convention attendees. You have 39 hours of high-quality education on current and cutting-edge topics, dynamic live presentations, three thoughtprovoking keynotes, roundtable discussions, and valued business partners highlighting services and products in the exposition.

So, you are wondering how a virtual convention and exposition works…

After you register, you will receive an email from notifications@mail.thoughtindustries.com; this email includes the link to the virtual convention platform. You will create a password for the virtual convention (your username is the email you registered with). Once you have created a password, you can access the virtual platform. While education sessions and the exposition will not be available until November 9, you can review the offered education, schedule of events, and start building your personal convention experience. Each education event will have its own page including title, description, learning objectives, speaker bios—and once released—access to sessions and handouts. When you see a session you wish to attend, add it to your dashboard. You can create a one-stop screen on your dashboard with your selected sessions and roundtables. On your dashboard, you will be able to join the event when it starts. • November 9-13: Six new learning sessions rolled out each day with LIVE Q & A with the presenters. Exposition opens. • November 16-18: LIVE premium days with keynotes, education sessions, roundtable discussions, and LIVE interaction with valued business partners in the exposition. • November 19-December 4: BONUS access to the convention on-demand library.

First, make sure you have your Care Providers of Minnesota username and password; you will need it to register online. Don’t know your login? Go to: careproviders.org/Loginhelp. Unlike previous years where organizations registered as a unit, this year, each person must register separately. Register at www.careproviders.org/convention

Will there be technical help?

Yes, there will be technical help on the virtual platform, plus, Care Providers of Minnesota will have a convention concierge to answer your questions and guide you through this new experience.

What do I need?

Computer with audio and high-speed internet. A web camera and microphone will be helpful to connect with your colleagues in roundtable discussions and valued business partners conversations.

CANCELLATION/NO-SHOW POLICY Care Providers of Minnesota 2020 Virtual Convention & Exposition registrant cancellation/no-show policy: Cancellations and no-shows will receive access to all convention archived recordings, handouts, and the exposition until December 4, 2020. Convention registrants may earn CEUs until December 4, 2020.

Roundtable Discussions While we cannot be together in person and have dynamic conversations with our colleagues on a topic, this year’s event will still offer the opportunity to connect with one another through roundtable discussions. Roundtable discussions are informal, virtual gatherings focused on a topic. While each group will have a moderator, there is no formal presentation, it is a sharing of best practices, questions—and hopefully a few answers—feedback, comments, and making connections. CONTINUING EDUCATION Care Providers of Minnesota is pleased to offer accredited continuing profession education. Education attendance is tracked by the virtual convention platform. To receive credit for your attendance, you must attend at least 30 minutes of a session—credit is rounded down to the quarter hour—and complete a session evaluation. Care Providers of Minnesota will add the CEUs earned at convention to your Care Providers of Minnesota user profile from where you can print certificates and reports.

Minnesota Board of Nursing Sessions are designed to comply with the Minnesota Board of Nursing criteria for contact hours. Nurses can apply their hours toward their licensure requirements; however, the Board of Nursing does not pre-approve continuing education. The Minnesota Board of Nursing rules on continuing education define the contact hour as 60 minutes. Convention offers nurses a maximum of 39 contact hours.

Minnesota Board of Social Work Some sessions are designed to comply with the Minnesota Board of Social Work criteria. The Board of Social Work does not pre-approve continuing education; however, Care Providers of Minnesota is a pre-approved continuing education provider, CE provider approval number: CEP-52.

Minnesota Board of Executives for Long-Term Care Services & Supports Application for continuing education credit for all sessions, including 60 minutes of the keynote and other live sessions, will be made through the Minnesota Board of Executives for Long-Term Care Services & Supports (formerly known as Minnesota Board of Examiners for Nursing Home Administrators) for a maximum 39 clock hours.

Housing managers Many sessions are designed to fulfill the 30-hour housing with services manager continuing education requirement. Other disciplines All other disciplines are encouraged to keep track of their attendance and submit them for CE credit to the appropriate agency for their discipline.

