FROM THE PUBLISHER This April 2010 issue marks the beginning of Lifestyles over 50’s fifth year in publication. It seems like only yesterday I was cold calling nonprofit organizations and community agencies to help provide me news and information relevant to older adults. Even more difficult was to sell ad space to prospective advertisers to support such a venture. I had to do much arm twisting to prove that a 25-year old kid with no industry experience could start and maintain a publication for those over 50.

From my first editorial until now, our goal remains the same. In April 2006 I wrote, “Today, I remain filled with the desire to help older people. I am overwhelmed by their needs and appalled

Understanding Palliative Care

What is Palliative Care?

There is a general misunderstanding of what palliative care is. Many people think it is only for people in the end stage of their lives. This is not the case. Anyone with physically and / or emotionally debilitating symptoms may benefit from palliative care.

Palliative care focuses on the relief of the pain, stress and other debilitating symptoms of serious illness. Palliative care can be delivered at the same time as treatment that is meant to cure you. It includes someone with a new diagnosis of a chronic illness (MS, Leukemia) where there is an expectation of a longer life (perhaps even a cure in the future) albeit with a need for continuing care treatment.

The goal is to relieve suffering and provide the best possible quality of life for patients and their families.

Different from Hospice Palliative care is NOT the same as hospice care. Palliative care may be provided at any time during a person’s illness, even from the time of diagnosis and may be given at the same time as curative treatment. Hospice care always provides palliative care, however, it is focused on terminally ill patients-people who no longer seek treatments to cure them and who are expected to live for about six months or less. As Dr. Ric Baxter of St. Luke’s states: “Palliative care grew out of hospice by recognizing that there are a lot of people who have a chronic illness that they will eventually die from that affects their lives in some way, but they are still doing treatments or have more than six months to live – so palliative care is not defined by a time frame.”

Who may need Palliative Care?

Those needing palliative care can be: • Patients with a new diagnosis of an illness that requires treatment of debilitating symptoms but have an expectation of being cured.

• Patients who do not have a curable illness, but are not dying from the illness (emphysema, advanced heart disease, chronic cancer disease).

• Patients who are dying and need care for the last months of life.

Treatment Palliative care can be used at any stage of an illness if there are troubling symptoms, such as pain or sickness. In advanced cancer, palliative treatment may help someone to live longer and to live comfortably, even if they cannot be cured. Palliative treatment can also mean using medicines to reduce or control the side effects of cancer treatments.

The treatment is provided for various diseases irrespective of

Health their stage or severity. The side-effects of certain medications can also be reduced with the help of palliative treatment.

Palliative care relieves symptoms such as pain, shortness of breath, fatigue, constipation, nausea, loss of appetite and difficulty sleeping. It helps patients gain the strength to carry on with daily life. It improves their ability to tolerate medical treatments.

Team Approach Where available, palliative care can be provided by a group of individuals - usually a team of experts, including palliative care doctors, nurses and social workers.

Chaplains, massage therapists, pharmacists, nutritionists and others might also be part of the team. Typically, you get non-hospice palliative care in the hospital through a palliative care program. Working in partnership with your primary doctor, the palliative care team can provide: • Expert treatment of pain and other symptoms • Close, clear communication • Help navigating the health care system • Guidance with difficult and complex treatment choices • Emotional and spiritual support for you and your family How can I tell if I or someone in my family needs palliative care?

There are some signs that may indicate a need for palliative care. Of course you must consult your physician first. The indicators of a need for palliative care can include symptoms that are physically and emotionally distressing, frequently accessing hospitals and / or emergency facilities, and dependence on others for basic life function or an inability to care for oneself.

Lifestyles over 50 Celebrates Its Fifth Year

by Jeff Tintle, Lifestyles over 50 Pennsylvania is the second oldest state (after Florida) and the Lehigh Valley has the fastest growing senior population in the Keystone state. As one circulates through the A-B-E area one sees many medical facilities and care targeted to seniors like personal care and nursing homes. It seems that almost every day a new medical building pops up.

