A recent Federal study shows that men die from heart disease and chronic liver disease at nearly twice the rate of women.

WHAT IS SLEEP APNEA? Sleep Apnea is estimated to affect 20 million people. Sleep Apnea occurs when the airway becomes completely blocked and air is not able to move through the airway. This is commonly called Obstructive Sleep Apnea because the airway is totally obstructed. This obstruction may occur anywhere from the area of the soft palate down to below the tongue. When the airway is obstructed, the patient will awaken many times during the night with a cough or gasping sound as the body attempts to restore normal breathing. Many major health problems that affect men and are preventable through lifestyle changes, early detection, and treatment. Still, when it comes to health care, some men are like ostriches. They bury their heads in the sand. That isn’t good enough. Just ask our own resident men’s health expert, Dr. William Higgins, Vice President of Medical Affairs at Hudson Valley Hospital Center at Cortlandt Manor. “Women drive healthcare,” Higgins said. “It’s not often a man drags his wife to the doctor, it’s usually the other way around. I do think it’s important for men to establish a relationship with a primary care physician when they are 40. The doctor should know your family and social history, your coping mechanisms. It’s a key process and can reduce your risk factors for heart disease and cancer.” Several things work against men when it comes to health. They tend to smoke and drink more than women. They don’t seek medical help as often as women. Some men define themselves by their work, which can add to stress. continued on next page

Here are some tips to help men maintain good health and long lives. Get routine exams and screenings. Ask your doctor how often you need to be examined. Ask about screening tests for certain diseases and conditions, including high blood pressure, high cholesterol, diabetes, sexually transmitted infections, and certain types of cancer. Get enough sleep. It can affect your mood and your health. See your doctor if you think you have a serious problem. Eat healthy. Nutritious foods give you energy and may lower your risk of certain diseases. Focus on fruits and vegetables, whole grains, and low-fat or fat-free milk products. Stay at a healthy weight. Being overweight or obese can raise your risk of diabetes, high blood pressure, heart disease, and stroke. Find out your body mass index, or BMI, to see if you’re at risk. Get moving. Regular exercise is one of the most important things you can do for your health. Find out how much physical activity you need. Be smoke-free. Smoking is linked to many of the leading causes of death, including cancer, lung disease, and stroke. If you smoke, quit today! Also, avoid secondhand smoke. Take any medications you need. Make sure to follow your doctor’s instructions for all medications, including those that help control conditions like high blood pressure and diabetes.

Avoid heavy drinking, which can lead to many problems, including high blood pressure, various cancers, psychological problems, and accidents. For men 65 and younger, drinking in moderation means no more than two drinks per day. Men older than 65 should have no more than one drink a day. Learn more about drink serving sizes.

Manage stress. Balancing work and family obligations can be challenging. But it’s important to protect your mental and physical health. Find health ways to cope with stress. “It’s just like everything else in life. If you can’t get an A, try for a B,” Higgins said. “You should integrate all of the tips mentioned and implement a plan for a healthier life.”

ASK AN EXPERT The Key to Successful Dentistry: Finding the Right Solution to a Problem By Edward Feinberg DMD When a diagnosis is made in dentistry, there are often several options for treatments. When choosing the best option, it is important to consider the potential outcomes and consequences. A perfect illustration is the case of Ms. S., a beautiful young lady who was born with missing lateral incisors and a malocclusion. Ms. S. had orthodontic treatment as a teenager. Her orthodontist solved her malocclusion (abnormal bite) by moving her teeth into alignment. The result is most certainly a tremendous improvement from her original bite. However, he inadvertently limited her restorative choices by not taking into account the spaces that were left by the missing lateral incisors. Her dentist addressed her immediate problem by restoring the missing teeth with a “Maryland Bridge”. The Maryland Bridge is a bar containing the missing teeth that is bonded to the inside surfaces of the adjacent teeth (in this case, the central incisors). This type of bridge periodically “de-bonds” and falls out. Certainly this is what happened to Ms. S. Worse, the leaking bridgework allowed extensive decay to occur on the inside surfaces of her two front teeth. (Fig. 1)

Figure 1 Unfortunately the type of orthodontic treatment Ms. S. had left spaces that were too small. The roots of the adjacent teeth were also positioned too close together to insert single tooth implants. It is unfortunate that planning for implant restorations was not included in her original orthodontic treatment plan.

