When faced with a major dental procedure, the last thing anyone wants is to worry about cost. With the Hybrid Plan, major procedures like oral surgery, implants or anesthesia are covered at 50% in-network after the deductible of $100 is met. Out-of-network, they’re covered on a maximum allowable charge (MAC) basis.

Basic services are common treatments like fillings and root canals. With the Hybrid Plan, these services are covered at a comfortable 80% after the deductible is met.

So, how does a member qualify for the Max Roll Forward benefit? It’s simple. First, the member must have an exam and teeth cleaning at least once each year. Second, the claims for the year must be less than half the plan maximum.

Example: Santa visits the dentist every year for his exam and cleaning. This year, he had no other dental services performed so his claims cost was under $500. Since Santa’s plan has a $1,000 annual maximum, he gets to roll forward $250 (25%) into next year. By year three, he has $1,500, and by year five, Santa has $2,000, double the maximum he started with!

This is successful Santa. He’s dancing into the future with a healthy smile and double the coverage he started with. Be like successful Santa - all year!

2019, Kristen finished T13 at the inaugural LPGA Q-Series to earn Priority List Category 14 status and was one of four rookies to make it into the season-ending CME Group Tour Championship. She finished second in the Louise Suggs Rolex Rookie of the Year standings that year, and in 2020, recorded a seasonbest T8 at the Volunteers of America Classic in The Colony, Texas.

This year, Michael recorded six top-25s, highlighted by a T5 at The American Express, and ended the season at No. 132 in the FedEx Cup standings. He made 16 cuts in 24 starts, making more than 15 in a season for the first time in his career. Michael is a two-time winner on the PGA TOUR, capturing the 2013 Honda Classic and the 2020 3M Open last July.

Through his career, he’s earned an impressive 19 top-10 finishes, and in 2012, he nearly won the U.S. Open, shooting a final round 67 to ultimately finish T-2.

Davis finished the 2020-21 combined season No. 9 in the Korn Ferry Tour points standings after recording two wins, seven top-10 finishes and 16 top-25 finishes across 37 starts. His victories came at the 2020 Panama Championship and the 2020 TPC San Antonio Championship at the Oaks. Davis earned his PGA TOUR card this year and will join the TOUR for 2022.

lthough 2021 was still a challenging year, many members of the BrightBenefits broker and business community came together a few times around the game of golf.

We are proud to partner with three professional golfers - LPGA player Kristen Gillman and PGA TOUR members Michael Thompson and Davis Riley.

Throughout the year, we hosted three virtual events with our brand ambassadors to talk discipline, goal-setting and life in the world of professional golf. BrightBenefits also hosted producing partners at the Zurich Classic Open with Michael in New Orleans and for a full day of golf in Michigan with Kristen.

The BrightBenefits brand ambassadors represent the company in a variety of ways, including on social media (namely Instagram and Twitter), branded apparel, special in-person and virtual appearances, and at golf events around the country.

As the world continues to emerge from the isolating clutches of COVID-19, BrightBenefits will expand on these events, cultivating and enriching our most valuable relationships to celebrate the brand we continue building together in the market.

In August 2021, Davis earned his PGA TOUR card by finishing in the top 25 of the 2020-2021 Korn Ferry Tour regular season standings.

While in town for the Meijer LPGA Classic in June, Kristen teed off with five BrightBenefits teams at the Thornapple Pointe course in Michigan.

BrightBenefits hosted nearly 25 producing partners in New Orleans in April 2021 to attend the Zurich Classic Open and play a few rounds with Michael at the Bayou Oaks golf course.

ELITE BROKER BONUS PROGRAM

• New group sales shall be defined as: new group (employee and dependents) whose initial BrightBenefits enrollment is effective between January 1, 2021 and December 31, 2021.

Broker eligibility • Participating brokers must be in good standing with a valid license, signed broker contract and required appointment(s) with National Guardian Life Insurance Company.

• General agents are excluded from participation; credit from group business submitted via a general agent will be credited to the individual broker.

• Program guidelines, terms and conditions Program terms and conditions • To be credited, a broker must be the broker of record at the time of the effective date.

• Annualized premium totals will be based on December, 2021 earned premiums which will be used to compute bonus payouts.

• Payments will be reported on ERISA Form 5500, Schedule A section 3(c) as other fees and 1099.

• Bonus totals are determined 60 days after the end of the program.

BrightBenefits will issue bonus payments in March 2022.

• Broker bonuses shall be payable in addition to commission and any other promotional offer or program BrightBenefits may initiate, unless otherwise stated.

• In the event a bonus was paid to the broker and not earned (i.e. any qualifying Large Group that cancels or is cancelled, e.g., for non-payment of premium, during the first twelve consecutive (12) months of plan term), one hundred percent (100%) of the • unearned bonus will be subject to chargeback.
