Ascension St. Vincent Tri State Clinics Employer Care Program What is Ascension St. Vincent Tri State Clinics Employer Care Program? Ascension St. Vincent Tri State Clinics Direct Care For You is now known as Employer Care Program. Employer Care Program is a service that delivers convenient high-quality primary care directly to your employees and dependent family members who are age 2 or older. Services include preventive and wellness focused primary care services, care for sudden onset illnesses, chronic condition management such as high blood pressure, diabetes, high cholesterol, anxiety/depression, and allergies. Many labs and medications are also available onsite at all Employer Care Program clinic locations.

What can I expect? • Convenient access to online or phone scheduling • Appointments are required. However, there are often same day/next day appointments available • Minimal wait times • Convenient locations and a variety of hours of availability • Clinic visits, and a limited number of lab tests, and prescriptions are provided at no cost in the clinic • At your request, all medical records including lab results can be sent to your personal physician or specialist • Initial office visits to establish care should be scheduled for 40 minutes • Follow up & illness appointments will take 20 minutes • The clinic offers you a 24/7 Patient Support Line to reach a nurse or medical provider to assist you with your needs outside clinic hours • All services you received are confidential and protected by law under the Health Insurance Portability and Accountability Act (HIPAA).

Who is eligible to use Employer Care Program? Employers are asked to communicate their list of eligible employees to Tri State Clinics. When you check in for each visit, the clinic staff will verify your eligibility. Please bring a photo ID.

Is there a cost? Access to the Employer Care Program clinics is provided to you for no out-of-pocket cost. If you are referred to another provider, lab or pharmacy outside of the clinic, your normal health benefits will apply and there may be a deductible, co-pay or other out of pocket cost.

Can I get my prescriptions at the clinic? Medications prescribed by the clinic provider, that are included in the clinic formulary, are available at the same time of your visit. If you need a medication not found in the clinic formulary, the clinic provider can send a prescription to a pharmacy of your choice to be filled. (This Rx will require payment at the pharmacy.) If a prescription is written by an outside provider and available in the clinic formulary, you can schedule an appointment with the clinic provider to receive the medication at no cost. Take the clinic formulary list to your personal physician appointments so they are aware of your free options. For routine maintenance medications, patients generally receive a 90 day supply at each fill. If you are needing a medication refill, please visit our website and make an appointment for a medication refill for your desired pick-up time and location. Medication refill appointments can also be made by calling 888-492-8722.

How can I schedule an appointment online? You have options when if you want to go online to schedule an appointment. You can go to the Tri State Clinics website or log in to your Ascension Patient Portal to schedule. See steps below for each option.

• Then select ‘Ascension St. Vincent’ For patients who do not have login information, please visit a Tri State Clinic or call the office at 888-492-8722 with your personal email address. They will send you the information needed to access your patient profile.