The implant option would have been ideal for Ms. S. because the adjacent teeth could have been left untouched. Now, in order to have this option, Ms. S. would require another round of major orthodontics to create enough room for this restorative option. Since she was on her way to college, she did not want to undergo additional years of dental treatment. Since Ms. S. was not a candidate for implants, her best option was restorative dentistry. The Maryland Bridge had to be removed immediately before it destroyed her two front teeth. However, her newly-corrected bite did not leave much room for conventional restorative dentistry. Most dentists are trained to “hang” their crowns on tooth structure that is above the gum. When there is not enough tooth structure above the gum, they build-up the tooth structure with various materials, pins or posts. Often this does not work and the restorative dentistry loosens and comes out. This approach could never work for Ms. S. because there is not enough tooth structure above the gum to work with. However, there is an approach to dentistry that has been around for over 70 years that does not require any tooth structure above the gum. This approach focuses on tooth structure below the gum. Crowns and bridges are created with copper band impressions that utilize all of the tooth structure below the gum for their retention. As a result, they rarely fall out. In fact, they are usually worn with Vaseline® or rubber for a trial period prior to cementation and they do not fall out. These crowns are also designed like the Mason Jar cover--the best invention ever devised for food preservation--so they rarely decay. Because the margins are all the way under the gum, they are rarely visible. The copper-band approach--which is not taught in dental schools--makes it possible to offer unique treatment options to patients.

As a result, Ms. S. was able to have her restorative problem solved by preparing only one tooth on each side. In order to create enough room for restorations so that they would not be oversized, root canal treatment on the two canines had to be done first. In a young person, the nerves (pulps) inside the teeth are very large. It is often difficult to remove enough tooth structure in order to make dentistry that is not oversized. As a person gets older, the nerves tend to shrink, so they are less of a complication for crown and bridgework.

Cantilever bridges have structures that extend out like a marquis from a central support. The first cantilever bridge was the Forthrail Bridge constructed in Scotland between 1882 and 1990. Since then, cantilever bridges have become commonplace. Dentistry is very akin to Engineering; and the same principles apply to bridgework whether in the mouth or across the river. The canine teeth are the cornerstones of the arch and usually have the longest roots--a great foundation for dentistry. In a young person, the canines can easily withstand the extra load of the cantilever pontic, as long as there are no destructive forces applied to it. In order to achieve an esthetic result, Ms. S.’s canine restorations were meticulously designed to give the illusion of two teeth. The restorations

were accomplished with a step-by-step approach that required precision. it is critical to a successful outcome that no step is skipped and the outcome of each step is perfect before proceeding to the next step. Luckily Ms. S. did not have to worry about her temporary restorations falling out while she was away at college for months in between visits. Her temporaries were designed with copper bands so they fit just as well as permanent restorations, even though they were made from temporary materials. ( Fig 2) The decayed insides of her two central incisors were repaired and left untouched. Ms. S. now smiles with confidence and she is thrilled to have her dental problem solved. (Fig 3) Her bridgework should last many years, even though her dentistry must function continuously 24 hours a day under water, under forces and in an environment loaded with bacteria. Amazing!

