Where do doctors go to treat their own pain?

Dr. Robert Sidlow MD, MBA—Internist, New York, NY “As a board-certified internist who has been in practice for over 13 years, I spend much of my time attempting to alleviate the pain and suffering of my patients. As a clinician, one of the biggest challenges I face is helping my patients deal with musculoskeletal and neuropathic pain syndromes-those alltoo-common, yet frequently debilitating ailments caused by bad backs, pinched nerves, arthritis, and the like. Unfortunately, much of what I have to offer my patients boils down to the following: take a pain pill, stretch if you can, wait, and hope for the best. After a failed trial of "conservative" therapy a call might go out to the orthopedic surgeon for assistance; a truly satisfactory result is not common. The simple fact is that physicians are not well trained to deal with musculoskeletal pain, and patients can suffer as a result. Thankfully, my views on this topic have been turned completely upside down since I have personally discovered Dr. Jonathan Donath's outstanding practice. After beginning a vigorous exercise regime, I started to experience awful neck and upper back pain, accompanied by numbness and tingling down my left arm and fingers. For weeks I tried to nurse the pain with non steroidal anti-inflammatory medication, but the pain persisted and got to the point that I was almost unable to work, sleep, or most importantly, play guitar. I was literally in tears from the excruciating pain. A neurosurgeon diagnosed a herniated disc and pinched nerve in my neck and offered to decompress it with an operation. In desperation I called Dr. Donath instead. My visit to Dr. Donath's office was an outstanding experience. He spent about an hour with me and combined multiple modalities to fix the herniated disc in my neck: deep massage, active release techniques, traditional manipulation, cold therapy, and finally spinal decompression. After that first hour with Dr. Donath my pain had decreased from a 10/10 down to a manageable 4/10, and I was able to comfortably move my head and neck. That very night I was able to play guitar again. Two days later I went back for another series of treatments and the pain decreased even further.

After about five days the pain was absolutely gone. Unbelievable! Since then he has used the same wonderful techniques to help me overcome painful bouts of sciatica as well.

The benefits of regular physical exercise are well known: improved health, improved energy and mood, better sleep, and weight control. Per the Centers for Disease Control and Prevention, every adult should aim for a minimum of 150 minutes a week of moderate-intensity cardiovascular exercise. For decades, one of the most popular ways of exercising was on a stationary bike, either at home or at the gym.

“A stationary bike offers many benefits,” says Dr. Kevin Plancher of Plancher Orthopaedics & Sports Medicine. “It gets the heart pumping without putting undue stress on the joints. And an indoor bike can be used in any weather, by novice and experienced exercisers and at varying levels of time and intensity.”

But for all their benefits, many exercise bikes bought for home use wind up as expensive clothes racks or dust collectors in the basement or attic. Why does motivation seem to flag after just a few months?

“One reason is boredom,” says Dr. Plancher. “Even watching TV or listening to music or a podcast while on the bike doesn't seem to be enough to keep a lot of people at it.” One attempt to address this problem is with spin classes offered at gyms. These programs use bikes that evolved from the stationary bike to more closely resemble a road bike and offer the camaraderie and competition of working out with a group. But, as Dr. Plancher points out, what is sacrificed is the freedom and convenience of working out at home on your own schedule.

New options now make it possible to combine the comfort and convenience of working out at home with quality hardware, expert instruction, metrics, and more classes and instructors than any gym could offer. One of the companies in this new market, Peloton Interactive, Inc., manufac-

tures a “smart bike,” which has a high-definition monitor attached to the handlebars. It uses home wi-fi to stream live classes from Peloton's studio and to provide an archive of thousands of past classes of every type, letting you choose from a wide variety of rides, instructors, and soundtracks. The system tracks your performance and compares your current ride to your personal best so you can push yourself to set new personal records.

You can also compete against others who have done the rides you choose and review user ratings of instructors and rides. If you participate in a live ride, the instructor sees your metrics and can address you as if you were in the room.

“This system incorporates all the performance and motivation factors of indoor cycling in the gym with the convenience of working out at home,” says Dr. Plancher. “This is an important new trend that represents the next generation of exercise technology, one that can potentially transform the landscape for home fitness.”

