Welcome to the Arizona Pharmacy Association’s 2024 Annual Convention at Hilton Phoenix at the Peak Resort! I hope you will indulge yourself in the resort’s many amenities. The theme of this year’s conference is “Where Pharmacy Excellence Takes the Gold”.
The AzPA Board of Directors and staff are excited to celebrate 101 years of AzPA Annual Conventions with you! We will be using a digital conference app again this year which will enhance your conference experience and provide enhanced networking capabilities.
In addition to the numerous continuing education activities, please take time to visit the Exhibit Hall where you will have the opportunity to network and socialize with your colleagues and learn about the latest pharmaceutical products, technology, and services Come enjoy the food and fun and visit with all our exhibitors for a chance to win great prizes like registration to the 2025 AzPA Annual Convention, AzPA membership, gift cards, and more!


Come join us as we celebrate our colleagues at the Awards Presentation on Saturday night. During this event we mark the transition of our Association leadership and honor our peers and colleagues. This is an exciting time to reconnect with old friends and make new acquaintances. We wrap up the convention on Sunday with more great speakers and sessions.

HOW TO REDEEM YOUR CE
LecturePanda is the learning platform that AzPA utilizes to submit your CE credits.
Getting Started
Locate your “AzPA 2024 Annual Convention Details” email which provides you with the link to access Lecture Panda. Look for an email from noreply@lecturepanda.com for further instructions.
Inside Lecture Panda
The top left corner of the event you are in should read “AzPA 2024 Annual Convention,” and there are 5 tabs running across the top of the screen. You will progress through each tab until you reach the end where you will submit all of your information:
Materials: Powerpoint slides will be posted here
Information: This tab lists all of your personal registration information. Double check your information to make sure it is correct. If you make any changes be sure to hit the blue [Save Changes] button at the bottom of the screen
Credits: Select the credits you earned. Click the box next to the credit and enter the CE session code in the box and hit submit.
Evaluations: All CE assessments and evaluations are stored here You MUST complete them and hit the green [Submit] button in order to get CE credit for that session
IMPORTANT! You have 2 attempts to take each quiz. If you failed the 2 attempts for a particular assessment, before you can submit all of your other CE Credits, you will need to uncheck the box for the session with the failed assessment attempts then click the GREEN submit button Complete: Now you will see all credits that have been claimed and certi cates will appear for you to download Note: This statement contains information provided to NABP from the Accreditation Council for Pharmacy Education (ACPE) via CPE Monitor® ACPE policy states paper and/or electronic statements of credit may no longer be distributed directly to learners as proof of ACPE credit. Accreditation:
The Arizona Pharmacy Association (AzPA) is accredited by the Accreditation Council for Pharmacy Education (ACPE) as a provider of continuing education.
How to Claim Credit:
To claim CE credit for the CPE program, participants must have actively participated in the entire session, submit the corresponding session code, and complete a session evaluation using the AzPA online CPE submission site. You may claim your CE credit until August 31, 2024.
Target Audience
AzPA Annual Convention education sessions are designed for pharmacists and pharmacy technicians from all practice settings Courses appropriate for pharmacist participation are designated by an ACPE universal activity number ending in the letter “P.” Courses appropriate for pharmacy technician participation are designated by an ACPE universal activity number ending in the letter “T”

Michael Blaire, RPhA graduate of St John’s University College of Pharmacy in New York, Mr Blaire founded and served as supervising pharmacist for three pharmacies in New York City Upon moving to Arizona in 1993, he served as Pharmacy Manager at several Walgreens pharmacy locations in the Phoenix area. In 2001, Michael moved on to co-found Diamondback Drugs, a veterinary compounding pharmacy that served zoos, aquariums, and veterinarians nationwide Throughout his tenure at Diamondback Drugs, Michael was recognized nationally as an innovator in the eld of prescription compounding for veterinary patients and was featured on the cover of the February 2012 issue of America's Pharmacist magazine. Diamondback Drugs was sold to Wedgewood Pharmacy in July 2018, where Michael now holds the position of VP Government and Regulatory Affairs
For over 25 years Michael has been an active advocate for the pharmacy profession. He has been Chairman of the Arizona Pharmacy Association Political Action Committee, and Chairman of COMP-PAC, the political action committee for the Alliance for Pharmacy Compounding (APC) He has been a member of the Political Action Committees for the American Pharmacists Association (APhA) and the National Community Pharmacists Association (NCPA) and has served as Chairman of the Compounding Special Interest Group at APhA. In 2015 Mr. Blaire was appointed to the Arizona State Board of Pharmacy and was Board President in 2018 As a member of the National Association of Boards of Pharmacy (NABP), he was appointed to the Committee on Constitution and Bylaws and served as Chairman of District 8. Most recently, Michael served as President and Board Chair of the Alliance for Pharmacy Compounding. Outside of the pharmacy, Michael is active in several animal related organizations In 2013 he received the “Unleashed” Award from Power Paws in recognition of his contributions to advancing canine health Most recently, he was invited to join the Board of the Arizona Center for Nature Conservation/The Phoenix Zoo.
Over the course of his career Michael has acted as a preceptor to over 50 pharmacy students With each, he has tried to instill a love for the pharmacy profession and a realization of the need to advocate for its survival. Michael is the 2017 recipient of the APhA Good Government Pharmacist of the Year Award and a 2019 recipient of the Arizona Pharmacy Association Hall of Fame Award In 2018 he became a Fellow of APC and in 2021 he received the Bowl of Hygeia Award from AzPA

