Community Care Services

In today’s ever-changing healthcare environment, it is a key focus in our community to assist our elderly to remain at home as long as possible.

This is only achievable if the environment in which they live in, is safe and supported.

Asking for help doesn’t mean losing your independence, it’s quite the opposite.

01. 1 Community Care Services

Government funded services such as a Home Care package, enable consumers to stay at home longer. The first step is to have your needs assessed and the risks identified. Home care assistance may vary for different people. It may mean getting help with shopping and cooking. Alternatively it could involve receiving personal care to assist with activities of daily living or getting in and out of bed. It may even mean getting modifications to your home to improve your safety and movement around the house.

2 Community Care Services

A referral to an Allied Health Practitioner is often necessary when home circumstances and physical capacity change resulting in an increased risk to the consumers safety. This service delivery could be completed by a Physiotherapist, Dietitian, Osteopath, Occupational Therapist, Podiatrist or an Exercise Physiologist, depending on the particular situation.

Home Care Package Implementation

Your Home Care Package is finalised and you have found your Home Care Package provider.

Your Home Care Package provider will complete their assessments & engage a preferred provider like BodyRight Healthcare to assist.

Assessment and Goal Setting

BodyRight Healthcare Allied Health Professionals will conduct a comprehensive assessment to understand your current health condition, limitations and goals.

Our BodyRight Healthcare practitioner will create a detailed report detailing a treatment plan in line with your goals.

The report will be shared with you & your Home Care Provider, ensuring that they have a comprehensive understanding of the assessment outcomes and treatment recommendations.

Communication and Review

BodyRight Healthcare will maintain open and regular communication with you & your Home Care Provider throughout your care journey.

Our compassionate communication fosters a therapeutic relationship contributing to the success of your treatment and promoting a positive care experience.

Allied Health Services Referral

As part of the Home Care Package implementation, your Home Care Provider will identify the need for Allied Health Services, such as Physiotherapy, Osteopathy, Exercise Physiology or Occupational Therapy.

Your Home Care Provider will consult with you and initiate the referral process by providing BodyRight Healthcare with the necessary information to commence services.

Upon receiving the referral, BodyRight Healthcare will promptly review the information and someone from the Community Care Team will contact you within 24 business hours to schedule an appointment for the initial visit.

Treatment and Support

Based on your assessment, our BodyRight Healthcare practitioner will develop a personalised treatment plan tailored to your goals and needs.

Regular appointments will be scheduled with our practitioner to provide therapy, guidance, and support in line with your treatment plan.

BodyRight Healthcare will monitor your progress and make adjustments to your treatment plan as necessary, and provide ongoing support throughout the care journey.

Our practitioner will actively listen, provide guidance, and make necessary adjustments to the treatment plan with your wellbeing as the top priority.