ABOUT KEVIN Kevin Kling, author, playwright, teller of tales. Kevin Kling, best known for his popular commentaries on National Public Radio’s All Things Considered and his storytelling stage shows like Tales from the Charred Underbelly of the Yule Log, delivers hilarious, often tender stories. Kling’s autobiographical tales are as enchanting as they are true to life: hopping freight trains, getting hit by lightning, performing his banned play in Czechoslovakia, growing up in Minnesota, and eating things before knowing what they are. Kevin describes his zodiac sign as “Minnesota with Iowa rising…” His storytelling started when a friend from the now-defunct Brass Tacks Theatre asked him to perform his stories. Since then, he has been awarded numerous

arts grants and fellowships. The National Endowment for the Arts, The McKnight Foundation, The Minnesota State Arts Board, The Bush Foundation, The Jerome Foundation, and others have recognized Kling’s artistry. In 2014, Minneapolis Mayor RT Rybak named Kevin “Minneapolis Story Laureate.” Kevin continues to write plays and stories in a rigorous fashion and travels around the globe to many storytelling festivals, residencies, and has been invited to perform at the acclaimed National Storytelling Festival for several years. Kevin has released digital collections of his stories and published five books: The Dog Says How, Holiday Inn, Big Little Brother, Big Little Mother, and On Stage with Kevin Kling.

The American Health Care Association/National Center for Assisted Living (AHCA/NCAL) National Quality Award Program is a progressive program that is based on the Baldrige Criteria for Performance Excellence. Alexander, RN, RAC-CT, Director of Clinical and Reimbursement Consulting, Pathway Health Ms. Alexander has over 25 years of nursing leadership in long-term care, including experience as director of nursing services , MDS, and reimbursement. She is an expert in the management of nursing operations, reimbursement systems, and performance improvement. Karolee is a strong proponent of residentcentered care, having established many programs that have led to positive resident outcomes. She has proven success with regulatory turn around, mentoring of nursing management, interim director of nursing roles and systems analysis, and implementation of policies and procedures. She possesses a strong technology interest and has experience with implementation of clinical software systems. Mr. Doug Anderson, Vice President of Marketing, New Perspective Senior Living Doug Anderson oversees the day-to-day marketing and branding functions for New Perspective Senior Living. He began his career as a news anchor and reporter for KTTC-TV in Rochester, Minnesota. Since that time, he has developed a 30-year career in marketing. His experience in senior housing dates to 1998 as an ongoing strategic and marketing consultant to New Perspective. Ms. Lisa Bahr, RN, NHA, Zellner Senior Health Consulting, LLC Ms. Bahr has over 25 years of experience working in the long-term care industry, including as a registered nurse, director of nursing, and a nursing home administrator. She has extensive knowledge of comprehensive home care, assisted living, and housing rules, practice, and standards that surround the senior care living environment. She has worked with the Zellner Senior Health Consulting team for over three years in a wide variety of positions and roles as an interim DON for skilled and assisted living services, an executive director and administrator, and provided client leadership for MDH stipulation.

Mr. John Barker, Administrator, St. Anthony Park Home John Barker began his long-term care career in 1978 as a custodian. He has been the administrator of St. Anthony Park Home, in St. Paul, since April 1991. Mr. Barker has a Bachelor of Music degree from the Juilliard School and completed his nursing home administrator schooling at the University of Minnesota. He also has a couple thousand degrees from the School of Hard Knocks and describes himself as being one of the smartest “C” students you will ever meet.

Mr. Doug Beardsley, Vice President of Member Services, Care Providers of Minnesota Doug Beardsley has been involved in the field of long-term care since 1982, when he became a nursing home administrator. Mr. Beardsley has a Bachelor of Science degree in healthcare administration from the University of Wisconsin–Eau Claire. His experience includes 20 years as a nursing home administrator, three years as a long-term acute care hospital CEO, manager of an in-house LTC pharmacy, and other long-term care related activities. Mr. Beardsley has been the vice president of member services with Care Providers of Minnesota since 2005, where he is responsible for the regulatory activities pertaining to nursing facilities, home care, housing with services, assisted living, and hospice. Duane’s professional experience spans 28 years, 16 of which are dedicated exclusively to senior housing design, spanning all levels of the continuum of care. As a founding team member at Community Living Solutions, Duane strives to create a better quality of life for older adults and those who care for them. She has over 20 years in the healthcare profession and began her career in long-term care while in high school working as a dietary assistant. After earning her college degree in social work, she began working as a licensed social worker in a nursing facility in St. Louis Park. From there, she obtained her LNHA education through the University of Minnesota’s Carlson School of Management and operated nursing facilities for about 10 years. For the last several years, she has been managing senior living campuses.

