Fertility facts you need to know Between 30 and 40 years, a woman’s chance of having a baby each month (if conceived naturally) falls from 22% to 6%. Even at your most fertile age, you will only ever have a 25% chance each month(3). The average age of a woman’s first birth in New Zealand is 30 years(1), however, a third of the women we see as patients are now over the age of 40.

For some, this is due to the fact that travel and career have meant that starting a family has been put to one side; for others it may be they have not met the ideal partner just yet. For others, it may be that they have been trying for a while now without any luck, but are not sure whether they should talk to a specialist now or wait a little bit longer.

Our key message is that fertility does not follow social trends – it’s hardwired into your DNA. Even though we are living longer than our ancestors, the fertility window for a woman remains the same, even if she is fit and healthy. Surveys have consistently shown that people over-estimate how long their fertility will last by about 10 years. In addition, there is a widespread belief that treatment like IVF can overcome the effects of age.

This is constantly reinforced by the glossy magazines who when profiling celebrities having babies later in life, often fail to declare that an egg donor was involved.

As you get older, you have less time to wait and see. Time waits for no woman (or man) – if you’re 30 and wait for 2 years your chance of a baby with IVF falls from 48% to 46%, but if you’re 40 and don’t do anything

for another 2 years your chances with IVF drop from 23% to 13%!

Baby’s shopping list Preparing for a new baby is enormous fun, but goodness... don’t they need a lot of stuff! Don’t be daunted by the list. Most items can be bought second hand, and relatives who have completed their family are a great source of baby gear. Apart from anything else, they will love helping you get set up for the new arrival.

Before you go out and spend up large, keep in mind gifts you may receive from friends and family, what you will need immediately and what can wait a short while. A high chair, for example, won’t be needed for several months down the track, and the covers for electrical outlets etc are a precaution once baby is crawling and inquisitive. Also, remember how quickly baby grows during the first few months and don’t buy too many newborn-size items.

About Lead Maternity Carer (LMC Services)

Pregnant women are required to choose a Lead Maternity Carer (LMC) who coordinates their maternity care.

Lead Maternity Carers can be midwives, GPs with a diploma in obstetrics or obstetricians. LMCs are contracted through the Ministry of Health to provide a complete maternity service to you.

The vast majority of women and their families choose a midwife as their LMC. These LMC midwives work with midwife partners (mostly in small group practices) and alongside midwives who are employed to work in maternity units to ensure you receive the care that meets your needs.

Midwives also work collaboratively with other health professionals such as obstetricians to meet any additional medical or health needs.

When you choose a midwife as your LMC, you can expect the following from her.

ON BOOKING OR REGISTRATION: • information regarding the role of the LMC and contact details including arrangements for ‘back up cover’ if unavailable • information about the standards of care to be expected, possible interventions,outcome, options for referral for additional care should you need it • a comprehensive pregnancy assessment including physical examination, assessment of general health, family and obstetric history • information about a range of screening tests (blood and ultrasound tests) and referral for these if you choose to have them • information about the availability of Pregnancy and Parenting Education courses • information about Paid Parental Leave • development and documentation of a Care Plan to be used and updated throughout your pregnancy, birth and after birth. (Your LMC will keep a copy and will give you one.)

• a copy of your clinical notes (updated at each visit) IN THE SECOND TRIMESTER (12 TO 28 WEEKS OF PREGNANCY): • monitoring of your progress including early detection and management of any problems, including referral to other care providers if necessary • updating your Care Plan • one-to-one education regarding healthy pregnancy, childbirth and parenting preparation • booking into a maternity facility or birthing unit unless you are planning a homebirth IN THE THIRD TRIMESTER (28 WEEKS UNTIL LABOUR BEGINS): • monitoring of your progress including early detection and management of any problems 8

