I have put together our response based on the listed requirements. While we feel this will best suit the overall needs of the University, we are excited to explore all options with you and your staff. While the RFP lists a 4 day period of use, we are offering a 2 day rate with 55% equipment discount because we want to show you what an Axxis experience is like. All equipment, labor, travel and misc expenses have been factored into this estimate. The only additional item not included is support labor for setup and strike, which we estimate to be in the ballpark of $8,000 pending schedule and final design choice. We are dedicated to meeting our clients budget and expectations and always err on the side of offering more. Our inventory includes things like way-finding with digital signage, LED uplights to celebrate the university school colors of blue and red along with anything else we discover together along the way that will improve the experience for the students and their friend and families. We also own and operate 5W, 10W and 20W lasers and have used them for outdoor events as well as arena for the University of Louisville Men’s and Women’s Basketball teams opening entrance to the court at the Yum Center in Louisville, KY. Your Axxis Team Stewart Davis, Managing Partner As Managing Partner for Axxis, Stewart oversees the strategic direction for the company. Much like the conductor of the Axxis orchestra, Stewart provides leadership for the Production, Design, and Account Management teams to facilitate a single vision. Stewart has been with Axxis for nearly a decade having previously held senior level positions in the Hotel and Resort industry in Marketing, PR, and Event Management. Stewart graduated Cum Laude with a dual major in Economics and Marketing from Ohio University. Justin Wardell, Director of Production Sales Justin Wardell has been with Axxis since 2014, and manages several of Axxis’ key accounts. Justin has been in the live event industry for over 15 years, and has played a key role in hundreds of productions that include conventions, concerts, festivals, corporate meetings, galas, theater performances, and permanent installations. Prior to moving to Louisville, Justin was Sr. Account Executive at Intelligent Lighting Creations, and Rental, Retail, & Production Sales Manager at DesignLab Chicago. Justin earned an MBA in Marketing from DePaul University, and a BS in Theater Design and Psychology from Washington University in St. Louis. Kate Schroeder, Sr. Account Director Kate flew to Axxis after serving over half a decade as a Senior Producer with ZFX Flying Effects where she executed the contract and managed the 2010 Vancouver Winter Olympics medal ceremonies. Kate also produced flying effects for hundreds of theatrical and special events for universities, schools, businesses and churches. For the past 7 years with Axxis, Kate has successfully managed our top corporate accounts including Humana, GE Appliances, Chevron, U.S. Equestrian Federation, University of Louisville and Louisville’s Holiday in the City/Light Up Louisville. Chris Burgin, Director of Lighting and Design Chris joined Axxis over 15 years ago as a Lighting Technician and quickly ascended to Director of the Lighting Department. He handles all lighting designs in-house, which allows us to provide a turnkey service to our clients. His extensive knowledge of lighting systems and CAD design make him a valuable asset to our team. Prior to Axxis, Chris worked as a Lighting Designer and Director for touring productions with Montgomery Gentry and Tracy Byrd.

Who We Are Scott Walker, Director of Audio and Video Scott has been with Axxis for over 20 years and currently runs our Audio/Video Department. Scott has an extensive background in audio and has developed a strong passion for video. Scott also takes care of inventory control as well as capitol purchases. Anthony Vaughan, Director, Scenic and Rigging Services Anthony began working with Axxis 7 years ago; his ability to save on resources and efficient planning launched him into his role as Rigging and Scenic Department Manager. Not only does he currently serve as Technical Director on-site for various events, he also manages our custom scenic builds and fabricated signage for clients. He and his team are always prepared to take on a challenge.

