Fingertips OCTOBER 27-30, 2020
AVA MEETING MOVES ONLINE On June 15, 2020, the Association for Vascular Access (AVA), in close collaboration with the Gaylord Rockies Resort & Convention Center, announced it will cancel its in-person 2020 Annual Scientific Meeting, scheduled to be held September 12-15, due to public health and safety concerns related to the coronavirus (COVID-19) pandemic. AVA is instead working with scheduled speakers, exhibitors, sponsors and industry partners to deliver a completely reimagined virtual meeting and exposition, AVA at Your Fingertips, from October 27-30. The cost for an AVA member to attend AVA at Your Fingertips is $199.00. Non-members can attend for $325.00, a fee that includes a 1-year membership to AVA. “After careful consideration and a thorough discussion of every aspect of the meeting and experience with our fantastic partners at the Gaylord Rockies Resort & Convention Center, AVA has made the extremely difficult to decision to move its 2020 Scientific Meeting to a virtual experience,” said AVA President Lori Kaczmarek, MSN, RN, VA-BC™. “Though we were eager to put on a robust, memorable in-person Annual Meeting in Denver this September, the public health and safety of our attendees, staff, volunteers, sponsors, exhibitors and the Gaylord staff greatly outweighed any other considerations in our collective decision-making process.”

Considering that many of those factors are either still unknown or constantly in flux, reviewing the feedback AVA received from the more than 1,000 participants of its 2020 Scientific Meeting prospective attendee survey, and with numerous AVA Annual meeting attendees typically coming from all over the country as well as internationally, all parties involved concluded that canceling the face-to-face event was the safe, responsible and appropriate action. “AVA and the Gaylord were eager to exceed the attendee expectation for the 2020 Scientific Meeting, and we look forward to working with our Denver partners in the future,” said AVA COO Tonya Hutchison, CAE. “As we shift gears, however, we are now fully immersed in providing the most dynamic digital meeting experience for our members and attendees possible. Attendees can anticipate the same great content we were primed to deliver in Denver, and we are anxious to structure this reimagined experience to fit a variety of schedules to further engagement and enhance their vascular access education.” Further details about the 2020 AVA at Your Fingertips will be communicated as soon as they are finalized and updated on the meeting’s website.

PEDISIG UPDATE Lisa Sheehan, MSN, RN, VA-BC™ | PediSIG ELC Director-at-Large, PediSIG D-TEAM Liaison
Amid these times of chaos and uncertainty, the leadership of AVA’s Pediatric Special Interest Group (PediSIG) is looking forward to getting together with our pediatric and neonatal members across the nation this year virtually to establish a new normal. Although the format of this year’s scientific meeting may be different, the one constant is the quality of content offered to the pediatric and neonatal populations. The recent announcement of pediatric sessions for AVA at Your Fingertips shared the content scheduled for presentations in October, and hopefully made it apparent that the pediatric and neonatal education was selected carefully to address current concerns in our area of specialty. The live content on Friday, October 30, will feature PediSIG specific content still designated by our logo on topics such as CLABSI reduction in 2 pediatric units, 26 gauge PIVCs, PICC tip location subjectivity, and, of course, the 6th Annual PediSIG Neonatal and Pediatric International Grand Rounds hosted by Elizabeth Sharpe, DNP, ARNP, NNP-BC, VA-BC™, FAANP. Although the PediSIG logo is only listed on Friday’s live sessions, many other days offer valuable information for the pediatric audience. Among them include Chronicles From the Front Line: COVID-19 Experiences of our Brothers and Sisters set for Tuesday, October 27 and the Herbst Awardee Session with 2020 recipient Matthew Ostroff, MSN, ACNP, VA-BC™. Don’t miss out! Be sure to also search for pediatric
sessions in the Educational Presentations section of the AVA at Your Fingertips app once it is available to access the content that carries the PediSIG seal of approval. Among the topics of interest: positive venipuncture experiences for pediatric patients, the effect of inappropriate vascular access decisions on future vascular access outcomes, preventing HAPI, and 10 years of pediatric mid-thigh femoral PICCs. If you have attended a previous in-person Scientific Meeting, you are aware that PediSIG likes to gather and network among like-minded practitioners, and we will have that opportunity once again this year. So, grab your cocktail and make sure you’re available for our virtual reception this year! PediSIG is still accepting nominations for the 1MPokes Call to Action award. Even though COVID-19 has somewhat slowed the pace of activity of the PediSIG leadership group, we have still been busy behind the scenes trying to make PediSIG something each pediatric vascular access practitioner can’t live without. In the spirit of inclusion ever present in our society today, PediSIG is also becoming more inclusive of the neonatal vascular access practitioner. Be sure to attend the webinar on August 26 titled, “A Tale of Two Algorithms” hosted by Angela Alderman, RNC-NIC, BSN, VA-BC™ from Carilion Children’s Hospital and Jane Hartman, MSN, APRN, PNP-BC
from Cleveland Clinic Children’s. This webinar will discuss two experiences of implementing neonatal algorithms and DIVA scoring tools to decrease the number of pokes for the smallest of patients. The presenters will go through the process of how the algorithms were developed and implemented to maximize their effectiveness in each NICU. Learn how one NICU was able to achieve a month with no neonate being poked more than twice for their vascular access by utilizing the algorithm and scoring tool. The goal would be that all neonatal and pediatric patients are scored for the difficulty of their vascular access to produce the most positive outcomes for our smallest patients to prevent chronic NICU babies from the scarring consequences of haphazard vascular access practices. Look for more information from AVA soon on social media and via email on how to register for this event. As a bonus . . . attendees at this webinar are in for something special. Just like a preterm infant, the reveal is a surprise, so join the webinar to share in PediSIG’s big announcement!