• updating your Care Plan • one-to-one education regarding healthy pregnancy, childbirth and parenting preparation • specific education for labour and birth including the role of your support people, coping with the pain of labour, options for the birth itself, care of the afterbirth, immediate care of the baby and breastfeeding • instructions for making contact when labour begins • arranging for you to meet any other maternity carers who may be involved with your care LABOUR AND BIRTH: • initial assessment at home (preferable) or at a Maternity Facility • regular monitoring of you and your baby’s progress, including referral to other care providers if necessary • all your primary care during your labour and the birth of your baby • all your primary care immediately following the birth including initial examination and identification of your baby, initiation of breastfeeding, care of the placenta, any suturing required and notifying your baby’s birth to the Registrar of Births If you are having your baby at home: • arranging for a second practitioner to be available to attend the birth • maintaining the necessary equipment including neonatal resuscitation equipment • providing the delivery pack and other supplies If you are having your baby in a birthing unit: • arranging for a second practitioner to be available to attend the birth • ensuring a practitioner remains with you until you are discharged AFTER YOUR BABY IS BORN (FROM THE BIRTH UNTIL YOUR BABY IS 4-6 WEEKS OLD): • detailed clinical examination of your baby within the first 24 hours • if you are in hospital, a daily postnatal visit prior to your discharge (unless another arrangement is made with you and with the facility) • one home visit within 24 hours of your discharge from hospital • a further detailed clinical examination of your baby within 5 days of the birth • a total of 5 – 10 midwifery home visits and more if clinically needed • a minimum of seven postnatal visits in total

• assistance with and advice about breastfeeding and nutrition for mother and baby • assessment for risk of postnatal depression and/or family violence with appropriate advice and referral • provision of the Ministry of Health information on immunisation • access to screening programmes for your baby as outlined in the Well Child Tamariki Ora National Schedule • advice about contraception PARENTING ADVICE AND EDUCATION • a final detailed clinical examination of the baby prior to transfer to the Well Child Provider (Plunket or other service) • a postnatal physical examination of you prior to discharge from the LMC service • transfer of your baby’s care from the LMC to the Well Child provider at a date agreed with you between 4-6 weeks after the birth, and formal notification of this transfer to the Well Child provider • notification to your General Practitioner of your discharge from LMC services.

Antenatal tests and screening During your pregnancy, you will be advised about screening for a number of conditions that may affect you or your baby. You will also be provided with information to help you choose if you would like to have this screening.

FIRST ANTENATAL BLOOD SCREEN When you first see your lead maternity carer (LMC) or GP during your pregnancy, you will be offered a first antenatal blood screen. This is to test for: • your blood group and rhesus factor (if you are rhesus negative, ask your GP or LMC to explain what this means) • your haemoglobin (iron content in your blood) • the presence of any antibodies that may be harmful to your baby • whether you are immune to rubella • whether you are a hepatitis B carrier • whether you have syphilis • whether you have HIV (your LMC or GP will talk to you about this).

ANTENATAL HIV SCREENING In New Zealand, all pregnant women are offered antenatal HIV screening. If you have HIV, treatment is available that can help keep both you and your baby well (the diagnosis and appropriate treatment in pregnancy, reduces the risk of the baby getting the virus from 30% to less than 1%).

Screening for HIV involves you having a blood test. Your LMC or GP will offer you the test at the same time as the first antenatal blood screen, usually during your first pregnancy visit.

The screening test for HIV is very reliable and picks up almost every woman with HIV. As with all screening tests, some women will get a positive screening result but will not have HIV. About one woman out of every 1000 who have the test will need to have a second blood test to confirm whether or not they have HIV. Your LMC or GP will discuss with you what the results may mean.

The second test is a nuchal translucency scan, which is an ultrasound scan performed between 11 weeks and 13 weeks six days of pregnancy (the best time is at 12 weeks). This scan looks at the thickness of the fluid-filled space at the base of the baby’s neck.

FIRST TRIMESTER COMBINED SCREENING (FOR DOWN SYNDROME) During the first 14 weeks of pregnancy, you will be offered two types of screening test.

These tests, when combined together, can give an indication of whether your baby has an increased chance of Down syndrome or another chromosomal condition.

Decorating the nursery If you will be bringing home a new baby soon, chances are you are thinking about how you will decorate the nursery. Here are some inexpensive and easy ideas to get you started.