Need a good night’s sleep? The Sleep Center at Phelps Memorial Hospital was recently fully accredited as a sleep center by the American Academy of Sleep Medicine. To be accredited, a sleep medicine provider must demonstrate compliance in areas such as patient safety and follow-up, testing procedures and policies, physician and staff training, and facility and equipment. “Our goal is to help people get a good night's sleep, which is key to proper development in children and contributes to overall health throughout life,” says Dr. Gary Lehrman, a pulmonologist and medical director of the Sleep Center at Phelps. “We are delighted that our efforts have been given national recognition by the American Academy of Sleep Medicine.” The Sleep Center, which was established at Phelps in 1997, performs approximately 100 sleep studies a month. The Center features six bedrooms with cable TV and private bathrooms. While patients stay overnight in the hospital, their sleep patterns are recorded, including activities of the heart, lungs, brain, muscle movements, air flow and oxygen levels in the blood.

Pediatric CME series offered to Westchester first responders

Blythedale Children’s Hospital, in coordination with the Westchester Regional EMS Office, Westchester County DES, and NYS EMS for Children, is proud to be offering a special pediatric CME series to pre-hospital providers of the Westchester EMS region. This series of symposiums, free to attendees, is funded by the Emergency Medical Services for Children Grant Award. Courses tackle subjects vital to first responders, including Traumatic Brain Injury, Autism, Technology and Care of the Infant or Child, and Child Abuse/Neglect. The subject matter is presented by clinical experts from Blythedale Children’s Hospital, including physicians, nurses, therapists and other clinical experts. “We are proud to partner with Westchester County Department of Emergency Services in providing this educational series to first responders, said Blythedale Children’s Hospital Chief Quality Officer Lois Allen. "It affords us the opportunity to share our wide-ranging expertise, as well as to give back to the community in a way that can benefit a broad pediatric population.” The programs are held on Tuesday evenings. Business Succession Planning: Business Owners Can Sell their Business to Children Tax Free Using Pension By Andrew J. Cavaliere, CLTC It is a classic American story. Grandfather started the business from nothing and worked his entire life from dawn to dusk. When it was time to retire, he left his modest but successful business to his son who had worked for him for years. After several more decades of hard work, dad took the business to the next level and it now has annual revenues of seven figures. Dad wants to retire but he can’t simply “give” the business to his sons. It is worth far too much money on the open market and most of his equity is in the company. He wants his children to take over the family enterprise, but he needs assurances that he will get his retirement money on time before he turns over the reins.