AVA: A LEGAL PERSPECTIVE Russ Nassof, JD | AVA and The AVA Foundation Treasurer
Vascular access is fraught with potential legal issues. Clinicians are increasingly exposed to situations where they could become liable, even before catheter insertion. As a result, I am always a little surprised when someone asks how an attorney got so involved with the Association for Vascular Access (AVA). Frankly, it was shocking to find out that I was possibly the only attorney in the organization when I became a member many years ago. Recently, I was reviewing the upcoming Infusion Therapy Standards of Practice. While I could not share guidance on the clinical aspects of the document, it occurred to me that there were so few sections where legal issues did not come into consideration. From non-clinical issues such as patient education, informed consent, and documentation, to clinical issues of adverse events, product evaluation, vascular access and device placement, vascular access clinicians must constantly deal with issues and make decisions that can have legal implications. Even the use of terms like evidence-based practice, clinical practice guidelines, and standard of care can have legal ramifications. As a result, vascular access clinicians must become knowledgeable about the law and must understand how the law can influence and actually improve
vascular practice. While clinicians often have distinct and unique preferences and differences surrounding products and practice, I believe there is a general consensus that the goal is to achieve compliance with an evidence-based standard of care. While this is the goal in theory as well as practice, I have found few clinicians who understand what compliance with an evidence-based standard of care looks like for vascular access. This failure can result in malpractice allegations and liability. David Sackett clearly defined the triad of evidencebased components as clinical judgment/critical thinking, the incorporation of patient values and preferences, and the utilization of relevant/current scientific evidence.1 While simple to recite, these components have proven to be extremely difficult to successfully implement across the continuum of medical practice. In the world of vascular access, critical thinking includes selecting the right product for a particular patient while utilizing best practice throughout the entire life of the device. In addition, it is important to involve the patient in shared decision-making with respect to catheter type, location, care and maintenance, access, and other clinical and lifestyle issues. It is, however, the last component of utilizing relevant/current evidence that may result in the greatest potential malpractice exposure. Put simply: If there was a readily available, reasonably reliable CONTINUED ON NEXT PAGE AUGUST 2020 | 13

when possible because the exposure to emerging and recent improvements can facilitate the inauguration of change, moving healthcare practice in the direction of evidence-based compliance. Knowledge of the law is a powerful tool in the hands of vascular access clinicians who can harness that knowledge to facilitate change and meet the evidence-based standard of care.