Our Story AIn 1981, at age 45, Reginald “Reggie” Smith decided to move his family from Northern Kentucky back to his hometown of Louisville and start an audiovisual rental business. He sold his house and purchased 6 overhead projectors, 4 slide projectors, 8 screens, a small sound system, a VCR, and a television. All of this (and the family) was moved into a rental house in Louisville and AV of Louisville was born. Reggie didn’t understand anything about the equipment he just purchased. However he did understand the power of hard work, customer service, and building relationships. He also understood that the company needed to provide dependable, good quality equipment and it had to be delivered on time. In the beginning, he ran the business out of his garage, using the family station wagon as the main mode of delivery. It was not uncommon at the time for the family television to be pulled out of the house if orders exceeded company inventory. In the first year the company grossed $15,000. As do most new entrepreneurs, Reggie thought of giving up several times but his continued attention to personal service; his ability to earn the trust of his customers; and his endless hours of work finally paid off and created the opportunity for growth. This growth took the company far beyond mere “audiovisual” so the name was changed to Axxis in the 90’s. Riding the constantly changing wave of technology Axxis continues to expand its capabilities by providing a full range of services, including full event design and production services, creative themes, program development, artistic design and technical expertise. Even though Axxis has far exceeded the Louisville hotel market, Axxis still remains committed to the core business that Reggie began in 1981 – audiovisual rentals; delivery of dependable, affordable equipment. Today, after more than two decades of service, Axxis has one of the region’s largest inventories of audiovisual and production equipment. The company employees over 55 full-time technicians and a rich network of freelance technicians who travel all over the country delivering riveting, state-of-the-art events and productions to some of the largest companies in the United States. Customers such as Conklin, GE, Humana, HBO boxing, and others have trusted Axxis as their partner to create unique experiences for their attendees. We consider this trust to be an honor and a testament of our work ethic. We have grown. And though growth has put a new face on the business, the fundamental principle that Reggie Smith began with still applies: “You have to show up on time and do a great job in order to earn the trust of your customer. July 31st, 2017 I’ve been producing and coordinating events for 22 years and will no longer use any production company other than AXXIS for my large scale events. Although it’s often more cost effective to use inhouse or local AV companies, you’re never sure of the quality of service you’ll receive. When the success of my events relies so heavily on production, I’m not willing to take a chance on the unknown. With AXXIS on-site, I don’t worry about the production elements and can focus my attention on other aspects of the event. From the sales process, to the set and design phase and throughout the execution of the event, I’ve been working with the same team of professionals for years. There’s such a level of comfort when you work with people that know your shows. They know the look I’m trying to achieve, my budget, what to expect on-site, etc. They are extremely responsive and approach a 10 person meeting with the same attention to detail as a General Session for 1,000. Axxis Owned Equipment Because we approach each job in a customized way we do not have a standard sales kit or catalogue. We offer solutions tailored to suit our client’s needs. That said, a sampling of major equipment in our inventory is listed below to provide an idea of our capabilities. We are able to handle a multitude of PA sizes for audio reinforcement including Arena; two (2) 10’ x 30’ projection screens that can be ground supported to save on rigging fees; two (2) large 9’ x 16’ LED video walls (5 mm) and we are able to support a show with a minimum of twelve (12) 14,000 lumen HD projectors and three (3) 25,000 lumens HD projectors without outsourcing any equipment. Because we have such an extensive inventory we are able to have a faster turnaround on event production and offer a significant cost savings to our customers. Depending on the complexity and size of event, we could receive a phone call the day before and have things turned around by the next day. We are also able to accommodate same day requests for smaller events with minimal needs.

Production Services Examples Company: Humana Event: Perfect Experience Multi-City Satellite Broadcast Conference Size: Large – Multi-state with 2 venues Location Site: Two (2) venues in the US (Kentucky International Convention Center and Hilton West Palm Beach) Brief Description of Production Services: This production is unique in that the conference spans over two (2) venues in different states. Axxis designed and built a custom stage and scenic package with glowing city-scape backdrops; Flown lighting with two (2) moving lights acting as spotlights in the audience; Three (3) 10’ x 30’ screens were used in each location along with twelve (12) 14,000 lumen HD projectors. We also had a redundancy built in to our plans by bringing an additional backup projector and video switcher. Axxis handled production management of all audio/visual services including the satellite broadcast trucks. Gross Cost: $600,000 Company: Kentucky State Fair Board Event: Kentucky State Fair Texas Roadhouse Concert Series Size: Very Large - Community Event Location Site: Kentucky Expo Center and State Fairgrounds Brief Description of Production Services: For over a decade, Axxis has provided audio, lighting and operating technicians for the free concert series at the Kentucky State Fair. Production Capabilities Axxis has a full scenic shop on-site at our main headquarters. Here we are able to fabricate custom scenic packages for our clients. Typical materials include wood, soft goods/fabrics, steel/welding and Plexiglas. Occasionally a project comes up that requires custom built truss from Thomas Truss or something outside of the norm. So, in some situations materials may come from elsewhere, but the fabrication is handled in-house by our scenic team. Because we handle all designs and fabrication in-house at our main headquarters, our sales team is able to be a part of all phases of construction. This allows communication to flow freely between designers, sales and our clients. We are also able to invite clients to visit our headquarters to see scenic designs in person. Our designers use a program called Vectorworks to mock up renderings. This program is used by professionals in the event industry all over the world. It enables us to show a variety of options to clients based on the artistic vision of the event. We can also insert the amount of people expected at the event, chair/table setups, etc. to show our clients how the event will truly look. The Axxis Scenic Department touts a 20’ automated turntable, cyclorama, a scrim, star drops, various soft good scenic elements, 14,000 square feet of stage decking and works closely with the lighting department to integrate lighting into scenic packages. We are constantly adding to our inventory to keep things fresh. In regards to transportation, Axxis owns several trucks which we use to transport equipment throughout Louisville and the United States. Scenic packages and their support elements are stored on-site at our main headquarters along with the rest of our staff.