• If you decide to invest in expensive cot bedding, your child will likely be out of the cot in two years and you will have to buy new bedding for their ‘grown up’ bed. On the other hand, if you plan to have several children, investing in quality items may be more economical in the long run.

• Choose fabrics and patterns that can be easily cleaned as you’ll be doing a lot of laundry and will want cot linens that are easy to clean.

• Consider buying a ‘cot bib’ that stretches across the head of the cot where you place

your baby’s head. If baby spits up, all you have to do is change the ‘bib’ instead of the entire sheet.

• Invest in a sturdy changing table with a strap.

Once baby is moving around, you’ll be glad you did.

• If you choose a ‘theme’ for the room, remember you are going to have to live with it for a while. Try to avoid things that will become outdated or babyish too soon.

• Make the room soothing for baby and he/she is likely to sleep better. Too much stimulation will do just that: stimulate.

• Consider a dimmer on the light switch for late night changes or invest in a night light.

• Consider blackout or room darkening shades, which will also help baby sleep when the sun is out.

• A bookshelf painted in bright colours is great for housing books and small toys.

• It is never too early to think about childproofing... make this a priority in all your decorating/furniture choices.

Settling baby to sleep

Settling baby to sleep in their own safe place on their back, with their face clear helps reduce the risk of SUDI (SIDS or cot death).Some newborn babies fall asleep being cuddled, rocked or fed, and then have a good sleep. But they may develop a pattern of needing you to feed them or rock them to sleep, and then they wake crying, soon after being put to bed.

TRY SOME OF THE FOLLOWING TIPS TO HELP YOUR BABY SLEEP: • Put your baby down awake so their bed becomes a familiar place, especially when they show signs of being tired.

• Have a quiet period before bed, such as a cuddle, talking in a quiet, soothing voice or singing.

• Relax your baby by patting, stroking, talking to them quietly or using a musical mobile. You can stay with them while they go to sleep, or leave the room when they are calm but still awake.

• If your baby starts to grizzle or cry when you put them to bed, gently stroke them.

• You don’t need to be quiet in the house. Most babies sleep through household noises, like a vacuum cleaner or music.

• If your baby goes to sleep in their car seat, always take them out when you get home and put them in a safe place to sleep. Babies can have breathing problems in a car seat if their head falls forward.

• Have a regular routine before bed, like a bath, massage, feed, and quiet cuddle.

Sudden Unexpected Death of an Infant (SUDI) SUDI can be a frightening topic for new parents to think about. Sadly, each year in New Zealand 60 to 70 babies die suddenly when they sleep. Most of these can be prevented. It is important to understand the facts and know what you can do to help protect your baby.

HOW CAN I HELP TO PROTECT MY BABY FROM SUDI? All babies, in all cultures and at all times, need these six conditions to protect their lives as they pass through the critical first 6 months:

VULNERABLE BABIES Risks may be built into the baby, the situation or the setting. The babies who will need the extra protection of their own baby bed every time and place they sleep, include:

• My baby is smoke-free.

• If not smoke-free, my baby always sleeps in ‘a baby bed’.

Plunket recommends babies sit in rearfacing seats until they are two. The American Association of Paediatrics says that in a crash babies in rear-facing seats have a 75% greater chance of surviving than forward facing babies. Keeping baby out of trouble Small children have a knack for finding trouble. Even when you’ve bought safe baby products you’ll still need to supervise your little one. Sometimes you need to think like a child to spot all the hazards.

BABY WALKERS With a baby walker, your baby can grab objects and get into situations faster than they would normally be able to. So you need to make sure you can see where your child is at all times.

Look out for hazards – including anything that could make your baby fall, anything that could fall onto your baby, or that your baby could crash into, grab or hurt themselves with.

IN THE BATH Babies have drowned using bath seats, and Plunket recommends you don’t use them at all. If you choose to use a bath seat then your baby needs your full attention the whole time. Baby bath seats aren’t safe enough for you to dash out of the room for the talcum powder. Get everything you need together before bath time and ignore any phone calls or doorbells. Don’t let older brothers or sisters be responsible for your baby’s safety.