Ms. Susan LaGrange, RN, BSN, LNHA, Director of Education, Pathway Health Ms. LaGrange’s 25 years of experience includes a diverse range of positions such as DON, NHA, and consultant. Locketz is a physical medicine & rehabilitation specialist with a subspecialty in pain medicine who has practiced medicine in Minnesota since 2006. Dr. Locketz is a licensed physician in the states of Minnesota, North Dakota, and Wisconsin. He is credentialed by the Minnesota State Medical Board and the American Board of Physical Medicine & Rehabilitation. Dr. Locketz is also a certified life care planner, as designated by the International Commission on Health Care Certification. Lundgren, RN, BSN, PHN, CWS, CWCN, President, Senior Providers Resource, LLC Jeri Ann Lundgren has been working in long-term care since 1990, specializing in wound care nationally since 1994. She is board certified as a CWS (certified wound specialist) by the American Board of Wound Management and a CWCN (certified wound care nurse) by the Wound, Ostomy, and Continence Society. Her extensive background in wound management in long-term care nationally includes: survey management, litigation support, wound care consultation, program development, and consultant/presenter for several state quality improvement organizations and departments of health. Currently Ms. Lundgren is the president of Senior Providers Resource consulting firm. Dr. Andrew Mayo, Chief Medical Officer, St. Croix Hospice, LLC Dr. Andrew Mayo was inspired to pursue a career in medicine by his family’s long history of dedication to community healthcare and innovation. After completing his family practice residency at Bethesda Hospital in St. Paul, Minnesota, Dr. Mayo spent 17 years practicing in St. Croix Falls, Wisconsin with an emphasis in geriatrics. Dr. Mayo has served on clinical and hospital leadership boards, and as medical director for Luck United Pioneer Home, and Frederic Nursing and Rehabilitation Community. In 2014, Dr. Mayo joined St. Croix Hospice as an associate medical director, where he is now the chief medical officer. Dr. Mayo continues to pursue additional medical training and has hospice medical director certification. Mr. Gregory R. Merz, Attorney, Lathrop GPM LLP Greg is a litigator and trial attorney who represents healthcare providers and other regulated businesses in regulatory and litigation matters.

Ms. Kelsey Nelson, Esq, Attorney, Voigt, Rodè, Boxeth & Coffin, LLC Kelsey Nelson is an attorney with the law firm of Voigt, Rodè, Boxeth & Coffin, practicing healthcare law including the areas of collections, regulatory compliance, and related business matters. Ms. Nelson has extensive litigation experience with collections, probate, discharges/evictions matters, and guardianship/ conservatorships. Ms. Nelson advises clients on issues of non-payment solutions, including Medical Assistance and hardship waivers, matters related to compliance, and corporate issues. She currently represents a number of nursing facilities, housing with services, and home care agency providers.

Ms. Dawn Nickleson & Mr. Patrick Nickleson, Pastry Chefs, Owners, Passion for Dining and Nutrition Dawn and Patrick Nickleson are the owners of Passion for Dining and Nutrition. Together they provide training for regulatory compliance and improving the quality of food for senior living communities. Dawn believes in the value of education and training, and in 2020 launched a culinary learning management system. She is a certified dietary manager, a ServSafe instructor, and the chair of the MN ANFP Education Committee. Patrick has been a culinary director, corporate culinary director, and has opened several senior living foodservice operations. His passion is creating good food and teaching others how to create good food. Patrick is a ServSafe certified instructor and in 2020 was co-creator and leading man in videos for the launch of a culinary learning management system.

The company bundles services such as payroll, bookkeeping, human resources, technology, process management, and others to provide small and medium businesses with the expertise of large companies at an affordable fee. The company also offers consulting services on leadership development, strategic development, process improvement, and technology. Ayodeji holds a doctorate in leadership and has professional experience in the healthcare and financial industries. He believes that an organizational system with efficient processes and good leadership will produce the desired outcome. Ms. Elizabeth Patton, Attorney, Fox Rothschild LLP Elizabeth represents clients in all stages of litigation, from pre-suit advice through trial and appeal. She devotes a large portion of her practice to representing employers in matters before administrative agencies and in state and federal courts. She is experienced in cases involving allegations of discrimination and retaliation; noncompete issues and breach of the duty of loyalty; and other employment-related claims.

Ms. Marit Anne Peterson, JD, Program Director, Minnesota Elder Justice Center Marit Peterson, JD, serves as program director of the Minnesota Elder Justice Center where she leads the victim services and professional training. Prior to joining the Minnesota Elder Justice Center, Marit practiced estate planning and elder law with a metro-area law firm and worked in contract administration and legal publishing. Marit currently serves as vice chair of the Minnesota State Bar Association’s Elder Law and Probate & Trust Law Sections. She is also an adjunct professor of philosophy at Augsburg College, where she teaches ethics in the undergraduate Augsburg for Adults program.