Tips for getting the most from your cycling workout

However you ride - on an upright stationary bike, a recumbent bike, or a new smart bike - Dr. Plancher offers tips to help you get the most from your cycling workout:

• Before you buy a bike, do your homework: Consider what kind of bike you want and factors like how much room you have and how much you want to spend.

• Have an expert ensure that you're buying a bike that fits you properly - seat height, distance from pedals and handlebars, etc.

• Make sure you understand how to safely adjust intensity and other options. Start slowly and increase the intensity of your ride gradually.

• The bike seat shouldn't be uncomfortable. Try padded shorts or a gel seat to improve comfort. Sit lightly on the seat.

• If you're not using one of the newer smart bikes that provide streaming and archived classes, look into buying or renting cycling videos that have a variety of rides and changes of scenery.

• Consider cross training with another activity once or twice a week to build up endurance in different muscles and prevent overuse injuries.

“The most important factor in a successful fitness program is finding the one that is right for you,” says Dr. Plancher. “Indoor cycling has worked for millions of people and new advances make it even more appealing. But the program that works for you will be the one that suits your personality, fitness level, and lifestyle and that you will enjoy and stick with.”

It’s with deep sadness we share the news with our readers and advertisers of the sudden passing of General Manager James Stankiewicz on February 1, 2017. Jim was only 52.

Jim began his career here at Belsito Communications almost 16 years ago with Hospital Newspaper and was also a part of launching Healthcare News and Boomers & Beyond several years ago into Westchester County, NY and Fairfield County, CT. As many of you who have had the opportunity to meet Jim at various events our publication was involved in over the years can attest, he had an undeniable enthusiasm about his work and was very proud of the publications he helped to build.

As passionate as he was for his job, Jim’s family was what made him the most proud. His wife, Susan, and children Robert and Brianna were his life’s motivation as well as other family members including his mother, brother and three sisters.

Jim’s work family will always remember the jokester he was here at the office and how he always went out of his way to bring a smile to everyone’s face. He was an ardent fan of the Jets and Mets and spent many years coaching baseball to scores of youth in the local area.

I had trouble believing this myself, but what if I told you that there exists a recently FDA approved laser that can grow your hair back?

Day Kimball Hospital Birthing Center among top ten percent in state and nation for maternity practices in infant nutrition and care

Day Kimball Hospital has scored among the top ten percent of all participating maternity care facilities in Connecticut and across the United States in the Centers for Disease Control’s (CDC) latest Maternity Practices in Infant Nutrition and Care (mPINC) survey. The survey is administered every two years to monitor and evaluate hospital practices related to breastfeeding. About 82 percent of all facilities that provide maternity services in the U.S. participated in this latest survey, which was conducted in 2015 with the results being published by the CDC in late 2016.

The mPINC survey evaluates how effective birthing facilities are at implementing best practices that, when implemented together as a consistent system of care, results in better breastfeeding outcomes.

These best practices help to support breastfeeding throughout seven key clinical care processes, from labor and delivery care to postpartum care, discharge care, staff training and the structural and organizational aspects of care delivery.

Day Kimball Hospital earned an overall quality score of 96 out of 100 on the survey, compared to an average score of 83 for all participating birthing facilities in Connecticut and an average score of 79 for all participating birthing facilities across the U.S. Day Kimball earned a perfect “100” score in four out of the seven measurement areas. Make healthy shifts in food choices at home during National Nutrition Month, says Academy of Nutrition and Dietetics During National Nutrition Month®, celebrated each March, the Academy of Nutrition and Dietetics encourages everyone to "Put Your Best Fork Forward" by making small, healthy shifts in food choices when cooking at home.

The 2015-2020 Dietary Guidelines for Americans recommend making small changes to eating patterns to include healthier ingredients while cooking at home. Choosing a variety of healthful foods across and within all food groups helps reduce the risk of preventable, lifestyle-related chronic diseases including cardiovascular disease, type 2 diabetes and obesity.

Vanessa Lloyd, 28, was home alone in Hartford with her 2-year-old daughter when she fainted several times. So she called her mother to drive her to the Emergency Department at UConn John Dempsey Hospital.

But as their car pulled up to the ED’s entrance, Lloyd passed out again. Her mom ran into the ED for help, and a team of nurses and UConn Health Fire Department paramedics rushed to the car.

