CareMate Home Health Care, Inc. 2236 Marshall Avenue St Paul, MN 55104 COVID-19 Vaccination Policy Policy CareMate Home Health Care, Inc. (CHHCI) best interests are in the safety and health of its employees and clients. Therefore, CHHCI provides all of its employees up to date education regarding COVID-19 and the COVID-19 vaccination. This information is linked directly from the Centers of Medicare and Medicaid Services (CMS). Procedure 1. CHHCI will ask all staff to complete the COVID-19 Vaccination Record where they need to answer answer yes or no to their vaccination status. A. If employee answers yes, proof of vaccination status is requested. A fully vaccinated employee means a person who has received their primary series of COVID-19 vaccines and has 2 weeks have elapsed after taking the final dose. B. Proof of vaccination must show the name of the individual, date of birth, manufacturer of the vaccine and the date(s) on which the vaccine was administered. 2. If employee answers no, then proof of exemption is requested and must be approved by CHHCI. A. Employees could provide a medical exemption that will need to be signed by a licensed physician; or. B. Employee could provide signed religious exemption in accordance of Title VII. 3. 80% of all employees are expected to be fully vaccinated and / or claim an exemption by January 27, 2022 with documentation of a vaccine card or provide documentation of a medical or a religious exemption. 4. 90% of all employees are expected to be fully vaccinated and / or claim an exemption by February 28, 2022 with documentation of a vaccine card or provide documentation of a medical or a religious exemption. 5.19 – 1 / 2