Few business owners look further than their IRC Sec. 401(k) as a succession planning funding mechanism. Mistakenly thinking they will have to shell out huge sums of money for their employees benefit, they are reluctant to review other options. There are IRC compliant plans that not only save the owner tremendous amounts of money in taxes, but also puts those same dollars into their own retirement plan. For small family controlled businesses, it does not get any better. What if the children can buy-out dad’s interest in the business through the pension plan? What if hundreds of thousands of dollars can be contributed into the pension plan by his children on behalf of dad. Not only does this transaction create a tax deduction for the children, who are the buyers, but it also creates a tax-free contribution for dad, the seller. This way over the course of several years, dad can be remunerated for the full fair market value of the company while assisting his children with the transition. Moreover, dad is not required to take a salary during the transition years, provided it can be shown that he had at least three (3) prior years of earned income. This allows his children even more corporate dollars to be utilized for dad’s pension contribution. Once the pension is fully funded, as per their agreement, dad retires and rolls his money out into an IRA. Qualified Defined Contribution & Defined Benefit Pension plans allow business owners to sock away much larger amounts of money than the $17,000 their 401(k) currently allows by the IRC. Defined Contribution plans, often referred to as Profit Sharing plans, “define” how much money an individual can contribute before taxes. The 2012 IRC Sec. 415 Defined Contribution Limit is 25% of salary up to $50,000 ($55,500 if age 50 or older). Please note that a 401(k) is a Defined Contribution plan usually with a small employer match, say 25% of the first 4% of an employee’s deferral. Many plans do not fully utilize the Profit Sharing component. Defined Benefit plans, often referred to as Pension plans, “define” a future “benefit” rather than defining a contribution amount today. The 2013 IRC Sec. 415 Defined Benefit Limit is $205,000 a year or $17,803 a month. As such, much larger contributions can be made in order to fund the benefit. The formulas include factors such as: salaries, plan provisions, assumed rate of asset growth and employee characteristics like age and job classification. Often, if the owner(s) are much older and much more highly compensated than the rest of the employees, the formulas permit them to put much more money away for themselves before taxes. Few business owners explore these options because of a pre-existing belief that previous tax law prohibited this kind of planning in the past. And here is where it gets even better. Due to the Pension Protection Act of 2006 (PPA), business owners can combine deduction limits Under IRC Sec. 404(a)(7). For plan years beginning in 2006, the IRC Sec. 404(a)(7) deduction limitation -- which limited deductions to 25% of eligible plan compensation when an employer sponsors both a Defined Contribution and Defined Benefit plan -- will be determined without regard to Defined Benefit plans that are covered by the Pension Benefit Guaranty Corporation (PBGC). So what does this mean? It means that a business owner can fully fund both a Defined Benefit plan and a Defined Contribution plan…! Simply put, much larger annual contributions and tax savings to the business owner. Moreover, Defined Benefit plans that cover at least one employee other than the business owners, partners and their spouses, provides protection of the contributions from creditors. This is of the utmost importance simply due to the litigious nature of the business climate we live in today. In summary, these plans provide a wide variety of limits and tax leverage that most business owners find desirable. In fact, because these plans maximize the share of the annual contribution to the owners, due to age and compensation factors, they are the perfect vehicle for family buy-out proposals. What do doctors do to treat their own pain? As a clinician, one of the biggest challenges I face is helping my patients deal with musculoskeletal and neuropathic pain syndromes—those all-too-common, yet frequently debilitating ailments caused by bad backs, pinched nerves, arthritis, and the like. Unfortunately, much of what I have to offer my patients boils down to the following: take a pain pill, stretch if you can, wait, and hope for the best. After a failed trial of “conservative” therapy a call might go out to the orthopedic surgeon for assistance; a truly satisfactory result is not common. The simple fact is that physicians are not well trained to deal with musculoskeletal pain, and patients can suffer as a result. Thankfully, my views on this topic have been turned completely upside down since I havepersonally discovered Dr. Jonathan Donath’s outstanding practice. After beginning a vigorous exercise regimen I started to experience awful neck and upper back pain, accompanied by numbness and tingling down my left arm and fingers. For weeks I tried to nurse the pain with non steroidal anti-inflammatory medication, but the pain persisted and got to the point that I was almost unable to work, sleep, or most importantly, play guitar. I wasliterally in tears from the excruciating pain. A neurosurgeon diagnosed a herniated disc and pinched nerve in my neck and offered to decompress it with an operation. In desperation I called Dr. Donath instead. My visit to Dr. Donath’s office was an outstanding experience. He spent about an hour with me and combined multiple modalities to fix the herniated disc in my neck: deep massage, active release techniques, traditional manipulation, cold therapy, and finally spinal decompression. After that first hour with Dr. Donath my pain had decreased from a 10/10 down to a manageable 4/10, and I was able to comfortably move my head and neck. That very night I was able to play guitar again. Two days later I went back for another series of treatments and the pain decreased even further. After about five days the pain was absolutely gone. Unbelievable! Since then he has used the same wonderful techniques to help me overcome painful bouts of sciatica as well. My own patients and family members have become “believers” in Dr. Donath’s abilities as well--- the results he achieves are real and lasting. He is my go to guy for the majority of musculoskeletal and neuropathic pain syndromes that I now realize are truly treatable. I am a pediatrician and in my work, I have to bend down a lot and I’m constantly lifting kids. I usually have one to three bad flare ups of low back pain every year. When my back would "go out", I could barely walk to the bathroom and would have to lie down in bed for a few days. I have tried everything: doing nothing and "toughing it out", muscle relaxants, physical therapy, chiropractic, therapeutic massage, and even painful steroid injections. Nothing worked.
