Senior Support Resources Directory Overview The 2013 Senior Support Resources Directory was created from the Topeka & Shawnee County Senior Health Fair & Symposium in October 2012. This Directory will help Seniors and their Support Teams find where to go for help, and insure they know how programs work to make the best decisions. The Symposium provided experts from the community in the following areas: • • • •

Financial & Legal Planning Living Options Recovery Options Programs (Medicare, Medicaid/KanCare & others)

These experts helped provide insights into how programs work and where they can go for help. The Senior Health Fair that followed allowed Seniors and their Support Teams to speak directly to the Providers. If a Provider was not the best fit for their needs, they referred them to an organization that was. The next logical step was to create this Directory, both in online and print versions, that would be a permanent guide and reference for Seniors and their Support Teams. The PDF and the link to the online version has been given to each Provider so they can place it on their website. We want to thank the community for your support of our efforts to bring the resources and support directly to you. A physician’s orders are required to receive Hospice care. Each patient is cared for by a team. Nursing and other medical and home-health personnel, social workers, chaplains, and volunteers provide services including pain relief, symptom management and supportive services for the patient and caregiver. Hospice staff assists the caregiver in caring for the individual. Staff can be reached 24 hours a day, seven days a week. Hospice agencies are certified and surveyed by the Kansas Department of Health and Environment PACE The Program for All-Inclusive Care for the Elderly (PACE) was developed by Medicare/Medicaid to meet the health care needs of individuals who wish to remain in the community. The PACE provider coordinates and provides all needed preventive, primary, acute, and long-term care services, so that older Kansans who need nursing home level of care can continue living in the community. The PACE service package must include Medicare and Medicaid covered services.

Continuing Care Retirement Community (CCRC)

Services Provided to Individuals Remaining in Home (Continued) Respite Respite care provides time off for persons caring for family members. Respite care covers a wide range of services that provides relief and allows the caregiver to take care of themselves, too. There are many types of services which may be used to provide caregivers the break they need. It may be asking a family member to sit with your loved one for brief periods of time. You may also choose to have a respite worker come into your home for a few hours, take your loved one to an Adult Day Care Center or an adult care home that provides respite care.

Improve Balance and Prevent Falls: Falls among the elderly are prevalent, dangerous, and can diminish their ability to lead an active and independent life. According to the National Aging Council, about one in three seniors above age 65, and nearly one in two seniors over age 80, will fall at least once this year, many times with disastrous consequences. A physical therapist can help you prevent falls by designing an individualized program of exercises and activities with an emphasis on strength, flexibility, and proper gait. Balance may be improved with exercises that strengthen the ankle, knee, and hip muscles and with exercises that improve the function of the vestibular (balance) system. Once a physical therapist has reviewed a complete medical history and conducted a thorough examination, he or she will develop a personalized plan of care. This may include a walking regimen with balance components such as changes in surfaces/terrains, distance, and elevations; Tai Chi (which emphasizes balance, weight shifting, coordination, and postural training); and aquatics classes geared toward balance and coordination. The physical therapist also may teach specific strengthening and balance exercises that can be performed at home. If necessary, the physical therapist will refer you to other medical professionals, such as an ophthalmologist or neurologist. Hours: 8:00 AM—4:30 PM, Monday—Friday Services/Support: Meals are provided to participants who are elderly, homebound, disabled, or otherwise at risk of food insecurity throughout Shawnee and Jefferson Counties regardless of age or inability to contribute toward the cost of the meal. In addition to meal provision, services include: • Nutrition screening • Education on nutrition • Nutritional assessment and counseling, as appropriate Home delivered meals also represent an essential service for many in-home caregivers, by helping them to maintain their own health and well being. Caregivers in many cases are eligible to receive a meal as well. Volunteers who deliver meals play a vital role by reporting to Meals on Wheels any health and environmental changes or crisis they may have observed during their visits. This includes participants that did not answer their door. Reported or suspected issues will be immediately addressed by Meals on Wheels staff to carry out next steps as previously outlined by participants. Meals served are required to provide at least one-third of the daily recommended dietary allowances established by the Food and Nutrition Academy of Sciences National Research Council. Physicians may also order a special diet to assist in the management of chronic illnesses up to and including: diabetes, kidney failure, congestive heart failure, and chewing/swallowing problems. In an effort to meet rural participant needs, Meals on Wheels has established a Multiple Meal Delivery program. Services/Support: • Downtown Branch: Educational programs and activities geared to the needs of the community. Facilities include three gymnasiums, swimming pool, whirlpool, indoor track, racquetball/handball courts, locker rooms, showers, and child wellness center. Services include aquatics (swim lessons to competitive swimming), Summer Day Camp, youth sports, programs for active older adults, community development, and wellness/fitness. • Kuehne Branch: Programs similar to the Downtown YMCA. Facilities include two gymnasiums, weight room, swimming pool, racquetball/handball courts, youth game room, saunas & whirlpools, locker rooms and five acres of outdoor playing fields. • Camp Hammond: 175 acres (15 acre lake) located 15.5 miles southeast of Topeka; maintained as an outdoor camp for boys, girls and adults. • Southwest Branch: Facility includes gymnasium, swimming pool, soft-play area, nursery, teen center, multi-purpose room, wellness center. Eligibility: Call for information. How to Apply: Call or come to any YMCA Branch. Scholarships are available. Fees: Based on type of membership or ability to pay. Sliding fee scale used for program and membership scholarships. Funding: Membership fees, United Way of Greater Topeka, individual contributions, program fees and other.
