Some parents in Westchester County are cheering the start of a new school year. However, others may be singing the blues, knowing they’ll be missing leisurely days of easy hugs or spontaneous family outings, as their little ones learn to navigate school hallways and unfamiliar social terrain.

Yes, parents can experience the back to school blues, too. If you’re dreading a new school year, there are simple ways to cope with the separation anxiety, even if you can’t watch your little one get on the bus: · Be a school volunteer. There are lots of opportunities for parents to be involved in the school.

Plan a school party, sign up as a reading volunteer or math aid or come in on career day to talk about your job.

It's also nice for the child. It gives them a sense of being special. · Pursue other interests. Parents who suddenly have time on their hands can embrace interests they may have put off, such as yoga, music lessons, a book club, volunteering at a food bank or signing up for some adult education classes. · Remember, it’s OK to cry. Don’t do it at the child's school, or in front of the child, but it’s OK to cry in private. Any kind of change can engender feelings of loss. Losing day-to-day contact does require a shift and sometimes that's a little uncomfortable. It will hurt, she says, but it will also get better.

If you feel more like cheering than crying at the sight of a little one climbing onto the school bus, go for it.

Free NY METS tickets were given out to all folks who donated blood. The blood drive benefits The New York Blood Center/Hudson Valley Blood Services, which is a not-for-profit blood center responsible for supplying 448,000 units of blood to Hudson Valley, NYC and Westchester hospitals.

From Urgent Care and Wellness Center helped them achieve their mission and give cancer, transplant, leukemia, newborn babies and trauma patients a fighting chance.

“Chief’s Challenge” scores again: Westchester County Police and Mohegan Lake Firemen win big for fitness

The Westchester County Police Department and Mohegan Lake Fire Department were each recently awarded $1000 as winners in the 2014 Chief’s Challenge, a program designed to improve the health and fitness of first responders by encouraging them to take advantage of discounted memberships at local health clubs.

Of the nearly 200 participants from 35 police and fire departments throughout Westchester and Putnam Counties and New York City, the two winning departments tallied up the highest levels of participation by their members.

Working in collaboration with Club Fit in Briarcliff and Jefferson Valley, The Chief’s Challenge was established and funded by the Heart and Health Education Foundation, directed by Franklin Zimmerman, MD, Arthur Fass, MD, and Dina Katz, MD, all members of the Phelps Medical Associates cardiology practice in Briarcliff Manor.

The program was inspired by research conducted by Dr. Zimmerman, a nationally-recognized authority in the treatment and prevention of cardiovascular disease in public safety personnel. His findings have been widely cited at national law enforcement meetings and in publications.

“My research shows that law enforcement personnel have a high risk of heart disease and cardiovascular risk factors. One only needs to listen to the nightly news to understand the high level of stress these men and women endure. Our program seeks to create an incentive for first responders to maintain cardiovascular health and fitness.”

For Dr. Zimmerman, his passion for the subject is both professional and personal. His sister, Shelley Zimmerman, is Chief of Police of San Diego.

Thomas Eade, Lieutenant in the Lake Mohegan Fire Department, was enthusiastic about the program. “Fitness is a paramount consideration for firefighters, and adding some competition about fitness gives people a push, and makes them strive to be better at their occupation,” remarked Lt. Eade. The money awarded by Chief's Challenge will help offset the cost of the fitness machines for their station.

“There are stresses associated with police work and the shifts they have to work,” said Stephen Smith of the Westchester County Police Department. “Staying in shape and eating a healthy diet helps officers address challenges they are confronted with every day. We consider health important and promote it in the department.”

A Police Benevolent Association vice president, he will use the Chief's Challenge award to purchase exercise equipment for the police academy, which will also enable other recruits and members of the department to benefit.

The 2015 Chief's Challenge is under way, with high hopes for an even bigger turnout of public safety personnel this year.

NYMC Phelps Family Medicine Residency Graduates First Class of Residents Healthcare News - Westchester September, 2015

The first six doctors to graduate from the NYMC Phelps Family Medicine Residency Program were honored at graduation ceremonies held recently at Phelps Memorial Hospital Center.

Three years ago, these first residents arrived at Phelps to begin their training as family medicine physicians.