“I was really scared,” recalls Lloyd, who only vaguely remembers the paramedics rolling her into the ED from the car. She figured her fainting was linked to her diagnosis of epilepsy back in 2011, but she was wrong.

While she was in the ED, Lloyd’s heart suddenly stopped.

“UConn saved my life,” she says.

“The nurses successfully performed CPR on me and shocked my heart with a defibrillator to restart it. I woke up, and immediately started talking. I recall asking the medical team, ‘What just happened’?”

Department at UConn John Dempsey Hospital, says Lloyd was lucky to be in the right place at the right time. “This young woman … went into a dangerous ventricular fibrillation arrhythmia, which our nurses immediately recognized, allowing us to defibrillate her heart into normal rhythm.”

Her doctors suspect that Lloyd may never have had epilepsy, because she never experienced epileptic seizures.

Instead, heart experts at UConn Health’s Calhoun Cardiology Center believe that Lloyd has Long QT Syndrome, a condition that can cause an abnormal and chaotic rapid heartbeat leading to fainting or even sudden cardiac death. It stems from a genetic predisposition, or may be triggered by certain medications. The fainting spells that people with Long QT syndrome experience often look similar to epilepsy, leading to confusion over the diagnosis. Lloyd is awaiting genetic testing to confirm her diagnosis.

Lloyd was completely surprised that her health was at risk from a dangerous arrhythmia: she works

daily in a hospital as a telemetry technician monitoring patients.

“Thank you to everyone at UConn that helped save my life,” she says. “I am so grateful and so very happy – beyond happy.”

While Lloyd was in the hospital recovering, an implantable cardioverter defibrillator (ICD) device was implanted under the skin of her

chest using a minimally invasive technique. The device has tiny wires that connect it to her heart.

Dr. Christopher Pickett, her electrophysiologist at the Calhoun Cardiology Center, says the device will help regulate Lloyd’s heartbeat, preventing it from going too slow, the situation in which her heart rhythm might become dangerously chaotic again – and can even restart her heart if it ever suddenly stops again.

The technology allows Pickett to monitor her heart rhythms from afar.

Lloyd says the technology not only protects her heart health, but also gives her peace of mind for when she is alone caring for her young daughter.

In recognition of the lifesaving work of the personnel at UConn Health, including nurses Amanda Costello, Sarah Urso, and Erica Gomez, Lloyd nicknamed her monitoring device “Jonathan” after UConn’s Husky mascot.

“I am so thankful to everyone at UConn, so there was no better name,” she says. “No matter where I go, I now have a little piece of UConn inside me keeping my heart beating.”

She advises others, young and old, to take their health very seriously, and to get checked out by a doctor if they feel anything uneasy or abnormal about their body.

Colon cancer is one of the most preventable and treatable forms of cancer when it’s found early.

Screening is the #1 way to prevent colon cancer because it finds cancer in the early stages, or, before it even has the chance to grow or develop.

While screening is the most important step you can take to prevent colon cancer, it’s not the only one. You may be able to lower your colon cancer risk with some simple lifestyle changes. Be proactive about keeping yourself healthy. It will pay off!

Eat Well Consistent evidence shows diets high in vegetables, fruits and other plant foods reduce risk for many diseases, including colon cancer. In addition to these foods containing lots of dietary fiber and nutrients, plant foods are good sources of substances called phytochemicals, which help protect cells in the body from damage that can lead to cancer. Plant foods can also help us maintain a healthy weight – also important in reducing risk of colon cancer – because many are lower in calories.

Diets high in red and processed meats have also been linked to colon cancer. Red meats include any meat that is red when raw, like beef, pork or lamb. Try to keep your red meat intake to 18 ounces (cooked) per week or less. Processed meats, on the other hand, should be avoided if possible. Processed meats include hot dogs, ham, bacon, sausages, among others, and cancer risk increases even with low consumption.

Get Your Exercise Research consistently shows adults who increase their physical activity, either in intensity, duration or frequency, can reduce their risk of developing colon cancer by 30 to 40 percent. It’s estimated that 30 to 60 minutes of moderate to vigorous physical activity per day is needed to protect against colon cancer.