A 2007 report from the Lehigh Valley Planning Commission, based on the 2000 Census, projects a 46.3% growth in the over 50 population, putting total Lehigh Valley residents over 50 at 266,708 or 37.9%. Surprisingly the largest projected age group in 2010 will be those 75+ with an estimated 58,502 residents.

This may seem odd, given that the state is constantly fighting “brain drain”, the attrition of young professionals, and the fact that the Valley’s winters are less than ideal for

active seniors. In addition to aging boomers, the Lehigh Valley has tremendous hospitals and healthcare professionals.

When this is combined with the local culture, there is no wonder why most residents do not leave as they age.

Additionally, the Valley attracts young families to the area for job opportunities, better schools, safer communities and a lower cost of living. Often, these families have parents and grandparents who will follow.

As the Valley has expanded and gotten older, many opportunities emerged to serve this growing market.

Lifestyles over 50 published its first issue in April of 2006 with a mission to encourage vibrant and healthy living among boomers and seniors in the Lehigh Valley.

At a time when print media is fading, the magazine has been holding steady as it begins its fifth year in publication.

In previous articles I have reviewed several conditions and problems that can affect the feet. I have offered tips on prevention as well. This month, I would like to discuss some interesting facts of the foot.

The foot has 26 bones, 33 joints and over 100 ligaments.

The foot is a very complex structure. Average shoe size for a male is 11 and for a female is 7. Your feet may not be the same size or shape as each other.

Remember to purchase your shoes for the larger foot. For most, the big toe is the longest toe, but often the second toe can be the longer toe. When this is the case, measure for shoe size using the longest toe.

And during impact sports such as basketball, tennis, racquetball, up to 8 times body weight of force is transmitted to the feet.

This is why proper shoes and insoles are so important.

They will provide not only support to the feet, but help absorb the shock or forces that you are subject to.

Additionally the foot structure and function often impacts the ankles, knees, hips and back. Thus, when your feet hurt, you can hurt all over.

Sudden Cardiac Arrest versus Heart Attack

Knowing the difference can save lives According to research from the Heart Rhythm Society, more than 70% of Americans believe sudden cardiac arrest, SCA, is a type of heart attack. It’s like comparing apples and oranges. The common misunderstanding between SCA and a heart attack can lead people to overlook important risk factors that contribute to SCA.

In an effort to raise awareness about SCA and promote better heart health, the Heart Rhythm Society, an organization representing specialized clinicians trained to treat heart rhythm disorders, has launched a multi-year campaign aimed at helping individuals identify risk factors, understand how to properly respond and how to effectively prevent and treat SCA. Chief among its priorities, the Heart Rhythm Society hopes to help people better understand SCA, and is working to eliminate the confusion between SCA and a heart attack.

So what is the difference? In basic terms, SCA is an electrical malfunction of the heart, while a heart attack is a blockage in the blood vessels interrupting the flow of blood to the heart – essentially an “electrical vs. plumbing” problem. Knowing the difference could help save thousands of lives each year.

“Sudden cardiac arrest is a serious health issue.

Unfortunately, the public knows little about SCA, such as whom it affects and what can be done to prevent it,” says Richard L. Page, MD, FHRS, president of the Heart Rhythm Society. “The Heart Rhythm Society would like to encourage all Americans to learn more about SCA by becoming aware of preventative and responsive techniques that will ultimately save lives.”

Unfortunately, not only is SCA misunderstood, it’s severely underestimated. SCA is a leading cause of death in the United States, taking more than 250,000 lives each year.

Unknown by many, SCA claims more lives annually than breast cancer, lung cancer or AIDS.

The Heart Rhythm Society offers the following tips and advice. Who is at risk of SCA? The following risk factors may indicate an increased risk of SCA: • Family history of sudden death, heart failure or heart attack

The Value of Volunteering - Making a Difference

by Pat Nemetch, President APRN, Member of the PA State AARP Long Term Care Committee After a long, cold winter, blooming flowers, the start of another baseball season and other rites of spring are a welcome relief. If you’re like me, you’re happy to put all that snow in the rear view mirror for another year.