The vision of the program has always been that graduates would choose to stay locally after they completed their three-year residency, providing the region with excellent family physicians. So far, three of the first six graduates have accepted positions in Westchester and one in the Bronx.

Anesh Badiwala, MD, will be practicing family medicine on the staff of Phelps Medical Associates, Phelps’ primary and specialty care medical group; Jorge M. Espana, MD, has joined the staff at Port Chester Open Door Family Medical Centers; and London Muse, MD, has accepted a teaching position on the faculty of the NYMC Phelps Family Medicine Resi-

dency Program. Sara Marie Paul, MD, will be continuing her education nearby in the obstetrics fellowship program at Lincoln Medical and Mental Health Center in the Bronx. At press time, Batel Isenstein, MD, and George Elkomos, MD, had not yet announced their plans.

The NYMC Phelps Family Medicine Residency Program is a collaboration developed by three major Westchester health institutions: New York Medical College, Open Door Family Medical Centers and Phelps.

Physicians at Phelps provide residents with the broad range of clinical training that is required for board certification in family medicine. In addition to rotations in the hospital, residents see outpatients at Open Door Family Medical Centers’ Sleepy Hollow location. New York Medical College in Valhalla is the sponsoring academic institution for the program, which received national accreditation in 2011 and was recently reaccredited for a period of 10 years.

Family physicians are primary care doctors who treat the whole person in every stage of life. They are trained to take care of the entire family, dealing with issues that may affect more than one family member and creating long-term, personal relationships with their patients and their communities.

The best breakfast foods give you fuel in the tank for energy that lasts. They boost your metabolism, fight disease and help you keep your weight down. The worst breakfast foods do the opposite. They lead to mid-morning crashes, wreak havoc on your metabolism, encourage disease and cause weight gain.

You shouldn’t skip breakfast — doing so can make you gain weight, lose energy and think poorly. But you should skip the worst breakfast options. If you see these five foods on the menu, run the other way.

1. Doughnut and pastries: A recipe for weight gain Doughnuts will cost you 250–550 calories, but the 20–50 grams of sugar in each is the real problem. With such a huge amount of sugar in a small package, your body pumps out lots of insulin to accommodate. A huge blood sugar peak leads to an even bigger sugar crash. This extreme up-and-down leaves you hungry soon after your breakfast — and you’ll crave more refined carbs. It’s a vicious cycle of unhealthy eating that starts with the first doughnut.

2. Sausage biscuit: Hypertension’s helper The sausage biscuit is basically a saturated fat and sodium bomb nestled in a trans fat sleeping bag. If your blood vessels could talk, they would plead, “Please don’t do it to us!” as you place your order at the fast-food drive-through. The sky-high sodium in the highly processed sausage can make your blood pressure surge.

If you have hypertension, it may increase your risk for stroke.

Nitrates and nitrites in sausage have been linked to increased risk of certain cancers, too.

3. Flavored non-dairy creamer: A coffee disaster If you think non-dairy creamer is a healthy option, think again.

Many non-dairy creamers simply swap saturated fat for trans fat (check the label for “partially hydrogenated” oil), plus sugar and artificial sweeteners. Trans fat increases your risk of heart attack and stroke by increasing LDL cholesterol. Predictions say decreasing trans fat consumption by even a little could help prevent more than 10,000 deaths a year. To perk up your coffee, try unsweetened vanilla almond milk, low-fat milk or a small amount of chocolate milk instead.

4. Bright, sugary cereals: A rainbow of hyperactivity Those magically colored kids’ cereals aren’t such a bright choice.

The FDA has noted that food dyes may contribute to hyperactivity in children with ADHD, even if not in other children. A 2012 study backed up that idea but said more research is needed. Even if food coloring’s effects aren’t fully understood, these cereals are usually loaded with sugar — empty calories for your little ones.

5. Loaded bagel: An invitation for diabetes Your body works hard to keep you functioning at night. Don’t thank it with inflammationcausing calories in the form of a bagel loaded with cream cheese or butter.

Except for the occasional 100 percent whole grain option, most bagels are 300–500 calories worth of starch. Slathering on cream cheese or butter adds more calories and saturated fat.