Exercise also helps you to maintain a healthy body weight, which reduces your risk for colon cancer. In fact, studies show people who are overweight or obese are more likely to develop colon polyps, a possible precursor to cancer, and higher weights are associated with higher polyp risk.

Know Your Family Health History A family history of colon cancer puts you at an increased risk for the disease. A family history is considered to be an immediate family member (parent, brother or sister) or multiple family members with colon cancer or polyps. If you have a family history, you may need to be screened at age 40, or 10 years before the youngest case in your immediate family, whichever is earlier. Talk to your family about their health history!

Avoid Alcohol & Cigarettes Although modest amounts of some alcohols have been linked to protecting against coronary heart disease, alcohol is known to increase cancer risk. According to the National Cancer Institute, people who regularly drink 3.5 drinks per day have 1.5 times the risk of developing colon cancer as nondrinkers or occasional drinkers. If you do chose to drink, try to limit your consumption.

Long-term cigarette smoking is also associated with increased risk of colon cancer. The longer a person smokes, the greater the risk.

Loud, sleep-interrupting snoring may seem like an adult-specific issue, but children aren’t immune. About 20% of kids snore from time to time, and in most cases, there’s no reason to be concerned by the nighttime noises.

But if snorting or gasping sounds accompany the snoring, it could be a sign of a larger issue: sleep apnea.

The condition, which causes pauses in breathing throughout the night due to the obstruction of airways, affects one percent to 10 percent of children. Untreated sleep apnea can lead to high blood pressure, heart trouble, and poor growth, but luckily, there are a number of treatments available.

One of the most common causes of sleep apnea in children is enlarged tonsils. If a doctor determines that this is what’s responsible for a child’s excessive snoring, he or she may surgically remove the tonsils. And because sleep apnea is more prevalent in overweight kids, a weight loss program could also help.If neither of these options is successful, doc-

tors may recommend continuous positive airway pressure (CPAP).

This involves the child wearing a CPAP mask during the night, which blows air into the airways to keep them open, helping to end the snoring.

Talk to your pediatrician if you suspect that your child has sleep apnea. In addition to loud snoring, you may notice your child gasping for air during the night, tossing and turning, or sleeping in strange positions in an attempt to keep airways open. Signs of sleep apnea may also show up during the day. Children with sleep apnea often feel tired, since the condition wakes them from their sleep periodically throughout the night.

In turn, they may have difficulty paying attention at school or be in an irritable mood. In fact, sleep apnea in children has been linked to a higher risk of depression and anxiety. The good news is that many kids outgrow sleep apnea, so talk with your doctor about which treatment method is right for your child.

Foods for a good night's sleep

Preparing for a good night’s sleep starts at mealtime. Certain foods, such as the ones listed below, can have a calming effect on both the brain and body. And if you're not sure how to incorporate each food into a dish, take a look at these suggestions.

Lean Proteins Tryptophan, an amino acid that increases serotonin levels to help you feel sleepy and relaxed, can be found in foods like turkey, chicken, fish, and low-fat cheese.

Combining foods that are high in tryptophan with healthy, complex carbohydrates also improves sleep.

An hour before bed, try having a snack of whole-wheat crackers and cheese or a small turkey sandwich on wholegrain bread. Or for dinner, chow down on grilled chicken and brown rice, a black bean and cheddar quesadilla on a whole-grain tortilla, or whole-wheat mac 'n cheese.

Herbs Certain herbs, such as sage and basil, contain chemicals that can improve sleep by easing tensions that would keep you awake. Basil can be sprinkled on whole-wheat pasta dishes or when topping sliced mozzarella and tomatoes. You can also chop it up and mix it with olive oil, garlic, pine nuts, salt and pepper, and pecorino cheese to make a pesto sauce, and then toss cooked zucchini in it.

Sage pairs best with fatty meats like pork and beef (which you should eat only in moderation). You can also put it on butternut squash soup or a cold bean salad.

Nuts Eating heart-healthy fats can also boost serotonin levels, and nuts—including walnuts, almonds, and cashews, as well as nut butters—are a solid source. Nuts are a great addition to salads and stir-fry dinners, and they can also be incorporated into meals as breadcrumbs on, say, a chicken cutlet. Or, mix sliced almonds with green beans as a side, combine walnuts with bananas in whole-grain pancakes, or smear almond butter on a whole-grain waffle.

