The Final Footprints Support Program is an end-of-life care coordination service designed to support people with a life-limiting illness to enhance their wellbeing.

The care coordinator works with the person and their carers to assist them to receive the support they require during this stage of life.

Services provided may include:

• Assistance in defining and achieving personal goals

• Information about specialised services and support, to access these services

• Coordination of care between services

• Education about palliative care and symptom management

• Support to develop advance care plans and directives, and appoint legal representatives

• Referrals to service that assist with everyday activities

• Assistance when moving between care settings

• Support to access culturally appropriate services and resources.

This program is fully funded by the Hunter New England and Central Coast Primary Health Network and there is no cost to access it.

How do I access the program?

The Final Footprints Support Program is available to anyone over 55, with a non-malignant life limiting illness, living in the Newcastle or Lake Macquarie Local Government Area.

Who can refer:

• The person with the life limiting illness

• Family / friends / carers

• GP’s, nursing and allied health professionals

• Local palliative care services.
