Where do doctors go to treat their own pain?

Dr. Robert Sidlow MD, MBA—Internist, New York, NY “As a board-certified internist who has been in practice for over 13 years, I spend much of my time attempting to alleviate the pain and suffering of my patients. As a clinician, one of the biggest challenges I face is helping my patients deal with musculoskeletal and neuropathic pain syndromes-those alltoo-common, yet frequently debilitating ailments caused by bad backs, pinched nerves, arthritis, and the like. Unfortunately, much of what I have to offer my patients boils down to the following: take a pain pill, stretch if you can, wait, and hope for the best. After a failed trial of "conservative" therapy a call might go out to the orthopedic surgeon for assistance; a truly satisfactory result is not common. The simple fact is that physicians are not well trained to deal with musculoskeletal pain, and patients can suffer as a result. Thankfully, my views on this topic have been turned completely upside down since I have personally discovered Dr. Jonathan Donath's outstanding practice. After beginning a vigorous exercise regime, I started to experience awful neck and upper back pain, accompanied by numbness and tingling down my left arm and fingers. For weeks I tried to nurse the pain with non steroidal anti-inflammatory medication, but the pain persisted and got to the point that I was almost unable to work, sleep, or most importantly, play guitar. I was literally in tears from the excruciating pain. A neurosurgeon diagnosed a herniated disc and pinched nerve in my neck and offered to decompress it with an operation. In desperation I called Dr. Donath instead. My visit to Dr. Donath's office was an outstanding experience. He spent about an hour with me and combined multiple modalities to fix the herniated disc in my neck: deep massage, active release techniques, traditional manipulation, cold therapy, and finally spinal decompression. After that first hour with Dr. Donath my pain had decreased from a 10/10 down to a manageable 4/10, and I was able to comfortably move my head and neck. That very night I was able to play guitar again. Two days later I went back for another series of treatments and the pain decreased even further.

After about five days the pain was absolutely gone. Unbelievable! Since then he has used the same wonderful techniques to help me overcome painful bouts of sciatica as well.

My own patients and family members have become "believers" in Dr. Donath's abilities as well--- the results he achieves are real and lasting. He is my go to guy for the majority of musculoskeletal and neuropathic pain syndromes that I now realize are truly treatable.”

Support efforts that help communities prepare for and adapt to the health impacts of climate change.

Encourage healthy behaviors and choices Avoid using tobacco, alcohol and other drugs. Eat healthy foods and exercise.

We need to make these and other healthy choices for ourselves. But it doesn’t stop there – we need to work together to create communities that make the healthy choice the easy choice for everyone.

Provide quality health care for everyone Health reform was just a start. To fulfill its potential, we must continue to pursue options for expanded access to quality care at the federal, state and local levels. But we also need to shift the main focus of our health system from one that treats illness to one that equally emphasizes prevention.

Strengthen public health infrastructure and capacity Strong and consistent funding levels are necessary for the public health system to respond to both everyday health threats and unexpected health emergencies. Support higher budgets for key public health agencies like the CDC and HRSA. April marks both National Public Health Month and Occupational Therapy Month – an ideal time to focus on two important aspects of healthcare.

Public Health Month emphasizes prevention, information and simple steps everyone can take to boost our personal lifestyle habits and help create a healthier nation.

Recovering from Heart Disease with Cardiac Rehabilitation

According to the American Heart Association, approximately every 40 seconds an American will suffer a heart attack. Cardiovascular disease is the underlying cause of about one out of every three deaths in the U.S. However, many people not only survive a cardiac event, they manage to return to living normally—especially if they participate in cardiac rehabilitation. Cardiac rehab is a supervised program of exercise, education and counseling designed to help people recover from a heart attack, heart surgery or heart condition.

The American Heart Association has found that people of all ages who have a heart condition can benefit from cardiac rehabilitation, which is covered by most insurance plans as well as Medicare.

Yet despite the fact that cardiac rehab increases the chance of survival and regaining health, this therapy is underused due to low patient referral and a lack of public awareness of its importance.

“It is crucial that anyone who suffers from heart disease—or has a family member or friend with these conditions—is aware of the profound benefits of cardiac rehab,” says Richard S. Novitch, M.D., director of the Cardiopulmonary Rehabilitation program at Burke Rehabilitation Hospital in White Plains, NY. “Patients who participate in cardiac rehab tend to recover faster, while improving their overall health and quality of life.” Burke

Rehabilitation Hospital was one of the founding members of the American Heart Association in 1924.

Cardiac rehabilitation at Burke features three phases, encompassing both inpatient and outpatient care:

• Phase I is an inpatient program and begins when the patient is in the hospital or once they’ve been transferred to an acute rehabilitation hospital, like Burke. It involves limited daily exercise and activities, along with information about making heart-healthy lifestyle changes.