PYJAMAS New rules have changed the labeling of some children’s nightwear and daywear. A ‘low fire danger’ label or an orange caution label means the clothing has a lower risk of catching fire as long as the pyjamas fit snugly. But you still need to make sure children stay at least a metre from the heater. Using a fire guard or safety barrier is a good way to stop children sitting too close to the fire or heater.

COTS You’ll be leaving your baby alone in the cot so it has to be super-safe. Make sure your child can’t get their head, hands, fingers or toes caught in the bars or details. There shouldn’t be anything in or around the cot that your baby could choke on or be suffocated by. Put the cot far enough away from windows or shelves so that your baby can’t reach out for objects, cords or escape routes. When your baby starts

climbing there is a higher risk of falling out, and it’s time to change to a bed.

PORTABLE COTS AND PLAY-YARDS Portable cots and play yards are not as sturdy as regular cots. They are designed for convenience and are usually not suitable for using constantly. Carefully assemble the cot or play-yard using the manufacturer’s instructions, and each time you set it up check that it is sturdy and safe.

PRAMS AND STROLLERS Your child will probably be spending a lot of time in a pram or stroller and will be constantly exploring. Make sure the harness can firmly restrain your baby and that there are no cords or levers that your baby can reach or get caught in. Also check that there are good brakes and locking devices so the pram or stroller cant unexpectedly take off or collapse

Under Fives Pre-schooler drownings typically occur when a child is allowed out of sight and reach of a caregiver; in the bath, paddling pool, or bucket – or larger bodies of water such as in a beach, lake or river.

No children under-five should be drowning in this country.

Keep under fives within arm’s reach at all times.

It only takes sixty seconds and around five centimetres of water for a child to drown.

SAFETY TIPS: • Always empty and store paddling pools and water containers after use and ensure you have a safely fenced play area • Identify water hazards in and around your home and ensure your children can’t reach them

Effective parenting There are things that children need from you to help them grow up to be healthy and capable adults.

LOVE AND WARMTH. Show your child how much you love them. Give them lots of praise, be positive and try not to say negative things.

TALKING AND LISTENING. Talk to your child as much as you can in simple language they can understand, and listen to what they say to you.

GUIDANCE AND UNDERSTANDING. Be clear about what you want your child to do or not do, and give the reason why.

CONSISTENCY AND CONSEQUENCES. Children need to learn that words and actions should match. So it’s important that you don’t tell them one thing then do another.

SECURITY AND STRUCTURE. To feel safe, children need to know what to expect. Try to have routines, so things like bed time and bath time happen at the same time every day. Make your home safe and

Nutrition Foundation’s Guide to Healthy Eating Childhood is a time of rapid growth and development, and a time when many habits and behaviours are set.

A healthy lifestyle, including healthy eating, in childhood will help to ensure a healthy future .Here are some simple guidelines for healthy growth and development.

A VARIETY OF FOODS EVERY DAY This includes food from all the four food groups: fruit and vegetables, breads and cereals, milk and milk products and lean meat and alternatives.

Fruit and vegetables provide carbohydrates, are high in fibre and contain lots of vitamins and minerals, making them the perfect snack food. Preschoolers need at least 2 servings of fruit and 2 servings of vegetables each day.

School children need at least 2 servings of fruit and 3 servings of vegetables each day. A serve is the amount that fits into your child’s hand.

Encourage them to choose from a rainbow of colours, e.g. a red tomato, purple grapes, orange carrots, green broccoli and yellow capsicums.

cereals, very active school children need 5 serves and older children need at least 6 serves. Serving sizes for children are approximately 1 roll, 1 medium slice bread, 1 cup cornflakes and 1 cup of pasta or rice. Children under five years have small stomachs and can’t eat the same amount of fibre as older children or adults so white breads and cereals (such as cornflakes) can be suitable choices.