Diets high in refined carbohydrates have been linked to increased risk of type 2 diabetes and cardiovascular disease, so don’t make bagels a regular morning meal.

Save on Taxes! Defer Your IRA Minimum Required Distributions To Age 85!

By Andrew J. Cavaliere, CLTC I wrote about this topic earlier in the year, but I wanted to revisit it because it is so important. Do you want to be able to defer those unwanted Required Minimum Distribution (RMDs) from your IRA after age 70½…? Well now you can and at the same time avoid those unnecessary federal and state income taxes.

RMD rules require all individuals with qualified money to take distributions from those accounts beginning at age 70½, with respect to a portion of their total retirement savings, whether they need the income or not. In July 2014, the U.S. Treasury Department removed a significant impediment to the ability of qualified plan participants and IRA owners to balance their use of annuities and other investments in their retirement plans by issuing final regulations that allow individuals to bypass Required Minimum Distribution (RMD) rules.

The new regulations allow individuals to essentially defer a percentage of their RMD distributions and the taxes associated with those distributions with no penalty! The individual establishes a Qualified Longevity Annuity Contract, or “QLAC,” a guaranteed deferred annuity, under which the amount of annuity payments is locked in at purchase, and those payments can be deferred at a later date up to age 85.

These annuities are also referred to as Deferred Income Annuities, or “DIAs.” DIAs have been available in the marketplace for some time before these regulations became effective; however, when they were purchased under a qualifying employer-sponsored plan or Individual Retirement Account (IRA), the annuity payments generally needed to commence by age 70½, or the individual’s actual date when RMDs must commence. Thus, QLACs generally encompass such DIA contracts with income start dates after age 70½ and at or before age 85, while non-QLAC (or NQLAC) DIAs remain available for income start dates up to age 70½ in qualified employer plans or IRAs, and are not subject to the new QLAC limitations.

Limits on premiums / purchase payments

➢ Over their lifetime, individuals cannot allocate more than $125,000 of their qualifying retirement plan [401(a)/(k), 403(b), governmental 457(b)] and IRA (traditional; not Roth nor inherited IRAs) savings to QLACs. ➢ Additionally, no more than 25% of the participant’s account in any one qualifying retirement plan may be allocated to purchase one or more QLACs. This limit also applies to traditional IRAs; however, rather than applying to each separate IRA, the limit applies to the aggregate of the individual IRAs. Unlike Roth IRAs, Roth accounts within retirement plans are eligible for QLACs because they are also subject to RMD requirements. ➢ The 25% limit applies differently to plans and to IRAs. Under a plan the 25% limit applies to the most recent valuation of the plan, and under an IRA it applies to the prior 12/31 aggregated IRA balance. In both cases the value includes the present value of any previously purchased QLAC or NQLAC under the plan or the IRAs. The plan account value is also adjusted for any subsequent contributions or distributions. It is unclear whether this same adjustment applies to the 12/31 IRA value.

The bottom line is that individuals can avoid thousands of dollars in unwanted taxes every year. They can leave a portion of their required minimum distributions inside their IRAs or qualified retirement plans; 25% of your IRA money up to a maximum of $125,000 can be deferred until age 85 if you so choose.

Reflux disease treatment side effect risk – why take the chance?

Heart attacks, pneumonia, diarrhea, and hip fractures – does taking a medication for reflux increase the risk of these problems?

Reflux disease has been steadily increasing over the last few decades. Most doctors prescribe a proton-pump inhibitor such as Nexium, Omeprazole, Prevacid or an H2-blocker such as Zantac or Pepcid. These medications can decrease acid secretion and control symptoms.

However, they may do so with a price: high cost, long term need, possible drug interactions, and a host of potential side effects. These drugs have been linked to possible problems with calcium absorption and associated fracture risk, diarrhea, pneumonia, and most recently heart attack risk. These risks are likely quite rare and remain unconfirmed, but taking any medication certainly poses a potential risk of a complication or drug interaction.

PPIs, proton pump inhibitors, are a class of drugs commonly used to treat acid reflux disease to the tune of over 14 billion

dollars a year in the United States alone.

