My Health Record provides a safe and convenient way to manage your health information in one place, and easily share it with your health and aged care team.
BaptistCare at home is registered to access the national My Health Record as part of our planning and delivery of care.
Through My Health Record, our Registered Nurses and Allied Health team will have better and more timely access to your health information, particularly where treatment has been provided in health services outside our service or where emergency treatment is needed.
Your health information is uploaded into My Health Record by healthcare professionals, and includes documents such as:
y Immunisation and vaccination details
y Medical history and shared health summaries
y Discharge summaries from hospital visits
y Current medicines
y Radiology and blood test pathology results
My Health Record can also be used to store an Advance Care Plan if that’s something you have or are thinking about.
You probably have a My Health Record, unless you opted out, so it’s recommended you talk to your GP and other healthcare professionals and ask them to add more of your health information into your record.

You can also have a family member or carer access and manage your record on your behalf if you would like. Your representative can apply to be an authorised or nominated representative by going online at www.digitalhealth.gov.au and use the search function to find ‘representative’.