Milk and dairy products are an important source of protein and calcium for this age group. Calcium is particularly important for growing strong bones. Children need at least 2-3 serves every day. Serving sizes are 1 cup of milk, 1 pottle of yoghurt, 2 slices of cheese or 2 scoops of ice cream. After two years of age you can start to introduce reduced and low fat milk and milk products if your child is eating well, and height and weight are following the normal growth curves.

Meat and alternatives including lean meat, chicken, seafood, eggs, dried peas, beans and lentils are great sources of protein and provide important vitamins and minerals such as iron and zinc. Children need some of these foods each day: for example, 2 slices of cooked meat, 3/4 cup of mince, 1 medium fillet of cooked fish, 1 chicken drumstick, 1 egg or 3/4 cup dried, cooked beans. Parents of children following vegetarian diets should seek expert advice to make sure their diets are adequate in essential amino acids, vitamins & minerals, especially those on vegan diets which exclude all animal derived foods.

FLUIDS The best drinks for children are water and milk. Small bodies heat up more quickly so drinking plenty of plain water will keep them cool, and also help them concentrate at school. Milk is an important source of calcium, which is essential for healthy development of bones and teeth. If your child doesn’t like milk, they will need an adequate calcium intake from other sources such as cheese, yoghurt, nuts and calcium-

fortified soy products. Leave soft drinks and fruit juice for occasional drinks, in small amounts. Children should not drink energy drinks, which have high levels of caffeine.

Some children are reluctant water drinkers - a few ice cubes or chilling a jug of water in the fridge can be more appealing, or try squeezing a dash of lemon juice or adding a splash of fruit juice to water for a bit of extra flavour.

REGULAR PHYSICAL ACTIVITY Physical activity is very important for healthy growth, and to help prevent your child from becoming overweight. Swimming, outdoor games and walking or cycling to school are great ways to fit in regular physical activity.

Some schools have a Walking School Bus programme where adults walk children to school. Remember that sitting for extended periods of time in front of the tv or computer screen is not good for health. Encourage

children to move regularly and do some exercise that causes them to huff and puff. This will improve their physical fitness.

FUSSY EATERS Fussy eaters can cause a lot of distress to parents and other family members. It is good to remember children are very good at responding to their appetites and therefore may eat most of their food as snacks rather than at meal times.

Tips: • Involve children in shopping and food preparation as this will increase their interest in eating.

• Offer nutritious snacks – for example a pottle of yoghurt, a slice of toast, a piece of cheese, a glass of milk or some fruit or vegetables.

Smaller sized portions can be easier for children to manage so try fruit chopped up and vegetable sticks.

• We can associate foods with certain occasions, positively or negatively, and children can do this too. Children can learn to prefer foods they associate with being happy, or conversely dislike a food if they associate with being unhappy, anxious or unwell. So try to make mealtimes enjoyable and if a food is not eaten just take it away rather than making it the focus of an argument. If the child is hungry later, re-offer the food provided previously rather than something different.

• Make meal times a time for eating together, where your child sees others eating the same foods they are being offered. You can also be a good role model if you are eating together.

• Regular mealtimes can also help, so a child knows when to expect to eat, rather than feeling uncertain or unsure • It can take up to ten times for a new food to be accepted, so don’t worry if a food is rejected several times before being eaten sometime later – and it can be months, not just days or weeks.

TALKING AND LISTENING Between the ages of 3 and 5 years children are learning to say and understand many words.

This involves a range of skills. Your child may be able to understand and use more complex words and sentences. They may be asking lots of ‘what’ and ‘why’ questions to find out new information. They may know the names of some letters and recognise their own written name.

HELP YOUR CHILD TO SPEAK BETTER There are many things you can do to help your child to speak better. For example, when your child says something that is not clear, say it back for them so that they hear it the right way rather than ask them to repeat it.

Have fun with words and sounds (eg, make up games about words that start with the same sound or make up silly words that rhyme). You could talk with your child about exciting things that are going to happen, to encourage talking about the future.

Let your child write and draw with you (eg, writing shopping lists, making special cards).

You could also play games that help them with taking turns and concentrating (eg, ‘What’s the time, Mr Wolf?’, hide and seek, snakes and ladders, and memory).