• Phase II takes place after the patient is discharged from the hospital and is done in an outpatient setting under close medical supervision.

Burke offers a 12-week program that includes ECG telemetry-monitored exercise sessions and nutritional guidance in choosing a heart-healthy diet, along with advice on weight loss, smoking cessation, and stress reduction.

• Phase III, which requires medical clearance, is devoted to maintenance of a patient’s new, healthier lifestyle. This phase includes an exercise regime supervised by trainers, as well as yoga, tai chi, strength training classes and education.

Benefits of

Occupational Therapy for

Autism

Occupational therapy may combine a variety of strategies. These can help your child respond better to his or her environment. These OT strategies include: • Physical activities, such as stringing beads or doing puzzles, to help a child develop coordination and body awareness • Play activities to help with interaction and communication • Developmental activities, such as brushing teeth and combing hair • Adaptive strategies, including coping with transitions

What are the benefits of occupational therapy for ASD? The overall goal of occupational therapy is to help the person with autism improve his or her quality of life at home and in school. The therapist helps introduce, maintain, and improve skills so that people with autism can be as independent as possible.

These are some of the skills occupational therapy may foster:

A person who has autism spectrum disorder (ASD) often has trouble communicating and interacting with other people; his or her interests, activities, and play skills may be limited.

Occupational therapy may help people with autism develop these skills at home and in school.

Occupational therapists study human growth and development and a person’s interaction with the environment through daily activities. They are experts in the social, emotional, and physiological effects of illness and injury. This knowledge helps them promote skills for independent living in people with autism and other developmental disorders.

Occupational therapists work as part of a team that includes parents, teachers, and other professionals. They help set specific goals for the person with autism. These goals often involve social interaction, behavior, and classroom performance.

Occupational therapists can help in two main ways: evaluation and therapy.

How is occupational therapy useful for evaluation of ASD? The therapist observes children to see if they can do tasks they are expected to do at their ages -- getting dressed or playing a game, for example.

Sometimes, the therapist will have the child videotaped during the day in order to see how the child interacts with his or her environment so that he or she can better assess the kind of care the child needs. The therapist might note any of the following: • Attention span and stamina • Transition to new activities • Play skills

• Need for personal space • Responses to touch or other types of stimuli • Motor skills such as posture, balance, or manipulation of small objects • Aggression or other types of behaviors • Interactions between the child and caregivers

How does occupational therapy help a person with autism spectrum disorder?

Once an occupational therapist has gathered information, he or she can develop a program for your child. There is no single ideal treatment program. But early, structured, individualized care has been shown to work best.

• Daily living skills, such as toilet training, dressing, brushing teeth, other grooming skills • Fine motor skills required for holding objects while handwriting or cutting with scissors • Gross motor skills used for walking, climbing stairs, or riding a bike • Sitting, posture, or perceptual skills, such as telling the differences between colors, shapes, and sizes • Awareness of his or her body and its relation to others • Visual skills for reading and writing • Play, coping, self-help, problem solving, communication, and social skills

By working on these skills during occupational therapy, a child with autism may be able to:

• • • • • •

Develop peer and adult relationships Learn how to focus on tasks Learn how to delay gratification Express feelings in more appropriate ways Engage in play with peers Learn how to self-regulate

How can someone obtain OT services for autism spectrum disorder?

You can obtain occupational therapy services either privately, through a statewide early childhood intervention program, or at school. Public law requires schools to provide certain types of occupational therapy to those who need it. Private insurance also usually covers OT. In addition, Medicaid may cover occupational therapy for autism, even for families with higher incomes.

How To Reduce Medical Bills On Any Health Plan

The United States is in a time of transition.

Anxiety over changes in health insurance and paying medical bills is rising as the Senate took steps to repeal “Obamacare” and the administration made an executive order about health care on January 20th, 2017. Advice from a medical bill advocate on how to lower health costs on any plan is needed now more than ever!

Some of the rights and protections of Affordable Care Act policies are coverage for people with pre-existing conditions, dependent coverage to age 26, an annual out-of-pocket limit: capping the amount individuals are expected to pay each year, prohibit annual and lifetime limits: stopping coverage from being cut off from use, as well as, prevention services without cost sharing: giving people access to preventative care. These benefits make it difficult to replace “Obamacare”. Without a replacement to ensure that services continue to

be subsidized and the self-insured continue to be insured the market may become unstable.

Any instability in the market and coverage would be unfortunate because as the ACA has matured the preventative services market has expanded, improving early detection and healthier living.

Medicaid expanded under the ACA. Usage information about service habits and illnesses have increased. While working with the government, the San Francisco startup NUNA has taken the data of the Nation’s 74 million Medicaid patients and developed a cloudcomputing database. With this data the ability to track illnesses and service usage has improved the preventative care services that insurance carriers encourage, medical professionals advise and patients benefit from. The Act has caused the development of higher value care over rewarding volume of services paid out.