Straightening out 4 Myths about adults and braces

Country music fans noticed something different about Faith Hill’s smile a few years back when she showed up for the Country Music awards.

The popular singer wore braces, adding her name to the growing number of adults who decided to straighten their teeth through the traditional “mouth hardware” usually associated with children and teenagers.

There may be a good reason behind the trend.

“A smile plays a critical role in how we feel about ourselves and how others see us,” says Dr. Leslie Pitner, an orthodontist and author of Improve Your Smile, Transform Your Life: A Life-Changing Guide to Orthodontics for Adults (www.drpitner.com).

“When people have an extreme overbite, crooked teeth or some other problem with their teeth, they hide their smiles. I’ve had patients who literally have forgotten how to smile after years of trying to conceal unattractive teeth.”

The result, Pitner says, is they can be seen as less friendly and less approachable.

Meanwhile, research shows that smiling can have an impact on numerous aspects of a person’s life, including their mood and how well they handle stress.

But Pitner says adults whose teeth could benefit from braces often don’t think about that option. Even when they do, they are discouraged from following through because of dubious information that they have come to regard as fact.

A few of the myths that cause many adults to hesitate include:

• Braces are just for kids. That’s simply not true, and hasn’t been for awhile, Pitner says. About 27 percent of people seeking orthodontic treatment in the United States and Canada are adults and the number has been growing, according to a study by the American Association of Orthodontists.

• A referral is needed. While a dentist can refer a patient to an orthodontist, it isn’t a necessity, Pitner says. Often, dentists don’t bother to suggest an orthodontist to adults because they assume their patients don’t want or need braces. “We’ve also been conditioned by the medical system to assume we need a referral to see a specialist,” she says. “That’s not so for orthodontics. You can make the decision yourself and it won’t affect your coverage if you have insurance.”

• Braces need to be worn for years. Many patients tend to overestimate how long they will wear braces. “It varies based on how complicated a particular situation is, but usually people will wear braces between six months and two years,” Pitner says.

“Treatment did take longer in the past than it does today, but technology has helped accelerate the process.”

• Braces are ugly and everyone will stare. This is another example of how things have changed. Braces are much smaller than they once were and ceramic, tooth-colored braces are available. Another option is clear, removable aligners such as Invisalign. Invisible braces attached to the back of the teeth also are available.

“Anyone who’s unhappy with they way their teeth look shouldn’t let misinformation get in their way of improving their smile,” Pitner says. “The process is much faster, easier and more comfortable than the myths would have you believe.”

Lemon Sipper With less than 10 calories, this is an ideal low-calorie, vitamin C–rich thirst quencher.

Connecticut Children’s Medical Center first in state to connect electronically to prescription monitoring program

Connecticut Children's Medical Center has become the first healthcare organization in the state to have successfully integrated the hospital's electronic health records system with the state prescription drug database.

As we are all aware, prescription drug abuse continues to receive local and national attention.

Legislation requires prescribing providers review a patient's controlled substance dispensation history prior to prescribing controlled medications for longer than 72 hours.

This accomplishment is evidence of Connecticut Children's commitment to patient safety and compliance with prescription drug monitoring.

“We are proud to take the lead on this initiative in the state of Connecticut. The important integration makes it easier for our providers to do the right thing for our patients,” said Chief Medical Information Officer, Dr. Richelle DeMayo. “Integration improves both patient safety and provider efficiency, enabling providers to fulfill their duties in the context of their normal workflows and allowing clinical staff to focus their informed attention on delivering excellent patient care.”

Connecticut Children's can now more easily provide our physicians access to controlled substance prescription histories in real-time, at the point-ofcare to improve patient safety and prevent overpre-

scribing. Without this integration, prescribers need to log into the PDMP separately to retrieve a patient's controlled substance dispensations and then cross-reference that data with a patient's other health information in our electronic health record. deMayo also notes the prevention of prescription drug misuse and abuse is a matter of enormous personal and public health significance that demands collaboration across settings. “Connecticut Children's Medical Center and our providers are excited to lead the way for others in the state to make responsible use of available information to protect patients and families from overdose and drug diversion,” said deMayo. “We are proud to work with the State of Connecticut to make it easier for physicians and other prescribers to do the right thing.”