While April represents the first full month of spring, many local organizations also spend the time publicly recognizing the many contributions of volunteers. In fact, April 18-24 is officially designated as National Volunteer Week to spotlight the imaginative ways volunteers are contributing to communities. That’s not to say most true volunteers limit giving to a special week or time of year. For many organizations, it’s just a time to step back and thank volunteers for all they do throughout the year.

With volunteerism on the rise-- particularly among older adults-- it’s easier than ever before to find opportunities to help your neighbors and the community. One example is to visit AARP’s Create the Good website (www.createthegood. org) to become involved with local activities, including ways you can do things in your own way and on your own time.

Whether you have five minutes or five hours to offer, Create the Good can help you connect with a network of people, tools, and ideas that will help you make a difference here in our community.

No matter how you choose to volunteer, the rewards can be immeasurable. Sometimes the one soul you touch may change someone’s life entirely. In other cases, you’re lending your time to help a cause you support and think is right.

Just last month in Harrisburg, advocates for Pennsylvania’s elderly, blind, disabled, and faith-based communities packed the State Capitol Rotunda to ask Governor Rendell and legislative leaders to restore cuts in the State Supplemental Payments (SSP) for 345,000 of the state’s most vulnerable citizens, including many here in the Lehigh Valley.

Back in January, the Department of Public Welfare announced plans to reduce SSP payments for an individual by $5.30/month and by $10.40/month for a couple. The cuts were included in last year’s state budget agreement that was signed into law last October, and are continued in Governor Rendell’s proposed 2010-11 state budget.

Now I know those cuts don’t sound like much. However, individuals living on SSI are either too disabled or too elderly to work and are already living well below the poverty line. A reduction of $5.30 per month is a missed meal, a medical copay that cannot be met, or a paratransit ride that cannot be taken. I am optimistic that after hearing from advocates and those affected by the cuts in person, the Governor and state lawmakers will see fit to reinstate these much- needed funds for the poorest among our poor.

So as you gear up for spring, remember that when you pick up the phone or send a letter on behalf of a cause, you are advocating and volunteering at the same time. You’re also getting to know your community better, meeting new people, helping improve the lives of your neighbors and working toward the goal of creating good for us all.

Each month, about 6 residents of Traditions of Hanover volunteer for Meals on Wheels of Northampton County delivering as many as 28 meals to area residents. The group uses the Traditions of Hanover community bus to team up to deliver the meals. One volunteer drives, one navigates, some assemble the meals, and others take the meals to the doors.

Pauline and Bob, husband and wife volunteers, have been volunteering for Meals on Wheels for 2 years. They say that they feel good helping others visiting and chatting with shutins, seeing the valuable help that the volunteers provide and seeing the recipients’ appreciation.

One volunteer, Ellie said that many seniors are in need of help. “The outside of the homes are deceptive, they could be very neat but inside its very sparse with no furniture and very dark. Although the volunteers are the ones giving, it is obvious that they feel blessed. “It’s the least that we could do,” says

Once I Was Blind and Now I Can See

Steven Super was a 26-year old accountant with a bright future until he suffered a sudden life changing visual disability that left him legally blind in both eyes. It changed the way he looked at life and forced him to learn coping strategies and sharpen his other senses to live independently.

He made daily trips to the library for the blind to listen to books on tape to help improve his ability to hear and improve is memory. Through the process he empathized with others dealing with injury and illness and found a new passion for physical therapy.

In the fall of 1991, just 10 months after being diagnosed with a permanent visual disability, Steve enrolled in Temple University. Using only his auditory sense and the generous volunteers whom read class notes and books on tape, Steve graduated in the top 1% of his class. In 1996 he earned a Master’s degree in Physical Therapy at Hahnemann University which was one of the top three Physical Therapy schools in the country. He was one of only a few visually handicapped students in the United States to graduate from a Master’s in Physical Therapy program.

In the fall of 1998, just before the birth of his first son, Steve experienced a slow but miraculous recovery of most of his vision. The medical specialists were at a loss to explain such a recovery since his optic nerve was significantly damaged. In the summer of 2007 Steve earned a Doctorate degree in Physical Therapy at Temple University.

