Accident Reporting How Does it Work?
1. Locate the QR code on-site – this could be a poster on your Health & Safety notice board. Cards and vehicle stickers are also available for fast access.
2. Scan the QR code using your device to open the form on a website called Assure.
3. Follow the easy steps, giving us much detail about the event as possible.
4. Submit the accident report – this will automatically get sent to your manager.

You can also access the from your computer – a link will be saved to your ‘Axis CLC favourites’ tab on your browser.
App Access
A mobile app is also available for you to be able to submit reports, where you can also track it’s progress and outcomes
Accident Reporting What Happens Next?
1. Your manager will receive your report and start investigating the accident, using the information you have provided in the form.
2. They will access the accident and be able to put lessons learned into place and feed these back to their teams.
3. If necessary, the report can also be passed on and escalated to the Health and Safety team.
4. Evotix captures all this data to be able to provide us with analytics that are vital to tracking trends and ensuring the lessons learned are being implemented.