Turnaround and Setup Time Axxis is always prepared for short notice special events. We have an extensive inventory and dedicated staff to accommodate any last minute requests. Depending on the complexity, turnaround time may vary. It is preferable that we have 30 days prior to the event date to plan and work out logistics with our clients and venue; however, we are always able to move quickly when time is of the essence. Most productions are setup the day (or days) before the event. In some instances setup happens the day of the event. The amount of time required for setup depends on the complexity of the project. Scenic packages may require more time or labor for a faster install. This is part of the reason why we approach each job in a custom way. Sometimes the venue will designate when we can setup based on other clients using the room the day before. We always lean towards the most efficient and cost-effective setup based on the scope of services. An example of our fast turnaround time occurred just recently for the U of L Red and Black Dinner. We received notification at 8:00am that a revised rain plan may be needed to deal with acclimate weather. Within a few hours we had held a production meeting, explored options and rendered up a new design for the Brown & Williamson club. We sent two (2) staff members to discuss this revised plan with the Facilities Director at the Football Stadium to ensure all logistics were met including power needs. It was determined later that day the weather may not be as bad as anticipated so this revised plan did not go into effect. But this is a perfect example of how committed our team is to ensuring the Universities needs (whether planned or unplanned), are met in a timely manner. In good faith of our partnership with the University we did not charge any fees for this last minute request. We will never let an event or your expectations fail.

Standard Terms and Conditions
To confirm the job and reserve all necessary equipment and personnel, the Client must issue a non-refundable deposit. All pricing is subject to availability; delays in issuing the deposit may affect pricing. Client will ensure a safe working environment free from environmental and health hazards. Axxis personnel follow OSHA guidelines for working in extreme weather, including but not limited to: extreme heat, extreme cold, lightning, tornadoes, hurricanes, earthquakes, and flash flood warnings. Axxis personnel onsite will make the final determination regarding crew safety, and will seek shelter or suspend activities based on OSHA guidelines and in consultation with the client. The Client is to secure sufficient access to the venue as needed to complete the work. This may include passes or credentials for the crew. Client is to provide security measures necessary to protect the equipment and personnel involved. Client is responsible for providing storage space for the AV Production, including a secure, designated location for empty case storage. Our labor estimate is as accurate as possible given the information provided at the time the quote was produced. It is agreed that any and all labor expenses incurred, including but not limited to changes in design or scheduling shall be the responsibility of the Client. All labor scheduled will be billed; cancellations or changes must be received in writing by Axxis personnel at least 48 hours in advance. The Client is financially responsible for any and all fees charged by the venue. This may include rigging fees, service charges, union labor, electrical service, etc. Axxis, Inc. employees must be allowed 10-minute breaks for every two hours worked and a 60-minute lunch break between their third and fifth hours of work. If employees cannot leave the job site for 1 hour at meal break, a nutritious meal must be provided at the work station at the client's expense. For all events outside of the Louisville Metro area, single-occupancy hotel rooms for all Axxis employees, Per Diem based on the published IRS rates, and applicable travel expenses are the financial responsibility of the client. Client agrees to provide Axxis with a list of personnel authorized to make changes and/or additions to the order. Every effort will be made to accommodate changes made less than 14 days prior to load-in, these changes may incur additional redrafting and/or prep charges. It is agreed that in the event of any dispute, both parties will engage in good faith mediation before pursuing arbitration or litigation. It is understood that this contract shall be governed by the law of the Commonwealth of Kentucky. All designs, renderings, and materials produced by Axxis Inc. are copyrighted; these designs are available for licensing upon request. Axxis must receive written notice of cancellation at least 30 days prior to load-in. If notice is received less than 30 days prior to load-in, Client agrees to pay a penalty equal to 25% of the contract total. 15 days prior to load-in, the penalty will be 50% of the contract total. 7 days prior to load-in, the penalty will be 75% of the contract total. The performance of either party under this Agreement is subject to acts of God, war, government regulation, terrorism, disaster, strikes, civil disorder, interruption or loss of utilities, curtailment of transportation facilities, any government, health, or other advisory that travel or meetings not be undertaken, or any other emergency beyond the parties' reasonable control, making it inadvisable, illegal, or impossible to provide the facilities or to hold the Event provided for under this Agreement. Should Axxis personnel or equipment be delayed or cancelled due to any of the provisions detailed above, the Client agrees to reimburse Axxis for expenses incurred, including but not limited to hotel rooms, Per Diem, car rental and other travel expenses, as well as any additional costs due to equipment shortages, or delays.