Steven Super has been a physical therapist for 12 years working in multiple capacities with all ages and varying disabilities. Steven has a uncommon appreciation for life and desire to help others. For these reasons he decided to open his own practice, Super Physical Therapy, in December 2008.

Understanding Long Term Care Insurance

We hear it all the time; people are living longer, the system is not equipped to handle the increasing number of older Americans, and health care costs so much.

One thing that can be done to safeguard your future and your finances is to take out a long term care insurance policy.

LTC provides coverage for health care issues that go beyond hospital care. There are different types of plans with different eligibility requirements and costs. In this article we’ll take a look at the details.

Eligibility: To qualify for LTC benefits, there should to be a need for help completing one or more ADL’s (Activities of Daily Living which include but are not limited to bathing, dressing, transferring and toileting) or have some cognitive impairment.

Some policies also require an elimination period of 20-120 days where the consumer pays for services before the policy will go into effect. In some cases, there may be no elimination days if the covered services are provided in-home and in accordance with a plan of care.

Benefits: Long Term Care Insurance generally covers services such as Home Care, assisted living, adult day services, Respite, Hospice, nursing home and dementia facilities.

Many people like this because they know they will not have to depend on children or family.

Another benefit is that it can keep seniors from exhausting their savings. In addition, premiums paid may qualify as an income tax deduction and benefits are generally not counted as income.

Types of Policies: Remember that premiums can be costly if you wait too long to purchase.

1. Tax Qualified (TQ): most common type. This type of LTC requires that a person need care for at least 90 days and be unable to perform two or more ADL’s without substantial assistance or they need to require care for at least 90 days and need substantial assistance due to a severe cognitive impairment. Benefits paid are not taxable

2. Non-Tax Qualified (TQ): Requires a “trigger” condition or some type of medical necessity. The doctor of the patient or one from the insurance company must state that the patient needs care before the policy will take effect.

The inability to do just one ADL is required for this type for policy. Walking is considered an ADL for NTQ policies. The taxability of these benefits has not yet been determined by the Treasury Department and is open to interpretation.

Fewer Non-Tax Qualified policies are available because consumers want tax deductions. When choosing a type of policy, it is advised that you seek good council and thoroughly review the pros and cons of each type.

Once a policy is purchased, the language cannot be changed and the policy cannot be cancelled due to health reasons, only non-payment.

Most benefits are paid on a reimbursement basis. Some policies offer different per-day rates. In any event, consumers are encouraged to get all the facts and figures of any policy before choosing to purchase.

Movie Review: Julie & Julia

If you have ever boiled water or cooked a dinner for eight you have heard of Julia Child. Knowing about the main character of a biography can sometimes take away from a movie.

You tend to compare what you know with what is shown on the screen. No worries about that in this movie. Everything that can be said about Merryl Streep has been said so I will not try. Her Julia Child is flawless. She captured the spirit of Julia Child.

The plot of the movie is simple. Based on a true story it follows Julie Powell (played very well by Amy Adams) deciding to make one of Julia’s recipes every day for a year in modern day Queens, New York.

The movie flips back and forth between Ms. Powell’s efforts and Julia Child’s own growth from a diplomat’s bored wife to an accomplished French chef in postwar Paris.

Julia Child proved to be an incredible woman and far ahead of her time. Before her, women were rarely accepted as French chefs, let alone an American. Her iron determination and sense of humor overcame every obstacle, and she eventually became an American icon.

Life is Good

for ten years or more; January and February were so nice there.

Time rushes by so fast, and it seems like yesterday we were wishing Happy New Year to everyone we met. It’s been very cold outside and I ask myself, where is the global warming that has been mentioned

We swam in a heated pool; and the beach was on our agenda, as we hunted pretty sea shells and enjoyed watching the Pelicans and long legged birds as they scanned the water for fish. The many species of birds were a real treat to our eyes, as we observed birds different from our birds back home.

Occasionally, we saw armadillos and very pretty butterflies of beautiful colors.

I say this because it’s cold right now, but it will soon warm up again and we can shed some layers of clothing and life goes on, and then we will find a different reason to complain and fret.