He, along with his co-presenters, was successful in the postponement of the implementation of these chapters that would have resulted in negative impacts on compounding pharmacies and the patients they serve

Dr Stein retired from US Public Health Service Commissioned Corps as a Captain having been on active duty as a pharmacy of cer for twenty-eight years. During his PHS career he was assigned to the Indian Health Service as a staff pharmacist, pharmacy resident, senior clinical pharmacist and regional pharmacy consultant and clinical pharmacy educator Then-Captain Stein provided leadership on several pharmacy practice initiatives including the IHS Pharmacy Standards of Practice, “A Report to the Surgeon General: Improving Patient and Health System Outcomes through Advanced Pharmacy Practice,” and the PHS National Clinical Pharmacy Specialist certi cation and credentials In March 2020, CAPT Stein was tapped to take part in the IHS’s COVID-19 response as the regional Incident Command’s Logistics Section Chief, which he did for 15 months.

He then attended the University of Hawaii for pharmacy and graduated with his doctorate and bachelors degrees in 2014. After graduation, Jimmy became a member of AzPA to help improve the pharmacy practice From 2018-2023 he served as Co-Chair of the Continuing Education Committee He was voted by his peers as Arizona Pharmacist of the Year in 2021. Currently, he sits on AzPA’s board of directors. He is a preceptor for the University of Arizona, University of Hawaii, and Midwestern University in Glendale, AZ Jimmy’s passion is to learn, teach, and share his knowledge so that others can develop into valuable leaders In his off time he enjoys traveling, reading, and being a devoted friend, husband, and dad to his daughter Tegan


Yousef Toma, PharmD, BCPS, CSP, AAHIVP
Yousef, an assistant professor of pharmacy practice at Creighton University School of Pharmacy and Health Professions, Phoenix, AZ, earned his Doctor of Pharmacy degree from Roseman University of Health Sciences, Henderson, NV He completed a PGY-1 residency in Specialty Pharmacy Practice at Optum Specialty Pharmacy, Phoenix, AZ Dr Toma is a Board-Certi ed Pharmacotherapy Specialist (BCPS), a Certi ed Specialty Pharmacist (CSP), and a Certi ed HIV Pharmacist (AAHIVP) He was recognized as a Distinguished Young Pharmacist by the Arizona Pharmacy Association (AzPA), awarded the 2022-2023 Educator of the Year at Creighton University, and named the 2023 Alumni Roadrunner from Roseman University.

Captain Holly Van Lew is an advanced practice pharmacist working in the Indian Health Service (IHS) She is a national trainer for multiple pharmacist and pharmacy technician immunization certi cate programs, training more than 2,000 pharmacists, students, and technicians to administer vaccines. CAPT Van Lew works at the local, regional, and national level to educate pharmacists, providers, and nurses about immunizations with a focus on meeting patient vaccine needs at every encounter She is currently working at IHS Headquarters as an HIV/STI/HCV and Communicable Disease Pharmacy Consultant.

Scott is an Infectious Disease Research Fellow and Visiting Instructor in the Department of Pharmacy Practice at Midwestern University College of Pharmacy – Glendale Campus. He earned his Bachelor of Arts in Biochemistry from Arizona State University and Doctor of Pharmacy from Midwestern University College of Pharmacy-Glendale Campus

Adrienne Waibel, PharmD, CDCES
Adrienne earned her Doctor of Pharmacy degree from Midwestern University in Glendale, Arizona in 2021. She completed a community-based PGY1 residency at Sun Life Health Center in Casa Grande, Arizona. Dr. Waibel is a Certi ed Diabetes Care and Education Specialist (CDCES) She currently practices as an ambulatory care clinical pharmacist at Village Medical in Mesa, Arizona
Diane Wasley, BS (Public Health)
Diane is the Athlete Leadership Assistant for Special Olympics Arizona (SOAZ) She got her bachelor’s degree in public health from Northern Arizona University in Flagstaff AZ She has been an athlete for about 15 years. She has competed in bowling, track and eld, swimming, golf, and cheer and dance. She is an athlete leader and health messenger and was an intern in health programs and community outreach prior to becoming an employee She also continues to compete as an athlete Diane assists the Athlete Leadership Senior Coordinator to expand the athlete leadership program and educates community members on the importance of inclusion.

Kaylee is a current PGY1 Pharmacy Resident at Mayo Clinic Hospital in Arizona. She will be completing a PGY2 in critical care next year. She completed her Doctor of Pharmacy at The University of Kentucky in her home state Her interests within critical care include pharmacokinetics, antimicrobial stewardship, and hemodynamics She hopes to be involved with research and teaching throughout her career

Samantha Zimmerman, PharmD, MLS, BS
Samantha is a PGY-1 pharmacy resident with Sun Life Health in Casa Grande, AZ She completed her undergraduate degree in Child Development and Nutrition at Arizona State University where she was a member of Gamma Phi Beta. Samantha began pharmacy school at University of Arizona in 2019, where she rst became interested in law She opted to complete her Masters of Legal Studies while earning her PharmD, and graduated from both programs in 2023 Later this summer Samantha will begin her PGY-2 residency in ambulatory care at Whiteriver Indian Hospital in Northern Arizona.
