Quicker access to support and more joined-up care • People with multiple major issues, such as cancer, heart and lung conditions, mental health concerns, dementia and arthritis, face long waits for referrals and appointments to specialist services. They want to get the support they need faster. • People told us that services don’t work together well enough. Individuals often see different professionals for each of their conditions and receive conflicting advice.