MANAGING BEHAVIOUR Children do best when they feel loved and valued, are well supervised and know how they should behave.

Give positive attention. If you want to encourage a behaviour, notice it. Let your child know how proud you are of them. Ignore little things. Respond only when your child is behaving really badly or is in danger. Giving attention to a child’s bad behaviour usually makes it worse.

Ensure that your child understands what is expected of them. Be clear and consistent. If your expectations are the same over time your child is more likely to learn to meet them.

Choosing an ECE service It can be hard to know where to start when choosing an early childhood education service for your child.

With so many different ECE services available, it may be hard to choose the best one for you and your child.

Children do best where they can: • form bonds with people who care • be around people who have plenty of time to spend with them • enjoy having fun • be given clear guidelines for behaviour that can be backed up with reasons A safe and secure environment with access to a variety of activities is also important THINGS TO THINK ABOUT When you are choosing an early childhood service for your child it might help to think about what will work best for you and your child and what’s important to you both.

You could consider: • How long you want your child to attend • The kind of service you want • The fees you can afford • The location of the service • You and your child’s needs When choosing an ECE service, do some research; think about what you need. Do you want a service that offers:

• Early childhood education in a home setting or at a centre?

• The option of attending the service with your child?

• Your child attending without you?

• A location close to home or work?

• Opening times that suit you?

• Lots of structure or lots of free play?

THE DIFFERENT SERVICES Early childhood education services can be divided into two main types: • Teacher-led: where teachers provide the education and care for children • Parent-led: where parents, family/whânau and/or caregivers provide the education and care for their children Within these two types, there is a wide variation of services. Some services: • Include children from birth to school age and others have a specific age range • Have a special character or focus • Are run by community groups and others are run by private providers • Are open all day, part days or a combination • Are in a home setting Every licensed early childhood education service is reviewed regularly by the Education Review

Office (ERO). When choosing an ECE service for your child, it might help to read their latest ERO report. This gives you an independent view of the quality of the centre and its strengths and weaknesses. ERO reports are available free online (www.ero.govt.nz).

Naylin Appanna Womens Health Centre Dr Naylin Appanna MBChB (Natal), Dip O&G (Otago), FRANZ COG Dr Naylin Appanna is a leading Gynaecological Keyhole Surgeon (Laparoscopic Surgeon) who has been performing advanced Laparoscopic Surgery in Hamilton since 1996 and for 3 years in England prior to this. Naylin did his basic training in South Africa, going on to a Diploma in Obstetrics and Gynaecology at the University of Otago, before completing his specialist training in New Zealand. He then worked at the Musgrove Park Hospital, Ryde Hospitals and the Royal Sussex County and Brighton General Hospitals in England.

During this time, in the early 1990’s, he attended Advanced Laparoscopic Courses at the Leeds Institute of Minimally invasive surgery as well as Hamburg and started performing advanced Keyhole procedures well before most other gynaecologists had contemplated Laparoscopic Surgery.

Naylin Appanna returned to New Zealand in 1996 and has been working as a private gynaecologist specialist at the Womens Health Centre; a private practice in Hamilton, with clinics in Tauranga and other locations throughout the Waikato. Not content with his surgical skills, Naylin has attended further courses in Laparoscopic Surgery including the Minims course in Brisbane and mentored other specialists in learning laparoscopic surgical techniques.

Dr Appanna understands the full advantages of Laparoscopic surgery and the benefits for his patients. Also called minimally invasive surgery (MIS) or keyhole surgery, this modern surgical technique in which operations in the abdomen are performed through small incisions (usually 0.5–1.5 cm) as opposed to the larger incisions means reduced pain, less risk of haemorrhaging and a shorter recovery time. Being the most advanced Laparoscopic gynaecological surgeon in Tauranga, Dr

Appanna is the first and foremost for many women in the Bay. Being predominately based in Hamilton, Dr Appanna identified a gap in the services available to the women in Tauranga and now continues to conduct a clinic at 49 Fraser Street every 2nd week to cater to new and existing patients.