Some of the high volume care services provide for: technological innovations that make services more patient-centered, alternative financing to allow for payment flexibility, core metrics for measuring collective progress and aligning providers and payers, as well as, increase collaborative efforts to connect health with social services improving health outcomes at the community level. These developments are reshaping the system to better care for people and cost everyone less in long term health care.

Avoid getting ripped off by insurers, medical providers and the government with advice from a medical billing consultant, who helps lower medical bills by spotting overcharges by providers, under-reimbursements by insurers, and inappropriate denials by insurers and government employees. As the ACA has encouraged medical professionals to collaborate and increase value care to decrease medical costs.

The fair gave children a chance to familiarize themselves with an Urgent Care environment, as well as with the staff and procedures. Organized by AFC Urgent Care’s Outreach Staff and YCNS,

the event provided children with a fun activity and valuable information, helping to alleviate any fears they may have of the healthcare environment.

Many people with cancer are interested in trying anything that may help them, including complementary and alternative cancer treatments. If cancer makes you feel as if you have little control over your health, alternative cancer treatments may offer some feeling of control. But many alternative cancer treatments are unproved and some may even be dangerous.

To help you sort out the good from the bad, here are 10 alternative cancer treatments that are generally safe. Plus, there is growing evidence that these 10 alternative cancer treatments may provide some benefit.

Hypnosis Hypnosis is a deep state of concentration. During a hypnotherapy session, a therapist may hypnotize you by talking in a gentle voice and helping you relax. The therapist will then help you focus on goals, such as controlling your pain and reducing your stress.

Acupuncture During acupuncture treatment, a practitioner inserts tiny needles into your skin at precise points. Studies show acupuncture may be helpful in relieving nausea caused by chemotherapy.

Acupuncture may also help relieve certain types of pain in people with cancer.

Aromatherapy Aromatherapy uses fragrant oils to provide a calming sensation. Oils, infused with scents such as lavender, can be applied to your skin during a massage, or the oils can be added to bath water.

Montefiore pilots new model to advance integration of behavioral health services in New York primary care practices Grants from the United Hospital Fund and New York State Health Foundation to help people overcome barriers to behavioral healthcare

Montefiore Health System, the University Hospital for Albert Einstein College of Medicine, has been awarded two grants to improve the ability of primary care physicians in small practices to treat common behavioral health conditions, such as anxiety and depression. One in five New Yorkers experiences mental distress, which frequently affects their physical health. Most do not receive adequate care due to stigma and limited access to behavioral health services.

With $188,320 from the United Hospital Fund (UHF) and $215,670 from the New York State Health Foundation, Montefiore will support and evaluate a new guide it has spearheaded, “Advancing Integration of Behavioral Health into Primary Care: A Continuum-Based Framework,” to help primary care providers implement and improve access to behavioral health services in six primary care practices in New York City and five more elsewhere in New York State.* “When healthcare is fragmented, people fall through the cracks,” said Henry Chung, M.D., vice president and chief medical officer of Montefiore’s Care Management Organization (CMO), associate professor, Clinical Psychiatry, Albert Einstein College of Medicine, lead author of the framework and the principal investigator of the pilot. “We will be working with small primary care practices that are critical to the overall health of so many New Yorkers. By collaborating closely with these providers, we can help amplify their behavioral health services offerings and better address their patients’ mental health needs.”

Up to 25,000 individuals will receive depression screenings and 1,000-2,000 individuals will benefit from integrated services in these practices. Using the framework, the practices aim to improve their integration activities based on their resources and geography.

Montefiore has been a leader in the integration of behavioral health in primary care. Through a Centers for Medicare and Medicaid Services innovation grant

led by Dr. Chung, Montefiore patients have benefited from primary care site visits that are supported by a dedicated behavioral health collaborative care team, which may include a psychiatrist, psychologist or a social worker and care manager. Patients receive in-person introductions to behavioral health providers and participate in therapy sessions that focus on problem solving techniques. If medication is required, prescriptions are given by primary care providers with reviews and consultations from a psychiatrist. This has led to more than 2,000 patients with depression experiencing a nearly 50 percent improvement in severity of depression and associated symptoms, according to a standard patient health questionnaire.

To advance the new pilot, Dr. Chung and his team will support the use of the framework among the participating practices, advise on primary care sitespecific goals and lead monthly knowledge sharing and problem solving webinars for improving behavioral health integration. Practices will provide information about their progress, successes and barriers to advancing integration; information which is required to develop quality and sustainability strategies for small practices.

The pilot will result in practice changing recommendations and an updated guide that will inform clinicians, policymakers, and payers on how to best advance behavioral healthcare.