H2-blockers are an older class of drugs also used to block acid secretion in the stomach.

Both are used to treat GERD, gastroesophageal reflux disease (heartburn, indigestion, chest discomfort) as well as LPR. LPR, or Laryngopharyngeal reflux disease, is reflux that affects the throat and other head and neck structures. The most common symptoms are throat clearing, hoarseness, a lump in the throat, chronic coughing, difficulty swallowing, and post-nasal drip sensation. LPR can also affect sinus disease, dental problems, asthma and other lung problems.

Diagnosis of LPR is based on the above symptoms and examination of the throat with a small camera placed through the nose by a laryngologist (ENT doctor specializing in voice, swallow, and cough disorders).

The standard dietary and lifestyle changes include: small meals, waiting 3 hours before bed after eating, raising the head of the bed, loose fitting clothing as

well as stopping coffee, tea, chocolate, soda, greasy food, fatty food, fried food, alcohol, salt, and smoking.

PPI treatment, when used appropriately, can help prevent prolonged symptoms, ulceration, and even possible progression to cancer. However, PPIs should not be a substitute for diet and lifestyle changes.

Lifestyle and diet, drinking alkaline water – pH>8.0 and eating a 90% plant based Mediterranean style diet can effectively control symptoms and prevent progression of reflux disease without medication and all of its possible risks and side effects.

As a board-certified internist who has been in practice for over 13 years, I spend much of my time attempting to alleviate the pain and suffering of my patients. As a clinician, one of the biggest challenges I face is helping my patients deal with musculoskeletal and neuropathic pain syndromes—those all-too-common, yet frequently debilitating ailments caused by bad backs, pinched nerves, arthritis, and the like. Unfortunately, much of what I have to offer my patients boils down to the following: take a pain pill, stretch if you can, wait, and hope for the best. After a failed trial of “conservative” therapy a call might go out to the orthopedic surgeon for assistance; a truly satisfactory result is not common. The simple fact is that physicians are not well trained to deal with musculoskeletal pain, and patients can suffer as a result.

Thankfully, my views on this topic have been turned completely upside down since I havepersonally discovered Dr. Jonathan Donath’s outstanding practice. After beginning a vigorous exercise regimen I started to experience awful neck and upper back pain, accompanied by numbness and tingling down my left arm and fingers. For weeks I tried to nurse the pain with non steroidal anti-inflammatory medication, but the pain persisted and got to the point that I was almost unable to work, sleep, or most importantly, play guitar. I wasliterally in tears from the excruciating pain. A neurosurgeon diagnosed a herniated disc and pinched nerve in my neck and offered to decompress it with an operation. In desperation I called Dr. Donath instead.

My visit to Dr. Donath’s office was an outstanding experience. He spent about an hour with me and combined multiple modalities to fix the herniated disc in my neck: deep massage, active release techniques, traditional manipulation, cold therapy, and finally spinal decompression. After that first hour with Dr. Donath my pain had decreased from a 10/10 down to a manageable 4/10, and I was able to comfortably move my head and neck. That very night I was able to play guitar again. Two days later I went back for another series of treatments and the pain decreased even further. After about five days the pain was absolutely gone. Unbelievable! Since then he has used the same wonderful techniques to help me overcome painful bouts of sciatica as well.

My own patients and family members have become “believers” in Dr. Donath’s abilities as well--- the results he achieves are real and lasting. He is my go to guy for the majority of musculoskeletal and neuropathic pain syndromes that I now realize are truly treatable.

I am a pediatrician and in my work, I have to bend down a lot and I’m constantly lifting kids. I usually have one to three bad flare ups of low back pain every year. When my back would "go out", I could barely walk to the bathroom and would have to lie down in bed for a few days. I have tried everything: doing nothing and "toughing it out", muscle relaxants, physical therapy, chiropractic, therapeutic massage, and even painful steroid injections. Nothing worked.

Since that last back episode, Dr. Donath has helped me with my tendonitis in my right hand and with my chronic knee problems. I recommend Dr. Donath to everyone with back, muscle or joint pain. Even if you have not had relief with other treatments and other Chiropractors, give Dr. Donath a chance. I did and he changed my life.
