At Bluegrass Care Navigators, our mission has always been to provide expert, compassionate care to those facing serious illness and the end of life. Over the past year, we have continued to serve patients and families across the 32 counties we call home, ensuring that no one navigates this journey alone.
Through our hospice care, we have been honored to walk alongside thousands of Kentuckians, offering comfort, dignity, and support in their final moments. But our mission extends beyond hospice. We are committed to reaching even more individuals through programs like Bluegrass PACE Care, which provides seniors with the comprehensive support they need to age safely at home, and Bluegrass Dementia Care, a new program that imagines a future where those with dementia and their caregivers receive seamless, compassionate care in their homes and communities.
This year, our commitment to quality and compassionate care has been recognized on a national level. Bluegrass Care Navigators has been ranked sixth in Overall Quality among the 50 largest hospices in the United States, according to a recent study published in the American Journal of Hospice & Palliative Medicine. This ranking reflects our excellence in caregiver experience, employee and caregiver satisfaction, and sentiment scores from those we serve. Additionally, we are honored to be a 2024 Hospice CAHPS Honors Award recipient, recognized by HEALTHCAREfirst for our dedication to providing outstanding patient care and exceptional support to caregivers. These accolades are a testament to the dedication of our team and the trust our patients and families place in us.

Bluegrass Care Navigators Ranks 6th in Quality Among Largest U.S. Hospices
Bluegrass Care Navigators ranked sixth in Overall Quality among the 50 largest hospices in the United States, according to a 2024 study published in the American Journal of Hospice & Palliative Medicine. The ranking is based on three key quality indicators: caregiver surveys (CAHPS scores), caregiver and employee satisfaction scores, and sentiment scores from Google reviews and Glassdoor ratings. The study assessed both non-profit and for-profit hospices across the nation and BCN’s prestigious ranking reflects our dedication to providing high-quality, holistic care for patients and their families.
HDR Foundation Awards Grant for Bluegrass PACE Care
We are grateful to the HDR Foundation for awarding an $18,000 grant to Bluegrass Care Navigators to support Bluegrass PACE Care. This grant was used to acquire two mobile and four portable wheelchair stairlifts, supporting the safe transportation of medically frail seniors with mobility restrictions and allowing participants to age safely in their homes for a longer time.

BCN to Test Medicare Dementia Care Model
Bluegrass Dementia Care is a new program provided through The Guiding an Improved Dementia Experience Model (GUIDE) that imagines a future where those with dementia and their caregivers receive seamless, compassionate care. The program officially launched on July 1, 2024, and BCN was selected to participate by the Centers for Medicare and Medicaid Services (CMS).
Bluegrass Dementia Care provides a standard approach that
• provides comprehensive care coordination and caregiver education,
• improves the quality of life for people living with dementia and their caregivers,
• reduces strain on their unpaid caregivers, and
• offers a 24/7 support line.
In January 2024, BCN was one of three participants building Dementia Care Programs (DCPs) in Kentucky, working to increase care coordination and improve access to services and supports. With support from Bluegrass Dementia Care, caregivers and people living with dementia can remain safely in their homes and communities.