AVA BOARD DEVELOPMENT COMMITTEE, CONTINUED FROM PREVIOUS PAGE persistence, collaboration, and teamwork as we consider our call to action. AVA has added a board member position to serve a “patient role” in the organization. This Directorat-Large position brings the patient’s voice to the board to assist the organization with initiatives and strategies that engage the patient community. The AVA Foundation supports AVA’s mission, therefore the BDC supports AVA by vetting candidates for the AVA Foundation as well. The BDC interviewed and recommended a slate for leadership candidates earlier this year. A Foundation liaison will be joining the BDC as a full member this month. We look forward to considering the Foundation board candidates later this month to find the best people who can help the Foundation continue to provide the best innovation, research, and education to our communities. The candidates who fill the officer and at-large positions has been presented to the current AVA board who subsequently affirmed the slate. Your vote matters. Your vote as an AVA member supports the strategic process of identifying future leaders. Your vote allows AVA to follow best practice in association governance through the engagement of members regarding leadership decisions. NETWORK NEWS PERIPHERAL IVs: INFECTION RISKS, EVIDENCE & PATIENT SATISFACTION – DMVVAN’S FIRST-EVER VIRTUAL MEETING Sarah Mumma, RN | DMVVAN Treasurer
An unofficial slogan of the United States Marine Corps is “improvise, adapt and overcome.” As medical professionals, we probably feel as though this, too, has become our unofficial slogan for the last couple of months. DMVVAN, the DC/Maryland/Virginia Vascular Access Network of the Association for Vascular Access (AVA), conducted only seven meetings before the COVID-19 pandemic. As a network, we needed to move forward and find a way to connect to our members and provide them with valuable vascular access education. As a result, the DMVVAN Board began discussing our July 2020 meeting. Because an in-person meeting, unfortunately, wasn’t possible, a webinar was the best alternative option. We started by reaching out to a representative at Becton Dickinson and requested a specific speaker and topic from the list of AVA pre-approved presentations. The representative then reached out to her manager for approval and guidance. Once authorized, the sponsor contacted the speaker for approval. DMVVAN then created a flyer for the event, which included the objectives for the presentation. We subsequently applied for the CEs through AVA. This process, outlined entirely on the AVA website, includes uploading the flyer for the event, the speaker/ sponsor information, and a copy of the PowerPoint
presentation. Once the CEs were assigned and the flyer updated, this information was then distributed. Advertising was done through AVA and DMVVAN webpages, DMVVAN Facebook, the Vascular Access and Infusion Specialists Facebook page, emails to members and contacts, and through LinkedIn. It didn’t take long for DMVVAN to receive registrations for the virtual event. We asked the registrants whether they would like to pay for the single webinar or a yearly membership. Membership includes four events held quarterly. After receiving payment, participants were moved from a pending list to the approved list of registrants that was then forwarded to the sponsor. The day before the event, our sponsor emailed all participants with the link to the meeting. Overall, our first virtual meeting as a chapter was a success. Some joined from across the country, some undoubtedly joined in their pajamas, but we all were able to come together, network, and gain knowledge that will allow us to better care for our patients. We are very thankful to both Michelle DeVries, MPH, CIC, VA-BC™ for her wonderful presentation and to BD for their support and sponsorship of this event. Unfortunately, none of us know when we be able to be face-to-face again. So, as a network, I encourage you- move forward in these unprecedented times and host a virtual meeting for your chapter.