I read my Bible daily, and I just finished the book of Ecclesiastes. It holds one’s attention from beginning to the end. It tells us there is a time for everything and the list is long and true; and when I write a story taken from my notes, it speaks to me as I look in awe of Gods creation.

What helps to make it so interesting is that it changes with the seasons or length of time. Every passing day we see how Earth changes, and we learn something new every day.

I have my notebook with me, and it’s dated February 27, 1967. My wife Evelyn and I took a hike in the woods where we lived. We were avid bird watchers, but we also looked at trees, rocks, ponds, clouds, in general everything that held our interest. I took notes of these things, and now those notes have become a part of my story and are still interesting to me and also my readers.

When I retired 25 years ago, Evelyn and I were 62 years old and decided to see the many wonders of this planet we live on. We took notes, lots of them, and I read them from time to time. We went to Florida for two months every year

We did lots of walking and biking to places where few folks ventured, as they were off the beaten path. The wild flowers and weeds showed off their beautiful colors and aromas, and we enjoyed the surrounding beauty of God’s creations. If we just take the time to stop and observe close up, a whole new kind of scenery grabs our attention and it seems like we are in a different world and makes us see ordinary sights in a new and interesting point of view.

Thank God for our eyes and nose and ears. They make it all happen. Being able to enjoy the many facets of the world we live in was beautiful, and having my beloved wife with me made it even more beautiful and enjoyable.

She was a plain country girl and very gifted with the understanding of nature; and she taught me that what we call good or not good is here for a reason at some time in life.

As long as I live and can see and hear, I will always look for new sights and sounds as the seasons progress; and I always thank God for allowing me to live in his world and take time to thank and praise him for the blessing of life and family. Every day is special, so live it with gusto and love for all creatures great or small.

The Extended Hand

Teddy Roosevelt and William “Bill” Taft were best friends; in fact, Teddy groomed Bill to succeed him as President. Bill did succeed Teddy; however, things did not go as planned.

Teddy expected Bill to carry on Teddy’s policies in the White House, but Bill proved weak and ineffective. Their friendship broke down, and Teddy later ran against him in a bitter campaign. Both lost the race to Woodrow Wilson.

Five years later, Taft learned that Roosevelt was dining alone in a hotel, so he approached Teddy’s table. Teddy rose with hand extended. The two vigorously shook hands, slapped backs, and smiled and bowed to the cheering diners.

The former enemies were reconciled.

The concept of reconciliation has been much in the news of late, as the President scrambles to pass a health care bill.

We reconcile our checkbook to our bank statement. We may have to reconcile ourselves to a job or benefits loss.

Reconcile means to make friendly again; to harmonize or settle; to bring into agreement. Teddy and Bill were friendly again; the President wants to settle the health care issue; we bring our check register into agreement with the bank’s records. We harmonize our job situation with Psalm 103:2: “Bless the LORD, O my soul, and forget not all his benefits.”

The following verse focuses on the greatest of all benefits, which is the greatest story of reconciliation ever told: “Who forgives all your iniquities” (v. 3a).

In the Garden of Eden, God was grooming the first man and woman to carry on his policies in his kingdom. “And the LORD God commanded the man, saying, ‘You may surely eat of every tree of the garden, but of the tree of the knowledge of good and evil you shall not eat, for in the day that you eat of it you shall surely die’” (Genesis 2:16, 17).

Adam, Eve, and every person ever born has proven weak and ineffective, disobeying God, “for all have sinned and fall short of the glory of God” (Romans 3:23). Friendship with the Creator has broken down, “your iniquities have made a separation between you and your God, and your sins have hidden his face from you” (Isaiah 59:2).

Herein lies the reason for the Easter season. On Good Friday, we commemorate Christ’s death on the cross, bearing the penalty for our sins; “that is, in Christ God was reconciling the world to himself, not counting their trespasses

against them” (2 Corinthians 5:19). At the cross, God extended his hand to his enemies. He proved his sincerity by raising Jesus from the dead on Sunday: “He was delivered up for our trespasses and raised for our justification” (Romans 4:25).