Mr. Robert F. Rodè, Esq, Attorney/Partner, Voigt, Rodè, Boxeth & Coffin, LLC Robert F. Rodè is a partner and practices in the areas of health law, civil litigation, employment law, administrative law, and construction law. Mr. Rodè has a special emphasis on long-term care providers, home health agencies, rehabilitative agencies, and housing with services providers. In practice, Mr. Rodè represents and advises clients on issues such as regulatory compliance; the long-term care survey and appeal process; behavior issues; resident discharge; contract negotiations and drafting; dispute resolution; housing; civil and administrative litigation; arbitration and mediation; employment; and accounts receivable.

Mr. Mario Rodriguez, MS, Leadership Consultant, Integrated Leadership Systems Mario holds a master’s degree in industrial and organizational psychology from Capella University and is currently working on his PhD in the same field. Before joining Integrated Leadership Systems (ILS), Mario worked as an organizational psychologist for the state of South Dakota, working with executive leaders and teams to improve the state’s workforce in areas including leadership, culture, employee engagement, selection, and performance. As a consultant and executive coach for ILS, his goals and passion are to develop and transcend executives and their teams to help lead their companies into a sustainable future of maximum performance and health. Mr. Aaron M. Sagedahl, Attorney, Voigt, Rodè, Boxeth & Coffin, LLC Aaron M. MEET THE 2020 session presenters | exploring opportunities Ms. Jill Schewe, Director of Assisted Living, Housing & Home Care, Care Providers of Minnesota Jill Schewe has worked in and with assisted living communities her entire career. As a former assisted living manager, her experience was in startup, management, and operations of several assisted living communities. In her current role, Jill works with assisted living providers on any topic they need help with to aid in their success, including: licensing, policy, payment, and operational systems. She understands the needs of older adults and home- and community-based services alike. Jill has a BA in sociology and gerontology from Winona State University and an MA in management from St. Mary’s University of Minnesota.

Ms. Cynthia Siders, Executive Healthcare Consultant, Vaaler Insurance, Inc. Cyndi is an executive healthcare consultant with Vaaler Insurance. She has more than 30 years of healthcare, administrative, and insurance experience; 25 of those focused-on risk management and patient/resident safety. Cyndi provides customized risk management and patient safety professional consultation and strategic support, coaching, mentoring, and education for healthcare organizations and health systems; risk management, insurance, and claims management companies; and insurance agencies. She is a registered nurse with a master’s degree in nursing and holds certifications in healthcare risk management, patient safety, and TeamSTEPPS®. Mr. Randy D. Snyder, LNHA, Executive Director, Board of Executives for Long-Term Services & Supports Randy received his undergraduate degree in long-term care administration and business administration from Concordia College in Moorhead, Minnesota. He earned his Master of Health Administration from St. Joseph’s College, Standish Maine. He currently is the executive director for the Minnesota Board of Executives for Long-Term Services & Supports and the Minnesota Board of Optometry. Randy worked as an administrator at four Minnesota communities from 1982 to 2000. He served in various roles with both state associations. At the national level, Snyder is a former chair of the National Association of Boards and currently serves on its Executive Committee. Mr. Dan Strittmater, Vice President of Operations, Monarch Healthcare Management Dan is the vice president of operations at Monarch Healthcare Management. He has been with Monarch for four years. Prior to joining Monarch, Dan spent 19 years in a regional and operational role for Presbyterian Homes. He is a licensed nursing home administrator and adjunct faculty for the Masters Program of Health & Human Services Administration at St. Mary’s University of Minnesota.

Ms. Lisa Thomson, BS, NHA, Chief Strategy & Marketing Officer, Pathway Health Lisa Thomson is the chief strategy and marketing officer at Pathway Health Services. She has over 30 years of experience in the post-acute and long-term care arena, as well as other healthcare settings. Lisa is a soughtafter national thought leader in the healthcare arena. She presents to a wide variety of healthcare and other business provider types and is a known keynote and international speaker. She has written numerous articles and has been published in national communications across the healthcare provider continuum. She has been voted as a “Top Female Healthcare Executive” and serves as a mentor for healthcare leaders across the nation.

Ms. Lores J. Vlaminck, MA, BSN, RN, CHPN, Principal Consultant, Lores Consulting, LLC Lores Vlaminck is a provider of consulting, training, and mentoring for hospice, assisted living, home healthcare, and palliative care providing mock surveys, audits, regulatory consultation, education, mentoring, and advice, as well as assistance in developing post audit correction plans. Through Lores Consulting, LLC , Lores draws on her 44 years of professional nursing experience to empower her client providers to reach their greatest potential. Her extensive nursing background includes clinical practice in cardiac and intensive care, clinical services, as well as serving as a founder/director of a home care and hospice agency for 19 years in greater Minnesota. In addition, Lores is an approved trainer for the ELNEC curriculum.