INDUSTRY NEWS ivWatch SmartTouchTM Sensor Receives FDA Clearance for Continuous IV Site Monitoring FDA clearance expands product portfolio with the introduction of a miniaturized and disposable IV infiltration and extravasation detection sensor.
Newport News, VA, July 7, 2020 – ivWatch, LLC, a pioneer in biosensor technology, has received FDA clearance for its new SmartTouchTM disposable sensor for the early detection of peripheral IV infiltration and extravasation events. This new FDA clearance expands the ivWatch® sensor portfolio to provide hospitals, infusion centers, and other healthcare facilities with complementary sensor solutions to help improve patient safety and reduce the risks associated with IV therapy. With an estimated 46 million high-risk peripheral IVs given in the U.S. every year, ivWatch is the only company to provide hospitals with a continuous IV site monitoring solution.1 Designed with direct clinician input, SmartTouch is a single-use sensor that is cleared for all patient age groups. The new miniaturized and disposable sensor adds functionality options for IV site placement, particularly for neonates, for monitoring IV sites of more active patients, and for longer dwell times. The flexible, low-profile sensor includes a breathable adhesive to work seamlessly with existing IV dressings. When paired with the ivWatch Patient Monitor, a proprietary signal processing algorithm continuously measures changes in the optical properties of the tissue near an IV insertion site and simultaneously accounts for patient motion. The ivWatch Patient Monitor provides notification
to clinicians of infiltration and extravasation events often hours before they are detectable by visual or tactile examination.2
“At ivWatch, we believe that every patient deserves the highest level of care. We are on a steadfast mission to advancing patient safety by minimizing harm caused by IV therapy. Having continuous IV site monitoring available in all hospital departments is critical to our mission,” said Gary Warren, President and CEO of ivWatch. “We recognize that all clinical scenarios vary and we are proud to provide clinicians with IV site monitoring tools that are intuitive, complementary to current clinician processes, and equipped to provide reliable guidance and notification, so that together we can help reduce the harm associated with IV complications.” ivWatch sensors use visible and near-infrared light to measure changes in the optical properties of the tissue near a peripheral IV insertion site and SmartTouch can detect infiltrations of as little as 0.2 mL of IV fluid, with an average detection volume of 2.02 mL.3 Such acute perception can be the CONTINUED ON NEXT PAGE AUGUST 2020 | 23
INDUSTRY NEWS - IVWATCH SMARTTOUCH, CONTINUED FROM PREVIOUS PAGE difference between minor discomfort and a major patient IV injury. “The most important aspects in developing our cutting-edge SmartTouch Sensor were to ensure our design was just as reliable as our current offering, to incorporate clinical and user experience feedback, and to provide new sensor options to exceed clinician and patients’ varying clinical needs,” said Jaclyn Lautz, Ph.D., Vice President of Research and Development at ivWatch. “With our unwavering commitment to improve patient safety through both our biosensor technology and our user interface experience, we are helping hospitals achieve better patient outcomes.” A series of seven IRB-approved studies were conducted to test the efficacy and safety of the SmartTouch Sensor. Five verification studies were conducted to understand the performance and optimize ivWatch’s proprietary algorithm to maximize sensitivity to infiltrated tissues, while limiting the number of false notifications issued. Two validation studies were conducted to investigate device sensitivity and false notification rates. Clinical data shows that the SmartTouch Sensor issued notifications for 99.0%* of early stage infiltrations in less than 10 mL of infused IV fluid. Results also showed less than one false notification was issued every six days, therefore having minimal contribution to clinician alarm fatigue.3,4,5 The majority of the noninfiltration notifications were attributed to a force applied to the IV site, which may beneficially notify clinicians of conditions that could place the peripheral IV site at a greater risk for complications. ivWatch also has a proven record of detecting IV infiltrations an average of 15 hours before the clinician.2 In addition to the launch of SmartTouch, ivWatch is releasing an update for the intuitive user interface on the ivWatch Patient Monitor, which will enhance the clinician and patient experience. The interface will include upgrades such as a new event history log that clearly showcases the state of the IV, audiologistapproved tones and adjustable volumes, and the ability to adjust brightness levels to offer adaptability to various clinician and patient needs.

Along with FDA clearance, the new SmartTouch Sensor has also received the CE Mark and is available internationally. To learn more about SmartTouch and other ivWatch monitoring solutions, visit ivWatch. com and follow the company on various social platforms for the latest updates.