Accepting Jesus as Lord and Savior makes us more than friendly: “To all who did receive him, who believed in his name, he gave the right to become children of God” (John 1:12). Believing in Christ harmonizes and settles accounts: “For our sake he made him to be sin who knew no sin, so that in him we might become the righteousness of God” (2 Corinthians 5:21). Confessing our sins brings us into agreement with God: “If we say we have not sinned, we make him a liar, and his word is not in us” (1 John 1:10).

When we let sin come between God and us, we lose.

When we are reconciled, the spectators cheer: “Just so, I tell you, there is joy before the angels of God over one sinner who repents” (Luke 15:10).

Avoiding U.S. Census Scams

The official U.S. Census, which is conducted every ten years and is described in Article I, Section 2 of the U.S. Constitution, calls for an accurate count of people living in the United States and is used as a gauge in the allotment of each state’s seats in the House of Representatives and in the allocation of funds to citizens in need. The first official Census was conducted in 1790 under Thomas Jefferson, who was the Secretary of State. That census, taken by U.S. marshals on horseback, counted 3.9 million inhabitants.

Since that time, the Census has been conducted every ten years.

While participation is safe, a certain level of caution is necessary when divulging personal information. Citizens need to clearly differentiate legitimate U.S. census employee communications from fraudulent activity. It’s important for people to be on guard against predatory individuals and organizations who piggyback on official US Census activities with their own copy-cat schemes to solicit private information. As the Chief Security Officer of America’s leading physical security services company, I believe it is vital to ensure that the American public understands the importance of participating in the U.S. Census while not becoming victimized by con-artists.

The Census Bureau seeks to determine household information pertaining only to the occupants’ age, address, phone number, gender, race, birth-date and marital status and whether you own or rent your home. You can view the questions at: http://2010.census.gov/2010census/ how/interactive-form.php. No census information will be taken via the Internet. Information requested beyond the information on this form is likely being perpetrated by scam artists.

The Census Bureau employs thousands of trained Federal employees to conduct surveys in person, over the phone or via the U.S. mail. Requests for Census information will be labeled from the U.S. Census Bureau as “Official Business of the United States.” A notice from the Census Bureau will be mailed prior to receiving forms, phone calls or visits from Census workers. Americans will be asked to complete and mail back the 10-question census form when it arrives in mailboxes around March 15-17 2010.

The following tips will help you recognize fraudulent activity or unofficial data collections to keep predators at bay while ensuring your valid participation in the important census process: •

Don’t Get Social – The U.S. Census does not request your social security number so do not divulge this

information to anyone claiming to be a U.S. Census representative.

Avoid Phishing Trips – The U.S. Census is not conducting any surveying via the Internet so any emails or other electronic communications received are fraudulent “phishing trips” that should be immediately reported as spam and deleted. “Phishing” is the unlawful practice of attempting to acquire private information such as usernames, passwords, social security numbers, bank account and credit card details by masquerading as a trustworthy person or organization in an email or via instant messaging. Often the “phishers” direct you to enter this private information at a fake web site that looks almost uniform to the legitimate one.

Don’t Pay Cents (or Dollars) for Census – The Census Bureau never asks for money or a donation. Steer clear of anyone purporting to be with the Census Bureau that asks for monetary donations. Similarly, the Census Bureau will not request passwords or access to bank accounts, credit cards and other financial information.

Be an ID Watchdog – If a census worker visits your home, ask for identification. It is the duty of every U.S. Census employee to provide their identification to every person that is being visited. All Census workers carry official government badges marked with their name. A Census taker will never ask to enter your home. The representative can provide you with supervisor contact information. You can also call the regional office phone number for verification or the Census Bureau call center at 1.800.923.8282.

Civility is Central to Census – U.S. Census employees are educated to be civil with every citizen or nonresident that they contact. If you come across an official who is acting in an intimidating or aggressive manner, you have the right to refuse to divulge information and to report this person to the authorities.

Census workers are bound by law to keep your information confidential and are not permitted to share it with anyone.

If you prefer not to share your information in person, complete and return the form you receive by mail from the U.S. Census Bureau.
